"You... want to learn how to assess patients?" Hyrule asked hesitantly.
Wild shrugged. "What if I want to be an EMT?"
"You also said you wanted to be a chef in the hospital."
"I can do both, you know."
Hyrule laughed. "I suppose so. Well... we'll need a patient for this to work."
Wild immediately snatched Sky, who yelped as his friend snaked a hand around his wrist. "Sky's the patient, heaven knows he needs to be looked over anyway."
"Look who's talking, Mr. I-Have-Seizures-and-Don't-Tell-Anybody," Sky grumbled as he was manhandled to sit between the other two.
"Well, everyone knows now."
Hyrule and Sky gave Wild a scalding look. Adequately apologetic, Wild shrugged sheepishly.
"Anyway," Hyrule sighed, shifting his focus to Sky. "Assessments come in different forms. You've got a primary and a secondary assessment. Primary is kind of a general overview and checking for life threatening stuff, secondary is in-depth on what the issue actually is. Make sense?"
Wild nodded.
"Great!" Hyrule continued with a smile. "Okay. Sky's our patient. Sky, you got shot once, right?"
Sky nodded, and Wild balked. "He what?!"
"It was a long time ago," Sky waved a dismissive hand.
"Okay, so that's our scenario," Hyrule said, standing. "We're dispatched for a 21-year-old male with a GSW--"
"That means gunshot wound, right?"
"Yeah. GSW, conscious patient. That's all we've got. So, you get on scene, and the very first thing you do is check for scene safety. If the scene isn't safe, we're not going in. First thing you're taught in EMS - your own safety comes first, because if you're shot you can't help the patient. It's you, your partner, then the patient."
"How often do you actually listen to that rule?" Sky asked, raising an eyebrow.
"That's not what we're learning today," Hyrule waved off easily. It was pretty common knowledge that while he would never put his partner's life at risk, he'd gotten himself into dicey situations before. But he knew how to get himself out of those situations too. "So, we determine the scene is safe. Next, is our primary assessment. The purpose of this assessment is to check for life threatening things, and an overview of major body systems. Neuro status, bleeding, and your ABCs: Airway, Breathing, Circulation.
"The situation is pretty dynamic, like sometimes you walk up and somebody's got an arterial bleed and spurting blood everywhere, your assessment stops right there and you go fix that bleed. But generally you'll have time to do the entire primary assessment."
"Okay, so neuro and ABCs?"
"Yeah. And the good thing is that most of it happens all at once, you know? You walk up to Sky and he looks at you, then boom, you've got a good neuro - he's awake, he's alert. He may not be oriented, but you can figure that out by just talking to him. And by this point you can tell if there's life threatening bleeding. Then it's ABCs - is his airway patent, or open? Is he breathing, and is he doing so normally? Is his skin warm, dry, and normal tone for him? You can literally do al these things by just walking into the room and looking at him for five seconds. The primary assessment is done really fast and, the more times you do it, basically automatically."
"What would be an example of something being wrong?" Wild askd.
Hyrule glanced at him. "When I got on scene for your crash, you were unconscious and unresponsive--in other words, you were not only unconscious, but nothing would wake you up--and your breathing was gurgling sounding because you had blood in your airway."
Glancing at Sky, Hyrule said, "Sky can give us an example of a not great primary assessment, I'm sure."
Helpfully, Sky immediately flopped off the chair he was sitting on, collapsing to the ground with a crash. Wild laughed, and footsteps rushed from upstairs into the living room.
Twilight immediately froze in the entranceway, eyes wide and fixed on Sky. "Sky, what the--guys what the hell is hap--"
Sky perked up immediately. "Oh, sorry! I'm just helping Hyrule teach Wild!"
Twilight froze a moment and then sighed heavily, pinching the bridge of his nose and grumbling under his breath.
Hyrule smiled, pointing at Twilight. "He just perfectly showed a good primary assessment looks like! He walked in and saw the patient down on the ground, tried to figure out a neuro by calling out to him, and when Sky woke up he immediately could tell he was fine. Neuro intact, not bleeding, had a patent airway because he's talking, breathing normally, and skin looks normal."
"I hate all of you," Twilight groaned, walking out of the room.
"Okay, but by skin looking normal... what does it mean when it doesn't?" Wild asked.
"Your skin can tell a story," Hyrule explained. "If you're diaphoretic, which means sweating, something is likely wrong. Though it depends on context - if your patient's sweaty but they were just exercising, it makes sense. If Sky's sweaty on the ground after being shot, he's in shock. If the skin is cool, the body isn't circulating well - that can sap the color right out of your skin - the lighter your skin tone the more notable it is, but darker skin tones can become paler too. A lot of times with darker skin tones you'll want to look at their palms or their lips, that'll help you determine it. Another color is grey - that usually means cardiac and it's bad. So skin can tell you a lot!"
"How did my skin look?" Wild questioned, curious.
"Pale," Hyrule immediately answered. "Anyway. Sky's your patient. Look him over."
"Okay," Wild blew out a breath, approaching Sky and kneeling beside him. "So he's unconscious, that's my neuro so far."
"Can you arouse him at all?"
Wild poked Sky in the neck. Sky flinched. Wild poked again and Sky giggled. Wild's eyes widened in realization, and a mischievous smile crossed his face.
"Wild, wait--"
Sky started laughing hysterically as his friend tickled him, wiggling and trying to shove him away.
"Get--off of m--Wild you jerk--"
Hyrule chuckled. "Well, we're not taught to tickle our patients, but that works."

medical bitching beneath the cut
Long time followers know for the last 2.5 years I have been chasing jaw joint replacement surgery after my bones started disintegrating which has so far involved a) the surgeon ignoring my calls b) getting denied by insurance c) filing an insurance appeal and getting rejected again d) having to start the entire process over when my workplace switched insurances but at least getting approved this time e) having to do braces for the second time (the first time caused this problem in the first place) which have led to my jaw joints dislocating all over the place (not sure either one is in the socket rn tbh... you get used to a specific pain and then it's hard to tell when something's wrong anymore) and various other minor and major inconveniences including the time I lost 7% of my body weight after getting spacers.
Last week's drama included learning that artificial joints expire and I will have to do this all again in 20 years or so and also that after my braces are done I have to do extensive imaging and then it takes ~4 months to build the artificial joint, which really threw off my mental surgery timeline. The orthodontist had told me May (after many, many attempts on my part in getting any sort of timeline since your employers tend to like to know in advance when you go awol for 6 weeks) so *this* week I went back to him and went 'hey when you said May did you mean May is when you'll be done with me and I do imaging and then the surgery is 4 months later or did you mean May is when the joint will be ready'.
'Probably the first one,' he said 'because I didn't think you were going to have surgery, Adam called and said he didn't think you needed it'
me: ...who? I've never spoken to an Adam about this surgery in my life?
The identity of Adam remains a mystery! I informed my orthodontist that since the surgeon was explaining the build a bear joint process it sounds like surgery is in fact happening so hopefully that is cleared up (or is it???) but like
a) who the fuck is Adam
b) why didn't 'Adam' call me?
c) if I have been coming in for orthodontic treatment since February being very clear that it is because I am preparing for surgery and you think I'm not having surgery shouldn't you like. idk say something. I'm paying you thousands of dollars out of fucking pocket bc my insurance only covered 1.5k
d) if you are convinced I am not having surgery and I ask you hey when am I having surgery why would your answer be 'May' instead of 'never and let's clarify expectations'
e) who the fuck is Adam???
Anyway when am I having my surgery? Idk. Also don't know who (Adam??) how why or even where considering my surgeon works at like 4 locations and last month I was sent on a merry chase bc the ortho office gave me a phone number for a different location than usual (where he did not work, actually, they googled the wrong place. good times). Don't even know what because the ortho keeps talking about making space on my upper jaw for the 'orthodontic portion' of the surgery and as far as I'm aware the main bit goes through my ears. I guess I will just keep living my goddamn life waiting for someone to eventually call and tell me it's time. Or not! Maybe Adam will materialize in my home killing me instantly. I wish every medical practitioner the terror, confusion, and frustration of having years of your life spent in the hands of people who don't give a shit about you and can't be bothered to try. This is not even starting on the front desk staff.

Elizabeth II liked to drink a Martini before meeting guests when she was flying and always requested a particular type of mint sweet during take-off, it has emerged.
The late Queen also enjoyed following the route map while travelling.
Her flying habits are revealed in a treasure trove of memorabilia kept for decades by an air hostess who worked for British Airways for 28 years from 1970.
Elizabeth Evans, from Birmingham, served many famous travellers during her long career, among them the late Queen and Prince Philip during their 1989 tour of Singapore and Malaysia.
A collection of mementoes found in Mrs Evans’ study following her death contains confidential instructions on how to look after the royal couple.
The points of etiquette reveal that the late Queen liked a bowl of sugar-free Velamints beside her on take-off and that she should be left to doze if she was sleeping prior to landing.
She also preferred to use her own pillows, with her personal dresser advising on how to make up her bed.
Any guests joining the royals for dinner would be told what time cocktails were to be served in the royal compartment and the Queen would later be asked which wine she would like served with dinner.
“Her Majesty tends to like a Martini before her guests arrive,” the notes state.
Canapes were served with cocktails before the guests took their seats and staff were reminded that the late Queen must be served first.
One section of the notes is titled “Tips for Royal Compartment,” under which is listed: “Route map – Her Majesty enjoys looking at these.”
Members of the flight crew were told not to pass through the cabin and to restrict all noise.

The memorabilia, inherited by Mrs Evans’ niece, Jo Smallwood, is expected to fetch between £400-£600 when it goes under the hammer at Hansons Auctioneers on August 20.
Charles Hanson, owner of the auctioneers, said:
“Elizabeth must have been extremely highly regarded by BA because she served some of the most important people in the world. Take that 1989 British Airways royal flight.
Her memorabilia includes confidential instructions on how to care for our late Queen and Prince Philip – right down to the sweets Her Majesty preferred on take-off.”
Velamints were launched in the US in 1977 and are said to have become president Ronald Reagan’s favourite mint.
They were dubbed “the Official Air Force 1 mint.”

Meanwhile, both the late Queen and her husband were known to be fond of a Martini, which they would typically enjoy before dinner.
In late 2021, a year before her death, it emerged that she had given up drinking on doctors’ orders.
She was said to favour a gin and Dubonnet at lunchtime, and a gin and vermouth cocktail before dinner.
Mrs Evans also crewed Concorde, the world’s first commercial supersonic airliner, which would fly at twice the speed of sound, during the 1980s and 1990s.
She kept signed menu cards and other paraphernalia relating to many of her VIP passengers, including Rod Stewart, Cliff Richard, Arnold Schwarzenegger, and the late Patrick Swayze.

Mr Hanson noted that although the late Queen had flown to Singapore and Malaysia on BA, she had closely followed Concorde’s development.
“Elizabeth’s archive sweeps us back in time to the 1970s, 80s and 90s when flying on Concorde was a luxurious privilege enjoyed by the wealthy jet-set,” he added.
“Celebrities flocked to use its services and carrying royalty was the norm. Service had to be first class.”
He said of the late Queen:
“She enjoyed her first supersonic flight in 1977 and used the aircraft for state visits.
The Queen of the Skies was seen as fitting transport for the Queen of the realm."
“In 1991, Charles and Diana, then the Prince and Princess of Wales, flew supersonic during a trip to Oman and Pakistan."
“Elizabeth gathered mementoes throughout her career. Her memorabilia captures the life and times of Concorde at its height and the detail that went into luxury travel with British Airways.
No VIP wish was too small to be accommodated. Even smoking was permitted on board back then.”

Ms Smallword, 53, a retired police officer, from Solihull, West Midlands, said:
“I was aware of some of the things Elizabeth had kept from her career with BA but nothing on this scale.
She married late in life, moving to Devon and finally Hermanus, in South Africa.
The objects were found there in her study following her death at the age of 70 in 2017.
I came across them when I travelled to South Africa to arrange the funeral.
Having found this treasure trove relating to my aunt’s career and experiences with BA, especially the fantastic trips on Concorde and serving the Queen, I found it sad that none of it had seen the light of day.
I believe this collection is a little piece of history that should be shared and enjoyed by somebody.”
Mrs Evans was awarded a certificate to commemorate the time she served the royals on British Airways flights.
She also amassed signatures from the likes of Warren Beatty, Virginia Wade and Billie Jean King, which will also be sold by Hansons.

super incredible personal life-changing vital to me that Storybrook's clock tower is set to 8:15 aka Oceanic Airways Flight 815 in the Once Upon a Time pilot. Adam Horowitz and Edward Kitsis deserve a fun treat for that.
