Diabetes Insipidus

Vasopressin, a nonapeptide with a sulphide bond between 2 cysteines, is released from the super optic nucleus of the hypothalamus, which senses shrinkage due to a greater osmotic potential, as H2O is lost and salt remains. 

It acts on vasopressin receptors on the kidney, which cause AQP2 shuffling to the apical surface. 

ADH is degraded by serum peptides, with a T0.5 of less than 30 minutes. 

Treatment

Central:

ADH peptide analogues, such as Desmopressin (DDAVP), enter via nasal passages and vessels, through nasal mucosa to the jugular vein, avoiding the hepatic first pass effect. It can also be given orally and parenterally with injection. When given intranasally or sublingually, the max plasma concentrations are around 40 - 55 minutes after injection, and urine output decreases around 1-2 hours after ingestion, lasting 6 - 18 hours.

DDAVP is also known as 1 - desamino - 8-D argenine vasopressin, and it tricks enzymes by looking like a normal good ole ADH fellow. But he's not. The 8th amino acid is a D isomer: the stereoisomer of the natural L form. 

Kidneyal:

Bypass vasopressin receptors by using other Gs linked receptors, such as prostaglandins EP2 and EP4 to create a vasopressin-like effect. 

Viagra can inhibit phosphodiesterase 5, raising cyclic GMP to activate PKG, which also causes vasopressin exocytosis.
