FDA Indications/Dosages:
For the prophylaxis and chronic treatment of asthma in adults and children 2 years of age and older: The dosage for adolescents and adults 15 years of age and older is one 10 mg tablet daily to be taken in the morning. The dosage for pediatric patients 6-14 years of age is one 5 mg tablet daily to be taken in the evening. The dosage for pediatric patients 2-5 years of age is one 4 mg tablet daily to be taken in the evening.
For the relief of symptoms of seasonal allergic rhinitis for adults and pediatric patients 6 months of age and older: The dosage for adolescents and adults 15 years of age and older is one 10 mg tablet daily. The dosage for pediatric patients 6-14 years of age is one 5 mg tablet daily. The dosage for pediatric patients 6 months to 5 years of age is 4 mg daily.
For the prevention of exercise-induced bronchoconstriction in patients 15 years of age and older: One 10 mg tablet daily taken at least 2 hours prior to exercise. Wait 24 hours before taking another dose.
Pharmacology/Pharmacokinetics:
Asthma pathophysiology, including airway edema, bronchospasm, and altered cellular activity, has been correlated to the presence of cysteinyl leukotrienes (LTC4, LTD4, LTE4) in the lungs. Mast cells and eosinophils release cysteinyl leukotrienes upon metabolism of arachidonic acid where they bind to specific receptors (CysLT). Montelukast binds with high affinity and selectively to the CysLT1 receptor where it inhibits physiologic actions of LTD4 without any agonist activity. After an oral dose, peak plasma levels are reached in 2-4 hours and the mean plasma half-life ranges between 2.7-5.5 hours. Metabolism occurs via cytochromes P450, 3A4, and 2C9. Food does not alter the availability of an oral dose.
Drug Interactions:
Phenobarbital may increase metabolism of montelukast. May decrease the metabolism (via inhibition of P450 2C8) of paclitaxel, rosiglitazone, and repaglinide.
Contraindications/Precautions:
Montelukast is not indicated for reversal of bronchospasm in acute asthmatic attacks, including status asthmaticus. Do not replace oral or inhaled steroids with montelukast without tapering steroid dose. Montelukast for inhalation should not be used as monotherapy for exercise induced asthma. Rare cases of systemic eosinophilia have been documented during therapy usually after the reduction or stopping of oral corticosteroids. Use with caution in nursing mothers. Pregnancy Category B.
Adverse Effects:
Adverse effects closely resemble those found with placebo and include headache (18%), influenza (4%), cough (3%), abdominal pain (3%), dyspepsia (2%), and dizziness (2%). Rare cases of psychiatric disorders including agitation, aggressive behavior, abnormal dreams, depression, and suicidal thinking have been reported.
Patient Consultation:
This medication should be taken regularly even when the patient is asymptomatic.
This is not intended to treat asthmatic attacks and patients should have appropriate measures at hand to treat attacks.
Phenylketonurics should be aware that chewable tablets contain phenylalanine.
If a dose is missed, take it as soon as possible and return to normal dosing schedule.
Store in a cool, dry place away from sunlight and children.
Contact a physician if the above side effects are severe or persistent.
Granules can be given either directly in the mouth or mixed with a spoonful of soft food (applesauce, carrots, rice, or ice cream).

“is a plant cultivated for its grain-like seeds and as a cover crop. A related and more bitter species, Fagopyrum tataricum, a domesticated food plant common in Asia, but not as common in Europe or North America, is also referred to as buckwheat.
Despite the name, buckwheat is not related to wheat, as it is not a grass. Instead, buckwheat is related to sorrel, knotweed, and rhubarb. Because its seeds are rich in complex carbohydrates, it is referred to as a pseudocereal.
The name "buckwheat" or "beech wheat" comes from its triangular seeds, which resemble the much larger seeds of the beech nut from the beech tree, and the fact that it is used like wheat. The word may be a translation of Middle Dutch boecweite: boec (Modern Dutch beuk), "beech" (see PIE *bhago-) and weite (Mod. Dut. weit), wheat, or may be a native formation on the same model as the Dutch word.
Common buckwheat was domesticated and first cultivated in inland Southeast Asia, possibly around 6000 BCE, and from there spread to Central Asia and Tibet, and then to the Middle East and Europe. Domestication most likely took place in the western Yunnan region of China. Buckwheat is documented in Europe in Finland by at least 5300 BCE as a first sign of agriculture, and in the Balkans by circa4000 BCE in the Middle Neolithic. Russian-speakers call buckwheat гречка (grechka) meaning "of Greece", due to its introduction in the seventh century by the Byzantine Greeks.
Historically, the Russian Empire was the world leader in buckwheat production. In 1970, the Soviet Union grew an estimated 4.5 million acres (1,800,000 ha) of buckwheat. As of 2016, it remains a key cereal.
In the northeastern United States, buckwheat was a common crop in the 18th and 19th centuries. Cultivation declined sharply in the 20th century due to the use of nitrogen fertilizer, to which maize and wheat respond strongly.
Buckwheat noodles have been eaten by people from Tibet and northern China for centuries, as wheat cannot be grown in the mountain regions. Buckwheat groats are commonly used in western Asia and eastern Europe.
