Abnormality Analysis Framework
When any abnormality occurs, the root cause(s) of this abnormality must be known or analysis to be done & necessary corrective actions are required to be taken to prevent the recurrence of this particular abnormality. This corrective action brings in a gradual improvement in the performance level.
The occurrence of the abnormality can happen due to the following reasons:
SOP’s are not available
SOP’s are available, followed by the people but still, abnormality occurred.
SOP’s are available but not followed by the people
Analyze Abnormalities in the Industry Through Daily Management- Once an abnormality occurs, an analysis of the same must be done to conclude one of the above reasons due to which the abnormality happened.
Let’s Understand all the above three reasons:

More than 95% of the organisations give less importance for managing their human assets.  This is no surprise if we come to know that nearly 100 years back, Henry Ford also commented which had a meaning that
“ why is it what when I want to hire a pair of hands- only physical labour, a brain comes attached (to people) ?”.  
This concept has been highlighted through a fun movie Modern times where people are expected to do physical work and not contribute beyond that. Thinking, creating, improving and developing were activities were kept with very few – specific job profiles such as engineers, managers.
It is known fact that native born gifts of a person accounts to less than 10 % of the overall talent.  90 percent of the talent in the life of company employees is learned through own efforts, training and repeated practices.  Self-learning is definitely an option but can take long time to learn. Success at best organisations can be traced to planned and effectively delivered talent development programmes.  Companies like Toyota who believes in people development have invested dedicated training centres such as Toyota Technical Training Institute where they take competent and trainable people from various locations around their factories and with considerable time and efforts develop high levels of talent.   It is hard to find ideal people who have exact skills that an organisation is looking for and that too in readymade form.
Many companies have a thinking that
“what if we train people and they leave us?”
For such people, there is also need to ask what if untrained people gets retained in the organisation and they will never want to leave?  We end up with deadwoods.
Lack of training or ineffective training results in ineffective and suboptimal results. This results in too much firefighting which is not only time consuming but also results in loss of effectiveness and energies.  This further leads to no time for training. This becomes a vicious cycle. We have to start with small investment of time in right training which inturn can give back multiple fold time to the organisation.
One of the Toyota’s principles mentions:  Develop exceptional people and teams who follow your company’s Philosophy.  Many organisations claim that they give importance of developing people within the organisations with a slogan on everywhere “People are our most valuable resources”. However, on ground the reality is people are ill-equipped to perform their basic jobs.  There are no structured talent development plans and no skill matrix of operators too. We have seen many organisations who keep doing firefighting at senior management levels struggling to make on time dispatches/ order fulfilments even after having state-of the art manufacturing facilities and best in class machines. The missing link here is not having right skill of direct value adders – shop floor associates. Every company must have an effective method to train people.  Rather than self-learning of key job roles which is very unorganized, the training courses should have direct reflection on key day to day jobs. This is where the so-called Human resources development members can facilitate the transition. Training job skills through On job training is one of the starting point to improve organisation performance. Quality Culture in an Organization | Quality Improvement in the Workplace
Quality is the result of collective effort of every employee in an organization. Quality cannot be achieved only through stringent quality control methods and use of sophisticated instruments or by highly competent quality department. Dr. Deming claims that 85 % of quality problems are created by people who never touch the product. Also quality cannot be achieved using flavor of the month approach. When individuals make quality as a habit, it becomes a culture at organization level. In this paper we are making an attempt to summarize traits of an organization where quality is a culture.
1: What is Quality Culture and how we measure it?
During one of the plant visit, we entered their canteen and found beautiful visuals for keeping the used food plates, cups, waste food etc…And in practice most of the employees mixing cups and plates, partially clearing waste foods from plate etc… We came to a conclusion that this company may be having serious concerns of quality. When we expressed our view with one of the manager he was not convinced. Then we explained him the concept of culture. If your employees not serious about mixing cups and plates, what is the guarantee they are serious about mixing of parts in the shop-floor? Later he was convinced with our logic. In the next stage we observed employees not following lane principle while going out of canteen. They suppose to walk in the left hand side but most of them were not complying with the rule. Simple examples like this will tell us if quality is a culture in an organization.
Below is the Wikipedia definition of Organization culture as a summary.
Organizational culture is  collective behavior of people belonging to organization and the meanings that the people attach to their actions. organization values, visions, norms, working language, systems, symbols, beliefs and habits are part of its culture.
It is also the pattern of such collective behaviors and assumptions that are taught to new organizational members as a way of perceiving, and even thinking and feeling. the way people and groups interact with each other, with clients, and with stakeholders depends on its Organizational culture.
If we extend this definition to include quality, it becomes, “Organizational culture affects the way people and groups interact with each other, with clients, and with stakeholders and ultimately quality of the product
That means “Quality Culture depends on Organization culture”
Now let us look at the other side. Imagine that you entered an organization where quality is a culture; you feel the difference right from security gate. The way security personnel interact with you, the systems they maintain, the time management etc… gives you a feel of world class organization. Once you enter the reception, you will continue to experience the difference. The courteousness of receptionist, the self help displays, hassle-free path ways etc… You will appreciate how much attention given even for micro level things. No need to explain about the behavior of rest of the organization.
Once a company was audited by its Japanese customers for Quality System implementation. The auditors specifically wanted to visit three areas which took the company management by surprise. According to them, the areas customers selected were nothing to do with their quality systems.
The three areas selected were; Security, Scrap yard and workers toilets. Later during the closing meeting, one of the auditors explained to top management about the reason behind selecting those three areas. The logic is, most of the companies give least importance to these areas. According to them, if any company gives importance to these areas, it is obvious that they are serious about other areas.
2: Factors influence Quality Culture
We asked a question to our linked-in group members. In the equation Y=F(x), where Y= organization culture and x1, x 2 etc… are factors influence the culture, identify those factors. Also what are the 3 xs strongly influence the culture? We received various answers like Geographic location, Value system, Vision, Constancy of purpose, leadership, Management commitment, external environment, Quality Management systems, Human Resource development etc…most of them are interrelated.
Example: Poor implementation of quality management system is reflection of poor management commitment and leadership and so on…  Let us discuss about three important factors which according to us have strong influence on quality culture. They are Leadership, Training and development and value system. One may argue that the second and third factors can be influenced by Leadership. However, for ease of analysis we considered three factors separately.
2.1 Role of Leadership for creating culture of quality – The tale of two leaders
One of the organizations was engaged in manufacture of electric motors. All the completed motors were required to go through final auditing process. It was a month end and the factory was yet to deliver more than 50 % of the orders. In such situation one of the quality auditors rejected the entire lot of motors due to abnormal noise. The plant manager was very upset hearing this. He called the particular auditor and asked what is the problem and after knowing the problem, he suggested to release products under deviation.
The reason given was – These motors are used in textile industry and the abnormal noise of this motor will not be audible in the huge noise created by textile machinery. You can imagine what will happen for the quality in future. The quality auditor next time takes his own decision to pass the defective parts and spread the wrong culture.
Another company was making fabricated structures for construction of bridges. CEO of this company used to do surprise audits at different stages to ensure delivery of quality products to customers. Once while doing one of such audit, he found one of the structure having abnormal bend being in the vehicle ready for dispatch. He immediately asked concerned people to stop the shipment and correct the problem. Not only that; he also asked his team to do a root cause analysis for such escape. Though it is un intentional, the response by leader shows his commitment to quality.
One of the key roles of a leader is “Lead by example”. In the above example, the company may lose one delivery but it sends a strong message to all the employees that any deviation to quality is not acceptable at any cost.
Another key point here is continuation of these leadership principles irrespective of change in leadership. This is a big challenge for many organizations.
2.2 Training and Development – Key in creating culture
Cultural transformation is a journey. Training and development plays an important role in creating quality culture. It should happen at all stages recruitment to retirement and for all level of employees from operator to CEO. Values of an organization communicate what is acceptable and what is not acceptable behavior. For example one of the companies had the following values

Respect for people
Sense of urgency
Now this will be the guiding factors for every employee. They will be questioned by any one if they deviate from this. Like a compass in the ship showing direction to the captain, the value system tells organization the right and wrong direction. No one can question this including CEO. Once again, it depends on leaders, how they drive it and communicate it.
3: Quality Culture and Quality Management Systems
In our article “IS ISO 9000 CERTIFICATION LOSING ITS CREDIBILITY? We tried to demystify the various factors responsible for dilution of quality management system. It is quite obvious from our study that, organization which has poor quality culture give least importance to Quality Management Systems like ISO 9000. If the systems are weak, naturally product quality deteriorates. The following triangle illustrates the effects of deviation in quality systems.

4: Quality Assurance (QA) and Human Resource Development (HRD) should not just remain as nameplate change:

If your employees not serious about mixing cups and plates, what is the guarantee they are serious about mixing of parts in the shop-floor? Later he was convinced with our logic. In the next stage we observed employees not following lane principle while going out of canteen. They suppose to walk in the left hand side but most of them were not complying with the rule. Simple examples like this will tell us if quality is a culture in an organization.
Below is the Wikipedia definition of Organization culture as a summary.
Organizational culture is  collective behavior of people belonging to organization and the meanings that the people attach to their actions. organization values, visions, norms, working language, systems, symbols, beliefs and habits are part of its culture.
It is also the pattern of such collective behaviors and assumptions that are taught to new organizational members as a way of perceiving, and even thinking and feeling. the way people and groups interact with each other, with clients, and with stakeholders depends on its Organizational culture.
If we extend this definition to include quality, it becomes, “Organizational culture affects the way people and groups interact with each other, with clients, and with stakeholders and ultimately quality of the product
That means “Quality Culture depends on Organization culture”
Now let us look at the other side. Imagine that you entered an organization where quality is a culture; you feel the difference right from security gate. The way security personnel interact with you, the systems they maintain, the time management etc… gives you a feel of world class organization. Once you enter the reception, you will continue to experience the difference. The courteousness of receptionist, the self help displays, hassle-free path ways etc… You will appreciate how much attention given even for micro level things. No need to explain about the behavior of rest of the organization.
Once a company was audited by its Japanese customers for Quality System implementation. The auditors specifically wanted to visit three areas which took the company management by surprise. According to them, the areas customers selected were nothing to do with their quality systems.
The three areas selected were; Security, Scrap yard and workers toilets. Later during the closing meeting, one of the auditors explained to top management about the reason behind selecting those three areas. The logic is, most of the companies give least importance to these areas. According to them, if any company gives importance to these areas, it is obvious that they are serious about other areas.
2: Factors influence Quality Culture
We asked a question to our linked-in group members. In the equation Y=F(x), where Y= organization culture and x1, x 2 etc… are factors influence the culture, identify those factors. Also what are the 3 xs strongly influence the culture? We received various answers like Geographic location, Value system, Vision, Constancy of purpose, leadership, Management commitment, external environment, Quality Management systems, Human Resource development etc…most of them are interrelated.
Example: Poor implementation of quality management system is reflection of poor management commitment and leadership and so on…  Let us discuss about three important factors which according to us have strong influence on quality culture. They are Leadership, Training and development and value system. One may argue that the second and third factors can be influenced by Leadership. However, for ease of analysis we considered three factors separately.
2.1 Role of Leadership for creating culture of quality – The tale of two leaders
One of the organizations was engaged in manufacture of electric motors. All the completed motors were required to go through final auditing process. It was a month end and the factory was yet to deliver more than 50 % of the orders. In such situation one of the quality auditors rejected the entire lot of motors due to abnormal noise. The plant manager was very upset hearing this. He called the particular auditor and asked what is the problem and after knowing the problem, he suggested to release products under deviation.
The reason given was – These motors are used in textile industry and the abnormal noise of this motor will not be audible in the huge noise created by textile machinery. You can imagine what will happen for the quality in future. The quality auditor next time takes his own decision to pass the defective parts and spread the wrong culture.
Another company was making fabricated structures for construction of bridges. CEO of this company used to do surprise audits at different stages to ensure delivery of quality products to customers. Once while doing one of such audit, he found one of the structure having abnormal bend being in the vehicle ready for dispatch. He immediately asked concerned people to stop the shipment and correct the problem. Not only that; he also asked his team to do a root cause analysis for such escape. Though it is un intentional, the response by leader shows his commitment to quality.
One of the key roles of a leader is “Lead by example”. In the above example, the company may lose one delivery but it sends a strong message to all the employees that any deviation to quality is not acceptable at any cost.
Another key point here is continuation of these leadership principles irrespective of change in leadership. This is a big challenge for many organizations.
2.2 Training and Development – Key in creating culture
Cultural transformation is a journey. Training and development plays an important role in creating quality culture. It should happen at all stages recruitment to retirement and for all level of employees from operator to CEO. Values of an organization communicate what is acceptable and what is not acceptable behavior. For example one of the companies had the following values

Sense of urgency
Now this will be the guiding factors for every employee. They will be questioned by any one if they deviate from this. Like a compass in the ship showing direction to the captain, the value system tells organization the right and wrong direction. No one can question this including CEO. Once again, it depends on leaders, how they drive it and communicate it.
3: Quality Culture and Quality Management Systems

It is quite obvious from our study that, organization which has poor quality culture give least importance to Quality Management Systems like ISO 9000. If the systems are weak, naturally product quality deteriorates. The following triangle illustrates the effects of deviation in quality systems.
4: Quality Assurance (QA) and Human Resource Development (HRD) should not just remain as nameplate change:

Here the definition of Excellence, I am limiting to Business Excellence journey using strategies like TQM, TPM, Lean, Six Sigma, etc..
Why should some organizations start any of this journey?
Crisis
If the organization is in deep trouble, its profits are negative, Liabilities increasing, No demand for the product, etc.. we can consider that this particular organization is in Crisis.  The current scenario of COVID 19 puts many organizations in Crisis mode.  Many organizations thinking of adapting practices like Lean, 5S, Kaizen, etc.. to reduce their losses.
The problem with this driver is it works until the crisis ends. The later organization thinks about excellence only when another crisis hits!
Customer Demand
Many organizations depend on a few key customers and those Customers mandate organizations on certain Systems. For example, some customers may ask its suppliers to implement Lean or TPM or Six Sigma. In such cases, suppliers have no options other than accepting it.
The problem with this driver is, it works only to satisfy customers.  So TPM- Total Productive Maintenance becomes Total Paint Management (Painting the factory during customer visits) or Total Presentation Management. ISO 9001 only lies in papers. So the organization will never reap the benefit of the business excellence journey.
Competition
Our competitor is practicing Six Sigma, so we also should launch it!  Some organizations closely follow their competition and copy whatever they do. They are too much obsessed with competition. The biggest danger in this approach is, what works for competition may not work for us.
CEO/ Chairman’s Vision
Very few organizations, Business excellence is driven by Chairman or CEOs vision. There is no crisis, no competition, no customer mandate but the leadership proactively implementing it keeping long term results in mind. Business Case :  Not able to meet Customer demand on delivery time, due to constraint in fabrication.  This resulted in adopting build to stock policy which added to inventory, last minute design corrections  and rework costs
Project Goal : To reduce Chassis Fabrication time by 30 %
Approach used : Lean and Kaizen –- Studying and Mapping current process, -Analysing VA and NVA – Identifying and implementing solutions – Training on new methods – Standardization + more than 35 small improvements (kaizens)

Result Achieved : Previous : 9 Hrs/ Chassis

Case 2 – Industry : Precision Tool Manufacturing
Business Case :  Huge order backlogs and not able to meet customer expected delivery.  Lead-time from order to Delivery was more than 12 weeks whereas customer expectation was less than 4 weeks.
The Challenge; It is a High Variety and Low Volume Business and Changeover time is much greater than Processing time.

To reduce Lead-time in Phases to meet customer expectations (Both Manufacturing and Pre-Manufacturing Lead Times)
Approach used : Lean & Value Stream Mapping – Identifying the Bottlenecks – Developing should be Map and Implementation . ( Key tools: QCO, TPM, 5S, Standard work and Quality Improvement)
Result Achieved :  Reduced overall lead-time in Phases from 12 weeks to 8 weeks in Phase 1 and from 8 weeks to 6 weeks in Phase 2.
Zero Backlog orders / Delighted Sales Team
Intangible Results : Internal Customer Satisfaction increased from 29 % to 76 % which improved cross-functional team work
Case 3 – Industry : Fiber Optic Cables for Telecom Industry
Business Case :  Need to double Productivity as the demand increased in for exports
Challenges : It is completely manual process and productivity dependent on individual skills
Project Goal :
To Improve Productivity by 50 % from 430 ends per line per day to 650 ends per day per line
Approach used : Lean & Value Stream Mapping – TWI – 5S – Kanban
Hourly output monitoring and PDCA. Select pilot line and horizontal deployment to other lines
Result Achieved :  Improved Productivity in Phases from 430 ends to 800 ends per line per day in  2 phases – 6 months duration.
This was achieved with existing workforce whose basic educational qualification is Class 10 or less
Case 4 – Industry : Apparel
Business Case :  To meet demand of the export market . This could be met only if  the lead-time from Cut-to-Pack is below 6 days.
Project Goal : To Reduce Lead-time from Cut to Pack from 9 days to less than  6 days
Approach used : A pilot line is selected, Takt time concept introduced,
VSM – Current and Future, Line Balancing  , TWI, Heijunka + more than 25 small kaizens
Result Achieved : Achieved the goal of  Lead-time reduction to less than 6 days in 2 phases

Case 5 – Industry : Process – One of the Leading FIBC Bags Manufacturer in India
Business Case :  It was a Mix of Labour and Process Oriented Industry. Labour Productivity was low compared to Benchmark companies in China. Quality and Delivery improvement at Optimum cost was the Goal

Implementing Lean Maintenance System to Improve Factory Performance

Need for 100% uptime of machines from the production department has never gone out of demand.  Day to day production needs are always to be met to meet delivery performance and revenue of the organisation.  One may ask if zero maintenance time is really possible. Imagining a condition when a flight is already in the air and it needs maintenance in mid air.
Can we send a maintenance engineer to repair things in mid air?
Any maintenance needs are to be performed when there is break time or when production is not scheduled.  Preventive maintenance time allocation in such cases becomes essential so that the machine performs to its fullest potential during its intended working hours.
Lean maintenance system works in combination of Total Productive maintenance, Reliability centred maintenance,  Lean methods and TQM as integrated system. The ultimate goal is to eliminate the need for maintenance during production or Zero maintenance time.
We can call maintenance strategies into three broad categories:
Maintenance done only upon a breakdown
Periodic or preventive maintenance
Condition based corrective maintenance

In a lean maintenance system, maintenance planning is done based on the priority categories.
Priority category : 1
This can be classified into a critical category of maintenance.  In this category, if the machine gets into a minor stoppage or breakdown, it can disrupt the production resulting in serious implication to customers.
Solutions to handle the machines in this category are:
Identify critical components of the machine
Do regular monitoring of the critical components for its effective functionality.
Where applicable, replace the critical component before it gets into failure. For this, MTBF and past records will help.
Have clear procedures and training to the designated people in handling these types of failures.
Keep critical spares always in stock with Kanban systems or through ERP with min-max triggers. No stock situation due to customer lead time, internal approvals, logistics issues and supplier issues should be considered in planning and FMEA.
Have a back-up maintenance plan with designated people availability during production operations too.
Ensure MTTR is well within the accepted time.
Have a redundancy  plan or back up items to take over.

Priority Category : 2
The criticality of components in this case is relatively lesser compared to Category -1.
Remaining solutions are still applicable as category 1 in this case too.
Priority Category : 3
In this, the machine is still critical for operations and it can disrupt the production. However, the implication  of machine failure is less on the customer and hence it can still be managed. Solutions are to be customised to the machines under this category.
Other machines are considered not very critical and hence moderate levels of maintenance planning can be recommended to optimise the costs and strike the right balance between performance and downtime implications.
There are two main strategic approaches for ensuring zero down time.
Maintenance during scheduled non-production time
Reducing maintenance time
