So two weeks ago we looked at Pathfinder Second Edition’s take on a medical professional-themed archetype, and now today, we’ll see how things change with a few millenia worth of medical advancement!
Indeed, today we’re looking at Starfinder 1E’s take on the medic, one that specializes in or supplements their magical healing with good old fashion actual medicine. After all, while we do have the biohacker class, it is more focused on how chemicals affect the body rather than focusing purely on healing.
Speaking of which, biohackers and mystics are both natural picks here, but as we’ll see, there are plenty of ways to play a medic. Imagine for example a mechanic medic that acts as a first responder in disasters, and has a drone to help carry patients, or even operating on them remotely through the drone if unable to safely do so.
Either way, in more sedentary settings these doctors may run or be part of clinics, helping patients when they come in, or they might be actual medics in armed forces, or perhaps doctors for hire catering to a wealthy client or serving aboard a starship’s crew. Either way, they are welcome wherever they go, as the healing arts are almost always respected, or at least required, by most every sapient species at least once in a while.
So let’s take a look at what they bring to the table!
Naturally as medical professionals, these characters bring some new uses to the medical arts. These include things like restoring stamina with excess healing, treating many patience with long-term care, returning heavily injured patients to consciousness, and treating diseases at a brisk pace.
Later on, this expands with even more bonuses and uses, such as healing vitality when performing mundane medicine, preventing a victim from degrading further due to a poison or disease, or even slowly restore patients in extremely poor health back to vitality.
Finally, the most skilled of these medical professionals can bring someone back form the brink of death, provided they reach them in time.
Whether they use magic or technology to heal, there is always going to be a use for a healer, which is why they can show up with any class or profession. As mentioned before, biohackers and mystics are obvious picks, but mechanics working with mechanical or exocortex assistance could be just as good with bodies as they are with machines. Meanwhile, combat classes might be cross-trained as actual medics either to keep their fellow soldiers alive and protect them, or perhaps out of a desire to bring more than death into the world. Even skilled classes like envoy and operative, as well as the other spellcasting classes can get involved, with the former two representing those that are extremely skilled in multiple areas (and maybe putting their extensive knowledge of anatomy to use to heal and harm for operatives), while spellcasters may seek mastery over every aspect of reality, even life. From precogs and witchwarpers envisioning a reality in which no one dies under their care, to the technomancers that won’t let a little thing like the limitations of their pseudo-arcane magic get in the way of helping others.
It can sometimes be hard to remember in the wealth-driven state of society (or the dystopian parodies of it that exist throughout science fiction, but the majority of people get into medicine not out of a desire to have great wealth, but to help people. Whether they are doctors in clinics, or a ships doctor or even a military medic, they all strive to preserve life despite living in a universe of deadly beasts and dangerous foes aplenty. That drive can be a good place to start when it comes to portraying them.
Turned into a biological experiment by an exploitative employer, Torvah the half-orc swore she’d never let anyone suffer a similar fate under her watch, and so she became a mercenary doctor specializing in breaking into questionably-legal facilities as well as preventing and rehabilitating victims of induced mutation, a role she excels at. However, she remains disgusted by her own shifting body, particularly the lethal bone spear she can extrude as a weapon.
The crew has been commissioned to hunt particularly deadly game, but luckily not to kill. Dr. Venagax is looking to retrieve blood samples from a xaxmellia, a colossal kaiju beast with acidic vital fluids, making even the process of extraction a dangerous prospect, let alone subduing the beast beforehand.
There has been a recent outbreak of implant rejections in the undercity. With resources being limited as they are most would write this off as a normal occurrence, but the infections all share the same genetic markings, suggesting that this isn’t a series of coincidental accidents, but rather deliberate contamination.

brief note, diluc and shade would be over in .2 seconds. putting aside the impossibility of shade leaving your side willingly, diluc has long since memorized the microexpressions on kaeya’s face, and knows that he isn’t him.
brief note two, shade is much better at passing as kaeya than red is diluc. aside from the very obvious detail of red borderline growling at people all the time, red carries himself differently. diluc, while often awkward and sometimes unintentionally cold, is still approachable, a trait that red could never manage. shade, however…
shade is a bandit. a master of disguise mixed with kaeya's already clever personality, able to mask himself as kaeya—and has, on occasion, to lead a patrol away or to snatch some medical supplies from the city for you or nikki. the difference is that he has a diluc to fall back on, one that he can trust with his entire self. kaeya can't let himself be honest with diluc and vice versa, but shade and red can. who better to tell your homicidal urges to than your fellow reflection?
imagine with me. post hunt. you're entering mondstat, without red at nikki's request. you, nikki, momo, and shade, making your way toward the gates. everyone knew about the old copy of diluc you had made, about the mirrors your friend taught you to use to summon the archons of old. even night, elusive as she was, was known throughout the knights.
but shade was new. shade was quick on his feet, always swift to pull you through the impossible tangles of forest, into thickets that would knit behind you so the patrols couldn't follow. shade was a well kept secret, only glimpsed in rumors throughout the city that the cavalry captain was occasionally seen in a new set of formal wear.
kaeya had heard these rumors. he'd also known that your 'diluc' was your closest guard, and had himself witnessed him going into the city. he'd thought nothing of it, assuming the people had simply seen someone that looked like him. they hadn't.
once youre close enough for him to actually recognize shade, its like his whole world falls apart.
"divine one..." jean, at his side, likely also recalling the rumors. "your company..."
"this is momo and nikki," you introduce, "and this is shade."
shade. some part of that feels ironic.
hes not surprised by the talking cat, barely registering nikki's polite hello as he locks eyes with shade. same height, same hair, same patch over the eye and sharp smile when he knows he's won.
"shade, say hello."
shade blinks, then flashes a fake smile. "greetings."
the meeting is a blur for kaeya. all he can see is shade, shade and how clearly you and him are attached. his hand held in yours, the adoring look in his eyes as he watches you speak-
he'd passed as kaeya. his mask was so paper thin someone he's never even met could imitate it flawlessly, putting on his identity like a costume on the world's stage and doing it better.
shade was the one at your side. shade led you away with a polite smile—you didn't see the way it dropped so he could shoot kaeya a glare—and a hand around your waist. shade was the one that believed you, that kept you safe.
kaeya had invented the frostwind swordsman. he'd crafted his persona by hand, making a shield out of his words and behavior, and yet it was somehow shade that had perfected it.

Quick take - Demerzel created a Cleon Operating Guide and sent it (anonymously) to imperial HR. Some of the scenarios were very detailed and very specific. Staff turnover fell dramatically and absolutely no one mentions the file to the fussy emperors trio.

Operative’s Arsenal Operative (Operative Alternate Class Feature)

I do enjoy the operative class, but offensively it can be often pigeonholed given that the trick attack is limited in what it can be used with. (which I get, given adding extra damage to already powerful weapons would be a bit much) Sure, some exploits can let you at least get the debilitating effects on a sniper weapon, but the only real exception is the sniper alternate feature, which gets you a lesser form of bonus damage on sniper weapons only, and the heavyweight skirmisher, which grants the damage but at the cost of some of the more defensive and agile abilities associated with the class.
If you’re looking for that holy grail of using trick attack damage with bigger guns though, you’re better off with the heavyweight skirmisher, but if you’d rather stay agile while also having some better use for bigger guns, today’s alternate feature may be for you.
While the heavyweight skirmisher feels like a true blend of paramilitary might with the sneaky tricks of the operative, these arsenal operatives feel a bit more like they’ve chosen to diversify and master a particular class of weaponry to give them more options in combat, though of course their actual origins can vary quite a lot.
So let’s take a look, shall we?
This alternate option is fairly simple, they trade the third and fourth attack they can get at later levels for training in a class of weaponry normally outside of an operative’s wheelhouse, particularly using them with trick attacks. While they don’t get extra damage with them, they do get to apply their various debilitations with them. Later on, they learn to do so with another category of weapon.
There are two things to consider when trying to make this alternate option work. The first is, the weapons you plan on using. They have to have some useful qualities that you don’t see in pistols or normal operative weapons, such as reach, or the ability to hit multiple targets at once with blasts, lines, explosions, or automatic fire. Second, you have to consider what debilitating trick exploits you plan on taking. Combine those together than you can potentially spread a lot of debilitating effects around the battlefield. I would definitely also keep a proper operative weapon on hand though, so you have some way to deal that bonus damage.
No matter how specific their training, a good operative knows that limiting oneself can be a death sentence on the battlefield or on the job. As such, there’s no doubt that plenty of operatives take this path. The question is, what sort of formative moment might have given them reason to? Was it just part of their training? Did they desperately need a weapon but the only ones available were bigger and heavier than their normal equipment? Or did they simply think it silly to not also wield some of the more impressive melee implements?
The party is meeting with a client in a crowded nightclub when halfway through the meeting assassins with concealed rifles and other weapons burst in, killing indiscriminately as if looking for something. Getting out alive with the client is the first step, then comes figuring out who sent these killers.
Going by the callsign “Sylvan Huntress”, the moyishuu assassin for hire is a mystery, but what she is known for is her lethal grace with a various advanced blades, toying with her prey with strikes to their limbs to cripple them before finally bringing them down.
The party thought that the danger was over when they finally took down that assassin, but they rose again as a voidshard vampire, becoming all the more dangerous and deadly. Now it has become a race to prepare before the next nightfall from this nocturnal threat with both deadly weapons and powers.
