TV stories where the Doctor has only male companions
Because there are so few of them
We’ll start with the ones that have regular series companions:
The Massacre (1966): 1st Doctor and Steven. Yes, Dodo shows up at the very end, but that doesn’t count.
The Return of Doctor Mysterio (2016): 12th Doctor and Nardole, featuring Grant Gordon.
Yep, that’s it as far as I recall. If we expand our list to guest/recurring characters, we can include:
The Next Doctor (2008): 10th Doctor and Jackson Lake.
The End of Time (2010): 10th Doctor and Wilf, who is not a full-time companion.
Closing Time (2011): 11th Doctor and Craig. Amy is in this one, but only briefly, and not in the role of companion.
Then, if we include episodes that start with a male companion before bringing in a female companion partway through the story:
The Evil of the Daleks (1967): 2nd Doctor and Jamie, introducing Victoria.
The Wheel in Space (1968): 2nd Doctor and Jamie again, introducing Zoe.
The Keeper of Traken (1981): 4th Doctor and Adric, introducing Nyssa.
Planet of Fire (1984): 5th Doctor, Turlough, and Kamelion (man-shaped android), introducing Peri.
The Pilot (2016): 12th Doctor and Nardole, but Bill shows up pretty much immediately.
Honorable mentions:
The Ribos Operation (1978): Starts with the 4th Doctor and K-9. K-9 is voiced by a man, but he is a robot and, perhaps more importantly for the purposes of this list, a dog.
The Five Doctors (1983): There are plenty of female companions in this one, but the 2nd Doctor is initially paired up with the Brigadier.
The Two Doctors (1985): The 2nd Doctor starts the episode with Jamie before the two team up with the 6th Doctor and Peri.
The Lodger (2010): Amy is sometimes there over an earpiece, but this one is mostly the 11th Doctor and Craig.
A Christmas Carol (2010): See above, but with Kazran Sardick instead of Craig.
The Power of the Doctor (2022): This one gets a mention for having a same-sex TARDIS team for most of the story.
Interestingly, the trend of the lone female companion doesn’t start to emerge until the later years. It took nearly ten years for the Doctor to travel with a girl unchaperoned, if you count the boys from UNIT as companions. Then the show gradually transitioned to the “Doctor + girl” formula that’s the norm today.

In 1851, the Doctor (Doctor 10) arrived in England and met a man who claimed to be the Doctor and the owner of his own TARDIS. Given the knowledge the man contained, the Doctor was starting to wonder if this man might be one of his future regenerations. To complicate things further, the Cybermen were lurking about. it's midnight, time to think about Jackson Lake from "The Next Doctor" special and how much of a great (and gay) companion he could've been
he was so enamoured by the tardis, the time lords, the adventures, THE DOCTOR??? There was no fear of the adventure, he stayed behind only because it was a bit too much for him to comprehend. Otherwise, Jackson and Doctor could've been SO GOOD but the writers were COWARDS

Episode Summaries under the cut

54: The Star Beast - 60th Anniversary Special 1: A newly regenerated 14th Doctor, unsure why he has regenerated to have the same face as his 10th self, lands in London and runs into his old companion Donna Noble, her daughter Rose, and husband Shawn. He tries to avoid contact as the last time he saw Donna, he needed to erase her memory of him to save her life after her DNA was combined with his Time Lord DNA. A spaceship soon crashes, which the Doctor goes to investigate. An escape pod lands near the Noble's residence and Rose meets an alien, The Meep who claims to be on the run from evil alien warriors.
Following UNIT's tracking of the escape pod, the Doctor shows up at the Noble's house, where Donna has discovered The Meep. The Doctor gets more information from The Meep, who says that Wrath Warriors are hunting the Meep for it's fur. The house is then attacked by both Wrath Warriors and UNIT soldier's who seem to be under alien control. While the two groups fight each other, the Doctor, the Nobles, and The Meep escape, but after noticing that the Wrath Warriors weapons deal only non-lethal damage he summons two to question them about the Meep.
The Warriors reveal that Meepkind were turned into evil world conquerors after their sentient sun went insane, and they seek to bring the remaining Meep to justice. The Meep drops it's cute facade and kills the warriors. The UNIT soldiers show up under the Meep's control and take them all to the Meep's ship, which will destroy the Earth upon takeoff. They escape capture and the Doctor goes to stop the ship, but Donna sends her family off and decides to go with him. As the Doctor works on disabling the ship, the room they are in is split in half. Knowing that it will kill Donna, he reactivates her memories, allowing her to help him stop the ship.
She grows weak and accepts her death, but then Rose appears, having been affected by her mother's reactivation and also has the knowledge of a Time Lord. The Doctor realizes that by having a child, Donna was able to share the burden put on her to a more survivable level. They defeat The Meep, who is taken into custody by the Wrath Warriors. Donna and Rose are able to release the Time Lord DNA, saving both their lives while Donna is able to retain her memories. The Doctor plans to take her to see her grandfather, but she spills coffee on the TARDIS console, causing them to fly to a mystery location.

75: The Next Doctor - 2008 Christmas Special: The Doctor lands in London on Christmas Eve, 1851 and meets a man who claims to be The Doctor investigating the Cybermen, who have been responsible for many missing children and murders, starting with the murder of a man named Jackson Lake. The other Doctor claims to be a future regeneration, but he has lost much of his memory and does not remember the 10th Doctor. The Cybermen and their human accomplice, Miss Hartigan, attack a funeral, where they kill most of the attendees but keep alive the owners of several warehouses and orphanages.
While investigating with the next Doctor, he sees that his "sonic screwdriver" is just a regular screwdriver and that his TARDIS is just a hot air balloon. They search the house of one of the Cybermen's victims and discover "infostamps", devices that the Cybermen use to store information. The next Doctor remembers holding one the night he lost his memory and regenerated. The Doctor finally deduces that the next Doctor is actually Jackson Lake. The Cybermen killed his wife and stole his child, but he managed to save himself by fighting them off with a damaged infostamp about the Doctor. The information also beamed into his head, and combined with the trauma of losing his family, caused him to forget his real life and take on the persona of the Doctor.
Miss Hartigan and the Cybermen have been using the stolen children to work in factories building a giant Cyber man warship, the Cyberking. Hartigan sits the throne and takes control of it and the Cybermen and begins to attack London. Lake goes to rescue the children, including his son, from the factory, while the Doctor takes his hot air balloon TARDIS. He uses the infostamps to reverse the conversion on Hartigan's mind and allow her to see what she's become. Her emotions overwhelm the Cybermen and self-destructs the Cyberking, which the Doctor teleports to the Time Vortex to safely disintegrate.

The Wheel in Space The TARDIS materialises on board a spaceship, the Silver Carrier, where the Second Doctor and Jamie are attacked by a servo robot. Jamie contacts a nearby space station known as the Wheel and they are rescued. Meanwhile, the Silver Carrier discharges Cybermats, which also travel to and enter the station. These pave the way for the invasion of the station by Cybermen, who intend to use its direct radio link with Earth as a beacon for their invasion fleet.
The Doctor sends Jamie and a young woman named Zoe Heriot over to the Silver Carrier to fetch the TARDIS' vector generator rod. Meanwhile, he frees the Wheel's crew from the Cybermen's hypnotic control and aims to destroy all the Cybermen on the station.
When Jamie and Zoe return, the Doctor installs the rod in the station's X-ray laser, making it powerful enough to destroy the Cyber-fleet. An approaching force of space-walking Cybermen is also vanquished.
The Next Doctor Christmas 1851, and Cybermen stalk Victorian London. The Tenth Doctor discovers a spate of mysterious deaths, and he's surprised to meet another Doctor! Are two Doctors enough to stop the rise of the CyberKing?

I am having FEELINGS about The Next Doctor and Planet of the Dead that I was Not Expecting
The Doctor and Jackson Lake have a platonic connection. Jackson, because he is part Doctor has a bit of the Doctor in him, has a unique understanding of the Doctor ("All the things a Time Lord has seen, everything he's lost—he must surely have bad dreams." "...Yeah."). He can problem-solve in a Doctorish way ("Only the Doctor could come up with something like that"). He Cannot Handle the TARDIS and distances himself from it rather than stay. He declines the Doctor's invitation doesn't want to go with the Doctor. He asks the Doctor to Christmas dinner; the Doctor refuses; he asks again; the Doctor references his last companion in his response. Because she's not here. In memory of her. Fucking hell how is it this perfect I cannot. HANDLE THIS
The Doctor and Lady Christina de Souza have a romantic connection. Christina insists on following the Doctor around to get answers. She questions the Doctor's name; the Doctor responds by reiterating it; they have a back-and-forth about it. They banter. She tells him he looks human; he tells her she looks Time Lord. She displays impressive physical skill involving a rope, and swings into the Doctor's arms on said rope. She thrives on the same sort of excitement the Doctor thrives on. They flirt. They save each other's lives. She wants every day to be like this. I am SCREAMING HOW DID I NOT SEE THIS BEFORE
