every service crucial to the functioning of society, namely government, healthcare, banking, etc, needs to at all times consider that people who are very stupid need to be able to navigate and use these services with minimal struggle, and people who don't have easy access to the internet or a smartphone also need to be able to navigate and use these services with minimal struggle. unfortunately many of the people who design these systems and services are in possession of a higher education a smartphone with consistent high speed internet connection and a critical lack of awareness about how universal these experiences are
#anyway i love cuchulainn#got a questionnaire from a healthcare provider asking about their digital services that did not consider it might suck

theres also at least in the canadian system the added complexity of like. we have a single payer system (with a lot of gaps in coverage). all hospitals are public . specialists who can only be hired if the hospital has capacity are essentially self-employed rather than being government employees. they have to set up corporations under their own name and then bill their provincial government each time they provide care to a patient. if we let the workers pretend they own the means of production at any meaningful capacity they will fight back any time theres a proletarian movement that is fundamentally in their interest.
#in the states the same physicians are on contracts from healthcare groups#like company owns a series of hospitals and they hire docs#and the companies may have academic affiliations but you're essentially employed by a private firm#in canada the provincial govts own the hospitals and they decide how many docs can be hired#but once hired the doc sets up their own private firm under their own name#and charges the govt an amount (set by the govt) per service provided to patient#and theres this reciprocal relationship of like. the docs wanting to maximize earnings and the govt wanting to minimize spending#so they dont expand hospital capacities etc to be able to hire more docs#and then tell the docs if their hours are reduced to normal hours they wont make as much money tm#so most docs past a certain point of seniority absolutely hate the idea of medical trainees and residents and more junior attendings#asking for the right to see their family at dinnertimes and such#my posts

Aetna intentionally denied multiple pre-authorization requests for my upcoming surgery with the SOLITARY purpose of delaying it until it was too late to schedule a surgery date in 2024. I was scheduled to have the 3rd of 3 surgeries on 27 NOV 2024. My remaining Out of Pocket MAX for the 2024 year is down to $509, and Aetna wanted to be able to put more of the financial burden (Deductible and reset out of pocket ax) back onto me. My doctor submitted pre-authorization requests on 30 OCT 24, 08 NOV 24, and 12 NOV 24. Each time, Aetna came up with a new and unethical reason to deny or delay their approval. On two separate occasions, AETNA cancelled the requests internally, and outright lied as they tried to claim that my doctor's office had withdrawn the requests. Only when pressed in a three way call with Aetna and my Doctor's Scheduler on the phone did they admit that teh cancelled requests were their doing. Then they asked for a new request submission, claiming that it was not a "Denial of Service." Next, they denied service stating that my doctor and surgical center were "Out of Network." not only are both "IN NETWORK," both are considered Tier 1 Preferred by Aetna, as stated on Aetna's own website provider listing. After more than a month of phone calls, clarification requests, cancelations, re-submitted requests, transfers to various Aetna Departments, my request was finally approved on 06 dEC 2024... AFTER my originally scheduled date had past, and ONLY AFTER Aetna fully realized that it would be too late to schedule a surgery date by 31 DEC2024, at which time all deductibles and fees due from me reset all over again.
At no time were any of these delays out of a concern for my health, wellbeing, or safety. Aetna had one goal at purpose... delay, deny, depose for the purpose of saving Aetna money and costing me thousands $,$$$. I'll not even go into all of the details about denial of service for procedures already performed... like authorizing cervical fusion surgery and paying for the fusion plates, but denying payment for the screws used to fasten the plates in place...

Call Your Insurance - benefit questions
I get it, phone calls are scary! Here's some scenarios to show you what to expect when you call your insurance company. Don't be afraid to ask follow up questions - the only 'stupid question' is one you don't ask!
[beginning of call] Insurance rep: Hi, thanks for calling your insurance company, my name is X. How can I help you today? You: Hi, I'm calling to ask about my benefits. Rep: Okay, can I get your member ID number? [you provide] and can you verify your name and date of birth for me? [you verify. they may also ask for your phone number at this point.] Rep: Alright, I've got you pulled up here. What questions do you have today?
[example one] You: I think I need to see a specialist and wanted to know what my benefits are for lab work. Rep: Okay, give me a minute to get that information for you. [answer will obviously vary! they may ask clarifying questions to help find an accurate answer, such as diagnosis/issue you are seeking help for. they may also have to provide a disclaimer that they cannot guarantee benefits quoted] It looks like you have a $45 copay for specialist office visits, and that can include some basic lab work if it is billed on the same claim. If they send it to a separate lab, then your In Network benefit is 80% after deductible. You: Can you check to see if the specialist is In Network? And do I need a referral? Rep: Sure, what's the doctor's name? [they may tell you to verify with the provider if they are In Network, as not all insurance reps will have access to 100% confirm a provider's network contract status, unfortunately.]
[example two] You: I'm going to need surgery. What are my benefits? Rep: I can look that up for you. Where will you be having the surgery? [surgery benefits can depend on whether you are inpatient, outpatient, or in-office] You: At the hospital, I might have to be there a few days. Rep: Okay, inpatient services require precertification. Your doctor will need to start that process by submitting a precert request to our fax number or through our website. The benefit for hospital services is 80% after deductible. It looks like you've already met your deductible, so that means you'll be paying 20% until you hit your out of pocket max, which is $4000 for In Network services. You: Does that include my deductible?
[end of call] Rep: Is there anything else I can help you with today? You: No that's all. Can I get a reference number for the call? [feel free to ask for their name again if you didn't catch it or forgot! some systems don't have reference numbers and just use the rep's name and the date, so its always good to keep notes of when you called, who you spoke with, and what they told you!]
#marie#op#advice#scripting#phone calls#social anxiety#insurance#healthcare#benefits#also btw re: disclaimers#it'll be something like 'this is a general description of benefits and not a guarantee of payment'#its for situations where maybe they quoted INN benefits but you go to an OON doctor.#or other cases when what you talked about on the phone isn't what ended up going down.#as someone who had to give that disclaimer a million times#i hated being like 'yeah trust me you're covered' but also 'probably idk who could say for sure not me your insurance rep apparently'#misquotes do happen#but most of the time you'll get accurate info if you provide clear details about what services you need.#if your doctor can give you the procedure and diagnosis codes that can help a ton when you call your insurance

why in the fresh FUCK are so many medical clinics in idaho operating on a monthly subscription basis. this is life-saving healthcare not fucking hulu
#trying to find resources for a client and have now come across THREE clinics in the area that are on a monthly subscription basis#it's actually so insidious because how it works is this:#you pay a monthly subscription fee (which CANNOT be paid by insurance wtf)#and that gets you access to better services than public health would have like same-day appointment scheduling and more qualified providers#and shit like that#i fucking hate capitalism more and more every day#the monthly subscription fee is on top of any copays and insurance costs too#so basically you pay money out of pocket each month for better healthcare#because idaho's basic healthcare is shit#capitalism is gross#beef speaks#healthcare#capitalism#anti capitalism#eat the rich#fuck capitalism

decriminalize:
sex work
addiction
criminalize:
golf
#decriminalize addiction PLEASE! theres like a billion studies on this and as it turns out criminalizing addiction does not get rid of it#the same thing with sex work. both of these will still invariably be around if the solution to them is just arrest people who engage with i#the decriminalization of addiction and sw must necessarily be accompanied with the expansion of universal/public healthcare and other#social welfare programs (including UBI)#i strongly believe that the roots of nearly all problems in the united states is the general absence or underfunding#of welfare programs; as well as the influences of corporations and dark money in government and legislation; both of these removed#absolutely will improve the usa in every way#the us needs to become a social democracy!#and dont even worry about the costs; the usa has enough money to fund these huge projects. especially if the government pulls out#funding from other areas such as from police departments and the military; and furthermore these projects can potenially pay for themselves#by uplifting millions of people who (now no longer severely impoverished; or homeless; or without adequate healthcare; etc.) can now provid#for society which pays for these projects and creates a virtuous cycle! government services can help everyone and everyone can help service#and the golf stuff needs to go! golf is a blight on this green earth; as are lawns (which golf-fields are a type of)#and (continuous) monocultural farms; all have terrible environmental effects#lawns use SOOOO much water just for maintenance; this water could be used for other things like DRINKING (which is#necessary to live)#and lawns are huge areas consisting of just one species of plant; while other species of plants are intentionally removed#from these fields for maintenance; and as it turns out! large patches of uninterrupted stretches of one single species of grass really hurt#the environment by literally taking space for other plants away and reducing biodiversity; these fields of only one plant act as deserts#lawns dont even have a real purpose! they provide nothing for the rest of society! at least monoculture farms give us food!#lawns just sit there looking ugly as hell and stealing our water and killing our environment for literally no reason!!!!!!!!#i unironically propose a georgist solution to golf and lawn: a progressive tax on the size (by square area) of fields; and criminalize#fields exceeding a certain threshold of size; and lifting certain regulations on the maintenance of personally owned lawns#or like just kill golf and lawns entirely#ok#rant#reblog

I’m seeing a bunch of posts that make me think most USAmericans don’t know about The No Surprises Act.
It was passed in 2021 (thank you Biden) and essentially states that if you don’t have insurance or your insurance doesn’t cover a service you need (or want) you are entitled to a Good Faith Estimate of the cost of care. (If your insurance does cover the service, you should be able to estimate the cost of care based on your deductible and co-pay.)
As a healthcare provider who does not accept any insurance, I am very careful to not violate The No Surprises Act. Why? Because for every penny more than $400 that the Good Faith Estimate was “off” (or if it wasn’t provided), you are entitled to a refund for that amount.
Y’all. Ask for a Good Faith Estimate. Get it in writing. Compare it to what you are paying. If you are not provided an estimate or if it’s wrong by more than $400, demand a refund.
