Tongue Cancer – Sanjog Singh/Cancer Care India
The cancerous growth that begins on the surface of the tongue or inside the muscles or matter of the tongue is what is known as tongue cancer. Since it is made of nerves and muscles that help in movement and various functions, the cancer of tongue affects the tongue movement and this affecting functions of speaking, eating, and swallowing.

Symptoms
The symptoms listed down here may occur when you are suffering tongue cancer:
● A red or white patch on the tongue which doesn’t subside and gradually increases in size
● A sore throat which won’t go away
● A sore (ulcer) on the tongue or a lump that doesn’t heal or increases in size
● Pain or difficulty in swallowing
● Falling or deviation of tongue on one side
● Pain or burning over the tongue
● You may have difficulty moving your tongue and speaking
● A lump in the neck
● Unexplained bleeding of the tongue that is not caused by biting or other injuries
● Pain in the ear (rare)
Causes of Tongue Cancer
● Tobacco use
● Alcohol use
● Teeth with jagged edges causing tongue bite
● Betelnut chewing ( supari ,pan ,chunna)
● Human papilloma virus 16 and 18
● You may also be at risk for tongue cancer if you have certain genes.
Men and those born male are more likely to develop tongue cancer than women or people born female. If you are 45 years old or older, and your immune system is weakened, then it’s more likely that you will develop the disease.
Grades and Stages
Usually, we classify tongue cancer using grades and stages as it is a measure of how far the cancer spreads. The doctors use the test results to determine the location and size of the cancer and if it has spread which is known as staging. Basically, this shows how cancer cells appear under a microscope and gives an indication of the speed at which it could develop.
The stage and grade of your cancer will help your doctor plan the best treatment as the TNM and the number staging system are most commonly used to stage tongue cancer.
Listed below are the three possible classifications for each stage:
● The size of the tumor is called T and a small tumor is T1 while a large one is T4.
● N indicates whether or not cancer has spread into neck lymph nodes, N0 indicates that the cancer has not spread, whereas N3 indicates the cancer spread.
● The M indicates whether there are additional growths (metastases) in other parts of the body.
The grade of cancer is a measure of how aggressive and likely the cancer is to spread. The types of tongue cancer include:
● Well differentiated (slow and unlikely to spread).
● moderate differentiated
● Poorly differentiated (very aggressive, likely to spread)
Diagnosis
A medical specialist will usually detect tongue cancer first during a routine health check-up. To diagnose tongue cancer, the specialist will follow some tests, while also considering your medical history and symptoms. Listed below are the few test options that the doctor may follow for tongue cancer diagnosis:
Examining your mouth and throat:
Examining your mouth, neck, and throat is what amounts to a physical examination which is usually the first step of diagnosis to check for lumps on your tongue, and swollen nodes in your neck.
Using a tiny camera to check inside the mouth and throat:
Next is the endoscopy procedure where a camera-equipped tube is passed through the nose to the throat to look for any tongue cancer signs. The test is also applied to determine if cancer has spread in other areas of the throat like the voicebox.
Remove a tissue sample for testing:
In this biopsy method, a sample of tongue cells is taken by cutting a small piece or area using a thin needle inserted into the area of suspicion to collect cells. The collected samples are sent to a laboratory for testing as the results can determine if the cells are cancerous.
Imaging tests:
The doctors in certain cases suggest imaging tests where images of the body are captured to determine the location and size of cancer and these tests , MRI is the preferred choice of imaging test for tongue
Treatment Options
Your medical specialist will plan up the treatment strategy based on the size and location of the tumor around the tongue area.
Surgery:
Surgery is the primary treatment of choice for tongue cancer .A surgeon will remove all or part of your tongue during glossectomy, depending on the size of the cancer. The surgeon will also remove healthy cells that surround your tongue, while also removing some tissue from the neck if it appears that the cancer has spread. You may require physical therapy or rehab after surgery if you have problems speaking or swallowing.
Reconstructive Surgery:
This type of surgery is used by doctors to replace part of the tongue which have been removed as part of a cancer procedure. The plastic surgeon can use skin from the forearm , submental region , chest ,thighs to replace the excised part
Radiation Therapy:
Radiation is used as an adjuvantbtreatment after the surgery if the final pathology report warrants the requirement of it .The treatment involves the use of high energy photons or protons to direct energy beams at cancer cells in specific areas on your body.
Chemotherapy:
Chemotherapy involves use of drugs or injections either in the beginning of the treatment before the surgery which is called Neo adjuvant chemotherapy or along with radiation which is called concomitant chemotherapy to potentiate the effect of radiation and have systemic effect on the cancer
Targeted Therapy:
The doctors use targeted therapy to kill cancer cells when attacked by specific chemicals and also to treat cancerous tumors that have returned or spread.
Immunotherapy:
Your doctor may suggest immunotherapy if other treatments don’t work. It helps your immune system find and destroy cancer cells.
Prevention

It was almost funny how many of Bruce's children seemed to think his life was divisible into not-yet-traumatized child in one half and the Bat in the other. It overlooked both his youthful anger and directionless rage, trysts with Harvey that were full of heavy risk and secrecy, fighting with other students and a smoking habit that was only sometimes tobacco.
And it overlooked his disappearance from Gotham.
Bruce's time away from Gotham and away from his identity, absorbing styles of fighting like a thirsting creature, had also been spent clawing through the underbellies of half a different places. He'd woken dry-mouthed and irrational in opium dens that felt as though they were from different centuries. Drunk teas distilled from mushrooms that were only lightly lethal and wondrously hallucinogenic. Tried what passed for moonshine in a Moravian village, distilled from sweet damsons and fermented to local slivovitz. Once he had sipped stewed, mascerated monkey brain from the cap of its own skull; an aftertaste of metallic herbs; ignoring the memory of a passage on prion disease from forgotten studies; an experience in South America.
Yaba in Phuket, cocaine in the Azores, crushed coral and betelnut in the Marshalls, Meow in Singapore, de l'héro in Paris, ketamine in Cork country. Bruce had wrecked himself thoroughly before returning to Gotham to face its own demons, its own addictions, its own chasing for oblivion, its own curiosities and its own angers.

"I've got you," Bruce said gently. "I'm here. It's going to be okay," he petted Dick's hair. Kissed the crown of his head. Steadied Dick's weight against him, just as he'd done so many times when Dick was a zip of a boy, light and resting on Bruce's hip following patrol.
"You're good," he said in a murmur. "You're good. I promise."

Sure you’d bring wine and whiskey home. But what if I told you I brought rice and chili home too?
Before you judge my choice of souvenirs that I took back from Bhutan, I would to make my case. Our guide Kelley, provided very compelling arguments on why alcohol, chilli and rice are their best produce – they are all locally and very organically grown.
Read until the end to get more details on where to pick them up from!
What Bhutan Souvenirs to get
1. Beer (USD1) or Whiskey
Alcohol, for one, is cheap. Enough said.
2. Peach wine (USD4)
Peach wine was a wild guess when I bought it. When I tried it, it tasted surprisingly sweet and tangy. If you’re into dessert wines, this is a good choice.
3. Rice
Rice is their staple food, and everywhere we go, we see rice fields. Even by the airport! Since their rice produce are so fresh and organic, I thought to let my folks back home be the judge for themselves. So yes, I brought back sacks of rice (plural), and in kilograms. They have different types of rice, depending on the extent of hulling. So you have the option of choosing from a range of brown-almost red rice to white rice.
4. Chilli
Since chilli goes with everything, and I mean everything, here, from mom (chinese dumplings) to rice, vegetables and meat, how could I pass up the offer to bring a slice of spice home?
I took back dried whole chilli on top of grounded chilli powder.
5. Yak cheese
Yak cheese are commonplace, especially around the food markets and food stalls. You’ll notice bits of white block hanging from a piece of string at the stands of food stalls – and you’ll wonder what it is that’s so damn hard. That’s yak cheese.
How can cheese be so hard? Beats me. Try it at the expense of your teeth.
6. Betel nut
Betel nut is a snack that you’ll notice every Bhutanese chew on. Sometimes you’ll see splats of red on the ground – that’s the juice being spat out.
Betel nut is usually wrapped in a betel leaf with flavourings such as cardamom, cinnamon and tobacco. I was apprehensive to try it initially, but I had to uncover the mystery as to why every Bhutanese would be willing to risk oral cancer and having permanently black/ red teeth for it.

So I put the betel nut to the test. When I first sunk my teeth into it, it was as hard as a rock! Still, I continued chewing to hopefully break it down. The juice that came from the betel nut + leaf combination filled my mouth with such a strong acrid taste that no matter how much I chewed, the betel nut wouldn’t ever break down. In fact, the flavour just became more pungent and unbearable. I eventually had to spit it out. (Don’t tell my guide!)
7. Lemongrass
8. Cordyceps
Cordyceps and lemongrass are the 2 single most popular items sold to tourists in Bhutan. Cordyceps are expensive, partly because of its healing properties and wide use by the Chinese traditional medicine. Also, the difficulty it takes in procuring it makes it valuable.
A visit to the National Museum (Ta Dzong) in Paro explains how cordyceps are formed. Cordyceps are formed when the fungus attacks moth caterpillars, infecting it with spores. Over time, the fungus completely kills and mummifies the caterpillar, resulting in the long slender body shape. Not too cool hearing that the expensive cordyceps is made of fungus and dead caterpillar aye?
9. Customised stamps
What’s interesting is that you can choose to put your own image on the stamp in the Post Office in Thimpu!
Here’s how mine looks:
Postcards are less than a dollar in Bhutan, so grab a few copies to mail them to your friends and family – even to yourself! Immortalise your memory in Bhutan with a letter written by you, for you.
Your guide will probably bring you to make your own stamps in Thimpu, but otherwise if he doesn’t, request to make a visit to the Post Office!
9. Books by local artists
I got a book written by the Queen Mother of Bhutan, Ashi Dorji Wangmo Wangchuck, herself, titled “Treasures of the Thunder Dragon”. Through the book, I learned more about the history and stories of Bhutan that my guide probably wouldn’t have covered in my short time there.
For instance, I learned that the Dochula Pass’ 108 chortens that Her Majesty The Queen built was not built for fun.

Chortens (or stupa in Sanskrit) are receptacles for religious offerings or sacred relics. They are built for a variety of reasons – to honour a great king, saint or lama, to commemorate a departed soul, to ward off evil spirits or danger, or to mark an important event or a great victory. You are supposed to walk around a chorten in a clockwise direction – not anticlockwise or you’ll get bad luck!
The 108 chortens built on the Dochula Pass was built as a way of protecting the country and the King at a time when they were faced with the threat of a war by their neighbouring country. They were symbols of her prayers for the safe return of the King and their soldiers.
108 is an auspicious number for Buddhists, representing the number of prayers that make up a complete cycle. The string of beads that many believers and monks use to chant their prayers are also made up of 108 beads!
I also never knew that chortens have symbolic offerings, such as grains and a bronze vessel filled with butter put into it. Sacred objects such as deities, prayer bells, clay stupas, prayer books, medicinal herbs and incense, precious stones and jewellery are also sometimes also tied on a pole which are installed into the chorten.
Looks are deceiving! Who would have known that a chorten is worth so much spiritually and physically just by looking at its physical appearance?
10. Acorns
This is a bit of a queer souvenir to take home but as we were driving along the roads from one town to another, I noticed a lot of fallen acorns on the ground. They make great decorations for the house and so I asked if I could take some of them back.
My guide and driver very promptly started being on the lookout for big, fat, wholesome acorns to pick along the way. They would stop in the middle of the long mountain roads and start bush-smashing their way to pick up acorns for me. I was so touched by their help, and also overwhelmed with acorns after their little acorn cleanup session.
Where to find these Bhutan souvenirs
Centenary Farmers’ Market in Thimpu
We bought most of our foodstuffs from the Centenary Farmers’ Market in Thimpu. Thimpu’s weekend Centenary Farmer’s Market is the biggest in the country.
Be on an olfactory overload, and assault your nostrils. This is where you’ll find fresh food produce such as vegetables, fruits, honey, lemongrass, tea, cordyceps, rice, yak cheese, and just about any other fresh food.
Wet market section
It is the market that the locals go to to get their groceries. They are sectioned according to which region they procured their produce from. Different parts of Bhutan have different climates and thus grow different food. To reach here we must drive through the open countryside, during which time you can enjoy the panoramic views and natural pools. Rynji Falls
And last but not least this fall is about 80km away from Shillong, these falls are situated in the middle of Jaintia hills. Rock formations around the river make a beautiful cascade of water which is what people from all around the country come here to see. There are pools beneath the falls where many people try activities like fishing, the Moopun falls are right nearby here around 3km away.

Meghalaya takes its holidays seriously and Sunday mass is widely attended by everyone. There is singing in the church and all men and women are smartly dressed while all shops and local taxis are on holiday.
Kwai is local betelnut that everyone seems to enjoy all the time everywhere in Meghalaya (and the entire northeast). You can try it too, remember to ask the locals to give you kwai with as little of the white accompaniment as possible.

The Scourge of Mouth Cancer Last weekend, my family and I visited our driver for a condolence on the death of his young son, who also worked in our Hotel. The man died of mouth cancer … due to chewing #paan (#betel nut) and #gutka. Gutka, a #tobacco-related #narcotic, is truly the scourge of mouth #cancer- and it is addictive! In October, the Provincial Government enacted a tremendous law banning Gutka, with a heavy fine & imprisonment … while the nascent law is a positive step, the government has yet to follow up with effective implementation. The problem is that for the consumers of this substance it is a narcotic. You can’t simply turn off a switch. Like other narcotics and behaviors, it is deeply ingrained in society and part of their lives – so much so that generations of families are consuming it concurrently. Besides, I don’t believe we have the infrastructure to provide medication, counselling or alternative #therapies to “un-learn” this behavior. Like the law, we have had strict rules restricting betel nut on the job (let alone gutka!)- but, like the law, we cannot effectively implement our own rule. We do physical checks and entry pat-downs – yet it makes its way into the staff canteen and staff areas. In spite of knowing their colleague’s cancer (which was in its final stage when diagnosed), no one has even reduced this consumption. Our own house maid, her mouth STUFFED, refuses to accept she’s chewing it – her mouth so full that she cannot even verbally defend herself! On our condolence visit, not only was our driver’s wife consuming betel nut but their entire family was chewing away on it … nary a care to the reason behind reduction of their family’s size! We snaked around the potholes and mud along the road through to Mount Tavurvur. Mother mountain stood higher than anything in my view. South daughter was asleep, covered in green. The station’s of the cross stood like stolen white picket fences in the volcanic ash. Frangipanis, brought by the Germans, dropped their floral bombs around their church, the cemetaries filled with an exotic mourning. Bougainvillea vine burst above emaciated dogs, breasts sucked long, feeding generations. Everywhere I looked, the leaves stood out, varieties so different to what I was used to. Watching Aleisha eat Betelnut was joyous. Opening up her amulet, she dipped into the white powder with a mustard stick. I asked her if tasted hot on her tongue and she said it was like a bean and passed me mine. It looked like she was dipping licorice into a sherbet fountain. She ripped open the pod of the fruit and bit into the soft pink flesh, spitting the husk out the car window. The spray hit her back and we laughed together as she wiped away the spit from her chin. The ground dust in her paperbark was coral or shell, still not quite sure, but she was comfortable and continued the ritual. Why aren’t your teeth black and mouth red like many others?. “I don’t do this as much as them, I have to study” she said. What are you studying, “I want to be a health worker”. Started to wonder what the health benefits of this bag of bliss was, but it sure did relax us. “Can I sing you a song?” Of course “This is called Bird Song”. When she finished, I turned my head to the car window so she couldn’t see my tears. Back on the shoreline, I slept dreaming of wooden lips that kissed me. “How old are you?” Well, if you add up all your ages and times it by two you might get close to mine. I left them counting and giggling as they climbed higher onto the rocks. “How many children do you have?” Do I lie and say three or tell them I never wanted any, hard to answer those six bright smiles.

Free your adventure: falling in love with Canggu, Bali
Travel more, worry less!
For as long as I can remember, Bali has always been on my radar as one of *those* iconic travel destinations around the world that can, and does, change lives.
A popular hub for tourists from Australia and New Zealand, being close by, it’s almost a travel mecca in this part of the world, attracting explorers, surfers, expats, and tourists looking for a good time in a chill place, a vibe Bali exudes in abundance.
At the risk of being a total travel cliché, I definitely have had my own fair share of Eat, Pray, Love moments from Bali – minus a fling with Javier Bardem! With my first journey there five years ago to go diving and have a holiday away from my constant holidays (LOL), Bali has safely held a place in my heart as a beautiful part of the world, even with the monkeys.
Stay away from monkeys.
Since moving to New Zealand, travel has been a big part of my life.
Back when I first visited Bali 5 years ago, I used to just either buy individual travel insurance policies for each trip if I was forced to, or better yet, not at all. Oh to be young and naive again!
But with experience comes knowledge and I’ve seen enough misfortunes and experienced plenty of my own too, to realized that not having travel insurance is just plain dumb, especially when so many things could go wrong that are completely out of control.
Bali is one of those places that while it’s filled with Aussies and Kiwis (cough cough, Kuta, I’m looking at you) and can easily feel like home, it still is another country and unpredictable to a certain degree. Also monkeys.
Just like packing a bathing suit, sunscreen and beautiful twirly dresses are a must for my trips to Bali, so is travel insurance.
I used to just buy individual policies for each trip that I was taking, but it was adding up so now I just buy an annual policy – and after shopping around, Southern Cross Travel Insurance (SCTI) is now my go-to provider for all things insurance related on all my travels around the world.
I was doing more and more “hardcore” trips that often required me to have travel insurance with a high evacuation coverage – this means if I have an accident in Antarctica where it’s SUPER expensive to get back to mainland for medical coverage or riding horses in Mongolia or even just hiking in the mountains in a place where, if anything happened, I would need to be medivac-ed out, I’m taken care of.
I LOVE saying yes to seemingly crazy new adventures and SCTI really gives me the freedom to explore the world with peace of mind.
While my last trip to Bali was all about diving and having a real holiday, it was marred by getting a serious case of Bali belly or food poisoning waylaying me for two days in Amed. Totally uninsured at the time, I was really lucky to be staying at a luxury hotel that took care of me until I could move again, and it didn’t really impact my trip too much. All I can say is thank god it wasn’t the day I was flying home – can you imagine? Wait, don’t.
Anywho.
This time around I was headed to the little expat hotspot of Canggu with my business partners to work for a week together. Cheaper for us to all meet in Bali, I don’t think any of us could have booked our flights faster.
But being the total clumsy person I am and bit of a magnet for misadventure, I’m more likely to trip and break an arm in Italy or get my bag stolen in Australia then have something major happen. I think. touch wood.
I can count on SCTI look after me whether I’m chasing penguins in South Georgia to scootering in Bali to shopping in Sydney, freeing up my anxiety so I can travel more and worry a lot less.
That one time I crashed a scooter in Bali
As a total worrywart and a person who has developed a lot of anxiety and stress as an adult, this is so important to me, and it’s worth every penny.
I was so excited to see how Bali had changed, after watching it explode on Instagram over the years, and I knew I was in for a treat.
It didn’t disappoint. Bali was DIVINE.
From what I remember, Canggu was just a bunch of rice paddies and a surf beach the last time I was there; now it’s transformed into a veritable hub of influencers, creators, surfers and expats, a chill and relaxed place where you can prop up your feet for a $9 massage, sip a coconut and afford to stay for a good long while.
And man, has the food scene stepped it up! The cafe and restaurant scene in Canggu (and next door in Seminyak) is just incredible.
I didn’t have time in my schedule for Bali Belly Round 2, and luck and lady hygiene was on my side and there was no bout of sickness by any of us, and we didn’t skimp out on eating all the things.

A beautiful place filled with lovely people, right by the beach, heaps of big comfortable and modern cafes where you can work all day in drinking the most lovely juices, heaps of healthy and clean eating that’s affordable, yoga everywhere, fast wifi and lots of creative people traveling solo too. It’s my place!
I really fell in love with Canggu on this trip, and my partners and I have already booked trips back later this.
