Evan is a surgeon that makes peoples body abnormal, for example give them claws, horns, wings, tails etc. He will scar your face or permanently open up your jaw by taking the flesh out. (somehow his patients survive) He also pierces and with every surgery you can get a piercing for free. He makes pretty good money with this sort of illegal buisness, and living good.

Now Barty starts to hear from people that evan does that kind of stuff, and he got curios, so he decided to try it out. After the first time, where barty got his ears reshaped, he absolutely was in love with evan. He came back again and again, until he got the courage to ask Evan out at some point. At first he hesistated, but said yes in the end. Since then they've been seeing each other for much more than just surgeries. ;)
