Star Trek Episodes 60-63
Spectre of the Gun: So our heroes pissed off an alien force...again, and are punished by being put into the Wild West and taking up the roles of the OK Corral shooting. You know what? At this point after the gangaters, Nazi’s, and Roman gladiators I’m not even gonna question anything anymore. I gotta wonder of they did this episode just to prank Kelley since apparently he’d been in reenactments of the OK Corral during his Western carrer haha. Anyways, it was fine. I’m not really into Westerns, but I’ve seen enough from what my dad used to warch to know what to expect. Acrually it felt more like a deconstruction since those normally put the Wild West in a bit of a glorious light which is VERY much not the case here. Chekov was a freakin’ trip in this episode and it acrually got me when he got shot even though having seen The Motion Picture I knew he came back to life. There’s nothing too notable about the episode, but nothing wrong with it either and it was fun to see the cast in a Wild West setting. Just a nice watch all in all. 3.5/5.
Day of the Dove: Well points are already deducted for the blatant use of brownface. IDK if thus was dome with the Klingons before, but the obvious use here is especially bad both makeup wise and for other obvious reasons. Moving past that, the concept is a decent one. We know of the frictions already between our heroes and the war-seeking Klingons, so sticking them all in one setting and letting the tensions implode due to a third party is a decent idea. Infact, the third party is essentially acting as an influence, altering everyone’s state of mind into further hatred amd madness just to keep fueling that fire, even between their own allies. That’s always a fun concept, although that influence causing Chekov to nealry do... what he did to Mara was... distasteful even if he wasn’t in his right mind. Seeing our heroes, who certainly have their darker urges but normally keep it in check, becoming more hateful even towards each other is... really uncomfortable. I guess this was meant to be an episode against being overwhelemed by hatred, and it did so fine enough. Though the brownface DOES kind of cut into the message. Otherwise, perfectly good episode. I can’t give it a 5 because of the brownface and some fo Shatner’s oveacting as well as feeling we disn’t get a proper wrapup at th end, but I can still give it a decent rating. 4/5.
For the World is Hollow and I Have Touched the Sky: McCoy centric episode! Yay! So... our doctor has conducted a terminal illness that has no cure. IDK what xenopolythemia is, but regular polythemia is a blood cancer. I’ve both lost family to cancer and seen the general paint hat it and treatment does to them, so... yeah. Of course McCoy’s symptoms are just his body beind weak, which kinda sucks since there was a lot of potential here for an emotional story (though tbf it’s likely early symptoms since he was just diagnosed), especially since it’s the caregiver being sick and needing to be cared for. But they decided to use it to have him get with an alien prieestess because he doesn’t have even a year left to live.. which tbf given what I’ve read on his backstory since the show never got into it, it DOES make sense for him and I guess it was his turn after Kirk and Spcok’s flings with women (McCoy’s only other time was back in the start of S1), but still. It wasn’t done basly, I fully understood why he’d make that choice since if he’s gonna die in a year, he should be able to have it with someone who’d care for him and Natira genuinely does and he still made sure that Kirk and Spock would be able to get out safely. Kelley did a lovely job with his performance this episode and it made McCoy’s decisions and thoughts very clear, and me having looked up his backstory probably helped me feel for him more than I would have otherwise. And because of the episodic nature, we got an easy way out. I guess I’m just disappointed at the waste of potential, but the episode is overall fine. Standard at this point, but perfectly fine. It did a good job making McCoy feel weakened and Kirk and Spock’s concern for him was really sweet. Spock holding onto McCoy’s arm after beind told was genuinely touching. It was a nice episode and I enjoyed it especially since it focused on my favorite character, I just felt it could have done more and the thing that helped the episode feel stornger was essemtially supplementary meterial (albiet canon material) and is just an okay episode if one doesn’t know it. 3.5/5.

Yeah it feels a little too similar to Day of the Dove, which was just two episodes ago especially with Chekov being the first to lose it. We also have them losing Kirk... again, though they make the wise decided to keep his fate in the dark and this time it truly looks like this is it for him (it’s not, but still) as Spock tries to retrieve him despite putting the ship more at risk. Pretty illogical, huh? Haha! This was good because we get to see how the crew reacts with him gone,a nd then when declared dead, without the audience alreayd knowing that he’s fine, which helps us feel for them as we’re as uncertain as they are. The highlight here is Spck and McCoy as with Kirk deemed dead, the one that keeps them together and the mental influence isn’t helping especially on McCoy’s end. Spock is trying to get Kirk back while McCoy wants to get out due to the crew having psychotic episodes due to remaining. The two come into conflict as per usual but without Kirk there, that sense of compromise is gone. Then the two see the message that Kirk recorded specifically for them in the event of his death, telling them to trust and help each other and their reactions after it especially hit hard. It shows how much Kirk knows and trusts the two and he knows that they can go on without him... which given that it’s canon that he’s the first to die for real among them (sorta... and depressing since Shatner is the only one of the trio still alive... wow I made myself sad), is even more heartwarming in hindsight. It was a really good episode and the ending got a good giggle out of me. But it was good to see what happens when Kirk is out of the equation, and how Spock and McCoy can go forward and work together with mutual respect. Excellent episode! 5/5.
I thought about doing five today... but I just wasn’t feeling it. But after a mediocre start, we got a pretty good round of episodes this time. Not as good as S2 (aside from the last one, that one was fantastic), but perfectly fine. Will that continue? Fifteen more to go folks, we’ll find out soon enough.

Happy thing: I got a new plant. If it survives long enough, I’ll name it.
I’ve been posting a lot about BLM, protests, Sars-Cov-2, tear gas, police brutality, and more of that will be coming. It’s hard to post about school when the world is on fire but also because class is all on-line and I don’t want to post zoom lectures (a photo of a fellow student or lecturer). Also time passes so slow and then suddenly it’s gone and I feel like I haven’t done anything. it’s 12:21 am, forgive me for my ramblings.

Personal life I probably shouldn’t share on the interwebs:
My grandfather got real sick and was in the hospital for 5-6 days after going to the ER. Just this week he got sick again and refused treatment, seems to be getting better but...that’s a thing that’s happening. My sister is 7ish months pregnant and found out one of her friends knowingly exposed her to someone who came into contact with someone with a positive covid test (so two people away). She found this out while at my house, with my mom, dad, me, and both grandparents. My cousin is symptomatic and he’s immunocompromised wife has moved herself into their guest room with their 3 year old son. Dad was individually exposed by someone else. And this Monday (6/22), me and my mom were in the exam room when they put one of our cats, Tigger to sleep. He was 18 years old. They think he had a stroke sometime in the night. He couldn’t really move. We tried to see if he would pass at home, but he kept trying to drag himself around, so we took him in. Needless to say, I’m not handling things well.
School:
I re-rescheduled my NBEO exam now that availability has increased. (Just a week later, but I need an extra week after the one I’ve just had.) The original process was absolutely insane! There was a national misunderstanding on the time zone and when the sign-up would unlock. It was supposed to be EST 12:01 am but they based it off EST without DST so it was actually an hour later than we thought (so 1 am east coast). Once we were let in to the scheduler, we expected times and dates from July 27th to August 15th. Those dates technically existed...nationally. Major cities (in major states) had some dates, the location I wanted had 2 days to choose from total. Other states had nothing until the alternate dates in November! Entire states with nothing! Some people got slots that said 12 am or 4 am and a few other times. They took the slots because there was nothing else and expected it to be a glitch, only to be informed the next day that those testing sites were open 24 hours! imagine taking an 8 hour exam at midnight. (I’m awake, but not mentally aware enough to a doctoral exam). Some people signed up out of state and plan to make the drive or FLY to the test. The schools nationally made them fix it and it eventually was, mostly due to the states decreasing social distancing regulations. Not sure how this will change with certain states getting hit hard and needing to bunker down again. My school actually did something for once and actually communicated on our behalf.
The town hall committee I was on to change the summer curriculum was half effective. We got two courses to alter (read: decrease) the course load from “Bat-Shit Insane” to “We only hate you a little” but still won’t start externships until August. My state’s current ...state of things has me worried if this won’t get pushed back again, if I won’t get to graduate on time at all. With the current plan, I still get to keep all my original rotations in their original order. I’m very happy about this, but it’s bittersweet in that I’ll probably miss the birth of my niece (though at this rate, it wouldn’t be safe for me to see her for some time as a healthcare worker) and I’ll be spending my birthday alone at my first site. Usually I’d drag the family to a theater production/performance of some sort the week of my birthday, but those are all probably cancelled by now anyways. We have been told that during the in-house rotation (at school) we won’t have to wear our white coats and are allowed to finally wear scrubs and black athletic shoes if we’d like to. These are to give us a better option for heath. If we choose to wear our white coat, it must be washed on a daily basis, which just isn’t practical. Multiple copies of the same scrubs and plain t-shirts to wear underneath is more accommodating to our current needs. In a few months, I’ll be dressing in a routine uniform as a healthcare working in a pandemic that can incubate in the organ I’ve decided to specialize in.
That post about “May you live in interesting times” and finally understanding why it’s a curse/threat. Yeah, I get it now.

Help! Our daughter hates ergs

I have a very different and specific case of Erg Hate that I am hoping I can get some input on finding a solution to. Please bear with me as this is very long and very tough.
The person who hates the erg with all passion is my 19 year daughter.
First the background:
My daughter rowed all 4 years in high school with considerable on-water success winning several high school national championship races and others. They also logged vast miles on the erg during the winter. Her erg results at competition and in testing were quite reasonable for her age group, particularly since she weighed only 115lb. She always placed in the top group but was never quite fast enough to make “elite” status. Of course, only a very few are that fast. That was fine with her as she had no aspirations to compete at the elite level or beyond university. Both her high school and the club she rows at used erg testing only to rank the athletes for seat racing. There were never any cuts and team and boat selection was never done based on erg results alone. My daughter was quite successful with seat racing and consistently made the top boats. Even though she wasn’t the strongest girl on the team, the coaches usually assigned her to stroke seat because of her tenacity, technical ability and her ability to pull an amazing finish out of nowhere.
Tragically, just before she was to head off to university for her first year and with the hopes of rowing on the varsity team, her 17 year old very close friend and crew mate died suddenly while completing a 2k erg test. As expected, this was devastating to not only my daughter but to our whole rowing community. One week before my daughter headed off to training camp, along with her 4+ crew, she spoke at and was a pall bearer at her friend’s funeral.

The first 2k test after tragedy
The first full day of training camp, the team hopefuls were required to complete a 2k erg test. The coaches had been made aware of the situation with my daughter and were most accommodating and understanding. Not wanting to appear wimpy in front of the others or having the others think that she was getting special treatment, my daughter opted to pull the 2k test despite having a great deal of fear. For the hour before the test, she hid in the bathroom vomiting with the anxiety. Despite this, she persevered and completed the full 2k. Understandably, her time wasn’t that great and was almost 20s off her PB. Her father and I were amazed that she completed it at all and to us, that was a tremendous win for her, regardless of her time. Thankfully, the coaches had a kind spirit and offered her a spot on the varsity team based on her exceptional on-water results. That fall she rowed varsity and enjoyed the warm team environment immensely. During the fall season (the main competitive season for Canadian universities), there was very little erging; they trained on the erg only on a few occasions of inclement weather.
The new rowing coach comes in
Once the fall season was complete, a brand new head coach took the helm and changed the rules for the team. Even if the rowers had made varsity for the fall season, they had to make a minimum erg standard in either a 2k or 6k to remain on the varsity team for winter training (keeping in mind that in Canada, the main university rowing season is the fall. There is no spring on-water season – once April exams are complete, the students head home for the remainder of the spring and summer and row with their home club). Again, the anxiety for my daughter resurfaced. With a lot of calls home with many many tears, she opted this time to abstain from doing the 2k even though in the past she had made the particular standard that was set. She was however, going to give the 6k a shot. She was almost successful with the 6k, coming up only 13s short of making the standard (which happened to be a new personal best for her despite what she was going through) and performing better than many of her more senior lightweight teammates. Interestingly, most of the other lightweight girls did not make the standards either and were subsequently cut. I believe that only 2-3 lightweight girls made the standard.
Like the other girls who didn’t make the standard, even though most of them were in their senior year and had rowed for the university for 3+ years, my daughter was cut from the winter varsity team. She was sad for herself but also very sad for all the other girls that had participated in the varsity program for the last 3 years and had enjoyed securing 2nd place wins in the province for the previous several seasons and now found themselves cut from the team. (First place consistently went to the university located where the women’s national team trains and many women who have high hopes of making the national team someday select this university to attend. They have an intense program and they work hard.)
Persevere with training
Despite the setback, my daughter wasn’t ready to give up on the sport and she committed herself to training by herself during the winter. With no direction from the new head coach, she made up a workout program for herself that involved a lot of cross training – running and cycling as well as weights. As time permitted, the now retired former head coach lent a hand to her. Erging was kept to a minimum of mostly 2 and but as often as 3 days per week. That was all she could bear of the erg. Because she wasn’t allowed to use the varsity ergs, she instead used the few poorly maintained ergs in the general athletic centre. Most of them didn’t have a drag factor that worked and the chain felt like it was being pulled through gravel. Despite the loneliness, she persevered over the winter, maintaining as much fitness as she could while juggling her demanding science courses and adjusting to university life away from home. She enjoyed her first year, loves the university itself despite the new direction the rowing team has taken and made loads of friends. I think she did a stellar job.
When she returned home in April after the school year ended, she hit the water right away. I had been lucky enough to purchase a lightweight single (for myself) that she trained in everyday. She continued to row and compete for our club all summer with mediocre results in her first year as a U23 lightweight. I think that she was a little disappointed, not realizing the depth of competition at that level, but she was the only U23 woman at our club and she sure doesn’t have a passion for racing singles! Rather she prefers fours and quads because of the wonderful camaraderie that they offer her. She is definitely a big team girl! Our club did move some younger girls up so she had an opportunity to row the bigger boats as well for which she is thankful for. Actually, she ended up stroking a U23 Womens 4x to a gold medal at our provincial championship regatta with these Junior girls.
Anyway, to bring this story to an end and get to my point, the new head coach continued his requirement for all rowers to achieve an erg standard to be granted a spot on the varsity team for the fall season. She attempted both the 2k and 6k and was short in both so didn’t make the team. The standard set is an aggressive “elite” standard and much more stringent than the previous winter standard and the women’s team was trimmed from previous years by more than 75%, the men by more than 50%. The new coach’s preference is to have a very trim elite level team that focuses only on small boats. Not my daughter’s cup of tea. No other university in Canada has this approach from what I can tell.
Like the other girls who didn’t make standard, my daughter rowed for the local club for the fall season where they competed in head races rather than the university circuit. She enjoyed it even though it didn’t have the same “glitter” as representing her university. Now that the winter is here again, she didn’t even bother trying to make the more relaxed erg standard set for the winter as she knows that the current team environment isn’t what she is after. The new university team no longer has the warm team environment that she enjoyed. She instead has joined some of the other rowing cast offs and is training with the university triathlon club for the wonderful team environment they have even though she has no intent of competing in that sport. And she continues to do weights on her own which she loves.
The ergo remains a problem
Sadly though, my daughter has developed an absolute hatred for the erg machine. While she no longer fears it as she did right after her friend died, the erg simply represents to her so much loss. Loss of her friend and crew mate and the lost opportunity for the warmth of a large team environment because of the new direction that the university team has made of making elite erg standards so necessary.
As we know, the erg can be a wonderful training tool and with her continued desire to row at the club level, she may miss the opportunity for more sport specific training. I also fear that if she never overcomes her feelings for it, it will haunt her in the future. I don’t want her to reach some point in her life where she has regrets.

I was looking through my twitter timeline on 12 June, when I came cross a tweet by Dietram Scheufele which said “��bend nature to our will.’ #CRISPR frame in new #Doudna book might resonate differently across audiences […] #scicomm”. The tweet made reference to an article by Sharon Begley in STAT News about Jennifer Doubna’s new book, co-authored with Samuel Sternberg, A Crack in Creation: The New Power to Control Evolution, in which they reportedly write that it “won’t be long before CRISPR allows us to bend nature to our will”. I became curious about this new book and its grand claim.
The book: publicity and reactions
On 16 June, Jennifer Doudna, a pioneer in a new way of modifying or ‘editing’ genes called CRISPR Cas9, came to the UK to talk with Adam Rutherford about her new book at a Hay Festival event held at the Royal Society. CRISPR Cas9 is based on exploiting the way that bacteria snip out parts of an invading virus’s DNA “and keep a bit of it behind to help them recognise and defend against the virus next time it attack”. Scientists, like Doudna, have “adapted this system so that it could be used in other cells from animals, including mice and humans”.
The blurb for the event (which I could unfortunately not attend) says: “’A Crack In Creation’ also asks us to consider what our new-found power means: how do we enjoy its unprecedented benefits while avoiding its equally unprecedented dangers? As Doudna argues, every member of our species is implicated in the answers to these questions. Somehow we must consider and act together. The future of humankind – and of all life on Earth – is at stake. This book is an essential guide to the path that now lies ahead.” (From what I can ascertain, unicorns were not mentioned at the event)
It appears that Jennifer Doudna wants to stimulate public debate about this new technique for genetic manipulation or gene or genome editing. This is a great example of what some call ‘upstream’ engagement with an emerging technology by one of its founders. This is also an example of what people would now call responsible innovation. As one reviewer of her book, Peter Forbes, says: ”It’s always difficult when something like this [gene editing] happens to sort the hope from the hype, but anticipation is now intense. Doudna does, though, sound many notes of caution.”
Another review of Doudna’s book by Nathaniel Comfort, is a little bit more critical and says: “Rather than guiding us through the ethical thickets of precision genetic engineering, or providing a candid, warts-and-all look at one of the great scientists of our time, the book mainly polishes her ‘good scientist’ image and rationalizes the unfettered self-direction of human evolution, within liberal bounds of safety, efficacy and individual choice.” I became more and more curious.
Unicorns, dragons and flying pigs
Let’s get back to the article mentioned in the tweet by Scheufele. After reading that tweet, I read the article by Sharon Begley for STAT news entitled, intriguingly, “CRISPR pioneer Doudna envisions a world of woolly mammoths and unicorns”. We are told that: “This Doudna doesn’t hold back. We are ‘on the cusp of a new age in genetic engineering and biological mastery,’ she and Sternberg write, dangling the prospect of ‘life-changing treatments’ and ‘lifesaving cures.’ She says she is ‘not kidding’ that CRISPR could bring about ‘woolly mammoths, winged lizards, and unicorns. … It won’t be long before CRISPR allows us to bend nature to our will.’”
Wow, I thought, these are some claims. I also asked myself whether this is really how one sorts “the hope from the hype”? And I wondered whether and how the unicorns were picked up by other news articles reporting on the book (they were not mentioned in the Forbes and the Comfort reviews).
I found an article for CBC radio that notes: “Imagine a world where genetic diseases can be edited and cured, where pigs can be ‘humanized’ to use their organs for transplants in people — and where we might even run into unicorns or winged dragons.”
Here lizards have turned into dragons. Interestingly, dragons had been discussed in the context of gene editing, but one of the co-authors of the book had intimated that they would not be created: “Dr Sam Sternberg – formerly of the University of California’s Doudna Lab, which pioneered work with CRISPR-CAS9 – said […] he is not hopeful genetic engineers could ever cross the Rubicon to create dragons.” (BBC interview, 3 January, 2016)
The other article appeared in The Daily Beast and was entitled “New DNA tech creating unicorns and curing cancer for real”. Unicorns and dragons are here joined by yet another mythical creature: “Scientists say that CRISPR-Cas9 may soon allow them to perform miraculous fixes to eliminate or alter mutations that cause everything from some cancers to Parkinson’s disease. More whimsically, the technology could be used to create, say, a unicorn, or a pig with wings; though it’s unlikely they could make swine fly.” Lizards have turned into flying pigs….
Unicorns in context
I was now thoroughly hooked; I ordered the paperback edition of the book; and read it. It is a breezy and enjoyable account of the whirl-wind in which Jennifer Doudna found herself after the discovery of CRISPR Cas9, a new way to target and edit genes.
This rather personal account of excitement and collaborations is interspersed with cautious reflections on the social and ethical implications of CRISPR, but also with references to mastery (“over the code of life”, p. 113; “to rewrite the very molecules of life anyway we wish”, p. 119) and control (chapter 4 is called “Command and Control” echoing the subtitle of the book: “The New Power to Control Evolution”). Control and caution jostle with each other.
I also found the unicorns. They make their appearance in chapter 5 entitled “The CRISPR menagerie” (paperback, p. 117). Here is the quote that got some people so excited: “Within a few decades, we might well have genetically engineered pigs that can serve as human organ donors – but we could also have wooly mammoths, winged lizards and unicorns. No I am not kidding.”
The next paragraph goes on to say: “It amazes me to realize that we are on the cusp of a new era in the history of life on earth – an age in which humans exercise an unprecedented level of control over the genetic composition of the species that co-inhabit our planet. I won’t be long before CRISPR allows us to bend nature to our will in the way that humans have dreamed of since pre-history”. We are back to the tweet that set me off in chase of unicorns.
I couple of pages later the book’s sometimes breakneck foray into a new CRISPR world populated by novel plants and animals, including unicorns, is somewhat slowed down by a note of caution: “We have a responsibility to consider the ramifications in advance and to engage in a global, public, and inclusive conversation about how to best harness gene editing in the natural world, before it’s too late.” This is a call for ‘Responsible Research and Innovation’ or RRI.
Unicorns and responsible language use
A Crack in Creation has, it seems, three aims: firstly and mostly to convey the excitement of CRISPR; secondly to point out emerging risks (especially with relation to germline editing); and thirdly to stimulate public debate. My question is: are unicorns the right way to go about stimulating this debate?
I have written quite a few posts about CRISPR, some more about responsible innovation, and some others where I try to include responsible language use in the RRI agenda. I have also written an article with Carmen McLeod entitled “The dilemma of raising awareness responsibly”. The book certainly exposes this dilemma.
Viewed from the perspective of RRI and responsible language use, A Crack in Creation might therefore not only stimulate public debate about CRISPR; it might also stimulate debate about responsible language use. As we have seen, unicorns grab public and media attention; but are they good examples of responsible language use, especially since they can easily morph into flying pigs?
Master builders
This episode in the history of gene editing and public engagement reminded me of the early days of nanotechnology, where excitement mingled with concern and public discussion. At the time (around 2003/2004), a former colleague of mine, José López wrote about the trope of the master builder which he saw running through some of the early nanotech discourses, a trope that is also commonly used in science fiction. In an interview he pointed out:
“’Looking at science fiction helps us understand how we talk about nanotechnology. Science fiction usually relies on revolutionary technology or a major event to define how society works. Nanotechnology gets talked about like that—some argue that it will change everything,’ López says. This world-changing technology or event is called a novum—time travel and parallel universes are classic examples. Science fiction then typically includes a master builder who interacts with the novum and drives the story line. ‘With this mindset, we perceive nanotechnology—and the scientists driving it, the master builders— as a way to reconstruct a new world,’ López says. ‘We’re speaking the language of hubris to think we can control the future in this way. We have to remember that nanotechnology is still in its infancy.’”
This is perhaps something we should reflect on in all the excitement and promise surrounding gene editing today, as it has implications for science, society and science communication.

Going under...again.

Well after much back and forth and strategic procrastination, I ultimately decided and finally got up the gumption and nerve to share this very private thing with you all...
2 weeks ago today, on April 11th, 2017 at 7:30 am, I underwent bariatric surgery for the second time in 8 years.
The procedure I had is not as common or popular as some of the more well known procedures: lap-band, gastric bypass, or gastric sleeving. I underwent a procedure known as a duodenal switch (DS) which is a combination of a restrictive approach (which is what the band and sleeve are) and malabsorption (which is what bypass is). In this procedure, the small intestine (the duodenum) is rerouted in order to prevent the absorption of certain fats and nutrients (malabsorption). During the second part of the procedure, a sleeve gastroectomy is performed in which a portion of the stomach is removed, leaving it resembling a banana. Hence, the name gastric sleeve. This portion is considered restrictive because it restricts the amount of food one can consume at one time, thus leading to weight loss. In layman’s terms, the surgeon goes in, rearranges some stuff and cut some stuff out to help you lose weight.
I can just about imagine the look on some of your faces as you’re reading and you’re probably wanting me to just cut to chase and explain to you how in the hell I got to the point that I would undergo this process not once, but twice. How does one’s weight loss struggle lead them down the path of risky surgical intervention twice in 8 years?
Nine years ago, when I first made it up in my mind that I wanted to pursue the surgical route, I was at my heaviest: seven pounds shy of 400 pounds. I mean I was literally looking 400 pounds in the face! Now up to that point, I had tried and had been somewhat successful at various weight loss interventions. I had done Weight Watchers, gone to a couple dietitians, tried my own weight loss and exercise regimen. In fact, I can even remember my mother taking me to Jenny Craig when I was in about fifth grade. ( Yes, fifth grade! I’ve been at this weight loss thing my whole life!) Anyway, none of them created long lasting and/or extensive results. I would get overwhelmed and discouraged at the thought of losing an entire person and a half “naturally”. It just didn’t seem rational or doable in my honest opinion. So I did some research, and I finally decided that surgery was the best route for me.
Now up until that point, the only bariatric surgery I was familiar with was gastric bypass and I was absolutely terrified of it. I had heard the horror stories of how they slice you down the middle and rearrange your guts and how painful it all was and about the risk of death from complications. But more than all that, I was discouraged by the way people who had undergone bypass looked. They all seemed to have the “lollipop syndrome”. You know, where the head looks really big and the rest of the body looks really small. And I heard about the skin. Oh Jesus the extra skin! I was terrified of losing a large amount of weight and not looking like myself; but a saggy, big-headed version of someone that used to be. So when I spoke with my doctor and told him of my concerns, he referred me to my hospital’s bariatric program. It was there that I first learned of the lap-band procedure which was all the rave at the time: a procedure in which a silicone band is placed around your stomach laparoscopically, reducing the amount of food one can eat, thus causing weight loss. It was a laproscopic procedure which meant no slicing me down the middle which meant shorter recovery time and less risk. The weight loss would be a slower process than bypass which meant my body would adjust to the weight loss easier which meant less saggy skin and no lollipop syndrome. I thought, “Yes, yes! This is the answer! Lap-Band it is!” Sadly I was mistaken...
In preparation for surgery, I went through six months of nutrition and group counseling with other bariatric patients (which I honestly hate but we’ll explore that on another day). I underwent a battery of medical and psychological tests to ensure I was fit physically and mentally fit to undergo a major surgery, and finally I had to go on a liquid diet for 3 weeks. Morbidly obese people (which I am unfortunately considered) have enlarged livers. An enlarged liver makes it difficult to operate on the stomach. So I was put on liquids in order to shrink my liver. Those were thee hardest 3 weeks of my life! Oh and did I mention it was Christmas time?? I was weak, pale, fatigued...miserable! While the rest of the family ate Christmas dinner, I ate chicken broth and drank V-8. It was pure torture. But my mind was focused on the end result: smaller, healthier, happier Tia! In that 3 weeks, I lost nearly 35 lbs! And on December 26th, 2008, I went under the knife and would come out a new woman!
The first few months after surgery were great. None of my clothes fit; I had to buy all new clothes. I had a collar bone. I could cross my legs. My knees weren’t hurting anymore. I was working out with a trainer. Eating right. I mean life was good... Except the vomiting. Oh and the times my band would slip out of place causing me excruciating pain. Then the weight loss plateau. Then the realization that I NEVER made it out of the 300s. And that, in fact, the most weight I had lost was when I was on the liquid diet pre-op.
Now, don’t get me wrong. I had lost about 4 dress sizes and had gotten down to a size 20 in jeans (I used to be a 26/28 or 30/32). But it bothered me that I was still holding steady at around 330. Then this past year, I saw my weight slowly creep back up to the tune of a 40lb weight gain. My knees began hurting again. My clothes no longer fit me. I noticed I got easily winded. Things that I was so glad to be able to do like cross my legs, I could no longer do. I was devastated. I was disappointed. I was disgusted...
So I began my research again. I began looking up the data on Lap-Bands. And lo and behold, the research now shows they aren’t really effective for the morbidly obese and that a great majority of people who had bands had them revised to other procedures. Ain’t this a blip! So here I am with this foreign object in me, that may times causes me discomfort, and it ain’t even working! WTF! I decided to go back to my same primary care doctor. He heard me out. He listened to my frustration and he understood. He told me he supported me and he would do whatever he needed to do to make sure I got what I needed. He referred me to the University of Chicago Center for Surgical Treatment for Obesity, and in December 2016 I began my bariatric journey all over again.
I have to say the team at U of C is amazing! They have taught me so much and they are truly passionate about their patients. At my initial consultation, I met with the psychological team, the nutrition team, my nurse practitioner, and finally my surgeon. They listened to me, they addressed all my concerns, they gave me tons of information and answered all my questions. The most significant moment was sitting with my surgeon one to one and going over my options and what he felt was best. It was during this conversation that I learned I should’ve never gotten a band. He explained to me that a person with my BMI (52) is not a good candidate for a band because bands simply aren’t effective in enabling significant amounts of weight loss. He told me that he doesn’t really even do them anymore because in his experience they don’t work. Period. He was visibly upset that a surgeon recommended and pushed banding on me without providing me other alternatives. He wished he had known me 8 years ago. He also expressed to me his disdain for surgeons who claim that the band is reversible; its not. It is removable, but not reversible because of how they stomach is altered due to the scar tissue caused by the placement of the band and the port. Before I knew it, I was looking this man in his eyes with tears in mine. I felt both anger and relief. I was angry that I wasn’t presented with all my options. But I was relieved at the fact that it wasn’t all me. I didn’t fail the procedure; the procedure failed me. It was in that meeting that we decide I would move forward with the removal of my band and would revise to a duodenal switch.
Again, I went through the pre-op medical stuff: medical tests to ensure I was physically able to withstand another surgery, nutrition class, and pre-op diet (no liquids this time: just high protein, low carb, low fat). And on April 11th I walked through the doors of the University of Chicago Center for Care and Discovery, said a prayer, kissed my teary-eyed mother goodbye, and went into surgery.
Recovery this time has been way more intense. The Lap-Band procedure is a relatively simple procedure. I went home that same day. However, the DS is way more involved and required a couple nights’ stay in the hospital. I was in a great amount of pain in the abdominal area. My energy is extremely low. I’ve been on pain meds and stool softeners and some other medicine for my gallbladder. I have to take give myself 2 injections per day in my side which has left my stomach a mural of blue and purple bruises. If I don’t inject myself, I run the risk of getting a blood clot which could cause death. Some days are good; others not so much..
So back to how I got to this point. I was tired of being fat yall. There is no other way to put it. I love myself. Don’t get me wrong. But my weight is something that I have never been totally comfortable with. I have dealt with it. I haven’t let it keep me from living life totally. But I wanted it gone. And this is what I felt was best for me. Yes, I know some people say, “just eat right. Work out. Be disciplined. You’ll lose it!” And while I don’t disagree with those methods, in fact I STILL have to do those things, that simply was not enough for me. People struggle with losing 10 to 20 lbs. Can you fathom losing 200?
Contrary to what others may think, weight loss surgery is NOT the easy way out. This is THEE hardest thing I’ve ever had to do and I’ve done it TWICE. Surgery is only a tool! It doesn’t work on its own. I have to still work it. I have to be disciplined. I have to be focused. I have to be determined. Now more than ever, because at this point my life literally depends on it. I have to take about 6 dietary/vitamin supplements for the rest of my life. If I don’t, I run the risk of malnutrition and other health complications. So no, this is’t easy. Not by a long shot. But when you’ve been fighting the war on weight literally all your life, there comes a point where you have to make some hard decisions. And this is what I decided. I know my family and friends were concerned and quite frankly some were not in agreement, whether they expressed it outwardly or not. But again, this was a decision I had to make for me; no one else
For those of you who may be considering, weight loss surgery, please take into consideration the above-mentioned things. But first and foremost, have a real conversation with yourself. Ask yourself, is this what you want? Why do you want it? Are you looking for an easy out? Are you ready for the changes that will come with it? Are you strong enough to stand in your decision?
Remember at the end of the day, this is about you, your body, your health, your happiness...
