That drug is sildenafil so called viagra. It was originally created to combat pulmonary hypertension which is a serious condition caused by heart failure. The blood pumping through the lungs backs up and the pressure on the vessels increases. This can cause shortness of breath and respiratory distress. Long term it can cause pulmonary embolisms or pneumonia which can lead to death.
When this drug was created, it was a cardiac drug. But the men had the side effect of helping prevent erectile disfunction. Pulmonary hypertension does NOT pay near as well as erection medication does so it was trademarked as viagra and marked for sexual dysfunction. This led to insurance companies denying claims, knowing full well that it was created for pulmonary hypertension, because they don’t have erectile dysfunction. To this day, many male patients will have erectile dysfunction as one of their disease just to get the medication easily covered.

Drug Orders and Doses

Cool, so I think the first thing to know is how medication is ordered.
Generally speaking, it will be ordered in 5 parts, known as the "5 Rights" of medication administration:
#1 What patient is getting the medication
#2 What medication is to be given
#3 How much medication is to be given
#4 What time it is to be given (or how often)
#5 What route it is to be given
So an order might be "Give John Smith (5/13/1995) lorazepam 0.5mg IV once prior to MRI"
In this example, John Smith is the patient and 5/13/1995 is his birthday to differentiate him from all the other John Smiths. "Lorazepam" is the drug's generic name, "0.5mg" is the amount of the drug. "IV" is the route, and "once prior to MRI" is the time.
Drugs have generic and brand names. For example, acetaminophen is a generic name. Many companies make acetaminophen, and each has their own brand name for the drug. Probably the most well-known brand name for acetaminophen is Tylenol, but there are others, like Calpol and Panadol. For most people, it doesn't matter which brand of a particular drug is used, just that the active ingredient (the generic name) is the same. For some people it matters because the non-active ingredients may be different between brands, and they may be allergic to a non-active ingredient that is in one brand, but not another.
In a hospital setting, we're going to use the generic name, because the brand of the drug that is cheapest to the hospital pharmacy varies contract to contract, and there are a lot of drug shortages these days. That's why if you're in the hospital you might get an oval green pill one day and a round white one the next day. They're the same drug, just different brands.
The dose is given in milligrams, usually abbreviated "mg". Milligrams are a measure of weight. Cubic centimeter (cc), on the other hand is a measure of volume. At some point we switched from volume based to weight based measures because we had a lot of different concentrations and using volumes for everything made mistakes really common. If you're using weights, it doesn't matter if the concentration you have is 1mg/mL or 10mg/mL for a given drug, you can do the math and come up with a volume that is right instead of just hoping you picked the one the doctor was thinking about when they wrote the order.
There are many routes a drug can take into the body. There is oral (a pill or liquid), IV (injection in a vein), IM (injection in a muscle), SQ (injection into fat), rectal/PR (a suppository, gel, or liquid inserted into the rectum), SL (under the tongue), TD (a paste or patch that sends medication through the skin) and many more.
Times can be once, once every x hours, once every x hours as needed (PRN), once under a particular circumstance, daily, or pretty much any other interval you can think of. "Stat" is a term meaning "right now".
Here's a list of common medications and their dosages:
CODE DRUGS:
Epinephrine 1mg IV for cardiac arrest every 3-5 minutes, 0.3mg for anaphylaxis
Amiodarone 150-300mg IV over 10 minutes for cardiac arrest
Lidocaine 75mg for cardiac arrest initially, if that doesn't work then 37.5 10 mins later
Adenosine 6mg given very quickly for PSVT, if that doesn't work, give 12mg
Atropine 1mg every 3-5 minutes for low heart rate until heart rate is normal
OTHER DRUGS:
Albuterol 2.5mg in nebulizer for brochospasm/asthma attack
Metoprolol 5mg IV every 5 minutes up to 15mg for severe high blood pressure
Furosemide 20-80mg IV for fluid on lungs
D50 25g IV for low blood sugar
Diphenhydramine 12.5-50mg IV for allergic reaction
Morphine 2-10mg IV or IM for pain
Fentanyl 50-200mcg for sedation
Mannitol 20-150g for increased pressure inside the skull
Nitroglycerin 0.3-0.6mg every 5 minutes up to 3 times for chest pain (angina)
Naloxone 8mg nasal spray every 2-3 minutes for opioid overdose
Flumazenil 0.2mg IV for benzodiazepine poisoning, if that doesn't work give 0.3mg, if that doesn't work, give 0.5
Diazepam 15mg rectal gel for seizures that don't stop
Phenobarbital 1-1.5g IV for seizures that don't stop
Etomidate 22mg IV for anesthesia (for things like intubating someone)
Midazolam 5mg IV for sedation prior to surgery
Olanzepine 5-10mg IV for agitation (emergency sedation)
Haloperidol 0.5-10mg oral or IM for agitation (emergency sedation)

Was it unprofessional? Undeniably. Did he care? Not one bit. How could he be expected to not fall in love with you? He knew intimate details about your body that no one else did, you confided in him, and most importantly, it was his job to care for you. On top of that, the details he was privy to as your doctor made it all too easy for him to grow even closer to you. He knew where you lived, he knew that you were single, he knew that you were an only child, and that both of your parents had died young from sudden cardiac arrest.
After your first appointment, he knew the two of you had a connection. So what if he ordered some blood work and scheduled a follow-up appointment with you to go over the results even though it was only an intake appointment and there was no clinical indication it was necessary? The number of people who have vitamin deficiencies and don’t know it is quite high. Was it really that bad if your yearly physicals happened twice a year instead of just once? That didn’t seem evil to him, on the contrary actually, he was doing his duty as a healthcare professional and taking your well-being seriously.
Of course, the repeat blood tests every three months might have been a little bit excessive, especially because you weren’t actually deficient in any vitamins and had no reason to be taking a supplement, but it gave him a reason to see you. Despite the fact that normal lab results were typically discussed on the phone, he couldn’t resist having you come into his office.
Because of your family's cardiac history, the health of your heart was of great concern. Getou always made sure to perform a thorough cardiac workup at your check-ups. The way you allowed him to listen to your heart and touch the tender flesh on your chest was a delight for him. You were so trusting. Getou would expect nothing less though, he was your doctor after all. As your doctor, one of the hardest things was the uncertainty of whether you would drop dead of a heart attack the same way your parents did, or if you would have no complications at all. At first, this didn’t bother him, but as time passed he found himself obsessing over it more and more. A need for control started to well up inside of him driving him to stock your daily life.
After one physical where you complained of new-onset chest pain, Getou couldn’t take it anymore. He knew that there was only one thing he could do to ensure his control and guarantee that you wouldn’t suffer. He had really been hoping it wouldn’t have to come to this. He loved you and he wished that you could have loved him too, but it seemed as though that was not in the cards you two had been dealt.
Due to your chest pain, Getou ordered a two-week continuous heart monitor. The data collected from the monitor wouldn’t change his resolve, but he had another important use for it. After placing it and reminding you numerous times not to remove it or let it fall off before the two weeks mark, he let you go on with your day. Little did you know that you and Getou had not actually parted ways when you left his clinic.
You had a terrible habit of leaving the door unlocked. Just more proof to him that your chances of living a long life were quite slim. At the same time, it was quite beneficial for him. He loved to slip into your closet and wait for the sounds of your vibrator to fill the room followed by your moans. For the first few minutes after the injection, you felt fine. Getou was able to clean up and dispose of everything, but then the room started to spin. You laid back and your vision started to get spotty, “Doctor Getou” was all you managed to squeak out before everything went black.
So sweet and trusting. Suguru looked at your unconscious body in the rearview mirror on his windshield, you looked so innocent laid across the backseat of his car. In a different life, he could have had the future he dreamed of with you. As your doctor, however, he was sworn to do what was best for you and to do no harm. In order to abide by these principles, Suguru needed to move you to a different facility where he could do what he deemed necessary. “Don’t worry, darling. Soon you’ll be cured.”
You awoke disoriented. Despite the fact that your eyes were closed, you could tell you weren’t in the clinic anymore. The sensation of an IV in your hand caught your attention and you wondered if you were in the hospital. You didn’t have time to question it for long when you heard Suguru’s voice, “I really don’t know if you can hear me, but I hope you can.” You tried to turn your head in the direction of his voice, but you found yourself unable to move. “The sedatives and paralytics in your IV drip are just enough to keep you from being awake and able to move, but not to the point where you need a ventilator.” You could hear him moving around the room as he continued to talk. “As your doctor, I only want what is best for you, and I know you know this. I love you dearly, but the oath I took when I graduated from medical school comes first.” Your mind was swimming, trying to understand what was happening and what Suguru was going on about.
He paused for a minute, letting out a heavy sigh “I just want you to know that the data from your heart monitor will forever serve as a reminder of you, and every time I finish to those perfect EKG waves, I’ll be picturing your beautiful face.” You wanted to crawl out of your skin. How could he say that? Your thoughts continued to race. “I think it’s time we say goodbye now.” A feeling of terror consumed you. “I’m glad I was able to create a plan to cure you, not many doctors out there would care enough about their patients to go to the extreme lengths I am going to for you. The best part about this cure is that it is completely painless. One injection of about 30mg of morphine and everything will be better.” His voice sounded almost cheery.
The next sensation you felt was a needle pricking your inner elbow and viscous liquid being injected slowly into your vein. “There we go, treatment administered.” After disposing of the syringe in the sharps box, he quickly returned to your side. Your skin was turning pale and the heart monitor began to beep as your heartbeat and breathing rate fell dangerously low. He muted the monitor. Nothing was going to disturb the two of you in these final moments.
He reached for your hand, it felt cold as he held it, gently running his thumb over your knuckles in a soothing motion. You wanted to open your eyes, your mouth, ANYTHING. You wanted to scream. Your brain was becoming too foggy to even remember how screaming worked, even if it did, it wouldn't matter though and you knew that. It was almost like Getou could read your mind, and as your ears started to ring and the world crumbled away, he pressed a soft kiss to your forehead, softly cooing, “Shhh. Don't worry, no harm will ever come to you again.”

Some tiny Amazon frogs defend themselves when threatened by releasing a substance that quickly incapacitates a predator. For centuries, a few Indigenous tribes in Colombia have siphoned this toxin from skin glands on the frogs’ backs to coat blow darts for hunting, because of its power to take down big game.
The poison instantly cramps the muscles in the body and forces the heart into a chaotic rhythm that ends in cardiac arrest. The bright coloring of the frogs warns animals to stay away. Their chemical weapon is called batrachotoxin, a name based on the Greek root word for frog.
“Predators that attack these dendrobatid frogs are paralyzed so rapidly that they can’t clamp down on the frogs to kill and eat them,” said one of the senior researchers on a recent batrachotoxin study, William Catterall, a professor of pharmacology at the University of Washington School of Medicine.
He noted that “it is so toxic that wiping the toxin secretion from the back of a single frog is sufficient to poison several blow darts, which will each immobilize or kill large prey.”
What makes their irreversible toxin more severe, compared with others that also target the signal-producing pores of nerve and muscle cells?
It has long been known that batrachotoxin affects sodium ion channels, which control electrical pulses by regulating the flow of charged sodium particles through the membranes of these cells.
“The toxin acts by hyperactivation of the sodium channel that generates action potentials in nerve and muscle, thereby causing a lethal storm of hyperactivity of nerves, synapses and muscle cells,” Catterall said.
By using high-resolution cryogenic electron microscopy, a research team led by pharmacologists at the University of Washington School of Medicine have determined, at the atomic level, the structure of the toxin derivative batrachotoxin-A benzoate when it is bound to its receptor site on the cardiac sodium channel.
Unexpectedly, the researchers discovered that, unlike other toxins, batrachotoxin actually binds to two separate but similar receptor sites in the cardiac sodium channel. This dual-receptor action, and its multifaceted effects, provide unprecedented insights into why this poison is so effective, powerful and deadly.
The toxin’s unique mechanism for high potency and lethality might also establish a new paradigm that helps to explain the actions of other toxins with high lethality, the researchers suggested.
Such studies might also lead to better therapies for conditions resulting from certain electrical signaling problems in excitable cells and tissues, such as those in the heart, muscles, nerves and brain that are caused by malfunction of a sodium channel.
For example, Catterall suggested, drugs designed to be robust inhibitors at both dual-receptor sites discovered in this study might also be valuable therapeutic agents in the treatment of disorders of hyperexcitability of nerve and heart cells. Such disorders include epilepsy, chronic pain and arrhythmias.

I'm gonna miss Halloween a lot this year, the seasonal depression is hitting pretty hard and it's tough not to have stuff like that to look forward to. After Itaewon last year it doesn't seem right to go out or dress up. It's still a pretty sensitive topic understandably. My school canceled their Halloween party again this year and I'm guessing there won't be much going on in Itaewon and Hongdae apart from some oblivious foreigners in costumes.
#i was supposed to be there actually but i went to visit some friends in gyeonggi-do instead#i know people who were and they all made it out safe thank god#i remember i was getting safety alerts saying that like 50 people suffered cardiac arrest#we were like wtf is this drugs or something#they were so vague about it#we didn't know what really happened until we saw the news
