#Creepy chatter#Hemosiderosis :3#Technically it's idiopathic pulmonary hemosiderosis but I'm starting base level and then going into spec manifestations#Still riding the high of my one and only ever case of diabetic myonecrosis#So unusual that there wasn't anything really in place to capture it#I BREAK the International Classification of Diseases 10th clinical modification#I FIND the myonecrosis I SLAP the guidelines around bc they didn't consider diabetic manifestations#The cool thing was the patient was THRIVING! :D what a kick ass doctor to whip that shit into nearly sub-clinical levels#Medical cw

a good time to reiterate that doctors who give transfem patients birth control as HRT get a thrashing for having not opened a book in the last decade and consequently putting their patients at undue risk
#like at that point i would grill them on whether they were deliberately trying to give their patients blood clots#as some form of malicious compliance or what have you#anyway current guidelines recommend two things: an estrogen and and anti-androgen (male hormone blocker)#estrogen can be oral as 17-beta-estradiol or injected as estradiol valerate or given transdermally as estradiol patches#and the anti androgen is typically spironolactone but there are other options if that is for whatever reason not a viable option for someon#periodic labs are also needed to make sure you remain within safe levels of estradiol and the lack thereof is what makes DIY so risky#its also worth noting that progesterone isnt recommended in the guidelines#it raises clot risk such that it outweighs whatever non-unique benefit it could provide#and no it does not carry a unique benefit not already covered by the estradiol in non-cis patients#yes they use subbed in data from cis women but can you imagine how to even begin going about studying drug effects in trans people#also HRT has only begun to become more common in the past decade#which is about how long a controlled clinical trial can take#also the ethics of a controlled clinical trial are questionable bc would you enter a study knowing part of your HRT might be placebo

They’re getting rid of the mask mandate at work (a hospital) and I really wish they’d keep it.
#if they waited until flu season was over I’d feel a bit better#only a bit better#since if you don’t have a flu shot you must wear a mask in hospitals during flu season per state guidelines#and vaccination for flu is down since they have to mask anyway#chaos this shall be#and masking when a patient requests? they won’t even ask doctors to wash hands#I feel uncomfortable asking them to wash their hands and that’s my job!#holly does epi

isnt it crazy how if i want to get treatment for an attention disorder i have to meet with multiple doctors over the course of 8 months (and counting), handle a mountain of paperwork, spend hours and hours answering repetitive questions, get my parents to do multiple testimonies, and fight the aderall guardian before they ultimately decide i’m not worth helping
but i can just walk into the same kind of clinic and talk to a doctor for 10mn before she decides to prescribe me dangerous antipsychotics with no diagnosis
#but hey people don't use antipsychotics recreationally so at least the streets are safe !!#this is not hyperbolic btw#these meds made my life fucking miserable#for no reason other than the doctor had a hunch#where are all the guidelines for that ?#it's almost like they don't care about patient safety hmmmmm#how do you expect me to stop being so angry about hospitals if you motherfuckers treat me like a lab rat

i’m rewatching hannibal rn for funsies and my only big gripe with it is the fact that they treat the distance between minnesota and baltimore like it’s NOTHING like they treat it like it’s a six hour car ride at most. that motherfucker is a three hour plane ride!!!!!! what do you mean hannibal and freddie lounds and everyone else goes to minnesota and no one notices!!!!!!! someone would notice!!!!!! do these motherfuckers have a private plane??????

Hello as someone who is a paediatric registrar (larval form of a paediatrician) - most paediatricians are angry about this too!
The recent NICE guidelines regarding gender dysphoria are stupid and not based in fact, they’re based on incredibly biased and limited studies. Puberty blockers ARE in fact well researched and perfectly safe. Having such outright transphobia enshrined in national guidelines gives transphobic providers an “easy out” because they can just say “oops oh no the guidelines” but also puts caring paediatricians in a very risky position because by going against the guidelines and actually providing medication based gender affirming care, they could lose their ability to practice and provide any care at all.
If you’re based in the UK please complain about these guidelines to your local politician and local children’s hospital, and emphasize that the guidelines are based on very leading, limiting surveys, with bits of data cherry-picked to give the impression that there “isn’t enough info” or that “there’s no clear benefit to transitioning or puberty blockers”, both claims which are patently false and transparently transphobic.
[British pediatrician voice] Ok so basically there is no fixed point of maturity and no real way of determining such a thing. However, conveniently enough there is an idea in pop culture that brain development happens exactly at age 25, which is based on pretty much nothing except for like, cultural expectations of what a person should be like at that age, so yeah that’s not very scientific. But because that’s an idea that people have already, I am pretty sure that I can use this to restrict the human rights of people under 25, and because it’s in the context of everyone’s least favorite group the [SLUR WHICH HERE MEANS ‘TRANSSEXUALS���], I think everyone will pretty much let this one slide. And im pretty thrilled with this because it’s actually an older age than I thought I was gonna be able to get away with. I thought I was only gonna be able to exercise my power and control over people under the age of 18 or perhaps 21, which wouldn’t have been very exciting to me because I pretty much already get to control those people’s lives. But now that I’ve slipped this one in there, man, this opens up a whole other age group for me to wield power over. So im really happy with how this whole thing is shaping up. Bangers and mash.
#my hospital’s gender clinic is fucking furious about the recent NHS guidelines and rightly so#paediatrics#paediatric medicine#transphobia#I dont work in the UK but in a country which… tends to follow the UK in everything they do#however at least my country DOES allow puberty blockers AND estrogen/testosterone for teenagers#also idk why any paediatrician cares what happens to a 25 year old. if i get a patient that’s 17 i’m like…. why aren’t you with adult med#there’s always 1 or 2 old cunts who refuse to work with the gender clinic in any way#and honestly our gender clinic is oversubscribed for the amount of patients they actually have capacity for#so what we REALLY need are guidelines for local general paediatricians to provide gender affirming care safely#because we only have gender clinics in major cities at the moment which means rural kids need to travel quite far to seek care#SORRY OP i’m just a cis paediatric reg who cares really deeply about this and hates the current NHS guidelines#and like. a lot of other things about the NHS but they’re not relevant at the moment.#this is a lot of tags 😅
