I think a good angst idea would be Marty getting hurt badly cause of one of docs failed experiments that doc KNEW was risky and still asked Marty to help anyways.

Imagine how GUILTY doc would feel for endangering not only his best friend/son figure, but the only person that treated him kindly and was willing to help him without judgement
