here are some of the reasons wilson reads as transgender to me:

Wilson is very image conscious. He still has a skincare routine, that definitely makes him dysphoric, but he wouldn't be able to deal with having bad skin so it continues anyway. He knows that men can do traditionally "feminine" things and that washing your face is gender neutral, but it bothers him anyway. He dresses like a banker, and always has beautifully ironed clothing and a tie. You can tell that he is performing masculinity in a conscious way, despite the broader social view of masculinity being the "default".

His shirts are often under or oversized, which speaks of someone that struggles with accurately perceiving what their body looks like. He was very skinny when he was younger, and his weight fluctuates a fair bit during the show. Many transgender men have issues with body image and struggle with eating disorders or their milder cousin disordered eating, due to the cultural association of androgyny with thinness. Rolling up your sleeves pushes the shoulders of a shirt up, making them look broader and adding bulk to the biceps. This added bulk also makes one's hips look smaller in comparison. He does this almost every time he takes his jacket off.

Personality wise Wilson is very compassionate and caring. He constantly puts other people's needs above his own, which is why i headcanon him as transitioning at some point during med school, after he is no longer living with his family. He feels excessively responsible for his younger brother, indicating he may have been parentified by his family in his teens. He has all the hallmarks of someone that has been taught he should be kind, considerate, and gentle above all else. To be fair my cisgender brother was also raised like this, but wilson would have been born around 1970, and is also american.

His wives also fit into this somehow. He is chasing the image of a "normal" man, part of which is being married. This is why he rushes into relationships so quickly, and after the honeymoon period is over feels completely unfulfilled by them. He's not pursuing them, he's pursing the idea of being a husband. I'm a strong believer in wilson having at least a crush on house, which bothers him because it deviates from the typical life path he is trying to emulate.
