Esperança contra a recaída: vacina anticocaína e anticrack.

Um verdadeiro drama! Talvez esta seja a forma mais branda de descrever o sofrimento dos dependentes e seus familiares na tentativa de se livrar das "garras" da cocaína e drogas derivadas, como o crack.

Segundo a Organização das Nações Unidas (ONU), 18 milhões de pessoas no mundo consomem cocaína. Aproximadamente 25% desses usuários vão se tornar dependentes da droga. O Brasil é o segundo maior consumidor de cocaína do mundo, perdendo apenas para os Estados Unidos.

Claro que a raiz do problema está na produção e tráfico ilegais de cocaína gerenciados por poderosas organizações criminosas. Tentar combater a epidemia de cocaína e de crack nas cidades, na sociedade, nas famílias, sem adotar medidas de combate ao financiamento e à logística do tráfico de drogas é "tapar o sol com a peneira". No entanto, o que queremos aqui é mostrar mais uma promissora medida preventiva contra o consumo de cocaína e derivados.

Muita gente vai dizer ou pensar de forma correta que a educação, o lazer e o esporte, além da convivência harmoniosa entre as pessoas são os principais instrumentos para a prevenção contra o uso de drogas e todo o caminhão de malefícios que elas trazem.

Por sua vez, a ciência está nos mostrando uma outra poderosa arma preventiva contra as drogas: a vacina! Porém, antes de falarmos sobre ela, que tal se déssemos uma lembradinha de como a cocaína age em nosso cérebro?

A cocaína (cloridrato de cocaína) é uma potente droga estimuladora do sistema nervoso central, que bloqueia os canais de recaptação de dopamina (neurotransmissor ligado ao prazer) localizados na membrana plasmática dos neurônios pré-sinápticos. Assim, a cocaína provoca indiretamente "excesso" de dopamina na fenda sináptica, o que significa maior tempo de estimulação dos neurônios pós-sinápticos. Ocorre que todo esse processo é mais frequente nas áreas cerebrais ligadas ao chamado "sistema de recompensa". Nessas áreas, quanto "maior for a presença de cocaína, maior será o tempo de permanência da dopamina, o que resultará em mair sensação de prazer!"

O fato é que o mundo pode parecer "chato" para um usuário de cocaína quando ele não está utilizando a droga. "Dois palitos" para que ele volte a procurar a droga para se sentir "poderoso". Essa é a força motriz da dependência química!

A cocaína dura para sempre no organismo humano? Não, o fígado degrada a cocaína transformando-a em outros metabólitos. Em geral, o cloridrato de cocaína tem meia vida entre 30 e 90 minutos, porém os metabólitos originários da degradação da cocaína podem permanecer no organismo atuando por períodos mais longos.

Claro que o tratamento ideal para diminuir a dependência de cocaína passa pelo não consumo da droga, tarefa nem um pouco simples. Apoio da família, supervisão profissional e modo de vida harmonioso são os alicerces para enfrentar a dependência química.

E sim, é possível se livrar do vício dessa droga!

A ciência acena com grandes possibilidades terapêuticas. Já pensou se, de alguma forma, algum medicamento bloqueasse as recaídas, isto é, a vontade de obter novamente cocaína? A boa notícia é que pesquisadores de vários lugares do mundo, inclusive do Brasil, estão tentando desenvolver uma vacina contra a cocaína e drogas derivadas. Isto é fantástico!

A ideia é "aplicar" um antígeno (no caso seria a união da cocaína com algum tipo de proteína transportadora) no organismo do dependente químico para que ocorra uma resposta imunológica, isto é, a produção de anticorpos específicos contra a cocaína, além de uma memória imunológica por meio da formação de linfócitos de memória.

Nessa perspectiva, o usuário imunizado (vacinado) que tivesse, por alguma razão, uma recaída e que se drogasse, desta vez não sentiria (ou sentiria bem pouco) os efeitos prazerosos da droga, pois seu organismo seria capaz de lançar (ou produzir) anticorpos que se acoplariam à cocaína formando um complexo molecular muito grande, incapaz de transpor a barreira hematoencefálica e produzir algum efeito psicoativo no cérebro.

Bem, talvez você esteja se perguntando: o que é essa tal de barreira hematoencefálica?

Trata-se de uma estrutura formada por células endoteliais (tecido endotelial) que revestem os vasos sanguíneos que irrigam o sistema nervoso central, especialmente o cérebro. As células endoteliais têm alto poder de adesão entre elas, pois são unidas por diferenciações da membrana plasmática que limitam a passagem de moléculas e íons do sangue para o cérebro. Assim, a barreira hematoencefálica protege o cérebro, impedindo que determinadas toxinas e micro-organismos presentes na corrente sanguínea consigam penetrar no sistema nervoso central. A cocaína consegue passar por essa barreira.

Tendo como base a premissa descrita, a vacina anticocaína induz o organismo a produzir anticorpos que se acoplam à cocaína (e proteínas transportadoras) para formar complexos moleculares grandes que não atravessam totalmente a barreira hematoencefálica. Isso é bom, pois em tese a cocaína fica impedida de agir nas áreas cerebrais ligadas ao sistema de recompensa, diminuindo a presença de dopamina na fenda sináptica e limitando a "vontade" de experimentar novamente a droga.

Certo, mais em quais lugares do mundo há cientistas pesquisando a tão promissora vacina anticocaína?

Estados Unidos: universidades e instituições de pesquisa estadunidenses têm realizado estudos e ensaios clínicos para o desenvolvimento de vacinas contra a cocaína, dentre elas: Mayo Clinic, Baylor College of Medicine e Scripps Research.

Europa: países europeus, como o Reino Unido, a Alemanha, a Suíça e a Espanha, também apresentam instituições de pesquisa envolvidas no desenvolvimento de vacinas contra a cocaína, dentre elas: King's College London e a Universidade de Basileia.

Canadá: a University of British Columbia anunciou linhas de pesquisa nessa área.

China: os chineses investem em pesquisas relacionadas à vacina contra a cocaína, além de outras abordagens terapêuticas para fins preventivos.

Brasil: a gloriosa Faculdade de Medicina da Universidade Federal de Minas Gerais (UFMG) vem desenvolvendo também uma vacina contra a cocaína, utilizando matéria prima importada dos Estados Unidos. Isso ocorre porque, por foça de Lei, é proibida a utilização de cocaína no Brasil, mesmo para finalidade científica.

A vacina "mineira" contra a cocaína, batizada de "Calixcoca", demonstrou segurança e eficácia em estudos pré-clínicos com a utilização de animais em laboratório, inclusive mostrou-se segura na gravidez, resultando na produção de uma série de anticorpos que impediram a ação da droga sobre a placenta e fetos de ratas grávidas.

A Calixcoca vem chamando a atenção das autoridades sanitárias. A Prefeitura de São Paulo anunciou ajuda financeira aos pesquisadores mineiros para o desenvolvimento das fases sequentes da produção dessa vacina, inclusive com ensaios em humanos voluntários. Mas até à aprovação da vacina pela ANVISA "tem chão"! Alguns anos ainda serão necessários.

Put simply, addicts would no longer get high from the drug. ​If the treatment gets regulatory approval, it would be the first time cocaine addiction is treated using a vaccine, said psychiatrist Frederico Garcia, coordinator of the team that developed the treatment at the Federal University of Minas Gerais. ​The project won top prize last week – 500,000 euros ($530,000) – at the Euro Health Innovation awards for Latin American medicine, sponsored by pharmaceutical firm Eurofarma. ​The vaccine works by triggering patients' immune systems to produce antibodies that bind to cocaine molecules in the bloodstream, making them too large to pass into the brain's mesolimbic system, or "reward center," where the drug normally stimulates high levels of pleasure-inducing dopamine.

Vaccines for the following diseases were available during this time:

Smallpox- first generation vaccines were available and work on second generation vaccines occured in the 1930s with production of an egg-based vaccine begun by the Texas Department of Health in 1939. DC schools required smallpox vaccination for children to be allowed to attend by 1930, so Nico would have gotten immunized for this even if he didn't get anything else.

Diphtheria- first vaccine was developed in 1913, then a cheaper version in 1924. Yep, it's the "antitoxin" they call for in Balto (1995)- it can be used to either treat active infection or immunize.

Pertussis (whooping cough)- first vaccine was licensed in the US in 1914, then another in 1931, and another that became the basis for the modern vaccine in 1932.

BCG vaccine for Tuberculosis (TB)- first available in 1921, but neither the US nor Italy mandated it. This vaccine is still given around the world today.

Tetanus ("lockjaw") - first vaccine was produced in 1924, then a more effective version in 1938. The combo DTP (diphtheria, pertussis, tetanus) vaccine was first released in 1948 and was used all the way up to 1996 when a new, safer version was released.

Anthrax- the veterinary vaccine in use today is based on the one developed in 1935 in South Africa. The Soviet Union developed a human vaccine that was available beginning in 1940.

Yellow fever- the vaccine still in use today (17D strain) debuted in 1939.

The first rabies vaccine was developed in 1885, for crying out loud!

Penicillin was used to cure infections as early as 1930, though it didn't hit mass-production until 1945. Other antibiotics (Salvarsan and Prontosil) were in use in the early 1900s (by 1910 and 1935, respectively).

Thyroxin (1914) and insulin (1923) were known quantities for treating endocrine disorders.

Medical radiography (x-rays) was a thing before 1900. There were portable ECG/EKG machines as early as 1927.

Cocaine was taken out of Coke in 1903. Like, not even Maria di Angelo would remember that.

Yes, a whole bunch of things changed in medical science between 1942 and the mid-2000s- plenty of fodder for "Will blows Nico's mind with modern medicine." I will even provide suggestions!

Vaccines for major childhood illnesses: polio (1952), measles (1962), mumps (1967), rubella (1969). IMO the polio thing is way slept on given how big the March of Dimes got in the public consciousness.

like say you’re a child in victorian england and you have reached the age of nine. even if you’re upper-class, you’ve survived swill milk, cough medicine with cocaine in it, mercury in all your clothes, arsenic in all your walls, probably a cholera/typhus/scarlet fever/whatever outbreak (with no vaccines !)… you’ve made it past five, which is a feat, considering the high infant mortality rates. it’s even worse if you’re lower-class, because you’d probably also, on top of all that, survived 10-hour workdays with no labour laws for an absolute pittance. these kids were resilient.

All this generic alt-history fiction is F-15s this, gunpowder that.

If I had the time machine and time to prep a know how packet, I'd go to ~1200AD and grow a mercenary-merchant fleet of catamaran hulled ships.

The cornerstone of my fleet would be super-ship-biscuit with mixed in coffee (kills the weevils, keeps the sailors sharp), fortified with vitamin C (lemons, not limes) and nicotine (keeps the men sharp and under my control - leaving the ranks means withdrawal). There would be forbidden red biscuit (added cocaine) and blue biscuit (added heroine) for when things get extra rough.

I'd grow penicillin to demonstratively protect my crews from plagues even god cannot or will not stop as a stopgap while running extremely aggressive vaccination research (It'd go so much faster with liberal usage of orphans for experiments/culture growth; you could get so many fucking orphans back then, they were just free)

All the ranks would be taught Marxism and a prophecy of the wold ending demon that will be unleashed after the gods are forgotten and kings are killed if money still is a focus then.

Build a network of merchant hubs that'll earn sustenance mostly through external trade and fishing, with avoidance of grabs of land if it involved any displacement of locals. While external trade is commercial, internally they should rely on money as little as possible.
