The Department of Defense’s health insurance plan, Tricare, must cover gender-affirming surgery for transgender dependents of active and retired service members, a federal judge has ruled. The exclusion of coverage for this care violates the equal protection guarantees of the U.S. Constitution, U.S. District Judge Nancy Torresen ruled November 1. The suit was brought by two transgender women in their 20s, identified by the pseudonyms Jane Doe and Susan Roe, who are both daughters of retired military members and are covered by Tricare. Doe and her father filed suit in 2022 in U.S. District Court in Maine, and Roe was added to the suit last year. Doe lives in Maine and Roe in Florida. Both of the women’s fathers served more than two decades in the military.
Defense Secretary Lloyd Austin, the Defense Department, the department’s health agency, and Tricare were named as defendants. The women challenged a 1976 federal law that excluded coverage for “surgeries ‘justified solely on psychiatric needs including, ... sex gender changes’ and other services deemed ‘not medically necessary,’” Torresen wrote in her ruling. “Congress considered all ‘cosmetic’ procedures (including ‘sex gender changes’) to be ‘nonmedical’ in the sense that they were ‘psychologically’ or ‘psychiatrically’ justified, and not strictly medically necessary,” she continued. But Torresen found that such surgery is indeed medically necessary and that the Defense Department had not shown that any important governmental interest was advanced by denying the coverage.
Good news: Judge Nancy Torresen issued a ruling on November 1st in Doe v. Austin that Tricare is required to cover gender-confirmation surgery for transgender dependents of active and retired service members.

I just wanted to show my appreciation for Taylor Swift. I’ve never talked about this publicly and I would really like to make an impact and show my appreciation. Although she may not see this, thank you. For backstory, I was diagnosed with brain cancer in 2022. Because it had been there for so long, doctors were unable to remove the complete tumor for my brain because it wasn’t safe.
Obviously, this changed my life. I was only 13 and scared. I didn’t understand whether I would live or not. Ever since then, I have appointments almost weekly due to the cancer and other health problems.
I found Taylor Swift’s discography around late 2022/early 2023. I deeply connected to her words and her experiences, which helped me get through my own. My favorite song, seven, helped me process my childhood, having me realize that my parents not being around still affects me to this day. I also deeply connected to the song Ronan. He had a very similar connection to what I had, and I was so so happy that Taylor had made a song about him and had so much sympathy for his family.
Taylor Swift has been there through the hard, rough parts of my life, and her words make me feel as though she is singing about my life. I’m not sure I would’ve gotten out of 2023 without her, after going through a whole separate mental health journey and connecting with her even more deeply.
Thank you Taylor Swift for the music you make. Millions of people worldwide can connect so deeply and it truly does improve everyone’s life. I am so grateful we have amazing women like you in the industry. We love you ❤️
If there are any other people who can relate in any way to this, I would love to hear and know about it. I hope to make updates to my health here so everybody has a chance and a community to speak to about problems like this.

The Florida Health Department released guidance “strongly advising against the use of mRNA COVID-19 vaccines” and urging doctors to avoid administering them. The state justified this recommendation by citing a lack of “booster specific…randomized controlled trials.” This stance sharply contrasts with the guidance from federal agencies and health officials. Notably, the state's rationale mirrors the approach it has taken in its anti-transgender attacks on gender-affirming healthcare—a tactic commonly employed by Florida agencies and many anti-transgender activists. “The most recent booster approval was granted in the absence of booster-specific clinical trial data performed in humans…Although randomized clinical trials are normally used to approve therapeutics, the federal government has not required COVID-19 vaccine manufacturers to demonstrate their boosters prevent hospitalizations or death from COVID-19 illness,” reads the press release from the Florida Health Department.
This is not the first time Florida has used the absence of randomized controlled trials (RCTs) to justify bans on best-practice medical care—a tactic familiar to the state’s transgender community. During debates by the DeSantis-appointed Florida Board of Medicine over whether to ban gender-affirming care for transgender youth and many adults—a decision they ultimately made in favor of—the Florida Agency for Healthcare Administration issued guidance against transgender healthcare, stating, “There are no randomized control trials on the effectiveness of ‘gender affirming’ treatment.” Randomized controlled trials are often difficult to perform, take time, and may not be practical. Over 90% of all medical care does not have “high-quality” GRADE scores, which often require randomized controlled trials to receive. This does not mean that the treatments are deficient or harmful, rather, they are driven by other forms of evidence, such as observational studies, which are easier to perform, and individualized patient responses to treatment.

A new program in Victoria will pair police officers with registered nurses to handle mental health-related calls.
Officially launched on Monday after a two-week pilot, the Co-Response Team (CRT) will respond to calls in Victoria and Esquimalt from 8 a.m. to 8 p.m., seven days a week.
The CRT consists of two teams, each with a specially trained police officer and a nurse experienced in psychiatry and mental health care.
"We're addressing a gap ... this is what our communities have been asking for, for quite a long time," said Victoria police Chief Del Manak at Monday's announcement.
Both 911 and non-emergency call takers will assess whether the situation warrants a police-only or a CRT response, Manak said. [...]
"The co-response team will resolve mental health situations collaboratively and in the least intrusive way," said McKelvey.
Rob Schuckel, the co-ordinator with Island Health's Mental Health and Substance Use Services, said these calls come from various locations, including shelters on the streets and in people's homes.
