I am once again thinking of a unified Mediterranean union / league / confederacy of some type.

NO but hear me out. It would be composed of the PIGS countries (Portugal, Italy, Greece and Spain), extensively also including Andorra, southern France (roughly the Occitanie region), Monaco, San Marino, Malta and maybe Cyprus (?). Here is my rendition of how the borders would roughly look like (Macaronesia not shown):

It would be called Mediterranean League, and its official language would be Latin, although it would be mandatory to learn at least another three regional languages at school. About the capital, I have thought of three options:

-Rome (very central, lots of historical importance)

-Syracuse or another Sicilian city (even more central, access to both the eastern and the western part of the country)

-New city built in an island if possible, my bet is on Cabrera island off the coast of Mallorca, it's neither too big nor too small and it's very open to communication to the rest of the Mediterraneum. I have thought several options for the name: Marenostria / Mare Nostria, Mediterrania, Gymnesia (the ancient name for Mallorca, Menorca and Cabrera islands), or just Cabrera (which means "land of goats", maybe the symbol of the city could be a goat lol). Also it's technically an Archipielago, so it could be like a cool modern and bigger Venice.
