I read your post about open enrollment for the ACA and was hoping you might expand on why you believe it would take years to dismantle. I've been terrified that with a Republican house/senate, Trump could just snap his fingers and make it go away within months of taking office. I'd love some reassurance that that's not possible.
Hiya, sure I can share some thoughts on the matter! First, it's very important to understand the ACA is a huuuuuuuuuuuuge system with subject matter experts in dozens of places throughout the process. I'm one of those SMEs, but I am at the end of the process where the revenue is generated, so my insight is limited on the public facing pieces.
What this means is that I am professionally embedded in the ACA in a position that exists purely to show what conditions people are treated for and then generate that data into what's called a "risk score". There's about 6 pages I could write on it, but the takeaway is that the ACA is
1) intricately interwoven with the federal government
2) increasingly profitable, sustainable, and growing (it is STILL a for-profit system if you can believe it)
3) wholeheartedly invested in by the largest insurance companies in the country LARGELY due to the fact that they finally learned the rules of how to make the ACA a thriving center of business
4) since the big issuers are arm+leg invested in the ACA, there is a lot of resistance politically and on an industry level to leave it behind (think of the lobbyists, politicians, corporations that will fight tooth and nail to protect their profit + investment)
The process to calculate a risk score takes roughly 2 years. There is an audit for the concurrent year and then a vigorous retro audit for the prev year - - this is a rolling cycle every year. Medicare has a similar process. These are RVP + RADV audits if you would like the jargon.
Eliminating the ACA abruptly is as internally laughable as us finishing the RADV audit ahead of schedule. If Trump were to blow the ACA into smithereens on day 1, he would be drowning in issuer complaints and an economic health sector that is essentially bleeding out. You cut off the RVP early? We have half of next RADV stuck in the gears now. You cut off the RADV early? No issuer will get their "risk adjusted" payments for services rendered in the prev benefit year (to an extent, again very complex multi-process system).
The ACA is GREAT for the public and should be defended on that basis alone. However, the inner capitalistic nature of the ACA is a powerful armor that has conservatives + liberals defending it on a basis of capital + market growth. It's not sexy, but it makes too much money consistently for the system to be easily dismantled.
Or at least that's what I can tell you from the money center of the ACA. they don't bring us up in political conversation because we are confusing to seasoned professionals, boring to industry outsiders, and consistently we are anathema to the anti-ACA talking points.
I am already preparing for next year's RVP for this window of open enrollment. That RVP process will feed into the RADV in 2026. In 2025, we begin the RADV for 2024. If nothing else, the slow fucking gears of CMS will keep the ACA alive until we finish our work at the end of the process. I highly doubt that will be the only reason the ACA is safeguarded, but it is a powerful type of support to pair with people protecting the ACA for other reasons.
I work every day to show, defend, and educate on how many diagnoses are managed thru my company's ACA plans. My specialty is cancer and I see a lot of it. The revenue drive comes from the Medical Loss Ratio (MLR) rule stating only 20% MAX of profit may go to the issuer + the 80% at a minimum must go back to the customer or be invested in expanding benefits. The more people on the plan using it, the higher that 20% becomes for the issuer and the more impactful that 80% becomes for the next year of benefit growth. It is remarkably profitable once issuers stop seeking out "healthy populations". The ACA is a functional method for issuers to tap into a stable customer base (sick/chronic ill customers) that turns a profit, grows, and builds strong consumer bases in each state.
The industry can never walk away from this overnight - - this is the preferred investment for many big players. Changing the direction of those businesses will be a monumental effort that takes years (at least 2 with the audits). In the meantime, you still have benefits, you still have care, and you still have reason to sign up. Let us deal with the bureaucracy bullshit, go get your care and know you have benefits thru 2025 and we will be working to keep it that way for 2026 and forward. This is a wing of the federal government, it is not a jenga tower like Trump wishes.

Anyone have tips for raising a fuss with your local government/press?
My primary care physician has adopted an absolute bullshit policy for appointments that is NOT at all in line with my state's "right to timely care", and I'm looking for ways to escalate this.
They are the biggest provider in this county and several neighboring counties and they put a LOT of effort into their public image. They are literally lying to patients as part of this policy (they claim this has always been their policy, that it's an unavoidable part of a teaching hospital, and that the residents' schedules are not possible to predict.)
I want to force them to rollback or adjust their policy by putting pressure where it hurts. You don't get to have a reputation as the local leading healthcare provider, and accepting new patients, if you are refusing to schedule existing patients.
What I've done so far:
Filed a formal written complaint through my insurance company
Had an in-depth mediated phone conversation with a "patient representative" as part of the insurance process, where I proposed several concrete changes
Brought up the issue to my doctors and nurses, all of whom agreed with me
Asked the front desk staff to file a complaint. Not sure if they did this.
What I'm considering but am a little overwhelmed by and not sure where to start:
Contacting local news outlets
Contacting local government
Starting a letter writing campaign to the organization with my local Mutual Aid network
Any ideas or tips would be welcome! I have a lot of physical and mental health issues, so my capacity is limited, but I am fucking pissed both for myself and on behalf of other patients, like the parents I overheard that were unable to book the recommended follow-up for their toddler.
If you're wondering why I don't just change:
I have multiple ongoing diagnostic processes and specialists
They have a fucking monopoly on the area and so finding a different one with in-network urgent care is hard unless I switch to Kaiser, which I can't change until next open enrollment
I have been with these doctors for 15 years and I loved them up until last year when they started this
When I do get an appointment, I really really like the doctors
Because of their monopoly in the area, this is affecting SO SO many people and that's not okay
The specific policy is that you cannot book any appointment with this primary care provider unless you call them during the 2-3 days a month where they are accepting appointments only for the next month -- they fill up completely within days. That means you cannot schedule ANY appointments, including follow-ups, any further out than a month, and that is if you're very on top of it AND lucky.
There are exceptions (urgent care followups and getting an appointment through the nurse triage line), but this means, for example, it took me 4 months to be able to schedule my follow up appointment for my newly-diagnosed asthma, where I then needed two referrals and new prescriptions. I can't schedule my gyno checkup. I can't schedule a visit about my new fatigue and pain symptoms that have been going on for 4 months.
This would be frustrating at a specialist, but it is unacceptable for a primary care provider.

You don't have to answer if it's too personal, but as poor, queer person I was wondering what you do for a living/ how you're able to afford gender affirming surgery. Posts like yours give me hope that one day I'll be able to. But I like to ask and take notes on how other trans people were able to get there
i work for starbucks full time im a shift supervisor i make ~$24-26 an hour. i work there for the benefits....during medical LOA they pay you a portion of your wages. my state also recently started a in-state short term disability program that pays more than starbucks program does, but sb still partially matches my weekly in state benefit so im receivving a lot more $ weekly than i did during my top surgery-- enough to pay my bills get food and save to pay off my insurance deductible.
starbucks after 90 days of employment or during open enrollment you can choose your insurance and cross reference it with the entirety of your surgical team to see if everything is in network. so my surgery cost $4500. it resets every calendar year so i will have to come up with it again for my secondary stages.
im gonna be in a lot of debt regardless lol and there has been a lot of fighting w insurance, failures w insurance, needing to get on another plan to have surgery, etc etc. my coworker who got his phallo had to file for bankruptcy. i have had 4 surgery dates and started the process 3 years ago. i also recieved money via fundraising and saving my electrolysis insurance reimbursements was very necessary for me to afford stage 1
its very inaccessible but i know a lot of loopholes at this point if you want to game the system to get yours i can help you dm me if you are interested! also i know ikea has similar trans benefits and probably treats employees better

I think one of my sangriest (sad angry) moments in recent memory was when my husband busted a crown, and we didn't know our dental insurance.
I asked 3 different folks I worked with (my school district has 2 choices for insurance, so it should be a safe bet ONE of them has the same as me) where the fuck to find out how to get it covered. One is general (like Blue Cross Blue Shield, which I grew up under when my grandparents took me in til I aged out) and the other is weird because it's an all in one network*
I happened to ask 3 dudes, and each one replied with "I don't know, my wife handles that."
After the third same reply within 20 minutes (I'm blessed to have my day bookended with prep periods) of hunting folks down, my first thought was "fuck me, I wish I had a wife", and the theoretical warm feeling of "someone knows what to do in an area I don't" washed over me. Then I was despondent because I immediately realized I have extremely little of that. Definitely due to my growing up without it/extremely compensatory nature of refusing help because nobody will ever understand what I'm thinking without extensive explanation (which would be awful for them since my ADHD makes that lengthy on my end/the understanding that comes from teaching and metacognition that only I TRULY know all the background knowledge that informs how I personally process and categorize my thoughts) which means it's just easier to handle things myself.

Could I just get a wife? Can I afford having a third person in my emotional life that I cover living expenses as like a secretary but that I also love and provide for?
That's just kinda the feeling I got. It made me feel gross. It made me feel inadequate. I'm obviously not a good enough wife (and adult, damn) because I didn't know these things, and I'm a bad person for hoping some magic woman (because honestly, I later branched out exclusively to women, married or not, and didn't get anything even REMOTELY similar in their responses to my male coworkers) would take care of me and things I needed legally/monetarily if my body were falling apart.
Still makes me sad, honestly. I grew up til my teen years with zero support on the medical end, and only got insurance when I moved in with my grandparents. BCBS is comprehensive (at the time at least) and well received, and my grampa got it through working for the Pentagon. My mom moved in later with us, and has Medicare, and my younger brother has never gotten insurance through his job.
It turned out that my husband and i (same county educational system) were paying independently out the ass when we could save TONS after marriage, but it wasn't specified. So we decided to have him added to my insurance because honestly, I look at the mail and could (theoretically) handle it. Turns out I fucked up/the emails I get spammed constantly happened to bury what I needed to do next under "congratulations, you're in", so I didn't continue reading. He was uninsured, and it was because I didn't finish reading an email. It's all my fault.
The only way around was divorce, then remarriage (which iirc woulda been like $300 round trip) but that wasn't flying. He was attempting to apply after the open enrollment (why the fuck is this a thing) so he's now paying through the nose for the same insurance we had individually. He also SHOULDN'T have qualified, but we still don't know why they accepted him.
I'm a bad wife, so I still don't understand/the system itself is confusing. Maybe I'm not a bad wife, but everything I know tells me I am.
This is all to say that driving home from one of my side gigs was the 1074283953rd throbbing thought behind my head at how much happier I'd be as a dude, but idk. I've never seen myself as a feminine person, and wonder if I'm compensating for wanting to be outside the binary by idolizing masculinity. I try to keep abreast in the whole "where are we going with masculinity/why do boys feel adrift" thing, especially as an educator who isn't afraid of it, so I'm aware of what's going on in those circles. I know passing is BS as a concept, but outside of accepting circles, know I'll never be taken seriously as a guy. I'm barely taken seriously as an adult as is being 5' and hourglass shaped in my early 30s with a goofy and open disposition, which I use to my advantage of "who cares" as a vibe. But I still know I'm taken as seriously as I take most other things. I'm not gonna perform more than I have to to keep my job, gender wise or in a "professional" sense.
LGBTQIA+ committe head (they deserve respect!) goes as Mx. And goddamn. I can't even imagine the shit they deal with. I'm at a super progressive school, but the shit we deal with and bring up to admin (tbh more supportive than I've EVER experienced, in every capacity in my decade of teaching, by a longshot) to is still bullshit. You can't change a community til their kids fucking go non contact, in my experience.
This got super pessimistic, but most boils down to I'm just trying to rile and pep up my queer kids and be an icon. Maybe one day I'll be happy but til now, I'm just jazzed I did the whole "abrathatfits" thing so my tits are mostly outta the way. They're way up (comparatively) but feel so much better though now I'm uncomfortably aware of them and only one of the 2 fits I have are similar to what I'm used to, but that's just getting used to not wearing sports bras for forever
Makes me feel good in a way, but geh
This post was a primer on like 95% of my insecurities. Long story shirt, wish I could be the Chilchuck coded fucker I wish I were and I'd be happy.
