Chapter five soon queen? Hope all is well!
Consider this my T-minus 12 hours or sooner announcement!!! I WILL be posting Chapter 5 tonight. Whether it be before or after midnight, I can't tell you, but I have tomorrow off and nothing due in the next 24 hours so I'm determined not to sleep before it's posted.
Thank y'all SO much for being so patient. All is well, just been so insanely busy that I haven't had a time to breath, let alone finish editing the chapter. I have learned so much in the last few weeks during my ICU rotations. Spent today managing hemorrhagic shock in a GI bleed patient (original bleed had been repaired and was about to be discharged when it was suddenly like a massacre š¬) and we gave 2 units of blood over the span of 1 minutes through a line in his internal jugular, then watched as they did a bedside emergent colonoscopy and put in 2 endoclips. Coolest shit I've seen in a while (well, not for him, poor guy). @fangirl-docintraining have you had your ICU rotations yet? As my fellow medical nerd who gets so excited/ enthusiastic about new stuff, you will love it!
The biggest reason this chapter has taken so long is my balance between keeping it completely accurate to 1960s medicine vs. incorporating modern elements in a realistic way because it's just hard not to and I really want to keep my plot/ outline intact. If anyone wants clarification about what elements are modern or not that I don't specify in my chapter notes, don't hesitate to ask. There are two articles that I would really love everyone to read before or after reading my chapter. For years, I have been so passionate about St. Jude Hospital and several specific doctors and the work they did/ continue to do to treat pediatric cancer. Dr. Emil Freireich essentially eliminated bleeding as a cause of death in leukemia patients after experimenting with his own platelets and finding the correct way to infuse them. He then proposed that the method of treating tuberculosis (using multiple drugs at once) could also cure leukemia. In 1962, pediatric leukemia was a death sentence. People thought it was crazy that Danny Thomas and these doctors were stating that they would find a way to cure it. Kids diagnosed would die within weeks of diagnosis from infection or bleeding to death. Their experimental treatment of combining two or more chemotherapeutic agents was seen as inhumane by many doctors, who thought that would make these children sicker and it was better to let them die peacefully. To everyone's shock, they saw success. This treatment was trialed in the early 60s, but didn't become something doctors would refer patients to St. Jude to until 1968ish. In 1970, the man behind this extraordinary discovery, Dr. Don Pinkel, was able to officially proclaim that childhood leukemia was no longer a fatal disease, with the cure rate at 50%. Today, ALL has a 94% cure rate, but the treatment remains harsh and lasts approximately 2.5 years. And despite all the hard work of researchers, other pediatric cancers such as Wilm's tumor, Ewing's sarcoma, osteosarcoma, rhabdomyosarcoma, medulloblastoma, DIPG, and more have high relapse rates and higher fatality rates (DIPG is something I encourage everyone to research and support higher funding for- a pediatric brain cancer with 0% survival rate). I actually went to a conference at St. Jude when I was 19 for childhood cancer advocates as I was a top fundraiser that year and interested in a future career there. I cannot say enough positive things about it!
So obviously, my story will deviate the timeline a little and the fictional Children's Hospital of Oklahoma (which in reality was not established until the 21st century) will be offering this experimental treatment in '65. Some of the medications and medical devices I include were not available until after '65 (for example, ports/ central lines in this particular model were not a thing), but I also make a point to use meds that were more common during the time, not include certain devices such as heart monitors, pulse ox, etc.
This has turned into a really long ramble. The point is, I hope everyone is ready to learn a lot this chapter and I encourage you to let this motivate you to learn more about childhood cancer, the fact that it is not as rare as many think, and that it is severely underfunded.

You know not all is right when you sigh in relief at the showrunner saying your fave has been appearing too much lately. Because maybe, just maybe it means he recognises there's more to them than the main soprano in the "Psychological Colonoscopy of the Doctor" cantata? Like, I don't think it's just the matter of quantity but quality as well here. Again, the Master's been forced into a linear story that absolutely doesn't work for them! Either resolve their issues or fucking kill them for good, because this masochistic orgy of "behold exactly what's wrong with them, but no, of course we're not going to just deal with it" works for fanfiction (artistic expression I fully admire, but there a production issues), but not a neverending tv series that people get paid for creating. Of course, this is the same issue that's plopped on the Doctor, too, and I doubt it's possible to go back to adventures with occasional angst format that's a lot of Classic Who fans' holy grail AND THAT'S GOOD! This more introspective poetic works! But there is such a thing as too much good and I think there's something that makes them less suitable for villains than heroes? Like, with a villain, there has to be a solid fatal flaw that doesn't get patted on the back in the end, and there are very few such flaws that can be used over and over. Desire for power or survival work here, because they're both extremes of end-of-the-day common not to say healthy drives, and the beauty of thoschei is how those drives, though usually in a more measured level, aren't absent from the good guy. Complexes, abandonment issues, and all the rest of the diagnoses that get dumped on the Master lately... look, I understand those are things many people live with and never fully resolve, BUT THIS IS A STORY, IT NEEDS A FREAKING CONCLUSION.
Like, I really get the impression when Moffat came up with the redemption arc for Missy, and then arrived at the "ideal" way for how the Master's story should end, he. maybe wasn't naive enough to think future showrunners won't bring them back, but he really expected it to be at least a couple of seasons? Because there, I ended a linear story. Take a moment to go back to it. But then Chibs needed an exposition device for his Doctor apotheosis and that required the Master to be as historically rooted as possible.
Also, don't get me started on the "the Master always just comes back without an explanation uwu" thing, yes they do come back in the same regeneration, but when there's a new body THERE'S USUALLY BEEN A WHOLE EPISODE DEVOTED TO HOW THIS HAPPENED (all hail the Tooth plot).
As for the revelations... I'm a bit more on the fence with this one, because a) dress ups are the Master's thing and b) I think they're the perfect SEASONAL big bad, they just don't work as an overarching villain. Not in the least because they work best with the Doctor when they do have to unite against a common enemy. Point: people sometimes do good things for selfish reasons. Inventing the specific scenario where the only choice is between participating or not in an expanded altruistic suicide (that won't really do much difference from individual altruistic suicide) isn't the dramatic hit you think it is.
Bottom line: LET. THE MASTER. BREATHE.

this is an angry post but
it is unforgivable that I was allowed to suffer years of misdiagnosis. Over a decade of misdiagnosis. GI disorders are so prevalent and under-diagnosed.
I am permanently damaged from years of having to seek emergency care for my vomiting and stomach pain episodes. I've had so many IVs in my lifetime, so many blood draws, several vein blowouts and infiltration episodes that I now have to get another possibly permanent medical device in less than a year.
And I am terrified of getting a port! Like legitimately petrified of complications. Sepsis is a very real consideration and I just. Monday is quickly approaching and I am so overwhelmed
I should not have gone years without a GES. I shouldn't have gotten several diagnoses that are supposed to be diagnosis of elimination. Endoscopy and colonoscopy doesn't always find everything.

Just got out of my endoscopy/colonoscopy and it went super well! The new team and nurses are really nice. So far nothing wrong with my esophagus and stomach nor colon! So as far as we know my Crohnās is not active right now unless itās in the upper intestines.
Still have to do MRI, abdominal ultrasound, bone density scan and blood work - but both me and Johnny feel tons better after moving (almost immediately), so it really could have been a toxicity issue aggravating my ability to function, at least partially.
Also we took a fun picture of my hands bc I obviously have Raynaudās or some circulation issue š

I saw your post about whump tropes/ characters waking up and I wanted to ask do you ever get people laughing when they wake up? Iv seen it happen with shock to others (and myself, whoops) and I was wondering if people cry do they laugh?
absolutely!
in the initial stages of emergence, we tend to more commonly see a discomfort response because of the surgery and because patientās emerge with their airway device still in situ as we cannot extubate until we know theyāre breathing on their own and responsive.
with ETTs, these are removed in theatre (unless theyāre going to ICU) and the airway is suctioned then we sometimes see patients laughing and sticking their tongue up against the yankaeur, biting it, etc. (they are sent to recovery with LMAs in situ)
once the device and been removed and their LOC increases slightly, we observe other responses. we also assess for delirium.
laughter can also be a response to discomfort, it can also be a response to some of the drugs weāre giving. iāve seen quite a few people giggling immediately after or hysterically laughing out in recovery.
iām sure youāve seen videos of people acting strangely after anaesthesia, it happens a lot, and like how people crying hysterically for no reason, people laugh too. (they also say some really random shit)
other responses as well can be specific to the procedure, i see people fart very often after a colonoscopy.
a good rule of thumb as well is if someone is anxious prior to induction, they emerge anxious.

I so envy a manās toughest decisions: blue bathing trunks or red? Clean pair or out of the hamper? Of course, with all the excitement that goes with swimsuit shoppingāsummer, you came back!āIām also furrowing my brow way too much and earning serious Botox units on the road to choosing the right suits. By the time I finally plunk down my credit card, here are the deciding factors that have already come into play: 1. My child Much to my chagrin, my daughter is often a key stakeholder in my swimsuit shopping. Ā Sheās only eight, but the feedback started last year. Yeah, Iām gonna have to pass on that low-cut bikini bottom in favor of the high-rise. Are my breasts nearing Coast Guard-approved flotation device status? Then Iād better look for a 5-hook-and-eye underwire supreme. 3. My vacation destination Okay, come to think of it, itās the destination that decides everything! Do I really want major boobage while hanging out at the town pool? Itās hard enough to find the courage to wear a bikini when your cup size sounds like an algebraic equation (U.S. DDD-cup = E.U. F-cup = U.K. E-cup = meltdown), let alone while worrying about other momsāor, heaven forfend, dadsālooking at you. But if Iām going to the beach alone or with friends, well, then itās balls to the wall. Or boobs to the shore. All I really need is an adults-only tropical resortā¦single, straight male readers of this ladiesā lingerie blog, are you picking up what Iām putting down? 4. The company I keep Itās not just about the age or life stage of the humans with me but often, the level of judgment those humans exhibit. So before I say yes to a suit, I try to take into consideration the comrades who may be along for the ride.Ā Showing up to the beach with my sarcastic and somewhat emo eight-year-old requires a different strategy than say, a really hot man. Or a family member. If children are present, Iād rather not have someoneās tweenage boy judging my boobs, be it out of trauma or fascination. If Iām with my somewhat conservative girlfriend, I will put on a moderately cute tankini so as not to offend. If my New Age-y girlfriend is with me, Iāll take risks, depending on whether my aura is feeling hot or not. 5. My blood sugar level Going to a store and trying on in a dressing room is enough to give me hives or pull my hair out in chunks, whichever is faster and less attractive. Personally, if I shop online with the moral support of a really empathetic, dedicated Bra Fit Expert, Iām know Iām bound to come away with a decent purchase. But if Iām left to my own devices, wandering rack after rack of suits hung without rhyme or reason, willpower fading, Iām typically back in the car faster than you can say, āGrande vanilla non-fat chai latte, please.ā 6. My shopping destination Once I finally realized I had way more options online, my swimsuit game changed. It was like that giddy happiness I got when my divorce was finalized, but better. The first time I found Bare Necessities, I couldāve cried. Options, options everywhere! Sizes, styles and silhouettes for summers until I retire. All it requires is a bit of self-control. Okay, a lot of self-control. 7. My skin tone Since my only complexion settings are barely alive or burned to a crisp, I know I need to lean heavily on the power of both a good pattern and diligent reapplication of bronzer. If I had the forethought and the time to use self-tanner, I might take a shot on a teeny bikini. But if Iām my normal shade of ātwo weeks into a morgue visit,ā Iāll play it safe and pick out a jewel-tone tankini. 8. My bikini line Letās be honest, my haircut downstairs also determines how Iām gonna turn up at the beach or pool. A clean look is always best, but sometimes time constraints (mom life, amirite?) or razor burn get in the way. And that, my friends, is when I reach for a pair of boyshorts. Now you know. 9. The price tag When it comes to buying swimwear, my dollars inform my choices as much as my derriere does. How many times have I cringed at the cost of a suit I just had to have? But still I would argue every time that a good bathing suit is worth the spend. My strategy is to pretty much stalk a suit online and obsesses over it, remembering all the details and visiting it regularly until Iām absolutely sure Iām in love with it and feel prepared to commit. (Side note, maybe I should try this approach with dating?) Usually, I nab the suit at the right time, but thereās a chance you take. I have returned to the link to find that my size is sold out in the top or bottom, dashing my dream look. The pain is unending, I tell you. Spare yourself if you can. 10. The stars Sometimes, my astrological chart is out of whack, and my plans for what to pack are dashed against the proverbial rocks.
