Reading this struck me to my core. This describes me to a T and sheds light on what is wrong with my brain, and I have never before heard it laid out so accurately. I have ADHD, and I have this form of it specifically - without hyperactivity but instead with paralysis. This problem, right here; has genuinely crippled me for many years and has ruined my life, particularly because it is alongside other problems like CPTSD and chronic pain and exhaustion. If I didn't have it, though? If I JUST had the CPTSD and the chronic pain issues and didn't have to deal with my will to act and function being paralyzed? I would be able to actually tackle those other problems... and so I am sure that my life would be radically improved in comparison to where it is now. Wow
I did get a wonderful doctor recently and was finally given an Adderall prescription. What it does for me is gives me the ability to act, it breaks me out of the paralysis. It's not a magic cure however, no drug is; it gives me a short window of maybe three or four hours where it really works its best (and has some irritating side effects.) It also doesn't fix the problem completely, just improves it considerably. But I am very happy to have it. I should look into getting an extended release version...
What I am really wanting to say, though, is to reflect on how utterly devastating invisible illnesses can be for people, and that those include mental ones; even ones like ADHD which people often don't take seriously. Having doctors who are understanding and empathetic, being able to easily get proper medication, and especially having a society that better accommodated disabilities like this and actually helped us out; would make a huge difference for many, many people who are suffering.
Some individuals with AD/HD, especially without hyperactivity, have an activation problem as described by Thomas Brown, Ph.D. in his article AD/HD without Hyperactivity (1993). Rather than a deficit of attention, this means that individuals can’t deploy attention, direct it, or put it in the right place at the right time. He explains that adults who do not have hyperactivity often have severe difficulty activating enough to start a task and sustaining the energy to complete it. This is especially true for low-interest activities. Often it means that they can’t think of what to do so they might not be able to act at all, or, as Kate Kelly and Peggy Ramundo say in You Mean I’m Not Lazy, Stupid or Crazy?!, they might experience a “paralysis of will” (pg. 65). “The clothes from my trip—a month ago—are just still lying in a heap in the suitcase.” “I spend a lot of time in bed watching TV but my mind isn’t watching TV. I’m thinking about what I should be doing, but I don’t have the energy to do it.”

Example of how everything is breaking simultaneously: my X-rays aren’t getting read. I had to wait three days for a chest XR read. This should take hours, not days. The clinical usefulness drops substantially if I can’t make quick decisions based on a radiologist’s expertise.
(Do NOT tell me “well just get good at reading chest XR, they’re not that hard.” Go fuck yourself. They’re a tiny fraction of what I do; I’m not inpatient looking at six of them a day. I’m outpatient, doing a ton of psychiatric care in addition to everything else. Unless you want to show up to my rural clinic and manage all my complex bipolar and or psychotic patients who’ve been tried on 10-15 meds before and are still on the verge of suicide, and then go do an endometrial biopsy on someone with postmenopausal bleeding, and then go adjust meds on someone in a heart failure exacerbation who refuses to go to the hospital because the ER is in shambles, don’t tell me how to prioritize my learning time.)
Earlier this week I got an image of a maybe-broken hand. Got a prelim read, but those don’t show up in my inbox. Again, it should be hours, not days, to a final read. Still hadn’t gotten a final read by end of day Friday.
Something is really, really wrong, and it’s breaking the foundational elements of US healthcare.
