Thanks for the ask 😊 you may get more than you bargained for here.... The SOC medical AU is very much a vague pipe dream idea but here's a little excerpt:
Dr. Kaz Brekker is the most capable hospitalist on the entire medicine service at St. Juris Receiving. Inej has seen it. He could manage just about anything nonsurgical in his sleep, and is up to date on the new studies before the specialists are even aware a new trial has dropped. So why he's consulting Cardiology for their post-op below the knee amputation patient with nothing but a little well-controlled a-fib, she doesn't know. It seems like a bullshit consult, but she hasn't worked a medicine ward since medical school. Who is she to say?
Inej and Dr. Brekker happen to be re-evaluating their mutual patient at the same time. They stand awkwardly in silence, just the two of them, outside the patient’s room as they wait for the Cardiologist, Dr. Hoede, to finish his interview.
Inej steels herself. She's on the case too, it's her right to know what the other teams are planning. Taking advantage of their solitude, a little jolt of anxiety rushes through her as she asks, “I saw you consulted Cardio on this patient. Maybe I'm missing something, but what was the indication for subspecialty involvement? Something with his a-fib on tele?”
She hopes it comes off more curious than adversarial. Dr. Brekker makes no indication that he's listening besides a tilt of his head and a slight lean of his cane, a sleek black rod with one liquid ink snake wrapped around it, also black as the night.
“If you'll read my progress note, it's there in the A and P,” he says tersely. It stings a bit, but he leaves no opportunity for her to explain herself. She watches as he produces a pair of black nitrile gloves from his pocket and pulls them on, an idiosyncrasy of his she's noticed. He always carries his own gloves and puts them on before he ever walks into the patient’s room.
“Cardio is doing a cath?” she asks meekly after they step back out into the hallway.
“Looks that way,” he says as he peels off his gloves. He keeps his eyes on his hands.
“Oh,” Inej says lamely. A weird silence hangs in the air as she doesn't know whether to drop the topic or further risk his ire by pushing for an explanation.
He drops the gloves in a little trash, and his gaze flicks to her briefly.
He pauses, then his gaze drifts back to her, settling on her like an ominous cloud.
“Shall I let you in on a little secret, Dr. Ghafa?” He looks down his nose at her, and for the first time ever, Inej sees the side of his mouth curl into a smile that could almost be described as glowing, conspiratorial and luminous like the low flicker of forbidden deeds in the night.
She only blinks her assent, and he continues.
“There’s a department meeting tomorrow. I have a whole lot to address, and Hoede has a penchant for inserting long-winded and irrelevant anecdotes. I hate meetings that include Hoede. Can't get anything done.” Brekker stacks his hands on the top of his cane and leans closer to her, close enough for Inej to see clearly a smooth, white scar interrupting vermilion border of his upper lip. A scar that would've never existed if, after whatever had happened to him, Inej were there to stitch that lip.
“There's only one slot in Hoede’s schedule tomorrow for a cath, and it's just before that meeting,” Brekker says. “He'll never make it.”
For Compartmentalizing, it's just a little creative writing submission to a medical journal that has a little humanities section. Might be fun to have an actual publication! If they pick it, that is:
About halfway through my intern year, I find my work starts to bleed a little. As I'm passing out after my 72nd work hour one week, I swear there are telemetry alarms sounding off in my kitchen. One night when my partner nestles his feet in my lap, my fingertips are searching out his dorsalis pedis pulses before I even realize it. And after the 4th lung cancer patient I've transferred to stepdown one week, I nearly succumb to the urge to rip the cigarette out of a stranger’s mouth.
It's pretty niche but for those that like medical AUs and the like, I hope my silly little medicine musings are entertaining.

Throwback to last week when I was on a more chill elective.
This week I’m on a different elective and struggling. It’s frustrating because it’s not as busy or as long hours as the floors but I feel like my knowledge is abysmal (cardiology has never been a very strong subject for me) and working with a new attending every day keeps me on my toes since they all want different levels of independent work and presentations and note templates and physical exam styles 🥲
Doesn’t help that my afternoon continuity clinic yesterday ended at like 6pm because even though I only had two patients (yay intern year lol) they were both scheduled late in the afternoon, showed up late, and took forever to get roomed. So after what amounted to an 8am-6pm day I just went home and rotted and forgot I needed to prep for journal club this afternoon 😓
Anyway it’s been a time… I’ll be on inpatient cardiology next week and looking forward to spending the weekend visiting family and friends, running (finally got my hands on an albuterol inhaler) and studyinggg!!!

David Diamond - Demonization and Deception in Cholesterol Research
For the past 60 years there has been a concerted effort to demonize saturated fats, found in animal products and tropical oils, and cholesterol, in our food and blood. Despite the well-established health benefits of diets rich in cholesterol and saturated fat, flawed, deceptive and biased research has created the myth that a low fat, plant-based diet is ideal for good health. Poorly conducted epidemiological research, U.S. government intervention and misinformation conveyed by contemporary lifestyle researchers have contributed to the current state of confusion on dietary influences on health. The public must educate themselves on how to optimize their diet and cardiovascular health.
David M. Diamond is a professor in the Departments of Psychology and Molecular Pharmacology and Physiology at the University of South Florida and is a Research Career Scientist at theTampa Veterans Hospital, where he has directed his research program on post-traumatic stress disorder (PTSD) and traumatic brain injury (TBI). He has also served as the Director of the USF Neuroscience Collaborative program and is a Fellow at the American Institute of Stress and the International Stress and Behavior Society.
Dr. Diamond has served on federal government study sections and committees evaluating research on the neurobiology of stress and memory, and has over 100 publications, reviews and book chapters on the brain and memory. He has served on the editorial boards of numerous medical journals and has received over 25 years of federally funded support for his research.In the past decade, Dr. Diamond has expanded his research program to include cardiovascular disease and nutrition. His controversial research is an extension of an advanced seminar he directs at the University of South Florida entitled “Myths and Deception in Medical Research”, which emphasizes the critical evaluation of methods and conflicts of interest in health-related research. In recent years he added to his list of publications controversial papers on diet, cholesterol and statins, including one paper published in the peer-reviewed medical journal “Expert Review of Clinical Pharmacology”, which described the deceptive practices employed by researchers promoting statins for the treatment of cardiovascular disease. Dr. Diamond has been invited to present his myth-busting views on nutrition and cholesterol to lay people and physicians at nutrition, cardiology, obesity and diabetes conferences all over the world.
Dr. Diamond received his Ph.D. in Biology in 1985, with a specialization in Behavioral Neuroscience, from the Center for the Neurobiology of Learning and Memory at the University of California, Irvine.

Here are my days.
5:30am: Lab comes to take blood. All of my bird veins are black, blue, mustard, indigo. If I am still sedated enough, I cry without realizing tears are rolling down my face. The exhaustion is too much to bother acknowledging crying.
7am: Shift change. My night nurse updates daytime of how low my oxygen is and to be careful with my blood pressure. She reminds them that I need plasma at an incredibly specific time to try to influence my INR numbers for the day. Adriana says goodbye for the morning and says she hopes she doesn’t see me later, but we both know I will still be here.
7:30am: Meds. Magnesium drip. Platelets. Potassium. Steroids. Anxiety medication. Muscle relaxers. Blood pressure medication. Nausea medication for the blood pressure medication. Iodine up the nose. Propanol shot down the hatch.
8:30am-ish: Breakfast. Eggs taste like plastic and I cannot have salt. My eyes are dry from my oxygen cannula blowing up in my face.
8:35am then every 15 minutes: Random person from respiratory pops their head in and encourages me to take deep breaths. I contemplate creative forms of murder. My liver hurts. I force myself to pee in a bed pan and act like my kidneys are not shriveled and my back feels like a vacuum trying to strain to urinate.
8:40am: I really do not want dialysis.
9am: Rounds. Pulmonary wants a chest tube. Respiratory wants me to “just breathe”. Hepatology is here and avoiding giving answers about my transplant. My Mom is crying. I am stoic. Nephrology gives themselves more gray hairs and stresses out over my kidneys. Cardiology says things about “shunts” and bubble tests.
I just think about inhaling and exhaling and how badly it hurts with one lung.
10am-ish: Internal medicine comes by and apologizes profusely for the disagreement between teams and why I have not been given a chest tube yet. I usually cry again here. This is the definition of being in between a rock and a hard place, but I’m circling the drain between these places.
11am: Probably lunch. Probably also gross. Probably fighting with my Mom and regretting asking for her support. I would rather be alone.
Twilight zone: This is usually the hours between doctors where I let my respiration get really low and rest. I sip on my protein shake. I nest with my squishmallows and blankets. I journal. I think about the man I truly loved and how when I die, if I die, I have to be sure to not take it to my grave how I feel. The bed feels warmer, pretending he’s here under my fall blankets and surrounded with flowers and creature comforts.
People visit after this usually. I laugh until I can’t because it hurts too badly. I feel self conscious in my pajamas with my dirty hair. I feel like I have to entertain, even though I don’t. I insist it does not make me tired, but it does - but it makes me happy.
5pm: Labs. Time to find another vein to stab. Usually a chest X-ray here too. It feels great laying on a hard board for an X-ray everyday on a ribcage that has been punctured five times in ten days. But please, I know your ribs hurt so bad. Meds. More diuretics. Have to pee every ten minutes. Hope visitors are not here, because I feel less than human peeing in a bed pan with friends here.
Somewhere through the rest of the night, my Mother will decide to remind me that I deserve to be dying. That I am ungrateful, or unappreciative, or I removed the joy from her life. I am humbled and reminded of how small I am. My nurse usually kicks her out then and brings me a cup of vanilla and chocolate ice cream, because who wants to pick?
I’ll slowly chew up my grisly dinner, take my sleeping medication, and get ready for my oxygen to plummet overnight. The bed is exploding with comforts and I know if I go in my sleep, I will be as soft as I was coming in to this world.
Then it will be September 25th, 2023, and I’ll do it all again.

Case Study: Patient Analysis The patient presents with dyspnea (difficulty breathing) and unilateral left leg pain with swelling and erythema, all of which suggest an underlying cardiovascular and cardiopulmonary condition (Chizner, 2023). Given her history of recent airplane travel, oral contraceptive use, and systemic lupus erythematosus (SLE), a likely diagnosis is deep vein thrombosis (DVT) which has contributed to a pulmonary embolism (PE). DVT is the formation of a blood clot, usually in the deep veins of the legs. Prolonged immobility during air travel, oral contraceptive use (which increases estrogen levels and clotting risk), and SLE (autoimmune disease associated with a hypercoagulable state) all increase the likelihood of DVT. The unilateral leg swelling, erythema, and pain support the diagnosis of DVT (Chizner, 2023). If the clot dislodges and travels to the lungs, it can cause a PE, which explains the patients dyspnea and increased respiratory rate. The elevated heart rate and mild fever may also be secondary signs of PE (Walke & Pohekar, 2021). Ethnic and racial factors can be affecting this case. African American women with SLE have higher risks of cardiovascular complications, including thromboembolic events, compared to other racial groups (Garg et al., 2023). There may also be genetic predispositions related to clotting for different ethnic groups. The interaction between these pathophysiologic processes will put the patient at greater risk. Her SLE induces a hypercoagulable state, and when combined with her use of oral contraceptives and prolonged immobility it further increases her likelihood of DVT. The DVT can then contribute to a life-threatening PE, affecting her cardiopulmonary system and causing the dyspnea she is experiencing.
