COVID-2019: Status of Favipiravir in the world

 ❶ China says: “Antiviral favipiravir effective against COVID-19”

 Randomized trials of the broad-spectrum antiviral favipiravir, marketed as an anti-influenza treatment, have shown "obvious efficacy" against COVID-19, according to Zhang Xinmin, head of the China National Center for Biotechnology Development. Zhang's comments were reported by Xinhua News Agency, China's official state-run press agency.

 In one trial, testing favipiravir with interferon in Shenzhen, results showed that patients treated with the combination had significantly reduced the duration of symptoms, as measured by viral load chest imaging, vs. a control group. In another study, clinical recovery rates were higher for COVID-19 patients treated with favipiravir vs. those in a control group.

 ❷ Russia says: “Favipiravir(Avifavir) has been approved to treat COVID-2019”

 Russia has approved an anti-influenza drug, Favipiravir, to treat Covid-19 and will start delivering it to hospitals this month, according to Russia’s sovereign wealth fund.

 Preliminary trials appeared to show that it could shorten recovery times for patients with Covid-19. The final stage of Favipiravir clinical trials involving 330 patients are ongoing, RDIF and ChemRar said Monday, but patients are ongoing, RDIF and ChemRar said Monday, but Russia’s Ministry of Health on Saturday already temporarily approved the use of the drug as a coronavirus treatment.

 Glenmark Pharmaceuticals has gained an approval from India’s drug regulator for the antiviral drug favipiravir for the treatment of mild-to-moderate COVID-19 patients.Glenmark is now set to manufacture and market its favipiravir product under the brand name FabiFlu in India, and is the first oral favipiravir-approved medication in the country for the treatment of COVID-19.

 The approval is based on clinical evidence of favipiravir demonstrating promising results in patients with mild-to-moderate COVID-19 disease. According to Glenmark, the drug offers rapid reduction in viral load with four days and provides faster symptomatic and radiological improvement. In addition, favipiravir demonstrated clinical improvement of up to 88% among those with a mild-to-moderate case of COVID-19.“FabiFlu has demonstrated an encouraging response in mild-to-moderate COVID-19 patients during clinical trials. Moreover, it is orally administered, and so it serves as a more convenient treatment option over other intravenously administered medications,” said Saldanha.

 Glenmark has also announced its intention to conduct another clinical trial to evaluate the efficacy of favipiravir and another antiviral drug, umifenovir, as a combination therapy in moderate, hospitalised adult COVID-19 patients in India.

 ❹ Attitudes from more countries on against COVID-2019 by Favipiravir

 The US Department of Defense developed favipiravir in partnership with MediVector, Inc. as a broad-spectrum antiviral and sponsored it through FDA Phase II and Phase III clinical trials, where it demonstrated safety in humans and efficacy against the influenza virus.Despite demonstrating safety in more than 2,000 patients and showing accelerated clearance of influenza virus by 6 to 14 hours in the Phase III trials, favipiravir remains unapproved in the UK and the USA.

 The Italian Pharmaceuticals Agency AIFA has approved testing antiviral drug favipiravir for the coronavirus, Health Minister Roberto Speranza said on Monday. The technical-scientific committee is developing a testing and research programme to assess the impact of the drug in the initial phases of the disease after an initial analysis of the available data on favipiravir.

Italy has had approximately the same number of cases three days in a row. This means that their strict lockdown/quarantine procedures are working! GOOD NEWS EVERYONE!

China is endorsing the use of favipiravir as an experimental treatment for COVID-19. It has been used to treat influenza before and is showing some promise in COVID patients, by decreasing the duration of the illness and the severity of their lung disease. Watch this space for more!

Virology Corner

Today’s question comes from @greenfairyarmadillo​ and @heturnedleft​. Both asked a variation of the same question: Do we know yet if people who recover from COVID-19 are immune to reinfection or not? Do we expect COVID-19 to be like measles, like flu, or somewhere in between?

We don’t know yet! We know that people have protective antibodies, but my guess based on what I know about other coronaviridae is that the protection will wane pretty quickly, which is more like the flu. But we’ll see! Watch this space for data from China, where we’ll see hard data first.

The other thing to consider is the way that coronaviruses can attenuate, or get less strong, as they spread through populations. So it may be mitigated in the future less by herd immunity and more by attenuation of the SARS-CoV-2 strain itself. But I’m not sure! If I could predict the future I’d be super popular right now. Like, super popular.

As you all know, the whole world is working round the clock to create a vaccine to cure Covid-19.  With some countries just starting to get approval for clinical trials, while others are re-purposing existing drugs after discovering their properties may end up eliminating the virus in at least %70 - %90 of their patients. USA for example re-purposing an anti-malaria drug. However the most prominent that's been talked about is the drug Avigan. Avigan is the brand name of the drug Favipiravir, designed by Fujifilm (Yes that Fujifilm).  It was originally designed as an influenza vaccine and was also used as a potential cure for the Ebola Virus.  However upon testing it in China, Japan and Italy, the scientists have yielded great results from it.  I believe that if it can be approved worldwide, that it could hold the key to saving our world.  I believe it to the point where I have written up a petition, in an effort to let the world know how many people want....no need this vaccine to be approved worldwide. A means of spreading the word as far as I can. For months we've been living in fear of this thing and this is our chance to fight back. We May have a Chance.

Italy's Hospitals struggle through Coronavirus outbreak

The pandemic outbreak put pressure on the Italian healthcare system. On 17 March Undersecretary at the Ministry of Health, Sandra Zampa announced the purchase of many new ventilators as well as the importing of one and a half million masks from South Africa. In order to deal with the numbers of COVID-19 patients, intensive care units were expanded, and new hospitals were created, especially in Lombardy. In Emilia-Romagna, professor Marco Ranieri developed a method to double the efficiency of ventilators in ICUs. The lack of a single protocol for hospitals was considered to be a problem. Due to hospitals overcrowding with coronavirus patients, thousands of cancer patients experienced difficulties in getting access to treatment and transplants, with their lives put at even higher risk. Dozens of cancer hospital sections were indeed wither dedicated to host coronavirus wards, or closed after personnel got infected. According to a study, cancer patients represented 17% of coronavirus fatalities in Italy.

 A letter published on the New England Journal of Medicine Catalyst Innovations in Care Delivery claimed that one of the reasons for the high death count in the area of Bergamo was hospital contamination. Progressively, different hospitals became dedicated to COVID-19 patients only, and more rigid separations were set up between hospital sections and triage structures. In some regions, hotels were used to host healthcare workers or patients, and in Liguria a ship was adapted to host people in quarantine. On 1 April, the first Italian drive-through testing facilities opened in Alessandria and in north-western Tuscany.

At least ten different clinical trials were ongoing in Italian hospitals at the beginning of April.  The supercomputer of ENEA in Portici was used to run advanced simulations related to other possible drugs.  Some of the treatments employed for COVID-19 patients involved the administration of antiviral drugs. Remdesivir was tested with promising initial results in Naples. Despite doubts from the scientific community, Avigan (favipiravir) was also included in testing protocols by Italian Medicines Agency even though it was not authorised in Europe. Similarly to France, hydroxychloroquine (the less toxic version of the malaria drug chloroquine) was also tested : in Lazio, its use was reported by the second week of April on a significant fraction of the roughly a thousand COVID-19 patients confined at home. Plasma treatment already tried in China (transfer of antibodies from recovered COVID-19 patients to sick people) was tested at the hospital of Pavia (and later in Mantua) under the supervision of professor Cesare Perotti; the first two donors involved in the research programme were a married couple, both doctors and among the first COVID-19 patients reported in the province.

Covid-19 : Life and death in Italy's intensive care units

On 31 January, the first two cases of COVID-19 were confirmed in Rome. A Chinese couple, originally from Wuhan, who had arrived in Italy on 23 January via Milan Malpensa Airport, travelled from the airport to Verona, then to Parma, arriving in Rome on 28 January. The next afternoon, they developed a cough, and by evening the man had a fever; the couple were taken to the Lazzaro Spallanzani National Institute for Infectious Diseases where they tested positive for SARS-CoV-2 and were hospitalised. On 2 February, a team from the same institute composed of scientists Maria Rosaria Capobianchi, Francesca Colavita, and Concetta Castilletti isolated the genomic sequence of the virus and uploaded it to GenBank.��On 31 January, the Italian government suspended all flights to and from China and declared a state of emergency with the duration of six months. Prime Minister Giuseppe Conte said Italy was the first EU country to take this kind of precautionary measure. The government also introduced thermal scanners and temperature checks on international passengers arriving at Italian airports.

It is easy to feel down and overwhelmed with the coronavirus pandemic continuing to rage on – especially with Brits stuck on lockdown despite the upcoming warmer weather.

But countries following official guidelines, including staying at home and social distancing, are seeing positive results with the virus slowing.

China, where Covid-19 originated, has reported continual drops in new cases while Singapore has also managed to contain the virus, reports CNET.

The likes of South Korea and Germany have seen spreads slow due to rigorous testing programmes.

Hong Kong and Taiwan have also demonstrated the importance of investing funds in thorough outbreak prevention measures, having learned valuable lessons during the SARS pandemic in 2002.

Clinical trials of the potential vaccines are underway in China and the first US clinical trials have now got underway in Seattle, which proved an early hot spot in February.

Biotech firm Moderna has used a piece of genetic code present in other coronaviruses and combined it with fatty nanoparticles as part of a possible vaccine.

Similarly, Imperial College London is working on another solution using virus RNA genetic code, and Pennsylvania biotech firm Inovio is making DNA strands with the goal of stimulating an immune response.

Pharmaceutical giant Sanofi, meanwhile, is looking at combining Covid-19 DNA with harmless virus genetic material as part of an effort to produce a vaccine alongside US Biomedical Advanced Research and Development Authority.

Similarly, Johnson & Johnson has also teamed up with the authority in an effort to deactivate SARS-Cov-2 as part of its own work to find a vaccine.

But with a vaccine likely to be months or even years away, pre-existing antiviral drugs – including remdesivir and favipiravir – are believed to potentially be effective in treating Covid-19.

This comes as engineers from Mercedes’ F1 team are working with University College London to create a breathing device so patients don’t have to go to ICU.

And despite the death tolls, each day people are recovering from the virus – including many of the most vulnerable patients.

They include 104-year-old Ada Zanusso who caught the killer bug at her care home in Biella, northern Italy.

Twenty of her fellow residents have died from the virus, though Ada – who also survived the Spanish flu against the odds as a child – has “lost none of her lucidity and intelligence” say doctors.

It is understood she is the oldest survivor in the world.

And while it’s not exactly clear what treatment Ada received prior to recovering, doctors in India have found success using a cocktail of drugs designed to fight HIV, swine flu and malaria.

Doctors in China and Japan, meanwhile, have reported encouraging results from treating patients with blood plasma from those who have already recovered.

More efficient testing methods are also being developed, including at John Hopkins University where researchers are working on a nasal swab test which could analyse hundreds of people each day and bring back results in a matter of hours.

CoVID-19 and Drugs Used

At the moment there is no medication that is specific for treating COVID-19 but yes, there are talks of drugs that have been repurpose for treating SARS-CoV2, the virus that causes COVID-19. Which are those drugs and how they work? Do they promise any hope for stopping this virus?

WHO has started a trial program with the purpose to analyze different medications and their effect on the virus. The medications that made the cut are:

Remdesivir - the medication was mainly created to treat Ebola, but it has been noticed in labs that it can treat SARS and MERS, two coronavirus that behave similar to COVID-19. This medication works by inhibiting the enzyme RNA-Polymerase. With this enzyme out, the virus can’t replicate its RNA. It has been noticed that it’s effectiveness is during mild to moderate signs and symptoms of COVID-19.

 Chloroquine and hydroxychloroquine - an antimalarial medication that reduces acidity in an endosome. An endosome is a vesicle that our cells use as transportation. Chloroquine and hydroxychloroquine prevents COVID-19 from using this method. There are two problems to this medication: first COVID-19 uses ACE-2 receptors as a way of propagation and not endosomes. Second, the dose to make it work is 600 to 800 mg for 10 days. The safe dose is 5 mg/kg/day, it is unknown if this high dose for 10 days causes problem. The French Doctor Didier Raoult, MD, PhD claims that hydroxychloroquine plus the antibiotic azytromicin cures COVID-19 and drops the viral load in a short period of time. Hydroxychloroquine is less toxic than chloroquine. This medication is used for LUPUS, Rheumatoid arthritis and other autoimmune disorders.

Ritonavir/Lopinavir - known in USA as Kaletra, this HIV medication prevents proteins from being transformed into smaller units that later will be use for translation and replication of the virus. It has been noticed that this medication works better at the beginning stage of the virus.

 Ritonavir/Lopinavir/Interferon Beta - this combination seems more promising as long as there is no severe symptoms of inflammation. In that case, interferon could cause more damage than help. The interferon beta is produced by Cuba.

ACEI’s and Angiotensin Converting Enzyme Receptors Blockers (ARB) - Angiotensin Converting Enzyme Inhibitors are meds for high blood pressure, kidney problems, or coronary problems. This medication is not being used by WHO in the trial but it is worth mentioning. SARSCOV-2 uses ACE2 to enter the body and it was hypothesized that blocking this receptors would help stop the virus but it has been noticed that the body increases the production of ACE receptors facilitating the propagation of the virus. If you or anyone you know uses any of the medications, please do not stop taking them. Please, self-quarantine and observe all the recommendations such as using a face mask, and keeping a 1.5 meter (6 feet) distance.

Tocilizumab or Sarilumab - It is not being used in the trial by WHO but, again, it is worth mentioning. This medication is an immunosuppressant and its purpose is to reduce IL-6 (interleukin 6) and prevent a cytokine storm in some patients. Some patients’ immune system go crazy before SARSCOV-2 and can cause more harm than help, therefore the medication.

Ivermectin - is an antiparasitic medicine that seems to clear the virus within 48 hours. Studies were done in vitro. Is not on the WHO list of meds in trial.

Favipiravir - an antiviral medication with a wide spectrum of action against different virus such as influenza, and ebola. This medication inhibits the replication of a virus and lowers viral load. China is sending it to Turkey while Japan has entered in talks with USA and other countries. This medicine is not part of WHO's trial.

In summary, there is no drug available. It takes time to develop and test one. At the moment, the medical community is repurposing some drugs and reporting to WHO in order to determine a best course of action. Therefore, the best action is, isolate, wash your hands, use a face mask, and keep a distance of 6 feet (1.5 meters).

Favipiravir and COVID-19

As of recent, favipiravir was one of the experimental drugs on trial for COVID-19, a viral infection caused by the novel coronavirus, SARS-CoV-2. so far the drug has shown an 80% efficacy and its moving to the third phase of the trial against COVID-19.

The underlying mechanism of the favipiravir is the metabolization of its active form which inhibits the action of RNA-dependent RNA polymerase stopping transcription and replication.

Today’s question is: Why 6 feet of separation? How does that help? How is COVID-19 spread?

Let’s start with how COVID-19 is spread. It’s a respiratory virus, so it is spread by respiratory droplets, aka the garbage you breathe/cough/sneeze out.

A good hard sneeze won’t launch your eyeballs out, but it will launch nasty shit up to 6 feet away from you at a shocking speed. (This is why you should Dracula when you sneeze.)

6 feet is a great rule of thumb to keep people out of each other’s sneeze radius, so they can’t get in contact with each other’s respiratory droplets.

Gilead halts ‘compassionate use’ of potential coronavirus drug

Gilead has said it has stopped accepting “compassionate use” requests for its potential coronavirus antiviral remdesivir because it cannot keep pace with demand as health systems become overwhelmed.

Remdesivir is seen as the best hope in the fight against the COVID-19 coronavirus, at least until a vaccine comes along and several trials are already under way.

But the US pharma said that it is facing huge demand for compassionate use requests for the drug, where the drug is given to severely ill patients as there are no approved treatment options that are effective.

California-based Gilead said there has been an “exponential increase” in compassionate use requests as the SARS-CoV-2 virus spreads across Europe and the US.

As a result this has “flooded” an emergency treatment access system set for very limited access to investigational drugs, and was never intended for use in response to a pandemic.

In response Gilead said it is moving existing individual compassionate use requests to an “expanded access” programme.

This will ensure these requests are met with a similar 72-hour timeframe to the compassionate use requests.

But the company will not accept new compassionate use requests, except for pregnant women and children aged under 18 with confirmed COVID-19 and severe symptoms.

Latest figures from the World Health Organization show the virus is still spreading rapidly throughout Europe and the US.

The data published on Sunday show 53,578 new cases in Italy, with 6,557 new cases, 4,827 deaths, and 795 new deaths.

Spain is badly hit with nearly 25,000 cases, 1,326 deaths, and 324 new deaths. In the US there are 15,219 cases, 201 deaths, but figures show no new cases or new deaths.

In China there are more than 81,000 confirmed cases, with 82 confirmed new cases and six new deaths, with 3,267 total deaths.

Gilead’s share price was down in premarket trading following the news, falling by more than $5, almost 7%, following the announcement.

Medical authorities in China last week said that a similar drug, favipiravir, developed to treat new strains of influenza appeared to be effective in coronavirus patients.

The Guardian reported that Zhang Xinmin, an official at China’s science and technology ministry said favipiravir had produced encouraging outcomes in clinical trials in Wuhan and Shenzhen involving 340 patients.
