good news: the doctor i chose to be my primary care physician based 10000% on her kind eyes and gentle aura was, in fact, kind and gentle
bad news: my white coat syndrome heart rate was 160bpm and the ekg was "probably fine but let's get a specialist involved" so now i have to go see a cardiologist to discover if i have an actual problem or if i'm just truly the most nervous chihuahua imaginable

This is Korben from this morning at the internist specialist. It was just a consultation today. Tomorrow, however, they are doing all SORTS of invasive things. He is gonna get a full endoscopy and colonoscopy, a liver biopsy, samples taken from his gallbladder and bile duct, and he’s gonna have a feeding tube put in!!! Our beautiful handsome boy is going to get all kinds of terrible bandages.
What’s wrong with him, you may ask? Well, after throwing up every day for over a week a couple weeks back, we did urgent care vet and then multiple ultrasounds and rounds of blood work, all showing an increase in his billirubin and inflammation of his pancreas, gallbladder, and liver. He stopped throwing up but then got a respiratory infection and was sneezing boogers for a week. Coupled with all of this he had lost over a pound in the past three months and had already been underweight. And then he stopped eating. He has only eaten tiny amounts in the past two weeks and is continuing to lose even more weight.
So, what could be going on? That’s what all the procedures are for tomorrow! They are going to take all the samples from his various bits and culture them so we know the exact kind of medicine to give him. And meanwhile, we will do steroids to help the inflammation, and be able to get food into his stomach and put all his medications in him through his feeding tube, instead of forcing pills and goop down his throat and traumatizing him multiple times a day. It could just be a worsening of his preexisting IBD. It could be an obstruction in his common bile duct or small intestine or gallbladder. It could be small cell lymphoma! All of these are treatable, though IBD is progressive and lymphoma would mean different kinds of cancer treatment and palliative care.
This is all costing an arm and a leg, but Korben has a very rich grandmother who would be downright offended if she didn’t pay for it, thank goodness. The less expensive diagnostic option was just more fuck around wait and see ultrasound bloodwork bullshit which would lead to months and months of trial and error. There are of course risks to all of this, but Korben has been under anesthesia before and done just fine, and other than his complete unwillingness to eat and his elevated liver values, his blood work looks good, his energy and personality is still there, he completely got over his runny nose, and he hasn’t thrown up except for once a week ago which was before we stopped the antibiotics that obviously upset him so much he stopped eating or drinking and all he would do is drool and try to spit them out. Sigh. Poor guy. Fingers crossed though, because although we have now interacted with four separate very competent vets and numerous techs, nobody has ever given us an indication that Korben is unlikely to recover.
Other good things: I’ve now learned how to give IV fluids and injections! It really is super simple, as long as your cat is mellow like Korben and you have a second person to be another pair of hands. The needles are like the least troubling part. I love the immediate relief and comfort Korben exhibits when he gets fluids. I know myself how it feels to be terribly dehydrated and in pain and then get an IV, so it’s a great feeling being able to do that for my cat. Yesterday I gave him his third vitamin b12 injection (he is getting them weekly for now), and he didn’t care for it one bit but it was fine. So now, I’m confident that if anyone else I care for needs injection meds, I can give it to them! I am honestly shocked I’ve never helped a trans friend with their hrt or anything before this, but now if it ever comes up can be like “let me help! Just act like a cat!”
So. I am currently in the “ignore it until shit happens” phase where I’m trying to just project calming normal energy at Korben, who is currently sleeping in a sunbeam by an open window. Tomorrow morning he is going to be going through it, so keep him in your hearts and all that. And then afterwards will be the notable experience of gooshing puréed food in through a tube in his neck! Eeeek! Then it will be some time to get the cultures done, and then it will probably be over a month of antibiotics and other supportive meds. Egad! Fingers and toes and eyes all crossed.
Thanks @penaltykeks and a few other people who have checked in and asked how me and Korben are doing. I’ve just been sort of trying my best to engage with pleasant things online and not write out my worries because I’ve learned it doubles down on my rumination. BIG UPS to Zoloft, which REALLY FUCKING WORKS for me, I would basically be a pile of quivering useless bones and fat without it. As it is I am a quivering pile of bones and fat that can give IV fluids to my cat and call my mom and do the dishes. 😽🤞

September 1st, 2023
This morning we welcomed the final piece to our family. After 17 1/2 hours of labor, Alexander James was born weighing 7lbs 6 ounces and 20 inches long. By far, our tiniest baby. Hes passed all his blood sugars and if we pass our billirubin tonight, well be heading home tomorrow morning.
I reached 9cm naturally before I could no longer handle the pain. We had one dose of IV medication while we waited for the epidural and within 20mins of the epidural, we were 10cms and baby was getting low. We labored down breifly and pushed a total of 6 times (2 contractions). I got to stop and feel his head as he was crowning and watch him come out with a mirror. D cut the cord and we got a full golden hour of snuggles. I didnt even tear this time! It was so low stress and just a beautiful ending of this chapter of birthing our babies. I feel so complete. Hes so beautiful. He looks so much like Rhyan did but so much smaller. D told me he felt his name was Alexander James and I told him if thats his name, then thats it ❤
Everything weve been through has been worth it. The scares, the appointments, the worry.

Gallstones form when bile stored in the gallbladder hardens into stone-like material. Too much cholesterol, bile salts, or bilirubin (bile pigment) can cause gallstones. When gallstones are present in the gallbladder itself, it is called cholelithiasis. Add fruit such as banana or berries or a handful of dried fruit on breakfast cereal. Add salad to your sandwich fillings. Have a healthy dessert: try whole fruit, fruit salad, fruit tinned in juice or stewed fruit. Have at least one portion of vegetables or salad with your main meal.

I’m gonna tell you guys a story - and I’d like you to know from the very begining it has a happy ending.
Through my last weeks at my Uni, on placement, we had a dog in critical care. I don’t think I can give you his real name so I’ll just call him Rey. He’d been carried in and had been unresponsive for days when I got there. 
He had immune mediated haemoltic amemia, or IMHA. It can be brought about by recieving blood from the wrong blood type in a transfusion, or, at least in dogs, spontaniously. It ment his immune system saw his blood as a pathogen and started destroying his blood cells as fast as it could, much faster than they can regenerate on their own. A major by-product of breaking down blood cells is billirubin - it’s a yellow pigment that makes faeces brown and urine yellow. When you haven’t drunk enough water, or a lot of blood cells have been destroyed recently, your urine gets darker. 
His was black. 
Too much billirubin effects the body as a whole, from the brain to the kidneys. There is no dyalasis machine for veterinary use so his blood was taken and seperated and put back into him with fresh plasma and all the dead cells removed manually, something so complicated there were only maybe 3 people on staff at a major veterinary university who felt comfortable doing it. He had so many blood transfusions he exhausted our stock of blood products which is pretty impressive considering there are donors we can take blood from too - we used all of them. Whenever he could, he was wagging his tail. No one expected him to live. He was only 4. 
You want to know what happened? 
He walked out of the hospital, tail wagging. No lasting damage. Nothing. Just a happy dog, excited to be able to eat something other than ensure or slurry, thrilled to be able to see his people again. 
I’m not really sure why I wanted to tell you guys this - but sometimes good things happen. Despite the odds, despite setbacks, everything just works out. It wont always, and there was so so much work put into that happy ending - but thats what it was. Happy. Everything worked out perfectly. 
Good things happen, even when it doesn’t feel like they can.

When I had my daughter I learned that our blood types were “incompatible” although I do not have Rh negative blood. I learned it was ABO incompatibility which is NOT Rh incompatibility where the mother has Rh negative blood. I started to research more about ABO incompatibility after my daughter was born. In simple terms my blood recognized my daughter’s blood as a foreign substance and started to react to it by creating antibodies against her blood cells. The destruction of the red blood cells causes an increase in billirubin levels which then causes jaundice which is common in newborns. However, my daughter had elevated levels due to ABO incompatibility. She had light therapy (which I did not like, I would have preferred having her in natural sunlight, it was still relatively hot where we live). Outside of that she had no other issues. Some of the things that I experienced during that time at the hospital I was not pleased with and felt was not necessary however I made sure I nursed her directly from breast the whole stay at hospital. The good thing I learned is that I am now aware of this if we decide to have another baby. In the case with my daughter, she ended up having my husband’s blood type. There are some places you wish the wifi didn't connect automatically and you didn't recognize lots of the staff. We were in the hospital over the weekend with Teddy for his billirubin levels. We're beyond thankful for our families who have swooped in to take care of us and be Teddy's cheer squad. I'm also personally thankful that it doesn't (physically) hurt to laugh anymore! Phytomedicinal and Ameliorative Effect of Ethanol Leaf Extract of Corchorus olitorius on Carbon Tetrachloride Induced Hepatic Toxicity | Chapter 01 | Recent Advances in Biological Research Vol. 5
Liver, the engine house for biotransformation of xenobiotics serves as a receiving depot, processing factory and redistribution center of the body. In the light of these roles, the effect of ethanol leaf extract of Corchorus olitorius on CCl4 induced liver damage in wistar rats was assessed. CCl4 (0.5 ml/kg) was used to induced hepatotoxicity. 25-albino rats of wistar strain (120-150 g) were used for the study and were divided into five groups of five rats per group. Group A serves as normal control and was given distilled water of treatment equivalence, group B serves as a negative control and was treated with CCl4 and olive oil as a vehicle at the ratio of 1:1. Group C, D, and E were induced with CCl4 and the extract administered through oral gavages at scalar doses of 500 mg/kg, 750 mg/kg and 1 g/kg daily. The administration lasted for 15 days period after which the animals were sacrificed. Serum enzyme assay revealed that there was a marked reduction in the elevated activity of the hepatic enzymes viz alanine aminotransferase (ALT) A (71.2±3.27), B (98.8±2.61), C (78.0±3.74), D (74.8±2.77), and 74.6±1.95) respectively. Similar trend was observed for aspartate aminotransferase (AST), and alkaline phosphatase (ALP) levels when compared with the controls (P=.05). More so, the result revealed that the extract lowers serum albumin, no significant change in billirubin but increased total protein levels in all treated groups when compared with the controls (P=.05). Furthermore, it showed a significant decrease (P=.05) in White blood cell count (WBC), and platelet but no significant decrease (P=.05) in packed cell volume (PCV) and Hb. From the above findings, it is obvious that the ethanol leaf extract of Corchorus olitorius could have hepatoprotective property.

Hepatit olarak bilinen sarılık hastalığı bir tur karaciğer iltihabi rahatsızlığıdır. Genel bir kani olarak sarılık halk arasında başlı başına bir hastalık olarak düşünülmektedir. Ancak bilenmesi gereken sariligin bir hastalık değil bir tur belirti olduğudur. Sarılığa neden olabilecek bir çok çeşit hastalık bulunmaktadır. Hastalıklar içerisinde en çok görülen neden olarak safra kanalının tıkanmasına bağlı olarak gelişen sarılık turudur. Bu tarzda oluşan sarılık bulaşıcı değildir. Hastalık vira kaynaklı olduğunda ise sağlıklı kişiye bulaşma durumu gözlenebilir. Sarılık gözlemlendiğinde yapılması gereken bunun bir sonuç olduğu bilincin de olmak ve bu donucu ortaya çıkarsan ana etken hastalığı tespit etmek olmalıdır.

Hepatit b (sarılık) ile ilgili bilenmesi gerekenler nelerdir?        Viral şekilde ortaya çıkan sarılık bulaşıcıdır. Bazı virüs çeşitleri vücut diskisi ile vücuttan atılmaktadır. Hepatit b yenidogan bebeklerden yenidogan bebeklerden yetişkin insanlara kadar herkeste görülebilir. Bu sebeple yayılma olasılığı yüksek olan hastalıklar içerisindedir. Ye doğan bir bebeğin annesinden doğarken kaptığı hepatit b nin kronik olma ihtimali  daha çoktur. sonradan hepatit b kapan yetişkin bireyin taşıyıcı olma ihtimali ise zayıftır. Hepatit b nin belirtileri nelerdir: Hepatit b öncelikle halsizlikle baslar. Sonrasında bulanti,karin ağrıları, kusma ,iştahsızlık, kilo kayıpları görülür. Bu belirtilerin bir kısmi görülebilir.
