Elrond With A Modern Medical!Reader in Valinor

Request: Not sure if this is fic or just headcanons but: Modern!reader in middle earth who was a neurosurgeon in her old life and is now in Valinor working with Elrond to translate medical concepts to elven language before she dies and help advance their medical practice. Some differences between elven and human biology are found out in the process, it’s all quite fascinating to them both. She assists in a few operations but I imagine they don’t have many surgical operations to do in times of peace because people just aren’t getting injured like they used to during the famed battles against Morgoth and Sauron in middle earth. She helps on occasion but them elves are graceful and not injury-prone. - Anon
A/N: I was having trouble turning this into a fic, and since you gave me the option, not minding if I did a headcanon, I went with the latter. I had fun writing this, I also made their relationship ambiguous. Enjoy!

When you happen to arrive in Valinor, dazed by its eternal beauty and tranquillity, Elrond, with all his calming presence and wise demeanour was the first to greet and help you settle down. He was kind to welcome you into his new home.
During your time there, you and Elrond spend countless hours in his extensive library, trying to translate complex medical texts and concepts into Quenya, while he marvels at the intricacies of human detail in neurosurgery. To him, the ability to heal the brain by getting so practical and up close was fascinating.
“You humans certainly love your precision and details,” he would say as he smiled while reading through a description of a delicate brain surgery. You on the other hand would laugh and tell him that not all humans are so meticulous, following up your comment by sharing more medical mishaps from your world.
It is when you discover the biological differences between elves and humans, that things in the work become all the more interesting. You discover that elves’ regenerative abilities allow for healing preparations to be cut down and rushed to the healing wing, unimportant. Just knowing this, leaves you speechless as he casually mentions recovering from an injury that would have left a human incapacitated.
“So you’re telling me that you can heal from a stab wound in a matter of days?” you ask, incredulously with jealousy lingering. Elrond would simply nod along with a serene smile as he continued to translate the prewritten text on the paper you provided. “I wish I had that. Would have saved me all those trips to the ER.”
Assisting in medical practices in Valinor is rare but rewarding. Elrond’s precise, yet holistic approach to medicine complements your surgical expertise perfectly. Together, you manage to save a few elves who came in with nasty injuries, mostly from hunting trips gone bad.
As time passes, you are further blown away when Elrond teaches you their famous art of healing through song and rare herbs, enchanting your understanding of medicine. You do find the elves’ ability to enter healing trances particularly fascinating.
“So you just…sing them better? Like kumbaya and poof! Healed?!” you asked one day as you attempted to wrap your head around the concept, prompting Elrond to chuckle. “In a manner of speaking, yes.”
As time passes, your collaboration deepens both your understanding and respect for each other’s knowledge and expertise. Elrond is continually impressed but your surgical and modern techniques, while you are captivated by the elves’ natural form of advanced healing practices. This welcomed late nights in the library often turning into philosophical discussions as Elrond is thrilled by your stories of modern technology, and you are equally captivated by his tales of Middle-Earth.
You even meet a few of his family members during your stay as words of another human dwelling in Valinor. “Wait, you all fought a literal Balrog? Those fiery beasts?” you asked one evening as you sat around a table chatting with those who encountered the creature. “If you all were in my world, you could have used a fire extinguisher to put the flames out, or just douse water on them.” You leave most of them in laughter and confusion.
Due to meeting other elves beside him, you get the opportunity to practice your healing methods on them, though, they rarely allow you to since they usually appear fine even when injured. “So you couldn’t have hit your head a little harder for me to have something to examine instead of magically healing?” you disappointedly asked one of Elrond’s family members.
That has been the relationship for most of the encounters when an elf decides to come in sporting an ‘injury’. “You said you cracked your skull four days ago while hunting but I’m not seeing any injury. Do you mind if I hit you so I can have an actual injury to work with?”
Like you, even Elrond has his moments of being light-hearted, despite his dry sense of humour, when things don’t go according to plan. If the technique is too complex, he’ll jokingly say, “Of course, if all else fails, we just use magic.” Of course, you blink at him wondering if he was being serious or joking.
The partnership between you and Elrond as your work in Valinor developed certain areas of elven magical was tedious but also worthwhile. Even the relationship between you brought each other comfort and upliftment. “I suppose I’ll be remembered as the strange human who brought surgical scalpels to the Blessed Realm and threatened to beat people in their heads,” you joked.
Elrond usual response is filled with a sense of gratitude as he makes a toast in your honour. “You will be remembered as a pioneer and someone we are grateful to have encountered. I am glad you were brought here,” he fondly cheered.
Even as your time in Valinor progresses, you and Elrond continue to explore new ways to incorporate your medical practices into their elven healing. Each time an elf stops by for healing, you sometimes have to threaten them to come in with noticeable head injuries or you’d give them, while other times, you are lucky to have something to deal with. At least, during your years there, you managed to get a lot done.

Let's talk adaptation theory, because I've been seeing a lot of accusations that criticism of HotD is just "wanting it to be exactly like the books" and "book purists" not knowing what an adaptation is. So okay, let's talk about what an adaptation is, then.
I'll mostly be quoting from Linda Hutcheon's A Theory of Adaptation, because this is the first book most everyone reads when going into adaptation studies. Let's look at several ways we can approach and critique adaptation.
ADAPTATION AS INTERPRETATION
The adapted text, therefore, is not something to be reproduced, but something to be interpreted and recreated [...]
No one expects HotD to be a 1:1 reproduction of F&B. Hutcheon often compares adaptation to the process of linguistic translation, in that there will always be an inevitable loss of fidelity when translating from one language to another. However, the translator is still expected to provide an accurate representation of the source text — hence, adaptation as interpretation and recreation. Some may call this approach "fidelity criticism," an evaluation of quality based on how much the adaptation aligns with the source text.
("Fidelity criticism" is not what GRRM did. He didn't criticize the show simply because it differed from the books, and often even praises changes from the source material if it "strengthens" the impact of the work. His priority was never fidelity.)
This approach has its detractors, but there is merit to pointing out that HotD and its audience will have a difficult time interpreting and conveying F&B's message (story) if the showrunners actively take out key words (characters) and terminology (plot events). If we view adaptation as translation (from one medium to another), then the role of the adapter is to convey the intention and meaning of the source text as accurately as possible. And people do have a right to criticize "accuracy" of meaning if we see adaptation as a process of translation and remediation — which you are free not to, but some people DO come from this angle and are often dismissed as "book purists."
If you see adaptation as interpretation, are you a book purist? Perhaps, depending on what the definition of "book purist" is, but to make it clear, the people who are coming from this viewpoint clearly do not expect a blow-by-blow reproduction, and to argue that they do is dismissing a whole school of thought when it comes to adaptation.
ADAPTATION AS SUBSTITUTION
Another way to look at adaptation is through a "process of substitution." Pretty simple to understand, right? Prose that says "red dress" is substituted for an image of a white gown but with ruby embellishments, two characters are merged into one for the show, and Aemond and Aegon working together in Rook's Rest is substituted for the former betraying the latter. Your mileage may vary on whether you find these acceptable substitutions.
I believe this is the camp GRRM falls into. He brings up fidelity only insofar that he's concerned a lack of it will lead to poor and unacceptable substitutions.
How does one know if a substitution is "acceptable?" Well, I'd like to use the analogy Hutcheon brings up about surgery:
Usually adaptations, especially from long novels, mean that the adapter's job is one of subtraction or contraction; this is called "surgical art."
Good adaptations are like good surgeries: the body remains holistically intact and ideally functions better with the replacements and removals. Bad adaptations are like bad surgeries — hence the oft lobbied critique of an adaptation "butchering" the source material. The body of the adapted text cannot function on its own, being maimed or crippled by the adaptation process.
For example, the adaptational change of making Rhaenyra and Alicent the "heart" of the story has been discussed a lot by fans and critics. It was praised in the first season because it gave the story an intimate and personal "face." But it was lambasted in the second season because it actively deterred the plot progression, "crippling" the pace and stakes of the show.
In GRRM's case, his argument was that while Maelor was an unimportant part by himself, his presence was necessary for the continued function of other more vital organs. He goes on to suggest possible replacements and reprecussions upon the text as a whole. While he expresses disapproval that Maelor was removed in the first place and mentions other potentially "toxic" changes, there's also the (albeit wary) admission that Condal and his team could very still find acceptable substitutes that may stave off the damage he foresees being done to the body.
Again, this is valid criticism and a legitimate approach to HotD as an adaptation.
ADAPTATION AS AUTONOMOUS
Perhaps one way to think about unsuccessful adaptations is not in terms of infidelity to a prior text, but in terms of lack of creativity and skill to make the text one's own and autonomous.
Basically, this approach to adaptation asks, "Is the show still good by itself? Or does it fall apart without its source text and paratext (interviews, podcasts, press releases, etc.)?" This mode argues that adaptations cannot be simply sequels, prequels, or any sort of expansion of the source text. They must be separate retellings that actively evolve and mutate into a species that can survive on its own — mainly, that it adapts to a new context and audience so to speak.
A critique lobbied at the season two HotD finale was that its impact relied solely on the legacy of the prior show and the A Song of Ice and Fire mystery of who truly is The Prince That Was Promised. If the audience had no connection to Daenerys, no investment in the question of who truly was TPTWP, and never watched Game of Thrones, would Daemon's decision to finally devote himself to Rhaenyra make sense? Or does its emotional resonance rely solely on the audience's investment to another story that is not this one? Is it an adaptation of F&B or a prequel to GoT?
There's nothing wrong with it being a prequel, but if it was billed as an adaptation, then the audience has the right to feel misled because both conventional wisdom and esoteric theory agree that prequels are not adaptations. I think this is the school of thought most people subscribe to when they say HotD feels like "fanfiction" — because while fanfics CAN be written as adaptation (like modern AUs, video game novelizations, etc.), a vast majority of them are not. Most fanfics are grafted on expansions reliant on the source text for context.
This is all to say that a lot of criticism levied against the show, including GRRM's, can't be chalked up to "people not knowing what an adaptation is." There are several different ways to approach adaptation — the question is does HotD succeed in any of them?

Transformation can be a wonderful thing. It can be a terrible thing. There must, however, be a basis.

To be transformed is, generally, a mixed bag. Some individuals yearn for change, for shifting into something new, to match who they are within their soul. Others fear such a shift, a holistic destroying of what was before, never to rise again. And then, there are those who seek to enhance others, to cause that anatomical rebirth, forging something anew from the old.
There is an innate bond between the creator and the creation.
A vulnerability, the whispering knowledge that there is one that will forever know you better than yourself. The animator knew that well, the discomfort of his craft assuaged by the fact, or what should have been the fact, that their creation would never live in a way to be aware of its miniscule place in the world; of its sheer pointlessness - a cartoon.
No, it would never be aware, sentient of the futility to carve out for itself a new meaning.
That was how it was supposed to be, at least.
However, regardless if the creature was truly a doodle come to life, there were... other people irreversibly changed in this gaping abyss. The obsession with change and perfection made Henry uncomfortable, certainly, though as long as he was left alone, he was content. As content as a man in a personal Hell could be.
The prophet - seeking to change himself for the Demon, for his Lord. Eviscerated by his own ambition, comprehending his own desires too late.
The angel - seeking to change herself for herself, to become Perfection and thus destroy everything. Already thrice dead from her relentless pursuit of her own reincarnation, she continued with tools of iron and bleeding ink.
And the park maker...
The architect...
The octopus....
There was an unnerving, disquieting aspect to that silent ride as Henry entered the room.
Silent and overbearing.
The obvious trap was almost humorous what with how transparent it was, like a thin sheet of paper illuminated by a light table. Yet there was no way but forward, onward into oblivion. With a soft breath, a whispering wind in and out, Henry approached the slumbering giant. His hand reached for the audio player on the table (such a clear, unmistakable snare) and waited.
No, he felt no serious desire to actually touch it. On second thought, why bother? There was no need - he had his ax, the blade honed and sharpened to cut through metal. With ease, he could simply break through the door sealing him in this tomb. Resolved, Henry turned his back to the still sleeping hulk of metal and made his way towards the exit.
A nigh silent creak behind him had Henry gradually turn back toward the ride. There was not a single sign of any motion, however, not even a puff of dust. Apprehensive, Henry continued to edge his way to the door.
Faster than a viper, a leg of the mechanism shot forwards, whipping Henry in the back. It was sheer luck alone which kept him from slicing off his own hand. Breathless, and knocked to the floor, the old man was incapacitated swiftly, ax flung out of his grip. It embedded itself in a cart across the room, and Henry grimaced as he prepared to retrieve it. Now that the fight was inevitable, he had to defend himself or die.
Ended up being a pipe dream- or even, a dream for a pipe with which to fend off the arms that assailed him. Instantaneously, he was swarmed, an uncomfortable grip that lifted him clear off the ground. Eventually, he found himself tumbling into one of the seats, nauseated and off kilter. The brass bar kept him in place as he was dizzyingly pitched forward to the front of the ride.
The doors hiding that waterlogged face swung forward to greet him, Henry lurching back as it felt as though they were eager to crack open his skull.
"If it isn't Henry!" the hulking mass cried, the bloated head staring at him unblinkingly. "I'm so very glad to see you, old friend. Dear me, you look worse for wear!"
Henry did not reply, only stared silently back at Bertrum. The bodiless man's smile, that of an electrocuted man, did not fade.
"It's been some time since I've worked on an upgrade," the rideman mused, bringing Henry closer for inspection. "I doubt that I've gotten rusty, though. Let's give it a shot, eh?"
The arm holding Henry spun around rapidly, then twisted to the side- all the way upside down. It deposited him atop Bertrum's tape player as he tumbled out, knocking the device to the ground as he tried to scramble away and escape. To prevent that, the bars on two carts cinched tight over his wrists, nearly cutting off the circulation. A quiet grunt of pain escaped him, converted into a huff of air as he was slammed back onto the table. A screeching, creaking groan followed the table being dragged across the room towards Bertrum. The man smiled down at him, with that empty, blown-eyed gaze.
"My, time has been kind to you, hasn't it?" Bertrum murmured, a sort of delighted lilt to his tone. Henry kicked and tried to use his legs as leverage to escape, but the bars pressed tighter, pain lancing up the center of his wrist, skyrocketing up his arm to the nerves in his elbow. A half-choked whimper of pain, and he went limp (yet uncomfortable) in the pinching grip. "Your muscle structure appears to be in remarkable shape, and your stamina is rather unyielding from what I've seen. Yes, you would make a fine basis for a roboticized specimen."
Henry decided that he very much did not like what Bertrum was suggesting.
With a renewed effort, he attempted to break free vigorously, only to gasp with the sharp pain of one of his wrists fracturing under the pressure of metal winches. Horrified, his head swiveled to face the limb, seeing red oozing into his flesh- the burst vein seeping through his body. The nausea from before resurfaced violently, swallowing down the need to retch. 
“It’s been quite some time since I’ve done this,” Bertrum hummed, raising another limb- and to Henry’s rapidly increasing horror, he saw the ax tucked between two carts. Without thinking, Henry desperately tried to escape once more. A cart pressed on his chest, pushing him back down, gradually winding him as it crushed his lungs. He gasped, feeling his ribs creaking under the force of pressure. “If you would stop squirming, Henry, it would be much easier for the both of us, you know.”

He tried to reply, but the cart pressed down once more and winded him. Then, he held his breath unwittingly as the ax pressed to his sternum, directly above his solar plexus. Oh, hell no. If there was one thing worse than a non consensual surgery without anesthetics, it was non consensual surgery without anesthetics by a man turned octopus ride with an extremely limited range of motion and practically no precision at all. 
The ax cut surprisingly straight for those facts, tearing through fabric, skin, bone, and flesh with ease- just as Henry knew it could. Blood welled up in the cavity, spilling out over his side. It was both hot and cold at the same instant, the chills of blood loss shock hitting him- especially with his broken hand; and the lava-like warmth of his ichor. Henry ceased any and all attempts at escaping, understanding now that it was futile. 
Bertrum used a corner of one of the carts to spread open the wound left, the bulky mass pressing now directly on Henry’s lung. It was electrified ice along his exposed veins. 
A giant hydraulic made itself visible, gripped to the point of cracking between another pair of carts. Henry stared at it, unable to even muster any horror as the cloudy shock took over. The hydraulic was carefully placed at the space made in Henry’s chest, not quite a cavity- yet. The pressure of the metal entering his body was unbearable, forcing a space for a huge, heavy cylinder that did not belong in his flesh. 
Bertrum, losing patience, slammed the cart onto the hydraulic. Henry’s vision went black as it jammed fully within his corpus. The ringing in his ears only slightly faded as the ax pressed against his arm. Henry was far too delirious in pain to fight as Bertrum slowly tore into each of his limbs, replacing the bones with those massive hydraulics. Blood splattered along the ground, pooling around the table. Henry could only hear the crack of his bones, the dripping of blood, his own heartbeat in his ears, his groans of pain, and Bertrum’s quiet humming. 
Cut, open, hydraulic. Cut, open, hydraulic. 
Eventually, each and every bone in Henry’s body was replaced. Against his will, he stood. 
“Behold!” Bertrum announced to the void. “The animating automaton!”
The husk of Henry stood, without paper nor pen to draw with, a silent and unmoving exhibit forevermore.

Values and principles of the Health at Every Size model
“Health At Every Size” is often taken at face value; misinterpreted to mean “all fat people can and should be healthy.” This is NOT what HAES is. Here’s a quick primer on the values and principles of the HAES model.
1. Weight inclusivity
Respecting and appreciating the inherent diversity of body shapes and sizes and rejecting the idealizing or pathologizing of specific weights
2. Health enhancement
Supporting health policies that improve and equalize access to information and services, and personal practices that improve human well-being, including attention to physical, mental, social, spiritual, economic, and environmental health and well-being for individuals and communities
3. Respectful care
Acknowledging our biases, and working to end weight discrimination, weight stigma, and weight bias, and providing information and services from an understanding that socioeconomic status, race, gender, sexual orientation, age, and other identities impact weight stigma, and support environments that address these inequities
4. Eating well
Promoting flexible, individualized eating based on internal cues of hunger, satiety, pleasure, appetite, and individual nutritional needs, rather than any externally regulated eating plan or diet for weight control
5. Life-enhancing movement
Supporting appropriate, life-enhancing physical activities that allow people of all sizes, ability, and interests to engage in enjoyable movement, to the degree they choose, rather than any externally regulated activity plan for weight control
6. Decouple health and weight
Advocating that health and well-being cannot be defined by body mass index, body weight, waist circumference, or percentage body fat
7. No body assumptions
Not assuming that a person’s body size, weight, or body mass index is evidence of a particular way of eating, physical activity level, personality, psychological state, moral character or health status
8. Oppose the pursuit of deliberate weight loss
Opposing the use of dieting, drugs, programs, products, or surgery for the primary purpose of weight loss
9. Challenge body size oppression
Challenging any form of oppression including bias, exploitation, marginalization, discrimination, powerlessness, cultural imperialism, harassment, or violence against people based on their body image, body size, or weight; and any approach to health, well-being, eating, or exercise, and any products, services, or amenities which perpetuate body size oppression
10. Reject healthism
Opposing healthism as an ideology in which individuals are deemed to have primary responsibility for their health are morally obliged to pursue the goal of perfect health, and are personally blamed if they get sick
The HAES model thus supports policies, processes, and environments that enhance the holistic health and well-being of people of all shapes and sizes. It does not propose that people are automatically healthy at any size, but that all people deserve fair access to opportunities and environmental conditions that will enhance their health and well-being, irrespective of their body size. 
Given its explicit focus on equity, the HAES model can be defined as ‘a social justice approach to improving the actions, conditions of living and environmental supports for the health and well-being of people at every size.’ Although it includes actions that can be taken by individuals (intuitive eating, joyful physical activity, body positivity), as a critical health promotion model (Gregg & O’Hara, 2007), HAES recognizes the significant limitations of a purely behavioral approach, and instead urges a focus on the social, economic, political, and physical environments that play a far more important role as immediate and underlying determinants of health and well-being. The HAES model includes values and principles that it supports as well as those it explicitly opposes (Association for Size Diversity and Health, 2014; Bacon, 2006, 2010; Gard, 2009; Kater, 2004; Robison & Carrier, 2004; Tylka et al., 2014).
In the past 15 years, several studies have investigated the impact of HAES programs on various health and well-being indicators. Two systematic reviews (Bacon & Aphramor, 2011; Clifford et al., 2015) and a subsequent randomized-controlled trial (Mensinger, Calogero, & Tylka, 2016) have demonstrated that the HAES approach was more effective in improving various aspects of physiological, psychological, and behavioral factors than usual care or alternative treatment options. Other studies have shown that HAES curricula at the school and university levels led to improved body image, self-esteem, and eating attitudes in children (Kater, Rohwer, & Londre, 2002; Niide, Davis, Tse, & Harrigan, 2013); intuitive eating, body esteem, anti-fat attitudes, and dieting behaviors in university students (Humphrey, Clifford, & Morris, 2015); and knowledge, attitudes, beliefs, and skills in teachers (Shelley, O’Hara, & Gregg, 2010). Such evidence suggests that the HAES model is superior to the weight-centric approach at improving a broad range of health parameters. The HAES model responds to the critique of the WCHP and provides an evidence-based opportunity for significant paradigm shift (Barker, 1993; Kuhn, 1970).

The decision comes after an independent review of services for children under 18 and a sharp rise in referrals to the Gender Identity Development Service run by the Tavistock and Portman NHS Foundation Trust, which is closing at the end of March.
Children will no longer be prescribed puberty blockers at gender identity clinics, NHS England has confirmed.
Puberty blockers, which pause the physical changes of puberty such as breast development or facial hair, will now only be available to children as part of clinical research trials.
The government said it welcomed the "landmark decision", adding it would help ensure care is based on evidence and is in the "best interests of the child".
It follows a public consultation on the issue and an interim policy, and comes after NHS England commissioned an independent review of gender identity services for children under 18 in 2020.
The review followed a sharp rise in referrals to the Gender Identity Development Service (GIDS) - a specialised service for young people who experience difficulties in the development of their gender identity - run by the Tavistock and Portman NHS Foundation Trust, which is closing at the end of March following repeated scrutiny.
In 2021/22, there were more than 5,000 referrals to GIDS, compared to just under 250 a decade earlier.
Dr Hilary Cass, who led the review, published an interim report in February 2022 saying there was a need to move away from one unit and recommended regional options be available to better support children.
She also said there was a lack of long-term evidence on what happens to young people prescribed blockers - adding that GIDS had not gathered routine and consistent data, meaning it was "not possible to accurately track the outcomes and pathways that children and young people take through the service".
After Tavistock closes, two new NHS services will open in early April, situated in Great Ormond Street Hospital, London, and Alder Hey Children's Hospital in Liverpool.
The NHS said children attending these clinics will be supported by experts in neurodiversity, paediatrics and mental health, "resulting in a holistic approach to care".
Around 5,000 children and young people are currently on the waiting list for referral into the new clinics, with 250 patients expected to be transferred to them when they are open.
Currently there are fewer than 100 children on puberty blockers, who will continue their treatment at Leeds and University College London Hospital.
Puberty blockers can be used to delay the development of physical characteristics which can make someone look male or female, allowing transgender young people to explore their gender identity and weigh up medically transitioning.
Taking them early in puberty may mean less treatment or surgery in the future. However, critics have raised concerns over issues including consent, mental health risks and bone density development.
Health Minister Maria Caulfield said: "We have always been clear that children's safety and wellbeing is paramount, so we welcome this landmark decision by the NHS.
"Ending the routine prescription of puberty blockers will help ensure that care is based on evidence, expert clinical opinion and is in the best interests of the child."
The consultation on the future of such services received more than 4,000 responses. Around a quarter were from members of the public, while 22% were from patients, 21% from parents, 10% from trans adults and 5% from clinicians.
John Stewart, NHS England's national director of specialised commissioning, said the responses were "polarised" in line with the debate around puberty blockers.
Mr Stewart said: "Many people said the policy didn't go far enough in terms of still allowing potential access [to puberty blockers] through research, and others saying clearly they disagreed fundamentally and that these should be routinely available to everyone who believes they need it."
Transgender youth charity Mermaids described the announcement as "deeply disappointing" and accused the NHS of "failing trans youth".
A spokesperson added: "Those currently prescribed puberty blockers won't see any changes to their treatment, and this is a pause on prescribing - not a ban.
"It's also important to note that puberty blockers can be just one possible part of a young person's gender journey. However, this news still comes as a blow and will deeply affect our communities."
Former prime minister Liz Truss "welcomed" NHS England's decision ahead of her Health and Equality Acts (Amendment) Bill, which is up for its second reading on Friday.
The bill includes a ban on the prescription of body-altering hormones to children questioning their sex, both privately and on the NHS.
The most commonly used puberty blockers suppress the production of hormones, including testosterone and oestrogen.
NHS England hopes to have a study into their use in place by December - with the eligibility criteria yet to be decided.

In January 2020, a Policy Working Group (PWG) was established by NHS England to undertake a review of the published evidence. As part of this process, the National Institute for Health and Care Excellence (NICE) was commissioned to review the published evidence on Gonadotrophin Releasing Hormone Analogues (GnRHa). Nine observational studies were included in the evidence review (NICE 2020). Overall, there was no statistically significant difference in gender dysphoria, mental health, body image and psychosocial functioning in children and adolescents treated with GnRHa (2020). The quality of evidence for all these outcomes was assessed as very low certainty using modified GRADE. There remains limited short-term and long-term safety data for GnRHa.

The primary intervention focuses on psychosocial and psychological support; for some individuals, the use of PSH [puberty suppressing hormones] in adolescence to suppress puberty has previously been a treatment option though no NHS clinical commissioning policy has been in place; this may be followed later with gender-affirming hormones of the desired sex (NHS England, 2013). If individuals fulfil additional criteria, they may have various types of gender affirming surgery from the age of 18 years through adult Gender Dysphoria Clinics (NHS England, 2013).
What we have decided
NHS England has carefully considered the evidence review conducted by NICE (2020) and has identified and reviewed any further published evidence available to date. We have concluded that there is not enough evidence to support the safety or clinical effectiveness of PSH to make the treatment routinely available at this time.
