Some people are hiring two surrogates at the same time to carry their babies.

Concurrent surrogacy can be complicated and costly, with prices reaching up to $500,000 or more.

Many people who do it are in their 40s and trying to build out their family quickly.

Bill Houghton still vividly remembers the moment he met his son.

He was sitting in the hospital waiting room, right outside the birthing room, when a nurse appeared carrying a little green bundle.

"I just held him in my arms and just started crying. It was so overwhelming. My husband was like, 'Oh my God, I can't believe that this is it. We're a family,'" Houghton told Business Insider. "This is my son."

Just one week later, Houghton and his husband would have the same experience all over again when their second child, another son, was delivered.

"And it has been like that ever since," he said. "To this day, I still look at them and I think, 'Oh my God, these are my sons.' My father had sons. I never thought that I would have a son."

Houghton and his husband opted to become parents via concurrent surrogacy — a process in which two surrogates are hired to carry two babies at the same, or overlapping, time.

The resulting children can be born anywhere from one week apart, like Houghton's, to nine months apart, and have been referred to by some people in the industry as "tandem siblings" or "twiblings."

Surrogacy agencies told BI that concurrent surrogacy journeys are not uncommon, with some saying it's a rising trend in a growing industry that was valued at $14 billion in 2022 by Global Market Insights and has attracted the investments of private equity firms.

All kinds of people — couples or singles, straight or gay, young or old — have opted to build out their family two at a time via concurrent surrogacy. But there is one thing that most parents of twiblings have in common: the ability to afford them.

While Houghton hired surrogates abroad, couples who choose to go through US-based agencies can easily spend $300,000 to half a million dollars or more on concurrent surrogates, according to five surrogacy agencies that spoke to BI.

"It is a luxury, absolutely," Brooke Kimbrough, cofounder and CEO of Roots Surrogacy, told BI. "Most American families don't have $200,000 in cash to go through surrogacy generally, and then $400,000-plus in cash to be able to go through that twice at the same time."

Still, the use of concurrent surrogates could grow as surrogacy generally grows in the US, in part because celebrities like Kim Kardashian and Chrissy Teigen have started opening up about using surrogates, as well as depictions in film and TV that have made the practice more mainstream. Teigen was even pregnant at the same time as her surrogate.

Surrogacy is also becoming increasingly relevant as more and more people are opting to have kids and start building their families later in life.

Concurrent surrogacy can help build a family quickly

Concurrent journeys typically look like regular surrogacy journeys, just times two. Gestational surrogacy, when IVF is used to place a fertilized embryo into a surrogate, is the most common form of surrogacy in the US today. Parents can use their own egg and sperm or that of donors.

Like many gay couples, Houghton and his husband each used their sperm for one of the babies, as well as the same egg donor, so their sons are technically half brothers.

While there has been increased awareness around what some call "social surrogacy" — using a surrogate when it's not medically or biologically necessary — the majority of people who conceive via surrogacy do so because they have to.

"Typically, when people come to us, they've been through a lot. This is not their plan A, it's often not plan B, maybe it's plan C," Kim Bergman, a psychologist and senior partner at Growing Generations, told BI. "They've had a lot of disappointment, and they've had a lot of trials and tribulations."

Many hopeful parents are in their 40s and are simply eager to build their families, the agencies said. A surrogacy journey can easily take one and a half to two years, so for intended parents who know they want multiple kids, concurrent surrogates can be appealing.

Certainly, some people who opt for concurrent surrogates do not fit the definition of medically necessary, at least according to the standards laid out by the American Society for Reproductive Medicine (ASRM).

Some people have mental health reasons or a fear of giving birth. Others are actors or brain surgeons who spend 12 hours a day on their feet and who can't get pregnant and continue to do their jobs. All the surrogacy agencies BI spoke with said it's essentially never the case that someone opts for surrogacy simply for vanity reasons.

David Sher, founder and CEO of Elite IVF, told BI they've helped coordinate surrogates for celebrities, politicians, and people in demanding careers like finance or tech. He said he currently has a client who serves on the cabinet of a Western country and is trying to have a baby via surrogate in part due to her demanding schedule.

Sher said he thinks concurrent surrogacy has long been an option for intended parents but that there does seem to be an uptick in people who are opting to do it.

Part of the reason for that could be because fewer and fewer agencies are willing to do double embryo transfers, which were previously more common and could result in a twin pregnancy. The ASRM recommends against them, as twin pregnancies come with heightened risks for both the surrogate and the babies. So concurrent surrogacy is a safer option for intended parents who want to have two kids at the same time or in close succession.

Costly and complicated

Though it's viewed as a safer option, concurrent surrogacy is controversial. The ASRM guidelines actually recommend against concurrent surrogacy, as well as against social, or not medically necessary, surrogacy. But all five surrogacy agencies that BI spoke to will facilitate concurrent surrogacies.

The agencies said they've seen many concurrent surrogacy journeys be successful and that a lot of care and prior planning goes into making them happen.

"It's not taken lightly," Bergman said, adding that concurrent journeys are rarely chosen by 30-year-olds who have plenty of time to build their families, though that does occasionally happen.

Surrogacy, in general, is expensive — commonly ranging from $150,000 to $250,000 for one child. The costs go toward surrogate compensation, agency fees, legal fees for contracts, and clinical bills.

The agencies BI spoke with said a concurrent surrogacy journey would essentially cost twice that. Meaning there's no two-for-one special.

But cost isn't the only factor to consider. Perhaps the primary drawback to pursuing concurrent surrogacy (that is, besides the high price tag) is the logistics of it.

All the agencies emphasized that concurrent surrogacy should only be pursued with full transparency and the fully informed consent of every person involved. That means matching intended parents to surrogates who are fully aware and OK with the fact that they will not be the only surrogate.

There's also tons of planning and talking through hypotheticals. Are the surrogates based in the same area? Can the parents attend both births? Are we staggering expected delivery times enough? What's the plan if one surrogate gets pregnant on the first try but the other doesn't?

There's also a psychological aspect. Will both surrogates feel fully supported? How will one feel if she doesn't get pregnant right away and the other does?

"All of these conversations are front-loaded. Anytime in the conversation, the surrogate can say, 'I'm not comfortable doing this,'" Bergman said, adding that sometimes, after thinking through the logistics, some parents will change their minds and plan to space the deliveries out further than they initially wanted, like to six or nine months.

Most agencies recommended staggering the planned deliveries by at least three months. But at the end of the day, parents need to be ready for the timeline to not go exactly as planned.

Houghton and his husband had actually planned to have their babies six weeks apart, but when one of the babies was born five weeks premature, they ended up with birthdays one week apart.

Concurrent surrogacy may not be for everyone — even if you can afford it

Although the cost of concurrent surrogacy makes it prohibitive for most people, that could change in the future as more and more companies expand their fertility benefits.

There are also more nonprofits popping up that will provide grants or partial funds to people who want to build their families via surrogacy but may not have the means to.

Jarret Zafran, founder and executive director at Brownstone Surrogacy, told BI that it's not necessarily only the ultrawealthy who pursue concurrent surrogacy. He said he currently has clients who are lifelong educators on the older side who are getting ready to start the surrogacy process. They recently asked about what it would look like for them to do a concurrent journey.

"I guess it is still a luxury in the sense that most Americans would not even be in a financial position to afford it the first time," Zafran, who also had a child with his husband through surrogacy, said. "But for them, this is not a frivolous decision, and they're scraping together every single little penny that they have, all of their savings, their retirement funds, and I get it."

By using surrogates abroad over a decade ago, Houghton and his husband, who are based in Spain, spent much less on their concurrent surrogates than they would have in the US. But he's still not totally sure why they chose to do concurrent journeys rather than space the children out a bit more.

"We just liked the idea of having two kids that were about the same age that would sort of grow up together," he said, adding, "I didn't realize at the time the challenges that would come with having two kids."

In reality, he said having the two boys grow up so close together in age, not twins but in the same class in school, ended up leading to a lot of conflict and constant competition as they were growing up. He said it has gotten better now that the boys are facing their teen years and developing their own identities.

Still, if he could do it over again, he thinks he would stagger them more.

"They're unbelievable young men, and I'm so proud of everything about them," he said. "But having the two together has been a challenge."

He's incredibly selfish and work is the only thing he really cares about, he's engaged to janus but they might as well not talk at all. Roman and Remus are hired to clean Logan's private yacht and surprise surprise, Logan treats his employees like shit. He mocks them for their low class life style and jokingly asks if they ever even went to college, Remus explains they actually did ans their majors were theatre and graphic design, Logan laughs so much he cries. Remus gets sick of this shit and calls Logan a Karen.

The two start arguing and Logan fires both of them, Remus gets so angry he tries to push Logan to the ground but he stumbles and falls over the side of the boat. Remus and Roman are horrified and quickly fish him out and take him to the hospital, When they get there Logan wakes up and doesn't remember ANYTHING, not even his name. Orange and Janus hear about this but Janus doesn't really care enough to tell anybody or pick him up ans Orange is already pretending like Logan was eaten by a shark so he can steal his job.

The doctor asks who the hell the twins are and say they have to leave if they aren't immediate family so Remus lies and says logan is his husband so he can see if he remembers getting pushed. Remus didn't mean for it to go this far but the hospital staff seem very eager to get Logan out of there since he's such an entitled brat even with amneisia so they just tell Remus to take him home.

Remus freaks out cause he doesn't know wjwt he's supposed to do but then he and roman get a great, possibly illegal, definitely immoral idea. Remus can't afford a baby sitter or a house keeper, but they sure as fuck can afford to make this wealthy jack ass think he's a house husband. They assume it'll only be for a few days since his family will be looking for him but that obviously doesn't happen caude of orange and janus's bs.

On the way home Remus explains his name is Logan Sanders and that they've been married for fifteen years, they met at a hurts donut shop in arizona when logan was working behind the counter and remus asked if he could glaze his donut. They have four kids, Virgil (13), Remy (10), Patton (7), and Picani (2). Logan's parents are both alive but he doesn't speak to them anymore for reasons he never told him. He also used to work at spencers gifts but quit to stay at home with the kids since they couldn't afford a baby sitter.

Logan is disappointed that he would both marry some one like remus and live in such a shitty house. Remus fills his kids on all the details and no one knows what to say to Logan. Virgil ignores him and goes back to listening to MCR upstairs, Remy is actively trying to make him uncomfortable, Patton is bugging him non stop to play, and Baby Picani pukes on him.

Logan asks how they have four kids if adoption and surrogacy are expensive and Remus explains that they are from an ex fling but Logan has started caring for them like they were his own. Logan feels like a beta cuck. He then asks to change out of the hospital gown he came home in and Remus lies that the clothes his ex wife left behind are actually his.

Logan is surprised to find out he's the kinda gay who wears flowery blouses and sweat pants with the words "boss babe" on the ass, but he just rolls with it ans gets dressed anyways. Remus finds it very frustrating that he actually fits those clothes very... well. Too well.

Logan is terrible at nearly all household chores. he mostly just ate jam out of the jar and had other people cook food for him before so he nearly burns the whole house down when he tries to cook. He also hated children and thought they were all bratty immature hell spawns so he's not enjoying taking care of the kids. he does however learn to strategize accordingly and play to his strengths a little (with some minor help from remus ofc). He reads a mechanics book at the library and fixes a pipe, he reaches out to each of the kids and helps them with school work, He learns to properly care for a baby, he makes a stew and it tasted pretty decent, he even takes a look at their finances and help them save money! He's doing great!

During all of this Remus starts to develop some complicated feelings for Logan, his kids really like him, he's made their whole life better, and his ass looks really good in his ex wifes sweat pants. He feels awful about this stupid lie, he wants to tell him the truth really badly but he knows Logan will hate them all and leave. It's even worse because Logan begins to recipricate these feelings and admits he's happy they got married, which just makes Remus feel even more guilty.

On their supposed wedding anniversary Logan tries to lure Remus into the bedroom for sexy funtime but Remus refuses since even though he likes logan he doesn't feel comfortable doing anything physical with some one who probably can't consent. Remus just suggests they go to buffalo wild wings instead and they have a very fun time where Logan becomes smitten with remus's weird but caring personality.

The whole lie comes very close to falling apart when Logan is cleaning out Remus's car and finds a blue tie laying on the front seat. He knows damn well Remus would never wear a tie and Roman hates the color blue so he instantly assumes Remus is cheating on him, completely ignorant to the fact it's actually his and Remus just stole it from him when they were fishing him out of the water. Luckily Roman lies and says he had sex with a business man in remus's car and he calms down. Remus wishies his brother would of just stfu and let the truth come out.

So anyways thats the au, things are pretty good despite the weird ass set up... for now. Just wait until logans dad finds out what actually happened and yells at everyone. Things will get very very messy.

To become a gay dad with a biological child, you’ll first need some eggs (human ones). You can buy some from an egg bank or try to find a donor who will cycle just for you. If doing the latter, it’s good to have contracts, so you’ll need a lawyer too.

Then you need to get your eggs to a fertility clinic. If you have a donor, you’ll be paying for their travel and appointments. If you’ve got the eggs, a freezer truck transportation service will do. Embryologists at the clinic do their thing with the eggs and your sperm, and voilà — you have embryos on ice. If embryologists sound expensive, it’s because they are.

We’re done with the “less hard” part. Now you need to find a surrogate, the uterus-bearing person who will carry your baby. You’ll either pay a surrogacy agency, which can be pricey, or find a surrogate of your own, which “sounds hard because it is,” says Janene Oleaga, attorney and founder of Oleaga Law, a family formation law firm. “Then there’s the actual embryo transfer, which may or may not be covered by insurance. You’ve also been paying this whole time to keep the embryos cryogenically preserved,” Oleaga says.

Feeling the cash crunch yet? There’s another cost to consider. The stigma against gay parenthood runs deeper than rhetoric alone. It’s woven into the very fabric of our health care system. 

As LGBTQ+ acceptance makes hard-fought progress, today’s young people are coming out earlier than previous generations. In a 2018 survey conducted by the Family Planning Council, respondents over the age of 60 said they came out at an average age of 37; for respondents in their 30s, the average age was 21, and for respondents ages 18 to 24, the average age was 17.

When Obergefell v. Hodges granted federal marriage equality protections in 2015, LGBTQ+ interest in family planning spiked, with solutions like assisted reproductive technology (ART), foster parenthood, and private adoption leading the way. Nearly half of LGBTQ+ millennials are actively planning to grow their families, a difference of just 7 percentage points from non-LGBTQ+ millennials, and a marked difference from previous generations.

If queer intended parents (IPs) take the ART route, financial challenges loom. The average cost of in vitro fertilization (IVF) hovers around $12,400 per cycle, according to the American Society for Reproductive Medicine. But the chances of success are less than 50 percent, according to ART data from the CDC, adding considerably to the financial pressure. Subsequently, an entire boutique industry has emerged to help intended parents navigate the medical, legal, and emotional terrain of the process.

“To be very frank and honest with you, a lot of this is a mega, mega business,” says Harout Cracchiolo, who together with his husband became a first-time father last year. “We went with a friend who donated her eggs and paid for all her treatments. Then we went with a family member who ended up carrying the baby for us and got a separate surrogacy insurance for her. We still had to pay all the hospital bills, anesthesia, and doctor’s visits [out of pocket], because insurance wouldn’t cover it. We ended up at around $175,000.”

(In other words they had a baby by taking eggs from one woman then exposing another women to the dangers of a pregnancy with heightened risks just to create a baby that neither the egg donor or the woman who gave birth will feel a connection to and will have a harder time in court if they sue for custody or visitation)

Cracchiolo says he and his husband were lucky, “because we did our IVF and got pregnant the first time.” He partially attributed this success to the care he and his husband received from their chosen fertility center, and he called some agencies “a scam.”

“One quote included a $60,000 agency fee,” Cracchiolo says. “Then it’s about another 20 or 30 grand for the egg donor, another 70 or 80 grand for the surrogate, and all the attorney’s fees.” Cracchiolo also says he and his husband calculated another quote’s projected total cost to be over $250,000, with the agency wanting most of that money in escrow upfront.

Family planning professionals said the $60,000 agency fee figure sounded high and noted that every industry has its bad actors. For many intended parents, finding and working with a good agency to mitigate the risks of surrogacy will be worth the investment. “There are a number of reputable agencies protecting both intended parents and surrogates alike,” Oleaga says in a follow-up email. “An ethical, knowledgeable agency provides valuable guidance and support throughout the surrogacy journey.”

Oleaga adds that many lawyers offer to review agency agreements with both intended parents and surrogates to ensure individuals understand what they are agreeing to, along with what services the agency will and will not provide.

Nevertheless, for gay male couples, the fertility financial benefits that come with health insurance are often stunted, since they may only apply when policy holders themselves are the ones who are pregnant. “The majority of gay men,” says Oleaga, “should expect a six-figure bill.”

The unpredictable costs of fertility have led some intended parents to seek out agencies whose financial safeguards help keep their family planning hopes alive, says Taylor Frey, who with his husband Kyle Dean Massey opened Elevate, an egg donation and surrogacy agency, after their own decade-long fatherhood journey.

“When Kyle and I went through the process, we were both working in Broadway shows, and we started a savings account to create embryos with an egg donor,” Frey says. “I was 25 at the time. We created embryos, then put them on ice for eight years so we could replenish funds.”

Frey and Massey say they founded Elevate to innovate on industry setback and cancellation policies. “If you do not end up with what you need to transfer to a surrogate, your recycle fees are on us,” says Frey, referring to fertility fees incurred when eggs turn out to be unviable.

“The reason why [surrogacy] is so expensive is that, from deciding you want to be a parent to having a baby, hundreds of people will touch your case,” Massey says. “All the attorneys, and psychologists, and counselors, and lab technicians, and nurses, and physicians, and monitoring clinics, and labs — it’s a lot of people.”

Oleaga agreed with the idea of meeting with several prospective fertility service providers as a best practice. “I always advise intended parents and potential surrogates to meet with multiple agencies and review their agency agreements before signing on,” she says.

For female same-sex couples, the dynamics are different, albeit no less demeaning.

“We knew we wanted to have kids. My only stipulation was that we be married first because we’re gay,” says Samantha Davis, who with her wife Leni had their son via intrauterine insemination (IUI) on their second attempt in 2019. “I just didn’t want the government to feel like we weren’t both equally the parent.”

If heterosexual couples have unexplained infertility, they need to have been trying to conceive for at least a year (or six months if over the age of 35) before a doctor will make the diagnosis official, a declaration that unlocks health insurance benefits for some fertility services. But for same-sex couples, the waiting period is waived. “Because we’re gay, I wanted to make sure that I was automatically considered infertile [from the start] so that the insurance would cover some of the stuff we would be going through,” Samantha says.

The couple’s infertile status unlocked an allowance of $16,000. “That’s not per child, or per year,” Leni says. “That’s for life.” The Davises’ expenses included storage for sperm and paying to have embryos created and tested, all of which quickly burned through the stipend. They also say they had to pay out of pocket for psychiatric evaluations and blood work.

“The first time, we weren’t in a great financial place,” Samantha says. “We were scraping every time we needed to pay for something.” Samantha touched her belly. She’s pregnant again, this time through IVF.

“After our son, I really wanted to have a girl,” she says. The couple decided to do IVF to produce a female embryo, but after two attempts, the only remaining healthy embryos were males. “We could have tried again to see if we could get that girl, but financially it wasn’t feasible.”

The path to biological parenthood shouldn’t be this expensive for anyone. For LGBTQ+ people, however, the journey remains especially taxing as we navigate systems ill-suited to our family planning interests.

“A lot of LGBTQ+ people don’t have children, so it’s not such a big movement,” Cracchiolo notes. “But it needs to be. You want to feel included. You’re having a baby — you don’t want to feel strange when you go to the hospital.”

Horror stories aside, the parents agree their journey was worth every step.

“It’s the coolest thing to wake up and know that every day you’re working towards making this child’s life something you didn’t have,” Cracchiolo says. “It’s just the best thing ever.”

The Davises echo this sentiment. “It’s rewarding in so many ways,” Samantha says. “I think I’ve learned so much about myself, my upbringing, things that I hope to do differently.”

Queer people deserve to become biological parents regardless of socioeconomic status. But reform is needed to make LGBTQ+ parenthood more financially accessible.

(No one is entitled to biological offspring. And how do they reform it to make it “financially accessible” without fairly compensating the women going through the egg donation process or the birth mother who will be carrying a pregnancy with a greater risk of complications?)
