The current model of inpatient psychiatric care, which primarily focuses on safety and crisis stabilization, falls short in promoting sustained recovery. The prevalent emphasis on ultrashort lengths of stay often overlooks the need for comprehensive treatment plans.
A proposed model of care advocates for rapid diagnosis, goal-setting, and treatment modalities before initiating treatment, organized into three distinct phases: assessment, implementation, and resolution. This approach emphasizes individualized treatment and active patient involvement in treatment planning, addressing critical psychosocial aspects that are frequently neglected.
As we strive to reform the mental health care system, it's imperative to prioritize effective, recovery-oriented treatment strategies. This includes ensuring patient comfort and preferences are accommodated within reason. Considering patient preferences, like comfort items (such as safe stuffed animals; Share-Bears, if you will) and rescue medications (like melatonin,) is essential to upholding rigorous standards of care and safety.
Let's advocate for reforms that enhance patient-centered practices while adhering to established treatment guidelines and advancing recovery-oriented care.
Say no to “loony-bins;” those archaic relics that should be relegated to the distant past.

[URGENT] UK NHS consultation on puberty blockers
VERY SHORT DEADLINE! (Wednesday 1st November 2023)
The UK's National Health Service (NHS) are about to ban puberty blockers for all trans adolescents, except for a small group who are eligible and willing to be research subjects.
"As part of this NHS England-led process, the National Institute for Health and Care Excellence (NICE) was commissioned to review the published evidence. Overall, there was no statistically significant difference in gender incongruence, mental health, body image and psychosocial functioning in children and adolescents treated with PSH." -- 6-page introductory PDF to consultation
That's because puberty blockers don't change gender characteristics by definition, you absolute donuts.
They're carrying out a consultation on this because they have to, and there's only two days left. If you are in the UK and you are a young trans person or know any young trans people, please check out this consultation.

Things Biden and the Democrats did, this week #22
June 7-14 2024
Vice-President Harris announced that the Consumer Financial Protection Bureau is moving to remove medical debt for people's credit score. This move will improve the credit rating of 15 million Americans. Millions of Americans struggling with debt from medical expenses can't get approved for a loan for a car, to start a small business or buy a home. The new rule will improve credit scores by an average of 20 points and lead to 22,000 additional mortgages being approved every year. This comes on top of efforts by the Biden Administration to buy up and forgive medical debt. Through money in the American Rescue Plan $7 billion dollars of medical debt will be forgiven by the end of 2026. To date state and local governments have used ARP funds to buy up and forgive the debt of 3 million Americans and counting.
The EPA, Department of Agriculture, and FDA announced a joint "National Strategy for Reducing Food Loss and Waste and Recycling Organics". The Strategy aimed to cut food waste by 50% by 2030. Currently 24% of municipal solid waste in landfills is food waste, and food waste accounts for 58% of methane emissions from landfills roughly the green house gas emissions of 60 coal-fired power plants every year. This connects to $200 million the EPA already has invested in recycling, the largest investment in recycling by the federal government in 30 years. The average American family loses $1,500 ever year in spoiled food, and the strategy through better labeling, packaging, and education hopes to save people money and reduce hunger as well as the environmental impact.
President Biden signed with Ukrainian President Zelenskyy a ten-year US-Ukraine Security Agreement. The Agreement is aimed at helping Ukraine win the war against Russia, as well as help Ukraine meet the standards it will have to be ready for EU and NATO memberships. President Biden also spearheaded efforts at the G7 meeting to secure $50 billion for Ukraine from the 7 top economic nations.
HHS announced $500 million for the development of new non-injection vaccines against Covid. The money is part of Project NextGen a $5 billion program to accelerate and streamline new Covid vaccines and treatments. The investment announced this week will support a clinical trial of 10,000 people testing a vaccine in pill form. It's also supporting two vaccines administered as nasal sprays that are in earlier stages of development. The government hopes that break throughs in non-needle based vaccines for Covid might be applied to other vaccinations thus making vaccines more widely available and more easily administered.
Secretary of State Antony Blinken announced $404 million in additional humanitarian assistance for Palestinians in Gaza, the West Bank and the region. This brings the total invested by the Biden administration in the Palestinians to $1.8 billion since taking office, over $600 million since the war started in October 2023. The money will focus on safe drinking water, health care, protection, education, shelter, and psychosocial support.
The Department of the Interior announced $142 million for drought resilience and boosting water supplies. The funding will provide about 40,000 acre-feet of annual recycled water, enough to support more than 160,000 people a year. It's funding water recycling programs in California, Hawaii, Kansas, Nevada and Texas. It's also supporting 4 water desalination projects in Southern California. Desalination is proving to be an important tool used by countries with limited freshwater.
President Biden took the lead at the G7 on the Partnership for Global Infrastructure and Investment. The PGI is a global program to connect the developing world to investment in its infrastructure from the G7 nations. So far the US has invested $40 billion into the program with a goal of $200 billion by 2027. The G7 overall plans on $600 billion by 2027. There has been heavy investment in the Lobito Corridor, an economic zone that runs from Angola, through the Democratic Republic of Congo, to Zambia, the PGI has helped connect the 3 nations by rail allowing land locked Zambia and largely landlocked DRC access Angolan ports. The PGI also is investing in a $900 million solar farm in Angola. The PGI got a $5 billion dollar investment from Microsoft aimed at expanding digital access in Kenya, Indonesia, and Malaysia. The PGI's bold vision is to connect Africa and the Indian Ocean region economically through rail and transportation link as well as boost greener economic growth in the developing world and bring developing nations on-line.

Erik Erikson’s 8 stages of the life cycle
Normative Crisis — Age — Major Characteristics
Trust vs. Mistrust — 0 to 1 — Primary social interaction with mothering caretaker; oral concerns; trust or mistrust in life-sustaining care, including feeding.
Autonomy vs. Shame and Doubt — 1 to 2 — Primary social interaction with parents; toilet training; beginnings of autonomous will.
Initiative vs. Guilt — 3 to 5 — Primary social interaction with nuclear family; beginnings of oedipal feelings; development of conscience to govern initiative.
Industry vs. Inferiority — 6 to puberty — Primary social interaction outside home among peers and teachers; school-age assessment of task ability.
Identity vs. Role Confusion — Adolescence — Primary social interaction with peers, culminating in heterosexual friendship; psychological moratorium from adult commitments; identity crisis; consolidation of resolutions of previous 4 stages into coherent sense of self.
Intimacy vs. Isolation — Early adulthood — Primary social interaction in intimate relationship (usually opposite sex); adult role commitments accepted, including commitment to another person.
Generativity vs. Stagnation — Middle adulthood — Primary social concern in establishing and guiding succeeding generation; productivity and creativity.
Ego Integrity vs. Despair — Late adulthood — Primary social concern is reflective: coming to terms with one’s place in the nearly complete life cycle, and with one’s relationships with others.
Erikson’s theory is called a psycho-self-reflexive, contextual process of finding and making meaning in the world.
It is not in conflict with scientific knowledge because, on the hermeneutic account, all psychological knowledge is concerned with this sort of understanding of our world (Verstehen), and not about explaining the universal laws of nature.
His theory charted changes in personality over the entire life cycle, rather than focusing on the early childhood years as critical to all later personality functioning.
Erikson proposed 8 stages of human development, each with its own normative crisis, by which he meant not a debilitating conflict, but rather a period of heightened vulnerability and potential.
He viewed human development as occurring in orderly stages, each with its own special characteristics and its own particular age relationship.
The stages always occur in a particular order and cannot be skipped.
His stages illustrate what Erikson termed the epigenetic principle.
Briefly stated, this principle proposes that critical elements of human personality have a ground plan from which they grow, similar to the physical growth principle by which the undifferentiated cells of embryos develop in orderly ways into organ systems. Thus, all human beings will face the normative crisis of trust versus mistrust in the period of infancy.
The resolution of this crisis leaves the infant with an abiding sense of either trust or mistrust that will become part of later personality functioning.
Each stage builds upon what has gone before and carries elements of itself into future stages.
Erikson’s contributions to psychology, particularly to the study of personality, can be divided into 4 major areas:
His emphasis on the importance of the entire life cycle in personality change
The psychosocial origins of personality
The centrality of self-identity in personality development
The importance of life histories for personality study

My Dads, Dan and Phil
Dan says that he is the strict parent and Phil is the lax dad, HOWEVER, I think that they would actually be very complementary parents.
Dan would be focused on us meeting our physical and emotional needs. For example, I imagine Dan would be a stickler for a bedtime because it's important to rest and recuperate from your day. Phil would be the parents that would insist that there is nothing wrong with staying up a little later for a sweet treat.
Conversely, I feel that Phil would be the more cautious parent when it came to new and risky experiences. Dan, especially in this post-WAD world, is an advocate for living your life to the fullest and taking chances because you've only got one life to live. Phil would be the more reserved parent in this regard, wanting to protect us from preventable harms that come with some riskier life experiences.
These hypothetical father figures are the perfect mixture to meet our physical and psychosocial needs. These parenting styles are the perfect mixture to raise a mentally well, physically healthy, well-rounded individual with a killer comeback and absolutely rancid sense of humor.
Overall, my point is that we have two amazing Dad's who love and care for us and we will never be able to fully appreciate everything they have and will do for us.

As a physician, I will preach to any who will listen about the harms of punitive action for drug charges. It is in opposition to modern understandings of addiction. I work in an area where both extreme poverty and substance addiction affect far, far more than they spare. On a given day, I'd estimate that 80% of my patients struggle with a substance addiction (if I include nicotine) but about 50% struggle with an addiction to hard drugs. The harm that is done to my patients when they go to prison for it is immeasurable.
Imprisonment does nothing to curb addiction because you cannot punish psychosocial problems away. Prison places the addicted person, most of whom got to this point due to a history of mental illness or trauma, into a violent and dehumanizing environment that will exacerbate underlying stressors and mental health problems. Not to mention that medical care is rarely very good in prisons, so people who have other health issues (related to or unrelated to drug use) experience gaps in care, compounding that stress. Prison separates people from any positive support that they do have. It harms their employment prospects and places financial strain on the families, who are generally already struggling financially. It does every single thing we should not be aiming for, and to what end? How can anyone see punishment as a logical solution to a problem as complex as addiction?
Of the many things that should be done to address addiction, not a damn one involves locking addicts up. We need significant improvements in social programs, safety nets, community resources, quality medical access to general healthcare as well as addiction-specific healthcare, and other interventions that target underlying causes and facilitate immediate harm reduction.
We know more now than ever in history about addiction and how to help those struggling with it. It is a hard and oftentimes ugly problem to deal with, but locking that ugly problem behind prison bars doesn’t make it go away. Proponents of war-on-drug style approaches to addiction may consider the problem “ought of sight and out of mind,” but those of us unafraid to look at the issue head on must work twice as hard to pick up the pieces left in the wake of damaging policy. I am sick of it.

GENEVA (17 October 2023) – Continuing violent displacement and threats of further attacks on the besieged Gaza Strip pose a major public health risk and emergency, a UN expert said today after at least 12 health workers were killed and 24 Gazan health facilities were affected since the escalation of violence in the region.
“All parties to the conflict and their international partners must ensure swift and unimpeded access to essential humanitarian supplies, including food, water and medicine, fuel, and electricity,” said Tlaleng Mofokeng, the UN Special Rapporteur on the right to health. “Gaza’s medical infrastructure has been irreparably damaged and healthcare providers are working in a dire situation with limited access to medical supplies and conditions that do not allow them to provide timely and quality healthcare,” Mofokeng said.
“Humanitarian workers, physicians, civil society, human rights organisations and journalists continue to work in the region while under bombardment themselves,” the expert said.
The World Health Organization has documented more than 111 attacks on health care services in the occupied Palestinian territory, including 48 attacks on the Gaza Strip which resulted in the death of at least 12 health workers.
Mofokeng said that Israel was preventing the entry of essential supplies into Gaza, including food, water, fuel, medicines, medical disposables and equipment. “The Gaza Strip is experiencing continuous bombardment and massive devastation,” she said. “The health sector in the enclave is at a breaking point.”
Mofokeng called on the international community to intervene immediately to prevent the escalation of conflict and protect and respect the right to health of all, by demanding access through humanitarian corridors, protection of the healthcare infrastructure and health workers. Palestinian families require urgent supplies of food, water, shelter, fuel, emergency health care, psychosocial support and psychological first aid, the expert said.
The latest escalation and display of aggression in Israel and the occupied Palestinian territory must not be de-contextualized, the expert urged. It represents a crushing moment of ongoing gross structural, systemic and sustained violence experienced by Palestinians every day since the occupation, she said. “I call for an immediate ceasefire and for Member States to stop beating the drums of war,” Mofokeng said.
“The Palestinian people have been displaced for more than 75 years. The occupied Palestinian territory has been under military occupation for more than 56 years – an occupation entailing a lack of accountability, ongoing displacement and demolition, movement restrictions, and systematic racial discrimination,” she said.
The Special Rapporteur recalled that UN Member States had reaffirmed their commitment to upholding the sovereign equality of all States and respect for their territorial integrity and political independence, recognising 2019 to 2028 as the “Nelson Mandela Decade of Peace.” The declaration supports a comprehensive approach to sustaining peace by preventing conflict and addressing its root causes, she said.
“Steps to realising the right to self-determination of Palestinians is linked to their dignity and sovereignty,” Mofokeng said. “You cannot extinguish the human desire for freedom.”

The world would be very different if people magically knew how to cope because they're adults
But coping isn't innate, it's a skill that is taught and learnt, and coping is very hard work, and for many sociocultural reasons most of us actually haven't been properly or at all taught how to cope or how to regulate our emotions
Also everybody knows how to cope until something happens that is beyond what this person has had to cope with until then, and what that is can look very different depending on the person, and then the person find themselves trying to learn on the spot how to cope with it to not get crushed by it and that's super fucking difficult to do
And I am responsible for some of this because I joked about him coping poorly in the tags of my OG post about this but more seriously everybody is always just doing their best as far as coping goes
And when their best comes short the first person who hurts is themselves so if someone is coping poorly it should elicit more care and concern from their social system rather than condescension and judgment
