The subject of the study was the clinical use and the reliability of modern transpalpebral transscleral tonometry in patients with refraction anomaly before and after keratophotorefractive surgery. IOP measurements were carried out with transpalpebral diaton tonometer and Goldmann Applanation Tonometer (GAT). In consequence of the study central corneal thickness (CCT) corrected GAT IOP values before and after corneal surgery differ from each other, while diaton tonometry values remain stable. In the whole group, CCT corrected GAT and transpalpebral IOP-values were in good correlation. Regarding the differences in lid thickness: The Diaton measures over the tarsal plate. Higher up on the lid, individual lids may be of varying thickness, however at the #tarsus, there is less variability between patients. Additionally, the way in which the Diaton tip is designed, it very slightly flattens the area where the measurement is taken, resulting in more consistency. Of course, if there is a pathology such as generalized #eyelid edema, the use of Diaton would not be suggested.
