I went diabetic earlier this year, since then I've had far more serious health concerns to really focus on it. I've listened to my primary care and reduced my average a1c from 13 to 7. I've recently been looking into diets and what not that are the best. Currently, I'm trying to cut out all carbs, on my doctor's orders. What I'm seeing though is that a plant based diet is best. It looks like a ketogenic diet is what my doctor wants me to follow? I've watched videos on both diets and I don't know, I agree with you that keto is evil. What are your thoughts about this?
I am not a medical professional so i can't give you medical advice, but I'd say that you should ask your doctor for a referral to a dietician (an RD or an RDN, NOT a "nutritionist" - RD/RDN are protected terms that mean they have completed specific training and have specific board certification) and ask the dietician for advice on your specific dietary needs for your specific medical conditions.
What I can say is that trying to cut all carbs is pretty dangerous - not only is it a macronutrient that our body uses as the most available fuel for your body processes (we *can* get fuel from protein and fat, and ketones can *theoretically* replace sugars for energy but nobody is actually sure how long our bodies can do that and we know it's a LOT less efficient, it's supposed to be less efficient, and what that means is it makes a lot of people feel exhausted when they try it because they literally have less available energy) but also there are certain nutrients that are fortified in the US that are going to be hard to get if you're cutting carbs completely. The example that I always use is folate, because when I had to cut wheat out of my diet (i have grain allergies and celiac disease) I didn't know to supplement it and ended up with a form of anemia and stuff like "fainting" and "dizziness" and "low oxygen saturation."
Which is part of why massive diet changes should be undertaken with the assistance of a dietician! That's why I started studying nutrition! Because nobody supervised my medically necessary diet changes and it went very poorly!
Your GP very likely doesn't have a ton of training on nutrition, and is even less likely to have training on nutrition specific to your condition. If your GP is telling you to cut all carbs, they are telling you to do something dangerous and not nutritionally sound (even really restrictive keto diets call for 20g of carbs a day). Ask either them or your endocrinologist for a referral to a dietician (again, you are looking for a Registered Dietician or a Registered Dietician Nutritionist, RD or RDN, NOT just 'nutritionist') who is familiar with helping diabetics manage their nutrition.
Now, all of that said, in the choice between two fairly restrictive diets I will always say to try the one that requires less effort. It is much easier to eat a plant-based diet long term than a keto diet, and it is vanishingly unlikely that you are going to end up protein deficient (the primary concern for most people who are starting plant based diets, and it's just not all that likely - we need a lot less protein than a lot of people seem to think; though if you're going completely vegan you do need to be careful to supplement your B vitamins and to ensure that you're getting plenty of omega fats)
Because the thing is, for a diet to "work" you have to be on that diet forever. If you stop being on that diet, and stop adhering to its restrictions, whatever benefits exist for that diet go away. So the best diet for *anybody* is one that will provide them with the nutrients they need in a way that they can access regularly and affordably, that they enjoy eating and can comfortably maintain for long periods of time, and that includes a variety of fruits and vegetables because the only diet advice that is nearly universally applicable is that people should be eating more fruits and vegetables and they should be eating a wider variety of them.
I am not a fan of "diets" as a concept and I think that people should think of nutrition in terms of "my diet" not "the diet that is meant to be one-size-fits-all for millions of people that I am attempting." Your diet is what you eat and drink, and that is what you should be looking at adjusting. If you want to reduce carbs in your diet it's better to tweak your consumption than it is totally replace your diet with a one size fits all keto diet. If you want to increase fat in your diet it is better to tweak your consumption than it is to replace your diet with a one size fits all atkins diet. If you want to go plant based I think it is better to start by adjusting your diet to include more plants and to slowly replace animal based products than by trying a one size fits all vegan diet right out of the gate. You can always (and should!) make adjustments to what you eat as circumstances change and you may end up at a vegan diet or a low carb high fat diet and find that that works for you, but part of the reason that I think nutrition studies on diets are so screwy and hard to pin down is because your body is going to *flip the fuck out* when you change from, say, an average american diet to a study-provided Mediterranean diet for a 12 week experiment. If you drastically change your diet all at once and get good results immediately it's very hard to say if those results will be lasting because your body may just adjust to the "new normal" of your diet six months down the line.
But like seriously if your GP is telling you to cut all carbs you need to see a person who specializes in nutrition, and to prepare for your appointment with that person you should make a list of your goals (for you it sounds like you want to manage your blood sugar levels, reduce a1c, and *ask about* low carbs if that is something that interests you), a list of things you think that you'll have trouble with or that you want to include in your diet because they're important to you (if you really like nuts but have to be on a low fat diet, ask if there's a way to work around that with your needs, for example; if there is a cultural staple that you will find difficult to cut from family meals, TELL THEM THAT), a list of questions that you have about different types of diets, and *VERY IMPORTANTLY* information about your food budget and cooking skills. Be clear about it if you can't cook. Be clear about it if you can't afford certain ingredients.

As multiple work stoppages continued across the United States, Democratic Sen. John Fetterman of Pennsylvania on Thursday introduced legislation that would enable striking workers to qualify for federal food aid.
Called the Food Secure Strikers Act of 2023, Fetterman's bill would amend the Food and Nutrition Act of 2008 to ensure that striking workers aren't excluded from receiving Supplemental Nutrition Assistance Program (SNAP) benefits. In addition, the bill would preserve food stamp eligibility for public sector workers who are fired for striking and clarify that any income-eligible household is entitled to SNAP benefits even if a member of that household is on strike.
"Every union worker who is walking the picket line this summer needs to know that we have their back here in Washington," Fetterman said in a statement. "The union way of life is sacred. It's what built Pennsylvania and this nation. It is critical for us to protect workers' right to organize, and that includes making sure they and their families have the resources to support themselves while on strike."
"As chair of the Nutrition Subcommittee and an advocate for the union way of life, this bill is just plain common sense," he added. "I'm proud to introduce this bill that will eliminate the need for workers to choose between fighting for fair working conditions and putting food on the table for their families."
Workers typically forgo pay when they exercise their right to walk off the job in pursuit of higher wages and better conditions. Although union strike funds sometimes provide workers on the picket line with a stipend, it is less than their regular income.
Under existing law, striking workers and their households are ineligible to receive SNAP benefits unless they already qualified for food stamps prior to withholding their labor. This gives employers significant leverage over employees who can only endure economic hardship for so long. By repealing the current restriction on striking workers securing SNAP benefits, Fetterman's bill would help restore some balance to the struggle between capital and labor.

"It's good to see lawmakers attempting to correct the wrongs of the past by reinstating a benefit for striking workers that never should have been taken away in the first place," said International Brotherhood of Teamsters president Sean O'Brien. "Congress should never pass laws that punish American workers and hopefully this amendment is a repudiation of that practice."
O'Brien spent the past several weeks preparing 340,000 United Parcel Service (UPS) warehouse workers and delivery drivers for what would have been the second-largest work stoppage at a single employer in U.S. history, trailing only a 1970 strike of 400,000 General Motors workers. Although a UPS strike has likely been averted after the logistics giant and the Teamsters reached a tentative five-year contract agreement on Tuesday, Fetterman's proposal comes amid a nationwide wave of ongoing and potential labor actions.
"The United Auto Workers have mirrored the Teamsters' militant stance, blasting CEOs ahead of their own contract negotiations slated for later this year," The Intercept reported Thursday. "And the truckers union has staged trainings in dozens of cities for a strike that could shut down shipping from coast to coast. In California, meanwhile, thousands of hotel workers organized with Unite Here are already on strike, along with tens of thousands of Hollywood writers and actors belonging to the Writers Guild and SAG-AFTRA, respectively."
The walkout of 160,000 writers and actors, who are fighting for improved remuneration and attempting to safeguard unionized jobs threatened by artificial intelligence-induced automation, is perhaps the most well-known of the current strikes.
Earlier this month, an anonymous studio executive admitted to Deadline that "the endgame is to allow things to drag on until union members start losing their apartments and losing their houses," drawing widespread condemnation, including from star actor Ron Perlman.
The Food Secure Strikers Act is designed to counteract the delay tactics that bosses rely on to crush workers.
"Workers who make the difficult decision to go on strike are coming together to lift the standard of living and gain more respect for all working people," said Becky Pringle, president of the National Education Association (NEA). "They are prepared to make sacrifices—but going hungry should not be one of them. The Food Secure Strikers Act of 2023 will help ensure that when striking workers stand in solidarity for better working conditions and wages they can receive SNAP benefits so they don't put themselves and their families at risk."
The legislation is co-sponsored by Sen. Bernie Sanders (I-Vt.) and 10 Senate Democrats, including Sherrod Brown (Ohio), Elizabeth Warren (Mass.), and Ron Wyden (Ore.). A companion bill was unveiled in the House by Democratic Reps. Alma Adams (N.C.) and Greg Casar (Texas).
It is also endorsed by numerous unions and anti-hunger organizations, including the Teamsters, NEA, the United Food and Commercial Workers, the Communications Workers of America, the Food Research Action Center, and Hunger-Free America.
"We need to get rid of the anti-union provisions in our code that starve striking workers," said Casar. "We're seeing workers exercise their rights across the country by going on strike to demand better wages and working conditions. That's why our bill, the Food Secure Strikers Act, is more important now than ever. We need to stop starving strikers, and ensure all working families are able to make ends meet."

It is unique as another elephantine monster akin to Gammoth, albeit smaller and less powerful. It distinguishes itself through the four stout tusks used for digging and the myriad of mushrooms grown onto specialised tissues on its back and forelimbs. These mushrooms are a specialised form of parashroom perfectly adapted to mutualism with Paraphan. Paraphan is a fungivore specialised to digging up the mycelium of large fungal systems found around forest and grassy habitats. Its trunk can also hoover up the spores given off by many fungi to supplement its diet. Occasionally it will sample plants, tubers and fruit, but mushrooms form the core of its diet. Paraphans can be unpredictable in their behaviour; some individuals seem receptive to human presence and curiously examine them, whilst others show aggression at the slightest hint of approach. Field workers must be extremely careful to take note of which Paraphan they are studying, lest it turn out to be a brutish personality. Males and females of the species live separately; the smaller females gather in herds of up to twenty individuals led by a matriarch, whilst the larger males live as roaming bachelors. While females are rarely troublesome, concerned only with keeping their young safe, the males can prove aggressive, especially when in musth, and so must be closely monitored. Paraphans are not normally destructive, but an irate male or uncontested herds of females can destroy flora, necessitating hunting or repelling. The key to Paraphan's success is the unique strain of parashrooms that have bonded to the fanged beast. Sustained on fat reservoirs stored in specialised tissues, these parashrooms benefit from the fanged beast by way of nutrition and spreading their spores wherever the monsters travel. In return, the Paraphan occasionally feeds on the spores and caps, as well as exploiting the paralytic potential of the mushroom to repel predators. The brush-like comb at the end of their tail is often coated in these spores, ensuring a quick lash whenever Paraphan has need of it. Beyond its capacity to inflict paralysis through launching its symbiont's spores, Paraphans are also incredibly large and powerful monsters. They can easily crush most other monsters underfoot or swings of their muscular trunks and huge tusks. They are also surprisingly intelligent in making use of their environment; Paraphan's have been known to inhale water through their trunks and spew it out at other monsters, or even picking up rocks and tree trunks to use as bludgeons. Paraphan is not considered to be an exceptional monster (Low Rank - 3, High/Master Rank - 2) though it is not to be underestimated. Combining its strength with paralyzing spores and surprising cunning allows Paraphan to catch even veteran hunters off-guard. Make use of pitfall traps to exploit the fanged beast's bulk, and fire-based weapons are good for destroying the spores and fungi that it relies on. While not invulnerable to predation, Paraphan's impressive size and defensive integrity usually makes it too troublesome for most predators to bother with. The exceptional is the brute wyvern Alkanodos, whose cunning and venom outplays the slower fanged beast. Fortunately, Paraphan is known to form mutualistic alliances with the temnoceran Jansilla. The fast and keen-eyed Jansilla sights danger promptly, and complements the might of Paraphan to repel opponents neither could handle individually.

ASAN is dismayed by the debt ceiling deal recently signed into law. While Medicaid remains protected, ensuring access to Home- and Community-Based Services (HCBS) and healthcare for many members of our community, the deal does harm to other safety net programs.
The debt ceiling negotiations were a manufactured crisis. Congress and the administration were not trying to agree on how the government will spend money in the future. Instead, the negotiations were about whether the US will keep its promises to pay for goods and services it already bought. If Congress had not raised the debt ceiling in time, the government would not pay all of its bills on time and the consequences of this would have been devastating. There would have been a recession in the US and possibly in other countries. Marginalized people— including disabled people— would have been hit especially hard by job losses. Benefits payments to individuals and federal wages might have stopped. Delayed Medicare and Medicaid payments could have caused chaos in the health care system and interrupted the HCBS many of us need to live in our communities. These consequences would have especially affected marginalized communities.
Congress should have raised the debt ceiling without trying to change federal spending, as it regularly did prior to 2011. It was irresponsible for some members of Congress to put Americans at risk of interrupted benefits and health care access, and people around the world in danger of job loss and poverty, just to try to cut benefits programs. Congress should decide what the country will spend in budget negotiations, not put millions of people at risk of hardship by arguing about whether to pay for what the country has already bought. 
The outcomes of the debt limit deal are mixed. We are glad that Medicaid was protected from funding cuts and new work reporting requirements. ASAN sends our thanks to everyone who contacted their legislators to protect Medicaid, and helped keep this vital program unchanged.
We are disgusted at the way the debt ceiling deal harms people who count on other lifesaving programs. The debt ceiling deal caps federal spending on many safety net programs for two years. This means that the programs’ budgets will not keep up with inflation. Because of inflation, these programs will need more money to fund the same amount of services, but the cap makes sure they will not get increased funding. This will work like a budget cut over time, hurting people who are part of these programs now or will need them in the future.
Congress also added more work reporting requirements to the Temporary Assistance to Needy Families (TANF) program and raised the age for work reporting requirements on the Supplemental Nutrition Assistance Program (SNAP). That means that people on SNAP will have to continue meeting work reporting requirements until age 55 from now on, rather than 49. Fewer people will be able to get these critical benefits. Those who can still get help through these programs will have to fill out more paperwork and go through complicated processes that are often inaccessible. While there are some exemptions for disabled people, this ignores the fact that many disabled people cannot get a diagnosis due to disparities in healthcare costs and access. This is especially true for multiply marginalized people, including people of color and trans people. People without a diagnosis will not be able to use these exemptions. 
People will lose access to the benefits they need as a result of the funding caps and new work reporting requirements. Many of those people are disabled or otherwise marginalized. The decisions made during the debt ceiling negotiations will not meaningfully change the government’s debt. What they will do is cause mass suffering for some of the most marginalized people in our society. Parts of the debt ceiling deal, like cuts to funding for tax collection, will increase how much money the government has to borrow to meet its financial commitments. It is clear that the members of Congress who demanded changes to federal spending in exchange for raising the debt ceiling did not prioritize improving the federal government’s finances. Instead, their top priority was slashing assistance to the most vulnerable Americans.

The fight for our community’s dignity and access to lifesaving essentials, like food, home- and community-based services, and access to healthcare does not end here — and we remain dedicated to seeing it through.
