I'm so extremely serious when I say doctors should be put through an extremely extensive reliscensing process every 10 years. Doctors should have their knowledge scrutinized against current medical research and be de-barred at even the tiniest discrepancy. Too many old doctors absolutely refuse to stay up to date on research and dismiss patients because of their personal experiences. Too many people die every year because doctors don't take us seriously and refuse to listen to people who KNOW something is wrong. Too many people are told their problems are nothing and come back in a year or more with serious illnesses and doctors are just like "lol everyone makes mistakes" but doctors mistakes routinely cost people their lives! I'm tired of medical malpractice being swept away under the guise of "mistakes were made."

.. Moon, sun? Is staying up till 4 am Bad?
I've been staying up for a while.. And I dont know how to properly sleep, or get a good comfy position to fall asleep quicker-
Sun: This is for you Moon! *turns light off*
Moon: It sure is! And it CAN be bad, but it can also be completely fine. It depends on when you are actually awake and what your schedule is. If you work the night shift or sleep all day, then staying up all night is completely fine! But based off of the rest of your ask I will assume you are supposed to be asleep at night.
One of the easiest things to do would be to get off any kind of electronic device, especially ones without a blue-screen filter, about an hour or at least half-hour before you plan to go to sleep. I say this is the easiest because it requires the least amount of effort technically, but I know how people do rely on their devices a lot more than they use to.
So if that is not an option, then I would say to try and get some more exercise in your daily routine. Working out even for 30 minutes can help your sleep schedule drastically. It doesn't even have to be very intense work-outs. I know there are people who are disabled or cannot physically work out, so ANY kind of movement can help, even just walking or lifting something a few times. Just don't try to push or strain yourself while exercising. Also try not to exercise right before bed, try to keep it closer to the middle of the day.
You can also try to keep a schedule once you are able to fall asleep. If you end up falling asleep around 10 pm one night, try to aim to get into bed by 9 pm or so, without using electronics, and try to fall asleep by keeping your mind empty and focusing on breathing or nothing at all.
Oh! Another trick that could help, is to tense up your body a whole lot! This is called Progressive Muscle Relaxation! What you want to do is choose a set of muscles to tense up, such as a thigh or bicep, and tense that muscle up for 5 to 10 seconds while inhaling. Then, when you exhale, release all that tension you built up. Wait 10 to 20 seconds and then repeat that with a different muscle group! Do this for most of your body! Many people with Insomnia have said this kind of practice can help them fall asleep.
And then there are, like I just said, sometimes mental blocks that stop us from sleeping, such as Insomnia or even ADHD. There are some medicines that you can use to help you sleep, such as Melatonin or a prescription sleeping aid from a licensed doctor! These will help if you literally cannot fall asleep because of a mental disorder, just make sure to read all labels and consult a professional if you do go the medical route because there can be plenty of adverse reactions that could potentially happen.
Other than that, I would say to keep your room to your comfort level, keep distractions to a minimum, and try not to eat close to the bedtime you want to have. Use pillows or blankets to help get into sleeping positions, such as a pillow under your stomach area and between your legs if you lay on your side. Everyone's sleeping position is different, and you should try to stick to one method for about 2 to 3 weeks before moving on to another one to see what works because bodies take time to adjust to things.
You can also look up something called Biphasic or Polyphasic sleeping methods! That's where you split your sleep cycle up into segments which can work better for some people than others. There's plenty of sleeping trick you can try! Just give them a shot!

im 31, and after over a decade of trying to figure out what is wrong with me, my therapist and I are finally thinking it’s ADHD. i’ve had a gut feeling about it for a while and every ADHD post is relatable. now the problem is finding an place that does adult assessments and is affordable (no insurance). do you have any tips on going through the assessment as an adult? and if i can’t afford it, and can’t get medication. how do i ever become the productive focused person i want to be? thanks.
Sorry in advance for the long post... I put the most relevant bits in bold for a TL;DR version.
I know getting diagnosed as an adult can seem daunting, but you shouldn’t worry too much. While ADHD was once viewed as something that only affected children, it is now widely recognized as a lifelong disorder and you shouldn’t have to fear being dismissed because you weren’t diagnosed early in life. It’s extremely helpful that you have been seeing a therapist, and they also think you have ADHD. Ask them to send over their notes when you do go to the doctor.
As for how to get diagnosed - I’ll start by saying I hate the way American healthcare is set up, as medication and healthcare in general are expensive. I have to move frequently for me and my husband’s jobs (we both work in hospitality, and as the saying goes, “You have to move out to move up!”). Because most (all?) ADHD medications are a Schedule II drug (highly regulated but still legal), I have to get rediagnosed in every new state. I always bring my past history, but most doctors want to complete testing as they are monitored for prescribing stimulants and can lose their license if found to be providing this medication without ample documentation. (All of this to say - I have been through the procedure many times as an adult.) Depending on the state, some doctors also require bloodwork and an EKG to ensure you are healthy enough to receive the medication (although some will accept past test results if done recently enough.) Also depending on the state and doctor, they may have additional requirements. In Florida, my doctor wanted a multitude of tests, and asked for a sleep study to ensure the medication wasn’t causing poor sleep. In California, as part of the Kaiser HMO system, I was required to do periodic drug tests to ensure I wasn’t also using street drugs, and to check that the Adderall was in my system (as a test that I was using it as prescribed, and not selling it). Some states are much easier – Utah, Alabama and West Virginia all were able to diagnose me in one appointment and prescribed the medication same day. Last, a General Practitioner won’t typically prescribe it and will direct you to a psychiatrist. Even if you did have insurance, most don’t cover psychiatrists or if they do, it comes with a different deductible (because obviously mental health isn’t part of regular health (heavy sarcasm)). After diagnosing, you have to meet with the doctor once a month to get the prescription refilled – due to the Schedule II status, they can’t have it on an auto-refill like other medications and they need to ensure you aren’t abusing it or having negative side effects. (although the one good thing to come out of COVID is that it normalized tele-health appointments, since an in-person meeting with a doctor once a month can be difficult to schedule). Even though I have health insurance, I typically pay out of pocket $120 a month for my visit with the doctor, and after insurance and a coupon I pay $73 for two medications (Adderall & Vyvanse). I’m fortunate now to be able to afford that expense – at the times in my life where I couldn’t, I would request a 30 day supply of the more affordable pills and only take medication on days where I couldn’t function without it (such as doing large amounts of paperwork) and try to use learned behavior techniques the rest of the time, to stretch out my resources.
As far as what goes into the actual diagnosis – doctors most commonly use a questionnaire about your daily life to assess you. Here is a link to commonly-used questionnaires: https://www.additudemag.com/adhd-assessments-and-tests/.
I know I just made it seem very daunting to get diagnosed and on a medication, but I want to be honest with you about what the process looks like, and again, depending on where you live it can be done in one session. Now that is out of the way, let me give you some information that is more helpful:
If you can, skip asking a regular GP for a referral and make an appointment directly with a psychiatrist. This will save you the extra cost of the doctor’s appointment, just to be told someone else will help. Many places have low cost mental health centers and ADHD falls into that realm, so I would check out what is available in your city. Before making an appointment, confirm the following:
- Do they diagnose ADHD?
- Do they prescribe medication? (Therapists don’t prescribe, only psychiatrists, and some will not prescribe ADHD medication at all so it’s important to be clear that it is your intention to receive medication if diagnosed)
- What tests do they require for diagnosing, and prescribing medication? (Some places may have more or less requirements, and it can even vary within a city or state. This way you will know if it’s something you can afford at the time.)
Talk with the doctor about your specific situation, and what medications are affordable without insurance. Adderall, for example, is past the 10 year exclusive patent and now has a generic version available. It comes in quick release and slow release, depending on your needs. You can also talk to the doctor about a prescription to both quick and slow release, so you take the correct medicine based on your needs for that day (marathon work day? Slow release that extends over the entire day. Afternoon project – quick release that lasts for four hours). Vyvanse is great but doesn’t have a generic version and is insanely expensive without insurance (to the tune of $350+). Use the GoodRX app to find deals on medication without insurance (Adderall is about $15 for a month supply with this app). There are a ton of drug options so look up the pricing during the doctor’s visit, so you can confirm that you can afford what they prescribe. Also keep in mind that getting a prescription filled is the same cost whether you get 1 pill or 30 (a fact I learned the hard way when getting a 10 pill prescription filled once.)
If you read all that and thought, Thanks but no thanks, here are some other options:
- My psychiatrist in Florida recommended that I take Rhodiola Rosea supplements in addition to medication, as it has clinically proven positive effects on ADHD symptom control. I found it on Amazon. Omega-3 fatty acid supplements are also proven effective.

- Practicing meditation is a great way to relax your body (increased stress, while helpful for short-term tasks, can make ADHD symptoms worse long term) and train your mind to hold onto singular, important thoughts (people’s names, why you walked into the kitchen, etc). I use the Waking Up app and love it – there are also many free options in the App Store and on YouTube.
- Regular exercise is another great way to manage ADHD symptoms, as it gives your body a natural serotonin and dopamine boost, two important chemicals your body has trouble producing and absorbing naturally.
- Caffeine is a great, easily accessible stimulant that has a focusing and calming effect on ADHD individuals. My doctor actually asked my parents to give me coffee each morning before school when I was a child, before we moved onto prescriptions.
- Often, there are other factors that go along with ADHD, such as anxiety and/or depression. Getting this under control can go a long way in managing ADHD as well. I’m not sure if you have any issues with those, but it can be helpful to treat both if you do. The medication Wellbutrin is used to treat depression and also has mild stimulants, which would be helpful for both conditions. It isn’t a Schedule II drug, so you can probably ask your doctor for a 3 or 6 month prescription.

So you want to be a veterinarian...
Ask a handful of children what they want to be when they grow up and there’s a good chance that someone will say veterinarian. It has long been a dream career of many animal lovers, but is it right for you?
I fall into the “it’s all I’ve ever wanted to be” category. For as long as I can remember I was telling my parents that I was going to be a veterinarian. Human medicine? No, thank you. While I thought I had a pretty good understanding of veterinary medicine before I started veterinary school, I soon realized I didn’t know it all. That being said, veterinary medicine is definitely the right career for me. This post will go over the process of becoming a veterinarian, and the pros and cons of the job.
How do I become a Veterinarian?
To become a veterinarian, you must attend veterinary school. In North America, these programs are typically four years long.
Pre-Veterinary School Depending on where you live, there are prerequisites you must complete before applying to veterinary school. Many programs require completion of a four year undergraduate degree, taking some specific required courses. Some schools only require 20 prerequisite undergraduate courses (two years) in order to apply. The admissions process is competitive so keeping competitive grades is advantageous, though a 4.0 is not necessary. Good, but not great, grades do not mean you’re immediately cast aside. There are other components to your application. Many applications include veterinary/animal experience and extra circulars. During high school and undergraduate you should participate in a variety of clubs and volunteer activities. This makes you a more well rounded applicant, and also can be quite enjoyable. Some examples: - volunteer at a veterinary clinic: I personally believe everyone should spend time in a veterinary clinic before applying to veterinary school, regardless of importance for application. This will allow you to see what the job actually involves. - volunteer at farms, wildlife clinics, SPCAs etc. - 4-H or similar. - Student government (leadership, communication). - Pre-vet society/club. - Any other club/society on campus, ie. biology society, GSA, yearbook etc. Some schools, especially in the USA, require letters of recommendation. The school may specify who they are looking for letters from (i.e. vet, professor, etc). Make sure you make positive connections with the people you work with in order to allow for a positive recommendation. Finally, standardized testing is required by most veterinary schools, specifically the GRE (Graduate Record Examination). More information about this can be found online.
As a Canadian student, you may only apply to your regional Canadian school, unless you would like to apply internationally. As an American student, you can apply to a large number of schools within your county. Research each school as many have specific areas of interest, different cost, different living style etc. After applying to veterinary school, many schools will then invite candidates for an interview. The type of interview varies depending on the school so make sure you look into the specifics of each school you will be interviewed at. Veterinary School I will give you specifics of my education as an example, though all veterinary schools differ. In general, be prepared for a lot of work. You will be taking many classes at once. For example, I was often taking 10 courses per semester. Many of these courses included a laboratory component. A large number of classes equates to a large amount of home preparation and studying. Midterm season at my school was typically weeks long. The work load is certainly manageable, don’t let that alone scare you off, though it does take a lot of dedication and planning. The most important piece of advice I have in regards to course load is to make sure you still find time for yourself. Without taking care of yourself and constantly studying, you will burnout. My school had three years of course work, followed by one year of clinical practice. During the three pre-clinical years, we did have exposure to animals throughout labs and shifts in the clinic. We first perform surgery in third year under supervision of the course doctors. For the most part, the pre-clinical years followed a pretty general schedule (other than surgery duties). Starting class at 8 and being done by 4. There are opportunities outside of courses to get involved with clinical/medical activities. Veterinary school have a large variety of clubs that may offer these extra opportunities - ie. pathology club necropsies, exotic animal club medical producers, internal medicine club cadaver labs... There may be opportunity with a local stray spay/neuter program through your school to get extra experience with surgery as well. Clinical year is a whole new ball game. At my school the year was broken into three week segments. Each segment you are placed somewhere new - i.e.. three weeks in the surgery department, three weeks in radiology, three weeks at an external clinic of your choosing... This year takes you away form the classroom and puts you into “doctor” mode. Keep in mind that each veterinary school operates on a different schedule (some introducing clinical earlier in the program) and that this is my personal experience. What your duties are will depend on the school as well. During my clinical year, student are involved in client communication, history taking, diagnostic planning, treatment administration, clinical skills, participation in surgery, etc. All under direct supervision. The hours for this portion of schooling are not predictable and definitely do not run 8-4. Many placements include after hours care of the patients, on-call work, lengthly paperwork, etc. All while studying for the dreaded NAVLE. Even though it was often dreadful, clinical year was my favourite year of veterinary school. Everything started to come together and honestly, it was a lot of fun.
NAVLE In North America, in order to practice veterinary medicine, you must past the licensing examination, the NAVLE. This is a 360 question multiple choice test that encompasses everything you have studied throughout veterinary school. Your school will help you apply and register when the time comes. You will technically be preparing for the NAVLE all of veterinary school but will specifically study for it in your final year. There are preparatory courses that you may purchase and your school may host review classes. The test is written in December and April.
So those are the cliff-notes of how to become a veterinarian. Feel free to reach out to me if you have any questions, would like clarification or would like me to go further into detail.
So after all of that, what are the pros and cons?
Lets not beat around the bush, starts with the cons:
Debt - veterinary school is NOT cheap and the income does not match the level of debt obtained. The majority of students will graduate with $100,000-$200,000+ in debt. This is not unmanageable, though it can be daunting.
Extensive training - veterinary school is not easy, and it is not for everyone. You will need to be knowledgeable about many species and many subjects.
Working with owners - “I want to be a vet because I don’t like people”, just no. This is a customer service career. You will speak to people every day. You will have to convince people to spend hundreds or thousands of dollars on their pets. You will get yelled at, told you’re in it for the money, probably threatened at some point.
Working within a budget - unlike human doctors, we really need to think about the cost of each and every thing we do. You will often have owners come in with $100 dollars to their name and a very sick animal, you need to figure out what the options are in each instance.
Euthanasia - many people find this one of the more difficult parts of the job. I personally believe it’s how you look at it. Euthanasia can be a wonderful gift. I do not euthanize healthy animals.
Work-place hazards - our patients can be aggressive and difficult to handle. This is often due to stress or being scared, but it is a danger to veterinary staff. You need to understand how to read signals and prevent dangerous situations from happening. Veterinarians are also exposed to chemicals, radiation, heavy lifting etc.
Hours - depending on where you work, the hours can be gruelling. Not everyone has the luxury of a 40 hour work week. We often stay late (for no extra pay) finishing cases, tending to patients, calling clients, doing paperwork, etc. Many veterinarians must perform on-call duties. This may lead to 2 hours of sleep between shifts.
Responsibility - every decision we make is life or death. The type of drug, the dose, surgery, etc. The job carries a huge amount of responsibility that can be very stressful for certain people.
Ok, so despite all of that, I’m still a veterinarian. I still love my job. Why?
Debt - yes, I have a huge amount of debt. Is it manageable, yes. There are many people in the world who have more debt than me. I hate that its becoming normal for people to have debt, but it is.
Extensive training - vet school sucked, but it was also awesome. I know so much about so many things. I’m proud of the level of knowledge I gained while in vet school. I’m proud of the person I have become through my extensive training.
Working with owners - the vast majority of my clients are absolutely wonderful. There are owners that light up my day, that I look forward to talking to, that remind me how much I love my job. We have many lovely reviews written about us, cards sent in, treats brought in for staff etc. For every person that gets mad at me for cost, there are many more that are understanding and work with me to find a solution within their budget
Working within a budget - this is a con no matter what way you look at it, though I can say that when you’re willing to work with owners and help them within their means, they are often very thankful. There are many cases you can help within a tight budget.
Euthanasia - I tell every owner that euthanasia is a gift. We get to take away pain and suffering. This makes it much easier for me to deal with this on a daily basis.
Work-place hazards - proper training and education can help minimize hazards. Low stress handling, proper restraint etc.
Hours - not every job has horrible hours. I work, on average 40 hours a week. Some days I stay a little late to finish up, but it usually doesn't bother me. Yes, I do on-call but it is not very often. I have a great work life balance. You need to take this into consideration when looking for a job.
Responsibility - you get to call the shots, you get to save lives, help pets, help owners. You do not have to do it alone. There are so many resources out there to help. It’s definitely scary and daunting the first few years out, but its also very satisfying. Confidence grows day by day.
You get to live you dream job.
You get to be a part of an amazing team that shares the same goals as you.
You get to impact the lives of so many people and their pets.
You get to be an ambassador of the animals.
You get to go to work and learn something new every single day.

For the past couple weeks, I've become more and more antsy to get up to Sara's again. With all this job searching and such going on, it's like all I can think about, so why not mention what fucked me up at my previous ones: Putting me in a position of responsibility and expected knowledge. “Presumed Consent” - A Harvest of Organs
I just watched an interesting episode of Law and Order: SVU in which the topic of presumed consent was introduced. I, before watching this episode, had never been aware of presumed consent and I found it great to hear both sides of the argument in a dramatised way. So, let’s first look at the definition of presumed consent:
Presumed consent is alternatively known as an 'opt-out' system and means that unless the deceased has expressed a wish in life not to be an organ donor then consent will be assumed.

What this means, in practice, is that unless explicitly stated otherwise by the patient or their medical proxy (power of attorney), a doctor has the medical and legal right to harvest a patients’ organs for others to receive. This practice can be found in several countries, especially European ones, including Austria, Belgium, the Czech Republic, Finland, France, Greece, Hungary, Israel, Italy, Luxembourg, Norway, Poland, Slovenia, Spain, Sweden and Turkey, according to the World Health Organisation.
Now, the case I was watching on SVU was that of a paediatric surgeon removing the heart, lungs, and liver of a young girl who had died from a brain injury, resulting in her brain being dead, but her organs were still alive. The doctor, not being told explicitly not to harvest the organs, made the medical decision to remove the girl’s organs. Just before the transport carrying her heart was to leave, the surgery was discovered and Lieutenant Benson had to intervene, as the parents had not consented.
Throughout the trial, and before when Benson asked the parents if the transport could leave anyway, even though the parents had not initially consented, the argument that the child’s organs had been ripped out of her body without the parents’ consent was made, in a time of great distraught over losing their child, and it was wrong and the doctor should be punished by the law. The doctor responded in a Thoreauvian way, explaining that a law that is wrong should not be followed. Furthermore, she cited the ethics and morality of this issue in which she had outweighed the plurality of the law in her state. She cited that many European countries use this practice.
The ADA, in this case, sympathised with the parents because he had recently lost his father, and he told Benson that he was getting Half-and-Half when his father passed away, and the discovery of his organs being harvested while he was gone would have affected him greatly. Benson, having a young child herself, a son, sympathised with the child about to receive this heart that was intercepted. In the end, they are informed that the child who would have received this patient’s heart dies. The defendant’s attorney cites the rarity of paediatric organs and several agree that the surgeon should be commended as a hero, not condemned as a criminal. In the end, the court finds her guilty on 30 felony charges, as she forged consent forms due to the legality of the crime, and as forgery is a felony, the surgeon’s medical license is revoked.
I wanted to look at two things in this: (1) the legal and ethical implications of presumed consent, and (2) the selfishness of keeping organs, especially by parents.
Upon further research and study on the issue of presumed consent, I find it so that the idea and legalisation of presumed consent outweigh the implications of non-consensual organ harvesting. Organ harvesting, obviously, is a very time specific thing. There is no time to wait or to let parents or proxies decide to donate or not when other’s lives depend on that organ. The surgeon in this fictional case points out that having lost a son herself, the idea of asking a grief-stricken parent these questions is unfathomable. Instead, the law acts on a “act now, ask for forgiveness later” kind of deal. Unless a parent/proxy or patient says, “I do not want to give mine/their organs away for x, y, z,” then the doctor can presume consent of that patient to undergo this procedure. Having such a law saves more lives, and this value has been seen in countries where this law is implemented.
To the Thoreauvian point made by the doctor in this case, I find it very commendable and a very different look at civil disobedience. It follows the same principles as civil disobedience, and I do see that it is a moral and just cause, which is why this type of practice could make it to the Supreme Court in accordance to potentially overturn state decisions to prosecute such an action. And to the argument of selling organs, well I can delve more into that at a potential later date, but you know my position on for-profit healthcare, so that would have to be a no for me.
The second thing I mentioned was the selfishness of not donating organs. I will put aside the religious reasons of not donating organs, and focus solely on the secular. First, let me start off with this case. The parents told Lieutenant Benson, upon being asked if the heart transplant could continue, that they (specifically the mother) were not comfortable with having pieces of their daughter missing. I practically screamed at my television throughout the episode as they kept bringing this issue up. Like Fin, a detective in the show, said, “Dead is Dead.” There is no logical reason to not donating your organs after you are deceased. Coming from personal experience, my grandfather was on a kidney transplant list and was very grateful to have a direct donor, a co-worker of my grandmother’s (who happens to be an ER doctor). If not, there was a possibility of dying while waiting for one. There are so many deaths in this country every day, but many of them do not donate their organs. As they pointed out in the show, practically no parent of a paediatric patient consents to this process, which is sad due to the amount of necessity amongst paediatric transplants.
This case seems pretty sensible to me that presumed consent is appropriate and beneficial to the greater good. I am a left-Libertarian, but I also am a strong believer in Rawlsianism, so I have no fear in “taking away” personal rights that do not infringe on the daily life of a person and in the end help someone else and better the lives of the needy. I do not stick to one simple principle when looking at government and economics. I do not think that taking the organs from a person who is dead is wrong, nor does it infringe on their personal rights, again setting aside religious reasons, not because I believe the principle changes, but because I do not wish to currently argue that position. I think it is selfish that more people do not donate in this country, and if they do not explicitly say no, then there should be no reason why that shouldn’t be a yes.
