Steven Moffat had always toyed with the idea of introducing a superhero in Doctor Who, as he believed the typical tropes would be a fun contrast to the Doctor's world.

He had planned to make 'The Return of Doctor Mysterio' an episode of series ten, but was ultimately convinced to do it more justice by making it a Christmas special.

Moffat himself explained that the title is actually 'Doctor Who' in Mexican, which he and Peter Capaldi immediately fell in love with when they visited the country.

They were keen to use it in a title, and Moffat realised how well it suited this story.

Crackpot dw headcannon : the harmony shoal corporation (body snatching brains from husbands of river song and return of doctor Mysterio and others) is a schism of the spinward corporation's pool who wanted to ascend to pure thought without loosing their individuality in the process and at some point in their history developed time travel and set operation in the post war 21st earth century (a different 21st century than the one in Wich the butler institute layed the foundation of that technology)
