not trans specific but @queercanada is the closest to what you're asking for that I know of

I find there are more canadian trans activists on twitter and insta than on tumblr, if you look in the trans activism or canada transgender tags. with tumblr you're more likely to find lgbt+ news or positivity blogs

on instagram I know of @/transgendersupportcanada
