Drill Day
'Attention participants, the drill will be starting in 1 minute. Take this time to prepare yourselves and to remember that everything that will happen is a simulation. Nothing can or will legitimately hurt you. Should you wish to stop the simulation tap the centerpiece of your vest and you will escorted off, should you be unable to do so simply say 'moon' and you will escorted off.'
"Okay the sound system is good, the holograms and drones are a go, and our actors are ready."
"I still don't know why we need actors when we have perfectly good holograms and drones." Glip didn't want to admit but she was a little hurt that Calis and the Captain didn't trust her work.
"We've been over this. As good as your holograms are they aren't physical which can lower the realism and while your drones are physical they can can be too predictable which is something we don't want. Hence the actors."
"Doubt they'll even be convincing." she grumbled.

To Glip's annoyance and Quip's entertainment the actor's where in fact convincing.
"What are you doing you idiot?! Your in a supply closet with tons of stuff around to use and you use a towel?? You deserve to be captured by enemy forces."
The poor Vrool get's tackled to the ground by an actor who roars in their face, fake fangs and mandibles making them ink themselves.
"Aw buddy, aw there we go! Good sportsmanship actor!" the actor uses the towel to clean the Vrool best they can before finding them a new uniform to change into them a before sending them to the 'jail' set up.
"Shlip who did they hire for this?" honestly this was some of the best simulation acting she's seen in a while.
"Some random ship members who had too much time and was down for a free dinner on Cap."
"Huh, any apexes?"
"Oh yeah, like two thirds of them. Those big ones dressed like a Mors Crawlers? That's a tighalax and a rextalian."
"Great asteroids no wonder those orcs were shaking."
"Yep," he said popping the p. "almost as terrifying as the real thing."
..
"Wait wait wait, hold the shlipping communicator, we got younglings doing this?"
"Oh quiznack. . .well it's probably gonna be toned down right, right?" he asked looking at his co-worker.
"Yeah, yeah it should be. Look. Like half the actors are leaving...leaving only the scariest ones left." they looked at each other concerned.
"CAAAPPPTAAAIN??"
"What!? Damnit Quip and Glip don't yell into the comms!"
"Are we actually sending younglings into this??"
"Isn't this like youngling endangerment?"
"The kids have been briefed about this and their parents signed off on this. Perfectly legal and safe. I also brought in some experts on this."
"Experts on scaring children?" who the shlip does that??
"Just do your jobs."
...
"OH MY GODS..HOW THE ACTUAL SHLIP?!"
"how the deq are these kids fine with this..."
Said younglings are being chased by actors, dressed as Mors Crawlers and Domitors, the actors giving them very little breathing room.
One of them swipe at the younglings with their prosthetic claws, catching on the little ones clothes. They yank back and the child is sent sprawling towards the jaws of the Domitor, the child's vest lights up red. They're out.
Some more children get caught by claws, tails, and wings. Each one of them are out.
The remaining few duck into an open vent too small for the predators to follow. They claw and roar at the entrance.
"HA! Can't get them now, score one nothing for the younglings!"
"Nope, look at screen 3."
While the actors at the entrance roared and thrashed a few of them ran through the halls until reaching the other side of the vent. Just as the children were coming out.
They could run but to where? Back into the vent? Forward into their pursuers?
Well they tried both.
The largest and strongest charged forward while the smallest ran back.
While the larger and stronger ones were from species known for their power and abilities, they were still younglings going against adults. After some struggling their vests turned red.
Only a handful of younglings reached the vent without getting caught. The ones inside were safe...for now.
....
'Only 5 minutes before the simulation is over. 5 minutes.'
"Well, looks the kids win this one."
"Good for them. They earned it."
"Yeah-hold up...what are they doing?"
The actors were...breaking the wall. They had grabbed chairs and any heavy objects they could and were using them to break down the wall.
"IS THIS ALLOWED?!"
"THERE'S NO WAY IT IS....gods wrath it is...JUST LET THE KIDS HAVE THIS!!"
When a foot of the vent was now revealed they grabbed it and started to pull. The other side, now aware of what was happening, started to push and increase their efforts in grabbing the younglings.
"They're pulling it...oh my gods they're pulling it out of the actual wall!"
"IT'S 15 FEET LONG THOUGH!!"
"HOW MUCH TIME IS LEFT?!"
"2 MINUTES."
Inch by inch the predators pulled out sections of the vent. Almost a third of the way done. The hole left behind now big enough to send over their smallest predator on the other side.
"BODY CAM BODY CAM"
"I'M DOING IT I'M DOING IT"
The beast crawled as fast as it could through the tunnel. The younglings yell and kick at its claws.
A child screams while being dragged out of the body cams view.
"They're dead."
"Yep, you owe me five tix."
"No, I betted on the other rextali-"
"MOON!"
Quip and Glip quieted. Then went into a flurry finding the right camera, searching for the child who said the safe word.
They found the child; Zyz, age 6 years old, species rextalian, being comforted by an actor. Face pressed into the adults fake fur while stroking the child's spine with their palms. Keeping their face and claws out of Zyz's view.
All the other actors around them softly put down the vent pieces and children already caught.
They then step back and then kneel or crouch. Claws are up or by their sides.
The children go to one another, checking up on each other, a few check on Zyz.
One of the actors slowly approaches Zyz on their knees. Hand out holding a tissue.
A tiny hand takes it. Along with the tissue.
And just like that all the other adults scoop up the children. Some are cradled into their chests, needing comfort and to not have them see their masks. Others are put onto their shoulders and backs, not as riled up or in a more playful mood. Some are tossed and dangled, to put them at ease despite the adrenaline coursing through their small bodies.
Soon the sniffles and hiccups are replaced by laughter and cheers.
The claws are replaced by warm sweaty hands.
Fangs and mandibles are replaced by sweaty smiling faces.
Wings and extra appendages are replaced by children hugging the tired actors.
"...they were HUMANS?!?"
"Hey it's Max! ...Ohhh that's whose a professional in scaring kids. That makes sense now."
#shlip is a space curse word#this is influenced by me learning that Maleficent's daughter was the one to play young Aurora#also by the video of that little girl who loved the monster costume#sometimes i play as a monster when with kids and they love it but sometimes it gets to be too much#so i either tone it down by kneeling and being quiet or by giving them space/a break#humans are space orcs#humans are space oddities#the adventures of kim and max running a space child centre#not gonna lie I also get pretty competitive when chasing them and trying to catch them to the point where I will almost tackle them#or use their friends as bait to lure in the faster ones

thinking about swiss getting in trouble for all the times he didn’t play his guitar on tour, all the times he didn’t stay on his platform like he should’ve, all of the times he went off and distracted his packmates during songs simply because he couldn’t control himself, and in turn being ordered to go to the practice room one afternoon and play. just play. prove he can work like the loyal ghoul he was summoned to be. so he does. and he's not happy about it, but he does.
but I’m also thinking about the others catching wind of his punishment and perhaps making it their mission to let him know just how distracting he can be.
swiss is trying so hard to practice. he wants to finish the set and go back to the den, pester cirrus while she reads or go search out whatever odd task mountain is busied with, but no matter how far he gets into a song he can’t help but notice his packmates in the corner of his eye as they sneak into the practice room with him. they come one by one and leave before the next enters, playing coy as they pretend to poke at their own instruments or adjust a sparkling tab. the first one catches him off guard—he’s about to be through the bridge in rats when long fingers wrap around his tummy from behind. they poke and prod at the soft flesh below his shirt, grabbing and kneading it in their palms before dragging upwards to grope at his chest, and he's about to stop playing when rain hooks his chin over the top of his shoulder and tells him to "keep going." then only a minute or two later he's finished with the song and rain is slinking out of the practice room before he can ask him what he could have wanted with him.
then he's cruising through absolution when aurora enters with cumulus trailing behind her. they come bounding in a pair to the front of the room, twirling and dancing with their fingers intertwined as swiss plays, and he can't help but smile as cumulus hooks an arm around her waist and dips her nearly to the floorboards, pulls her back up onto her feet and twirls her in a circle. aurora lets go of her hand. only to drop to her knees in front of the busy multi ghoul and wrap her tiny hands firmly around the backs of his thighs. he startles when she grabs his ass but doesn't stop playing. never misses a beat (or at least any that they notice). cumulus dances and twirls around the pair of them, acting oblivious to aurora as she puts on a show of humping the air and pulling at his thighs in manners that are comically obscene. swiss wants to laugh, he really does, but he also really needs to get through with the set first... before his papa comes back to check on his progress... again.
the girls dance and laugh. cumulus shimmies against his right shoulder and aurora pulls on his leg so insistently that swiss contemplates knocking her in the forehead with the body of his guitar. but then just like rain had, the two of them are gone by the time he finishes the last note of the song- the door shut behind them.
more ghouls pay him visits as he progresses. mountain shows up during year zero just to obnoxiously pluck at the strings of his instrument, phantom slinks in simply to pester him, and even cirrus makes a show during mary on a cross of wriggling around on the floor like a worm in the dirt.
he's about to be done with it all (thank the gods below) when the last ghoul saunters in. he's jamming through square hammer when dew sidles up to him, and there's no games, no teasing to his distraction. swiss gasps over the chorus, choking on his saliva suddenly, when dew presses his hard-on into his ass and snakes an arm between their bodies and between swiss' legs. he grabs him by his cock firmly and with his entire hand, squeezes and gropes at his soft length like he's trying to claw at it through the fabric. swiss hears the ugly noise that screams from his instrument before he realizes that his fingers have stopped working-- his entire brain has stopped working-- and dew is unrelenting. swiss feels his knees buckle when dew awkwardly reaches around his front with his free hand to grapple at his balls. and now, with his entire package in his hands, he can't possibly finish the last song.
"what the hell, dew?" he says, exasperated, and tries to get out of his grasp. but dew is stronger than he looks. he just grips him harder until swiss is sucking air between his teeth at the pleasurable pressure (and hardening up under his palms) and begins the exaggerated gesture of pretending to jack him off beneath his guitar.
finally, when swiss manages to break free from him, does dew speak.
"you fucked up the song, asshole."
swiss can only gawk as he releases him and leaves the practice room without looking back.

Every Little Thing She Does Is Magic
A ShadowxAurora One Shot
Shadow never meant to keep it. With the limited space in his apartment, a piano wasn't exactly practical. But he'd seen it sitting on the street while on a run, a pathetic little spinet that apparently wasn't worth the effort for repair according to the owner, so it sat in wait for the dump truck.
Omega thought he was nuts when Shadow had used Chaos Control to transport the piano into the apartment, and perhaps he was. The instrument had definitely seen better days, and it would take more than a simple tune up to get it in pristine condition again.
That didn't stop Shadow from shoving the spinet against the wall between his mattress and the front door and then going out to purchase the necessary items for piano repairs.
The spinet became Shadow's passion project over the next several weeks. Any spare moment between his mercenary work with Omega and dates with Aurora, Shadow could be found with the spinet piano, painstakingly doting over the instrument to set it to rights again.
"You never told me you can fix instruments." Aurora had noted once, sitting on the little bench with her legs swinging while half of Shadow's body was inside the back of the spinet.
"Never came up." Shadow had grunted.
"Where'd you learn?" She'd pressed.
Shadow had shrugged. "I did a lot of things while off world, Light. Sometimes I was asked to fix things, and music is universal." Aurora had accepted that answer, and Shadow minutely relaxed.
No way he was EVER going to tell her that some aristocrat across the galaxy had taken fancy to him and tried to get his attention by breaking her piano, just so he'd come and fix it. It was the fastest he'd ever fled a planet. Omega still hadn't let him live it down.
The plan for the spinet once he'd finished repairs was simple enough: take it to the resale shop and get a decent sum of cash for it. He'd contacted the shop, gotten a good offer, and was set to deliver and receive his rings, but when he arrived and saw the buyer...a mother and son duo, the latter whom was whining about how much he HATED piano lessons and was currently and carelessly swinging a baseball bat around in his fit....Shadow took his piano and left.
No way was Shadow going to let all his hard work repairing his baby go to waste on some ungrateful brat that lacked basic appreciation. So, the little spinet piano became a permanent fixture in his apartment.
Shadow had never considered himself a musician of any sort. He was a warrior, a mercenary, the Ultimate Lifeform, a guardian. Music...required a certain softness that Shadow, with all his broken pieces and jagged edges, simply did not possess. But, somehow, that didn't matter. Sitting at his little spinet, gingerly filling his apartment with the soft tones of the classics centered him with a kind of peace he rarely ever achieved...with one exception. When he played, Shadow could pretend that was all there was. Just him and his spinet, creating something beautiful together. It was almost magical, if he believed in such a thing.
Shadow huffed a quiet chuckle, gently resting his hand atop the keys but not pressing down, his thoughts drifting towards the other almost-magical thing in his life. Honestly, if it magic was a thing, Shadow could believe it, because of her. The way she pranced through life, with such light and arms wide open, eager and excited for whatever came her way...could anything else but magical describe his precious Light?
Almost without his command, his fingers gently drifted across the spinet's keys, a delicate melody that swirled and danced through the air. Shadow sighed.
"Though I tried before to tell her
Of the feelings I have for her
In my heart.
Every time that I come near her
I just lose my nerve as I've done
From the start."
How many times has Shadow looked into those emerald eyes, seen that smile, and choked? It was three simple words, why was it so difficult? He's made peace with the past, hasn't he?
"Every little thing she does is magic
Everything she does just turns me on.
Even though my life before was tragic
Now I know my love for her goes on."
A sniffle behind him had Shadow whirling around, Chaos Spear halfway formed in his hand and a snarl on his muzzle, when those same piercing emerald eyes damp with tears stopped him dead. Shadow gulped, his ears flattening against his head. Damn. How long had she- Shadow made get up, averting his eyes as embarrassment colored his cheeks rosy red.
And then she's right there, pushing him back down on the bench with pleas of "Please don't stop, don't mind me-," and she's still looking at him with those eyes, pleading and wet, her body pressed tight against his side, lips protruding in the most pitiful pout...
Chaos, he was screwed, wasn't he?
Shadow sighed and tapped her nose with his finger. "You will say nothing to anyone about this." He commanded, and tried to ignore how distracting that beaming smile was in order to return to the piano. He gulped, frozen with his fingers in position. He knew his voice was not what anyone would call gifted, hers was so much better, and he chanced a glance down to his shoulder where she'd laid her head. She smiled at him again, eager and encouraging, and Shadow gulped and resumed playing.
"Do I have to tell the story
Of a thousand rainy days
SInce we first met?
It's a big enough umbrella
But it's always me
That ends up getting wet.
Every little thing she does is magic
Everything she does just turns me on.
Even though my life before was tragic
Now I know my love for her goes on."
Shadow rested his cheek against the top of her head, mindful of the short grouping of quills that acted as bangs, closing his eyes momentarily and just breathing.
"I resolve to call her up
A thousand times a day
And ask her if she'll marry me
In some old fashioned way.
But my silent fears have gripped me
Long before I reach the phone.
Long before my tongue has tripped me
Must I always be alone?"
Her arms squeezed him gently, reassuringly, around his middle, and he pressed a kiss to her head in response, smiling at the growing damp spot on his shoulder.
"Every little thing she does is magic,
Everything she does just turns me on.
Even though my life before was tragic
Now I know my love for her goes on,"
Shadow dropped one hand from the piano and cupped Aurora's cheek, tilting her chin up to look into her eyes, shining with light and joy, and he knew his words wouldn't fail him this time. He smiled at her and leaned his forehead on hers.
"Every little thing you do is magic
Everything you do just turns me on.
Even though my life before was tragic
Know that my love for you goes on."
Shadow ended the song with a soft kiss to her lips, sealing his declaration of devotion with all the love and passion and dedication he had in his heart in the best way he knew how. Words always failed him, but somehow, in this moment, it didn't matter. Aurora wept through his kiss, and he smiled as they parted, a quirk of his mouth so gentle and loving that only she would ever get to see it.
Aurora pounced on him a single moment later, using her own gift of speed to press kiss after kiss on his lips, face, head, everywhere she could reach, glowing so brightly and joyfully exclaiming "I love you"s between kisses. Shadow briefly wondered how she wasn't suffocating before dismissing the thought and basking in their shared love, trading her kisses and words with ones of his own. It didn't matter anyway.

I'm feeling a lot more confident in my training with Rory than I ever did with Mav, which is really cool, and the single best thing I started doing this time around was Rory Appreciation Time.
Allocating mindful screen-free time to be silly with Rory helped me avoid the puppy blues, creates a lot of levity, diffuses tension, and helps me learn more about what she finds rewarding. It's been really cool to see.

Provider Discretion (LU in Healthcare)
(Lots of technical jargon in this one, lovelies, hope you don’t mind)
Something wasn’t right.
The patient herself was… okay. Mostly. She had called 911 because she’d had back pain that had just been getting worse, and she’d said she couldn’t even get around anymore.
Mo and Hyrule often exchanged a somewhat exasperated look when someone called an emergency line for something that had been an ongoing problem, but today this… was different. She just didn’t look well.
“I’m really sorry,” the patient apologized for the fourth time as Hyrule and Mo loaded the stretcher into the ambulance.
“It’s okay,” Hyrule quickly reassured her. His heart ached a little at how much this woman wanted to seem to shrink into oblivion. Even Mo, who, despite his big heart, often came across a little standoffish, had tried to make her smile multiple times. “This is what we’re here for.”
With a chief complaint of back pain, there wasn’t much to do outside of check vital signs. Mo could easily take this call. But Hyrule just… this felt wrong.
“Let’s get a 12-lead,” he said, already grabbing the cables for it while Mo got vitals. His partner didn’t argue, helping him place the leads in the right positions.
Vitals looked mostly fine. The patient’s blood pressure was high. She said she had a history of hypertension, so perhaps between that and her pain that would explain it. Though 180/98 did not make Hyrule particularly happy. But he couldn’t treat that.
The 12-lead showed normal sinus rhythm. Nothing wrong there. But something just didn’t feel right.
Grabbing the blood pressure cuff, Hyrule checked it again, but on the opposite side.
There was a discrepancy.
Hyrule and Mo looked at each other, eyebrows pinching. Mo took a manual on the left. Hyrule took a manual on the right.
They still didn’t match.
Feeling dread fill him, Hyrule told Mo, “I’m taking this call. Let’s get going. We don’t need lights but… just drive expediently, ok?”
The transport was blessedly uneventful. But the discrepancy remained. Her blood pressure was high, but higher on one side than the other. Coupling that with back pain…
Her aorta. Hyrule was worried about her aorta. The biggest artery in this woman’s entire body could getting ready to tear apart.
When Hyrule texted Warriors later, he got his answer.
Dissection. They rushed her to the OR. You pointing out the BP difference really tipped off the doc. Good catch.
Mo whistled. “Good thing she didn’t rupture in our truck.”
Hyrule blew out a breath. He was just thankful he trusted his gut.

The dispatch information had been for diabetic emergency. Fire had gotten there first, which Aurora was thankful for since she and Dawn were coming from the hospital and therefore farther away than if they’d responded from the station.
When they arrived, the house was a nightmare. The street was so narrow that the ambulance and fire truck blocked the road entirely, the stairs were so narrow Aurora felt like she had to squeeze her arms in just to climb up them, and the turns were so sharp she wasn’t sure how any kind of equipment could get up there. The patient was lying on his bed, altered, and unable to move.
According to the patient’s friend, he’d heard him fall and came up to check on him. He knew he was a diabetic and figured his blood glucose had to be low. Fire had already checked it, saying it was over two hundred. As the firefighter paramedic gave information to Aurora, he said, “He could be acting like this because of his sugar. Could be a stroke. We’re not sure.”
Honestly, Aurora couldn’t see the patient all that well from her vantage point. Dawn had already walked in and started assessing, they’d handed a reeves stretcher to the firemen, and they were working on loading him on to it. The girl went downstairs to prep the stretcher for their arrival. Once they managed to get the patient into the ambulance, Aurora stared.
This man’s entire right side of his face was noticeably drooping. He was moving his head a little to the left, eyes somewhat moving, pupils equal. Aurora quickly asked him to look at her, to follow her finger. While he could stare at her, he couldn’t track at all, and his eyes wouldn’t move to the right. He blinked once while attempting, and was only able to blink his left eye.
Who the hell thought this could be his sugar??
Once Dawn got in the truck, they were quick to get vitals and a 12-lead. He was hypertensive, all other vitals fine.
“We need to stroke alert this,” Aurora immediately said.
“But he was last seen normal three hours ago,” Dawn said uncertainly. “Isn’t that outside the window? Or is the window four hours now?”
“I think it’s four,” Aurora answered. “And it doesn’t matter either way. This is absolutely a neuro issue. Drive us hot, okay?”
Dawn nodded, heading to the front. She drove to the hospital with the lights and sirens on, allowing them a faster transport time, while Aurora called it in to the hospital. As they progressed, she tried to get the patient to follow commands, but he couldn’t. He held up his right arm but couldn’t hold his left up at all, and he still didn’t really track any movement.
When they arrived at the hospital, they were placed in a major room, transferring him quickly to the hospital bed. Warriors was charge that night, working on coordinating all the help they’d need for this patient. The ED physician entered, looking the patient over, and then turned to Aurora, asking, “So what makes you think he’s having a stroke?”
Aurora stopped in mid motion, looking at him with the most enraged and bewildered expression. “I’m sorry, what?”
“Why do you think he’s having a stroke?” The doctor repeated.
“What makes you think he isn’t?!” Aurora snapped, completely mind blown that this was even a debate. “His face is drooping so low it’s hitting the earth’s fucking crust, he’s altered, not tracking movement, blinking with one eye, down on one side, is hypertensive, and you’re asking why I—do you even know what a stroke is??”
“Let’s just alert it,” Warriors said calmly as he walked into the room, clearly sensing that the paramedic was about to explode. “It’ll get us a CT to rule it out.”
Aurora was fuming, and she stormed out before she could hear a response. Dawn tried to gently check on her, only to be subjected to her ranting for the next hour.
Later, Warriors texted Hyrule, who relayed the message. “You were right.”
“OF FUCKING COURSE I WAS!”

Legend wasn’t particularly a fan of working triage.
There were aspects of it that were exciting - he was the one to make first contact with patients who didn’t come in via ambulance, and he determined their acuity. But there was also a public relations aspect to it, a patience dealing with impatient people, a kindness and sympathy for those who were genuinely hurting or needing help but had to wait anyway. It was understandable, but public relations… was not Legend’s forte.
There was a reason he was put in triage, though.
It wasn’t always obvious, what was wrong with someone. But there were times when a patient just didn’t look right. Legend saw the man limp over, listened to him as he explained that he had some leg pain that had been going on for the last few days, how he thought maybe he’d strained a muscle but the pain hadn’t improved.
There were always signs to look out for. Little things, cues that something was off. The man looked resigned, reluctant; he clearly had been talked in to coming to the hospital, and he commented that his wife insisted on it. Legend saw the clothes he wore, heard the accent he spoke with, saw his muscles, and pieced together that he was probably a farmer.
Farmers never came to the hospital.
“We’ll get you back as soon as we can,” he finally said after completing his assessment. Usually, this patient would be low on the acuity scale—a muscle spasm or strain was not nearly as important as a heart attack, pneumonia, sepsis, strokes, traumas—but Legend made him a yellow rather than a green. Just to be sure.
That higher acuity score got him a room far faster. That faster room made a doctor assess him and notice that his left leg was bigger than his right. That doctor made sure he got an ultrasound of his leg, found clots in his leg. She also learned the man was short of breath sometimes, which his wife insisted was new, and got a CT scan.
Legend glanced at his chart later to see him being admitted. Confused and curious, he did some digging.
The man had a pulmonary embolism.
Huffing with a small smile of satisfaction, Legend closed out of the chart as another patient approached.

Time had to admit, he did not spend as much time assessing his patients as he should. His hours were stolen away in the OR, unpredictable and chaotic as his line of work was. So sometimes he didn’t get to round, sometimes he didn’t have a chance to walk in and chat with the patients and the nurses and the licensed independent providers who took charge of their care.
Today he was glad he did.
The patient was actually calm and pleasant, had little complaint of anything except for some lower back pain. In the world of uncomfortable hospital beds, it wasn’t a huge surprise.
But Time saw something. Some staining, bruising, around the patient’s groin. He peeked around their gown, turned them a little, and saw it.
Their groin was purple. He asked the nurse, who said they were told this had been baseline for a day or two, and that the independent providers over them had acknowledged the finding and moved on.
Time walked into the doc box where the providers were. “I want a CT abdomen for room 3. She’s got some bruising that’s concerning. Her H&H has been down trending steadily.”
“Her JP drains haven’t put out much,” the physician assistant noted, looking over the patient’s chart.
“She might have a retroperitoneal bleed,” Time pointed out. “Let’s just be sure.”
Years of education and even more years of experience had taught the trauma surgeon well. The war was especially humbling and educational. So when he got a text from the PA that the patient did indeed have a retroperitoneal, he wasn’t surprised. But he was disappointed that he had to be the one to notice it.
Sometimes, he supposed, it took the leader to point out the problem.

Four… didn’t like this.
Report had been bad enough. The day shift nurse spoke of how the patient had been previously septic and was recuperating well before her pressor demand had gone up during the day. She looked… not great. She was so edematous they were constantly changing the sheets underneath her arms because her body was leaking fluid from every inch of itself - they had dumped fluids into her over the last few days just to maintain her blood pressure. She was on a lasix drip to get her lot o pee off the fluid as best as possible, and her kidney function was… decent, but not great.
As Four assessed her, the clenching his chest only worsened. She was alert, oriented, a little miserable but trying to be in good spirits, bless her. She was peeing a decent amount, her pulses were present despite the swelling, her lung sounds were a little coarse but overall mostly clear. Her abdomen was soft and non-tender, her pupils were equal and reactive, and she didn’t have much complaint of pain aside from being sick of laying in bed, which Four could understand.
But still. This just… didn’t look great.
As the night progressed, the woman’s pressor need climbed. Four continued to increase epinephrine, increase norepinephrine. He tried not to increase the vasopressin too much as it had such a profound effect on vasoconstriction that it could cause necrosis. Also, the woman had a history of heart failure and had a pretty weak heart.
Four eventually went to the resident in charge of the patient for the night. “Hey. Can we maybe give 11 some albumin? She has plenty of fluid to give, but clearly it isn’t in her vasculature - she’s peeing ok but her pressure isn’t tolerating it. I feel like it could help.”
The resident shuffled on his feet uncertainly. “The surgeon really wants to make sure we can get this fluid off. I’d rather keep her negative and not give her more fluid, you know?”
“Yeah, I get that,” Four greed before continuing, “But albumin is only 250mL, and if it helps suck in the fluid that’s third spacing, it’ll still help. We’re dumping fluid in her through the pressors anyway.”
The resident continued to waffle, before the night attending asked, “She’s on vaso, right?”
“Yes.”
“Just go up on that.”
Four stared a moment longer, starting to doubt himself. He hadn’t been a nurse for long, and if an attending physician was saying this, then… it had to be true, right?
Sighing, he went back to the room and did as he was told. The patient’s blood pressure improved well enough, and the night progressed fine.
The next night was not as fine. At rounds, Four suggested that perhaps she should be lined for CRRT, a continuous dialysis that would allow for Four to control how much fluid they were pulling and would likely be better for the patient to tolerate. The night doctors shrugged, saying they’d mention it to the day team.
Again, the woman’s blood pressure was tanking. Again, Four had to increase pressors. Vaso had been turned down and was told to be left alone because the woman’s systemic vascular resistance was so high the attending was worried about her heart. (Four couldn’t help but feel a little bitter about it, because he knew that was going to happen)
This time, though, she went into atrial fibrillation as well. As Four called the resident and attending into the room, they deliberated the matter, muttering, “Maybe we should line her for CRRT.”
Four blinked. Stared. Was he… losing his mind?? Was he invisible? He’d suggested this earlier!
Ultimately, Four had managed to keep the patient stable enough so that it wasn’t needed. Ultimately, the shift ended uneventfully.
But when Four came back for his third night, he could hear the woman’s breathing from the door, he could hear how she was drowning in fluid because she couldn’t tolerate losing fluid but had too much for her lungs and heart to handle. The day team had lined her for CRRT, but her pressors were almost maxed out at their dosage, and she was so hypotensive that the renal nurse who had set up the machine was hesitant to start it up, saying it would further bottom out her pressure.
Tonight was different, though. Tonight, the provider in charge of making decisions and orders was a nurse practitioner, someone who was used to this unit. She walked in, saw the issues Four had seen, and she walked right back out, making a call.
Four struggled to keep the patient alive long enough for the ECMO team to arrive as the patient fell apart. He felt frustration boil his blood as he had to hand off her care after fighting for her, had to watch as the CV ICU nurse came in to take over while surgeons put large cannulas into the patient’s body to redirect blood flow around her heart so she could still perfuse her organs. He watched as they wheeled her out of the trauma ICU to go to the cardiac ICU where she would remain while on such extreme support, and he threw his pen on the desk, burying his face in his hands, fuming.
They should have listened to him.
