so one of the things that's so horrifying about birth control is that you have to, like, navigate this incredibly personal choice about your body and yet also face the epitome of misogyny. like, someone in the comments will say it wasn't that bad for me, and you'll be utterly silenced. like, everyone treats birth control like something that's super dirty. like, you have no fucking information or control over this thing because certain powerful people find it icky.
first it was the oral contraceptives. you went on those young, mostly for reasons unrelated to birth control - even your dermatologist suggested them to control your acne. the list of side effects was longer than your arm, and you just stared at it, horrified.
it made you so mentally ill, but you just heard that this was adulthood. that, yes, there are of course side effects, what did you expect. one day you looked up yasmin makes me depressed because surely this was far too intense, and you discovered that over 12,000 lawsuits had been successfully filed against the brand. it remains commonly prescribed on the open market. you switched brands a few times before oral contraceptives stopped being in any way effective. your doctor just, like, shrugged and said you could try a different brand again.
and the thing is that you're a feminist. you know from your own experience that birth control can be lifesaving, and that even when used for birth control - it is necessary healthcare. you have seen it save so many people from such bad situations, yourself included. it is critical that any person has access to birth control, and you would never suggest that we just get rid of all of it.
you were a little skeeved out by the implant (heard too many bad stories about it) and figured - okay, iud. it was some of the worst pain you've ever fucking experienced, and you did it with a small number of tylenol in your system (3), like you were getting your bikini line waxed instead of something practically sewn into your body.
and what's wild is that because sometimes it isn't a painful insertion process, it is vanishingly rare to find a doctor that will actually numb the area. while your doctor was talking to you about which brand to choose, you were thinking about the other ways you've been injured in your life. you thought about how you had a suspicious mole frozen off - something so small and easy - and how they'd numbed a huge area. you thought about when you broke your wrist and didn't actually notice, because you'd thought it was a sprain.
your understanding of pain is that how the human body responds to injury doesn't always relate to the actual pain tolerance of the person - it's more about how lucky that person is physically. maybe they broke it in a perfect way. maybe they happened to get hurt in a place without a lot of nerve endings. some people can handle a broken femur but crumble under a sore tooth. there's no true way to predict how "much" something actually hurts.
in no other situation would it be appropriate for doctors to ignore pain. just because someone can break their wrist and not feel it doesn't mean no one should receive pain meds for a broken wrist. it just means that particular person was lucky about it. it should not define treatment.
in the comments of videos about IUDs, literally thousands of people report agony. blinding, nauseating, soul-crushing agony. they say things like i had 2 kids and this was the worst thing i ever experienced or i literally have a tattoo on my ribs and it felt like a tickle. this thing almost killed me or would rather run into traffic than ever feel that again.
so it's either true that every single person who reports severe pain is exaggerating. or it's true that it's far more likely you will experience pain, rather than "just a pinch." and yet - there's nothing fucking been done about it. it kind of feels like a shrug is layered on top of everything - since technically it's elective, isn't it kind of your fault for agreeing to select it? stop being fearmongering. stop being defensive.
you fucking needed yours. you are almost weirdly protective of it. yours was so important for your physical and mental health. it helped you off hormonal birth control and even started helping some of your symptoms. it still fucking hurt for no fucking reason.
once while recovering from surgery, they offered you like 15 days of vicodin. you only took 2 of them. you've been offered oxy for tonsillitis. you turned down opioids while recovering from your wisdom tooth extraction. everything else has the option. you fucking drove yourself home after it, shocked and quietly weeping, feeling like something very bad had just happened. the nurse that held your hand during the experience looked down at you, tears in her eyes, and said - i know. this is cruelty in action.
and it's fucked up because the conversation is never just "hey, so the way we are doing this is fucking barbaric and doctors should be required to offer serious pain meds" - it's usually something around the lines of "well, it didn't kill you, did it?"
you just found out that removing that little bitch will hurt just as bad. a little pinch like how oral contraceptives have "some" serious symptoms. like your life and pain are expendable or not really important. like maybe we are all hysterical about it?
hysteria comes from the latin word for uterus, which is great!
you stand here at a crossroads. like - this thing is so important. did they really have to make it so fucking dangerous. and why is it that if you make a complaint, you're told - i didn't even want you to have this in the first place. we're told be careful what you wish for. we're told that it's our fault for wanting something so illict; we could simply choose not to need medication. that maybe if we don't like the scraps, we should get ready to starve.
we have been saying for so long - "i'm not asking you to remove the option, i'm asking you to reconsider the risk." this entire time we hear: well, this is what you wanted, isn't it?

Hello again, and welcome back to Shining Through PCOS! Today weāre continuing a topic that resonates deeply with many of us living with Polycystic Ovarian Syndrome (PCOS)āhirsutismāand how our hormone imbalances contribute to this challenging symptom.
The Hormonal Connection to Hirsutism
Hirsutism, the growth of coarse, dark hair in areas where men typically grow hair, is one of the most distressing symptoms of PCOS. While itās not uncommon for women to experience unwanted hair growth at some point, for those of us with PCOS, it often feels like a relentless battle against our own bodies. Understanding the underlying hormonal imbalances can help us navigate this challenge with more compassion for ourselves.
Previously, we discussed how some of the conditions we deal with as a result of PCOS overlap. At the heart of hirsutism in women with PCOS are elevated levels of androgens, often referred to as āmale hormones.ā While everyone has these hormones to some degree, women with PCOS typically have higher-than-normal levels (Rizvi et al., 2023). This hormonal imbalance is primarily due to something called insulin resistance, a condition where the bodyās cells become less responsive to insulin, leading to higher levels within the bloodstream (Barber et al., 2019). Elevated insulin can stimulate the ovaries to produce more androgens, which in turn contributes to symptoms like hirsutism, acne, and irregularities in ovulation (Rosenfield & Ehrmann, 2016).
Understanding the Role of Androgens
Androgens play a crucial role in hair growth, but not in the way we might hope. In women with PCOS, the overproduction of hormones like testosterone can lead to the development of male-pattern hair growth (Mihailidis et al., 2015). Common areas for hirsutism include the face, chest, and back, making it a source of significant emotional distress for many of us (Mihailidis et al., 2015).
I remember grappling with so much embarrassment over the abundance of body hair I dealt with as a child. I felt perpetually frustrated and humiliated in my own skin. Itās a tough battle to handle feelings of inadequacy, especially at a young age when youāre not only worried about your self-perception but also how others perceive you. Many people may tell you itās ājust body hair,ā but they underestimate the toll it can take. It affects how you perceive yourself, deprives you of your femininity, and impacts your ability to relate to others and feel a sense of normalcy and autonomy. Itās so much more than just hair.
For many women, including myself, it goes beyond mere appearance; itās about how we feel in our skin. The pressure to meet certain beauty ideals can lead to feelings of shame and isolation, particularly in visible areas where hair growth canāt be ignored. It's vital to remember that these feelings are valid, and seeking supportāwhether through friends, family, or professional helpācan be an important step in managing the emotional toll of PCOS.
Finding Hope in Treatment Options
While hirsutism can be a challenging aspect of living with PCOS, the good news is that effective treatment options are available. Laser-Based Hair Reduction (LBHR) has emerged as a popular choice for many women looking to manage excess hair growth. However, itās essential to remember that LBHR targets the symptoms, not the underlying hormonal imbalance. Combining this treatment with lifestyle changes and, in some cases, medications like anti-androgens can create a more comprehensive approach to managing hirsutism (Narang et al., 2018).
As a student dermal clinician, I believe it is crucial for service providers to be as transparent as possible. While LBHR is an excellent treatment for addressing the rate of hair growth, length, and density, it is not a simple fix. It becomes even more complex with how LBHR is marketed as āpermanent removalā when it is, in fact, āpermanent reduction.ā This requires a multidisciplinary approach, which can be a lengthy process. Providers must understand when a client needs this comprehensive approach and when to refer patients for aspects of a condition that may be outside our scope of practice.
Unfortunately, patients are often taken in without understanding the complexity of their condition and are not encouraged to seek medical advice regarding their hormonal levels. Itās essential for providers to discuss all aspects of PCOS instead of offering LBHR as a mere band-aid solution.

I hope and pray that one day the skincare industry will lose its grip on us.
"Celebrity X uses product Y!" They're literally rich and make money off of being pretty and I'm supposed to believe they use a $35 product that was invented four months ago?
Hollywood is a cesspool of looking as unnaturally attractive as possible while maintaining that this is just "how I look normally" because no one can know you had to work for your looks (because that somehow makes you less attractive?)
They use vitamin A supplements or some expensive treatment, or they get plastic surgery and like about it because they've been taught to be ashamed. And that's if their skin isn't already perfect, because;
It's all genetics!!!! That's all it is!!!
Yeah acne begins with puberty and is heightened during - because your hormone are out of wack! But if you have acne beyond puberty you'll probably have it for a while. I had acne so bad I would regularly get abscesses in more humid areas (re: my coochie and armpits). Once it got so bad that I have to have two drained and it was too painful even under local anaesthetic so I had to go under general anaesthetic. I got put on vitamin A supplements, which dry out your skin, which was the best my skin was for a while (at the cost of cracked lips). And while it was less severe after puberty I still have acne!!!
Don't get me wrong, skincare is good for you. But everyone's skin is different in thousands of small ways! The chances that the hot new product on the market is what exactly matches your skin are slim to none! Especially when most of the products are for one type of problem and people have more than one type of problem. Now why could that be? (hint: it's because they want you to buy more product)
All in all, short of literally altering your hormones (which is why the pill helps some people) skin products won't do much for acne. They can still help if you have dry, oily, etc. skin, and even help with acne (but not to the extent of completely clearing it unless your acne is bacterial/environmental), but your goal shouldn't be clear skin it should be healthy skin.
Don't trust the Hot New Product, try different things and see what works for you to get a personalised routine.

i hate to be that guy but ur post on acne is very much giving ālet men be masculineā. like a small community of people online saying that acne should be considered value neutral and thereās too much pressure to get rid of it by literally everyone in their lives is not materially significant at all in comparison to the billions of dollars spent per year to make everyone with acne ranging from moderate to severe and cystic feel evil and ugly so we buy their products
the post isn't about feeling ugly and evil, it's about being in pain. it was a post i made in about 5 minutes while irritated at someone in my irl life who was saying those things.
sure, acne is value neutral!! i agree! i mentioned this in the post!! i think the pressure to get rid of acne is bad and the marketing on products is bad as well.
it still causes me pain though, i still ruin shirts because it bleeds sometimes. i still have trouble putting on bras.
i am not insecure about my acne anymore, i don't care about that. i care about the physical pain it causes me.
i did not say that the community of people online is equivalent to the multi billion dollar market. i did not say this at all. i pointed out there is sometimes nuance in these things.
i am not saying "let people spend thousands of dollars towards companies and treatments to solve a non-issue!! it's their right!!" i am saying that sometimes acne is incredibly painful and wanting to get rid of it is not playing into that.
i don't really care if i'm fully acne free or not, i just want it to stop hurting!! if i had mild acne that wasn't painful, i would be fine with that.
