Status Post #11098: Their membership status as part of the Avengers were relegated to not officially sanctioned in 2020 with the Empyre Handbook despite being allies with the team or used their name, which also includes the likes of A-Force, Avengers Idea Mechanics, Occupy Avengers, Savage Avengers, SHIELD led Secret Avengers, Ultimates, U.S. Avengers and the second iteration of West Coast Avengers, which I strongly disagree with the decision to do so since it caused more problems than resolving them, especially that they were honoured with their own statues in The Avengers v1 #690 (June 2018).

365 Marvel Comics Paper Cut-Out SuperHeroes - One Hero, Every Day, All Year…
Supporting Character Supplemental - Pod
The extra-terrestrial entity known as Ex Nihilo had created a powerful weapon that he dubbed the Pod Systems Armor.  He sent it to earth and it crash landed in rural Norway.  A young woman named Aikku Jokinen had been nearby the crash site and the armor inexplicably bonded to Aikku, bestowing her the powers offered by this highly advanced technology.  This boding process also temporarily took over Aikku’s consciousness and caused her to bolt off to Australia to attempt to defeat the Avengers.  
The Avengers were overwhelmed by The pod’s abilities yet ended up saved when the villainess known as Superia trapped Pod in a pocket dimension.  Superia was the then head of AIM and she planed on reverse engineering Pod for her own nefarious purposes.  Before she could do so, however, Superia was ousted from AIM and the organization was bought and taken over by Roberto De Casta (whose plan was to transform AIM into a force for good).  
De Cast recruited scientist, Toni Ho, into AIM and she ended up worked tirelessly to free Aikku Jokinen from the Pod Armor.  Toni was unable to completed detach Aikku from the armor, but she was able to restore Aikku’s ability to be in control and operate the armor of her own volition.  The two women fell in love and became a couple; both opted to join De Casta’s USAvengers Team.  Some time after the disbanding of the USAvengers, Aikku and Toni became involved with the scientists of G.I.R.L. and The Blue Marvel in running a worldwide crisis relief as part of the Avengers World initiative.
The heroine first appeared in the pages of Avengers Vol. 5 #4 (2013).

I really like that line up! Stingray and Sandman are some of my usual go-to’s when constructing hypothetical Avengers’ line ups myself.
If I may?
1) Captain America: It just doesn’t feel like the Avengers without Cap
2) Wasp (Janet): Likewise, I consider Jan to be pretty much an essential Avenger.
3) Beast: Providing scientific expertise, I’ve always felt Hank worked better in a more public facing team than the X-Men themselves. (Granted, right now you’d need to do a lot of character work)
4) Spectrum (Monica): I like Spectrum as a name better than Photon, but Monica has long been a favorite character.
5) Firebird: Another I’ve just always liked and she’s one of the few truly explicitly religious characters at Marvel (Daredevil and Nightcrawler being the other big ones) so there’s some character mileage there too.
6) Rage: Comic book time is a hell of a thing, but he’s surely got to be 18 by now. Time to give him some growth and another chance. I do like Victor Alvarez, but I’d actually kind of like to see Elvin take up the Power Man name. Or at least get some new one that doesn’t rely on “angry black man” stereotypes.
7) Rescue (Toni Ho): From Sunspot’s USAvengers, likewise providing some tech support, some (slightly) pacifistic design philosophies, and none of Tony’s arrogance.
I also like your idea of reservists, though in my case I might call it “specialists”:
Hank Pym: For any radical science stuff the others can’t handle
Stingray: For underwater stuff
Nova (Richard Rider): For space stuff
Doctor Voodoo: For magic stuff, less likely to be out of the dimension when you call than Stephen
Blue Marvel: Science and heavy hitter, for when all shit has hit the fan.
a bit silly, but if you had to fill an Avengers roster, who would you pick?
Wooof, oh me oh my. Let's lay down some ground rules before I do this. 1). Only people who have been Avengers at some previous point in time. Doesn't narrow it down a LOT but this list would be a jigsaw of my favorite Z-Listers otherwise 2). Limiting it to seven people. That's the magic number with superhero teams and it gives me a reason to stop

Steve Rogers, AKA Captain America: Sometimes you just cannot beat a classic and when it comes to Avengers line ups there is no one that I think is more integral than Captain America. The pathos that he brings to the table no matter what character he is interacting with is palpable and reading through his first big volume has given me a deep respect and love for the character. He's our leader for sure, the axis of solid, steady service I can hang my weirder picks on.

Dr. Walter Newell AKA Stingray: You all saw this coming and don't act like you didn't. One of my favorite if not my FAVORITE Marvel Characters of all time. He's a doctor with an interesting specialization. His "I'm only a part time superhero" hangup is even funnier and more interesting if forced into the limelight on THE hero team. Not to mention he comes with his own swanky Hydrobase we can use for an HQ and with his wife and four kids running around underfoot we have the kind of domestic adorability I think any good team needs.

Miguel Santos AKA Living Lightning: The first time I can ever remember reading about a comic book hero being gay, as just like, part of who they are. A tiny detail in their rich inner life. Not to mention the less respect a character gets the more I want to lift them up on my shoulders. He could be the sweetheart with a little chip on his shoulder from not getting the respect his objectively awesome powers objectively deserve.

Angelica Jones AKA Firestar: Something you may not know about me. The first piece of media that really opened my eyes as to the potential and depth and scope of the Marvel Universe was Spiderman and His Amazing Friends. It was cheesy, it was cheap and yet Angelica was the first character that I felt SPECIAL for knowing and caring about. She's happy, she's passionate, her simple classic costume kicks ass and the New Warriors need their goddamn respect. 'Nuff said.

Sersi, Just Sersi: What little I have seen of this character fucking FASCINATES me. This woman is chaos incarnate. It's like she is actively making on the fly decisions with everyone she meets whether she's going to kill them, screw them, turn them into a small mammal or some combination of the three. She's *Instant Plot Complication Just Add Water* because she saw a butterfly and that somehow translates to her blowing the entire team's cover.

Dane Whitman AKA The Black Knight: I love everything about him. I love his vibe, I love the fact that his backstory is built partially around recontextualizing the lore of a mostly forgotten Atlas fantasy comic. I love that he has a wickedly evil cursed blade that comes with the side effect of basically holding him hostage to a heroic moral code. And on top of that he's a dorky intellectual who can't see a social cue if it's blaring at him from oncoming traffic.

Jennifer Walters AKA She-Hulk: ...I do not feel the need to explain or justify this choice. YOU know Jen is awesome. *I* know Jen is awesome. She-Hulk does not need justification. She shows up in stories and makes them better by existing. Also yes this is the bodytype I'd use. Yes, I have an addiction. No, I don't feel the need to explain that either. RESERVISTS: Characters I really like but either don't know enough about or don't think they make good Avengers
Marc Spector and System AKA Moon Knight: One of my favorite dudes but does NOT play well with others. Was interesting for about 10 seconds as a member of the West Coast team but I'd prefer he never touch the ranks again.
Flint Marko AKA Sandman: Marvel did Sandman fucking dirty by never letting him fully reform and be the good guy. I want Sandman to be the good guy dammit
Maria de Guadalupe Santiago AKA Silverclaw: I know literally nothing about her outside of reference books but her powers are dope and I dig her vibe.

Tony's surgeries in the cave: Part 2. First Surgery
All injuries caused by the explosion:

Shrapnel in the chest, mainly in the sternum area. Many barbs are too deep to be removed under these circumstances. Some of the shrapnel could have entered the heart through the blood vessels and damaged it, leading to sick sinus syndrome (arrhythmia).
Small wounds on the torso.
Long cut on the left collarbone.

A trail of shrapnel wounds on the right forearm (with which he covered his face during the explosion).

Several barbs also hit the legs.

Blast injury (signs that we can see: lungs - wheezing; ears - bleeding; brain - confusion). The explosion itself could also damage the heart, causing arrhythmia.
Interesting fact - in the scene below, you can see that the explosion sent shrapnel through Tony. Barbs entered his chest and came out of his back. And you can see how big the wounds are.

Although while in the long run it's better for the shrapnel to go right through rather than get stuck in the body, in the short run he'd be very dead very soon.
For this reason and for the reason that there's no any sign of damage to his back after this scene, we should disregard this as a mistake made by visual effects specialists. But it's still a good idea for a fanfic.

What Yinsen did during the first surgery:
Stopped the bleeding.
Dealt with the consequences of the blast injury.
Performed sternotomy (cutting through the sternum and then reconnecting it). Removed the most dangerous shrapnel from Tony's chest.
Bolted an electromagnet to Tony's sternum and plugged it to a car battery to create a magnetic field that would attract the barbs and keep them away from Tony's heart.
Removed shrapnel from the rest of Tony's body.
Treated and stitched up the wounds.

What Yinsen did not do during the first surgery:
He did not remove Tony's sternum (sternectomy) because he didn't implant anything deep there.
He did not implant a pacemaker.
He did not implant the titanium socket for the reactor, because he didn't plan to implant it.
