NYC EMS ordered to not to transport cardiac arrest patients who cannot be resuscitated in the field.
A lot of my Facebook friends are EMS providers, ED nurses, or ED physicians that I have worked with or currently work with. They’re among the many members of the frontline, fighting COVID-19. Many have expressed fear, some have expressed support, but no one has said they are ready for what lies ahead. Many things are changing rapidly in the medical field right now, and it seems to be almost a daily occurrence. One thing changing daily are protocols. What do we do to best protect ourselves? How do we best protect the patient’s we treat? How do we best protect our families? How to we best protect our community? How do we stop this deadly virus? All are valid questions, but there are no definitive answers.
One of my fellow EMS providers shared an article that I found interesting. FDNY is being ordered to not transport cardiac arrest patients, that cannot be resuscitated in the field. Some people may see this as a good thing, considering the massive COVID-19 outbreak. Others may say this is terrifying and a lack of care. However, reading into the situation, there are several reasons why this is a best practice of care. One is because FDNY is highly overwhelmed with the amount of calls they are receiving. If you do no know, it takes a lot of resources to run a code in the field. There needs to be at least 5 people on the scene to grab supplies, perform life-saving tasks, and chart the entire situation. This doesn’t include the police who come to help or keep the scene safe while EMS is working. My protocol in southwest Ohio is 20 minutes of CPR before calling for an ED physician. This call is me asking for a doctor, waiting for that doctor to answer (praying they’re not already busy with another patient), telling the doctor the patient’s last known well time, how long CPR has been in progress, the patient’s medical history, and all interventions performed including IV/IO access, airway interventions, defibrillation, and all medications, with doses. This takes about 5 minutes, if not longer. The doctor then says whether there’s more we can do in the field, to bring this patient to the ED, or call time of death. Transporting a cardiac arrest patient keeps the crew out of service for even longer. Not to mention, manual CPR is highly ineffective during transportation and extremely dangerous.
Another reason was probably that ED resources are already stretched thin, and limiting these patients may release a little of the burden. While this may seem morbid, the survival rate of CPR is very slim. Not to mention everything that is done in the ED can also be done in the field. When these patient’s are coming in to the ED, there needs to be a room open for them to go in. Most EDs have specific rooms that these cardiac arrest patient’s go to because they’re larger and can hold all of the equipment and staff that are needed. Because of COVID-19, there are so many people coming to the ED to get treated for symptoms or to even get tested so finding a room may not always be possible, especially if another CPR situation or other life threatening emergency is going on. There are only so many doctors and nurses in the ED, that care may also be stretched thin and the potentially unstable patient’s are not being monitored as closely.
Lastly, COVID-19 is highly infectious. 200 of FDNY’s own members are out with this disease already. If the patient with cardiac arrest has COVID-19, CPR will likely help spread the infection around. No one is certain how this disease is spreading but it is in oral mucosa and sputum and spread via contact with the patient or when the sputum becomes aerosolized (via coughing or sneezing). When doing chest compression, sputum becomes aerosolized. When suctioning how the airway, again, sputum become aerosolized. Proper PPE is not even a question at this time and most ambulances do not have enough gowns and goggles to treat the high number of calls. Not to mention that the entire ambulance has to be decontaminated, which takes a fairly long time. Eliminating transport of these patients also helps stop the spread of COVID-19 to other EMS personnel, bystanders, ED staff and ultimately other patients who are not already infected.
While it seems scary that EMS will not be transporting people with cardiac arrest, there’s a fairly appropriate reason why. NYC is already calling for healthcare workers to come help, so it eliminates thinning the staff further and preventing the spread of this disease, as well as giving priority to patients who are still alive. This order is not saying to not respond to calls, so EMS will continue to serve NYC but they need to stay healthy too, so they can serve as many people as possible and still be able to go home to their families as well.
