When a man with painful cystic acne came to dermatologist Eva Rawlings Parker for help in a Nashville clinic, she couldn’t prescribe him doxycycline or minocycline, two medications she’d typically use to treat this condition. This is because the man was a roofer, says Parker, and these medications would have impacted his ability to tolerate heat. 
Parker’s patient was far from alone. Other common medications for physical health, like beta blockers, can impact people’s ability to handle heat. Many medications for mental health do, too.
Conventional wisdom tells people with conditions that make them unusually vulnerable to the sun, like the autoimmune disorder lupus, or are on medications that lead to heat sensitivity, to avoid staying outside when the sun is at its strongest.
But for the one-third of US workers who must spend regular time outdoors, that advice bursts into flames. For some, such as farmworkers, hours and hours of heat exposure, with minimal or no reprieve, are just part of the job. Increasing heat waves and more frequent wildfires point to the need to find real solutions for outdoor workers—and highlight how labor and climate change are intertwined. 
Edward Flores, faculty director of the Community and Labor Center at the University of California, Merced, specializes in the conditions of low-wage and immigrant workers in California. He says the need for heat safety policy reform is acute. “We know that workers have been dying,” Flores says, “because of chronic conditions that accumulate through heat stress over many years and decades that lead to shorter life spans.”
Parker, the dermatologist, is acutely aware of how heat can trigger or worsen skin problems. She is co-chair of the American Academy of Dermatology’s group on climate change and environmental issues, and was an author of a 2023 review on the ways climate change can contribute to dermatological issues, including triggering flares of conditions like hidradenitis suppurativa—which causes painful lumps deep in a person’s skin—and skin cancer.
Workers do have some legal rights to breaks and water, depending on the locale. California, Oregon, and Washington are the only states that mandate those breaks. And roughly half of crop farmworkers have no legal work authorization. That lack of legal status, and the threat of deportation, gives many workers reason to fear complaining about working conditions.
In July, the Occupational Safety and Health Administration proposed a new set of rules which would help protect more than 36 million workers from heat-related illness or death. The proposed OSHA rules would require employers to monitor their workers for heat exhaustion symptoms, provide adequate water and shade, designate break areas, and provide mandatory rest breaks, among other things.

Prolonged standing at work can cause health problems too.
As an employee of the Planet Fitness team I have gained experience that will guide me in my professional endeavors moving forward. I am grateful for the opportunity as well as getting to work with some amazing people. Over the last few months I have been experiencing foot pain while standing at the desk and found that I was only able to sit down with a doctor's note, and even then I experienced grief from my employers. I think it's important that businesses know and understand that prolong standing at work can be just as harmful as prolonged sitting. Research on the health risks of prolonged sitting at work have been prominent in the headlines recently, but there are new studies that also highlight the hazards of prolonged standing at work. I want businesses to take the necessary steps and provide a healthy working environment for their employees. I'm not just speaking for me but for all workers who are forced to prolonged standing, I believe we can do better!

Thomas Waters and Robert Dick concur that Prolonged standing at work has been shown to be associated with a number of potentially serious health outcomes, such as lower back and leg pain, cardiovascular problems, fatigue, discomfort, and pregnancy related health outcomes. The purpose of this article was to provide review of the health risks and interventions for workers and employers that are involved in occupations requiring prolonged standing. A brief review of recommendations by governmental and professional organizations for hours of prolonged standing is also included.
A number of studies have been conducted examining potential biomechanical indicators of risk of Low Back Pain due to prolonged standing. Researchers have suggested that risk of low back pain is increased due to excessive co-activation of muscles and involved in postural stability during prolonged standing. Prolong standing results in a significant increase in co-activity of the gluteus medius muscles, a muscle group that serves to stabilize the pelvis during standing by abducting, medial rotating and laterally rotating the thigh at the hip. LBP is just one example of the many issues with prolonged standing. A number of studies have investigated the effects of prolonged standing on cardiovascular health outcomes. The measures studied have included carotid arteriosclerosis, leg edema, orthostatic symptoms and venous diseases. There seems to be agreement among the studies that prolonged standing plays a significant and potentially dangerous role in the development of vascular problems for workers who must stand for long periods during the work day. Whether these exposures lead to long-term chronic leg problems is not clear, but acute health effects clearly should be prevented to the extent feasible.
There are many effective ways in which these health issues can be avoided or reduced, one example being sit/stand workstations. A number of national guidelines have suggested that sit/stand chairs with or without footrests/foot rails and/or adjustable workstations should be used for workers who must work in jobs requiring prolonged standing during the work shift. They are known as ergonomic chairs and they are helpful in properly aligning with the user's individual needs. With the correct installation they help the user maintain a neutral body position to achieve comfort. Katie Pierce of CrmXchange writes about the important relationship between employee wellbeing and customer service. A positive attitude and disposition are crucial in customer service. Enthusiastic employees relate positively with customers and are more likely to be more helpful in resolving customer issues. When employee well-being is taken care of, you can expect your employees to provide a higher standard of customer service. When employees feel that their needs are met at all levels, they feel valued and competent. They think it is the right job for them, allowing them to be more involved and engaged in their jobs.
If employee well-being is central to your company culture, it can attract competent and skilled customer service candidates. You are also highly likely to retain your existing employees. Focusing on employee well-being translates to increased productivity and performance, higher employee morale, and overall positivity in the workplace. A well-thought-out employee well-being strategy must be in place to maintain good customer service performance.
It's important to remember that prolonged standing at work causes health issues such as LBP and cardiovascular problems. Businesses can take the necessary precautions in preventing these issues such as providing sit/stand workstations or ergonomic chairs. When employees feel that their needs are met at all levels, they feel valued and competent. Enthusiastic employees relate positively with customers and are more likely to be more helpful in resolving customer issues. When employee well-being is taken care of, you can expect your employees to provide a higher standard of customer service. Based on the evidence presented, I’m hoping the Planet Fitness headquarters will take into consideration these recommendations I have provided them.

A record-breaking heat wave is cresting across the United States, with about 100 million people under extreme heat alerts. Local TV news stations, governors and health officials advise to plan accordingly, drink water, go to cooling centers if needed and above all, refrain from excess outdoor exertion.
But if you pick fruit in a field, walk door to door delivering packages, stack boxes in an oppressively hot warehouse or do any number of other jobs without air-conditioning, you don’t have much legal protection against working under sweltering conditions. In 2022 alone, 43 people died from exposure to extreme heat while working, according to the U.S. Bureau of Labor Statistics. Last year, there were others, including a postal worker who died of heat stroke in Dallas, and at least one farmworker who died after falling ill while working in extreme heat in Florida. From fields to warehouses to restaurants, laborers are in danger of illness, injuries and even death in this heat wave.
Climate scientists warn that we are reaching a tipping point where the mounting harms of global warming, including more frequent, more severe heat waves, will become irreversible. The federal government is trying to address the fact that climate change is making working conditions more dangerous each year. But its efforts aren’t likely to bear fruit quickly enough.
The Occupational Safety and Health Act has a “general duty clause” requiring employers to provide safe workplaces, but it lacks specificity on what to do in extreme heat. The Occupational Safety and Health Administration may issue a proposed rule on workplace heat relatively soon that would be likely to require, among other things, rest breaks, drinking water and cooling measures, as well as medical treatment and emergency response procedures. But once issued, there will be a comment and review period, followed by inevitable challenges from business groups arguing that the rule is too burdensome.
