A character who's been shot is captured by enemies/bad guys. They're bleeding badly, and their captors need them alive. The bullet is removed, and the wound is poured with alcohol and bandaged tightly - none of it is done gently, and, being tied up (and maybe gagged), they can do nothing but scream and cry out in pain while enduring the rough treatment.

Okay, hear me out....
A caretaker with an extremely conditioned whumpee.
Caretaker is forced to "speak their language" to get through to whumpee. Caretaker has to start acting like Whumper to slowly undo the conditioning.
They have to start harsh and strict to slowly teach Whumpee what freedom is, or reintroduce them into their own agency.
Maybe Whumpee was a slave or a pet, and all they know are orders and discipline and reward. Before Caretaker figures this out, Whumpee is listless or catatonic, or maybe self-destructive and reckless. Without order and routine, Whumpee has no idea how to function, their mind is sent into chaos.
Caretaker has no interest in being a master, but to get through the conditioning, to even get them medical help, they are forced to get their hands dirty.... Orders are just necessary- "you're going to the hospital, that's an order" or "I order you to decide on what you want for dinner."
Caretaker never wanted this power and responsibility over another person.
With power, there is also the risk of corruption.
Maybe Caretaker is terrified about walking the fine line between helping their severely damaged and twisted whumpee and becoming the new whumper.
Maybe they get it right, maybe they get it wrong, maybe it's something in between, there's so many directions this could go and all of the threads are so interesting.

"You'll never hurt anyone again."
Caretaker captures Whumper and conditions them to associate the sight of blood, wounds or sounds indicating any kind of hurt (screaming, yelping, whimpering, begging, etc) with pain. This also means they’re not just scared of hurting others, but they hate looking at their own wounds and avoid making noise when they are hurt because the sounds upset them.

Thinking of Character A and B who have a troubled, dysfunctional relationship, who genuinely do care about each other's wellbeing and want to help each other but really struggle to gel well together.
Thinking about Character A relentlessly stuffing down all their feelings about every argument, every stressor, every infuriating thing B does/says, because taking care of them is more important than getting along, until something finally tips them over the edge and in a moment of blind rage they're a lot more like a whumper.
Thinking about afterward too. The first time these two abrasive personalities shrink, one shocked and scared, the other just as shocked at themself and ashamed. B knew they had their troubles but they didn't realize A hated them, much less that much. And A floundering to backpedal and fix it because they don't, they don't hate them, but is there really any coming back from what just happened?

When Caretaker isn't a good person.
Well, in some ways they are. Whumpee has seen the way they care for them. And those restless nights where Caretaker stood besides them, comforting them and promising that everything would be fine.
But Whumpee knows.
Even if Caretaker tries to hide it.
Those weird noises coming from the basement, the scent of blood coming from Caretaker, some days filling the house.
Caretaker isn't a good person. But Whumpee is willing to ignore that if that means staying on the good side of Caretaker.

Whumper was interrogated while caretaker watched from beyond the glass. Whumper gleefully spoke about everyone they tortured, recalling in great detail. Whumpee in particular was a prominent subject; they seemed to be whumpers favorite.
Each word ate at caretaker; whatever whumper spoke of, caretaker imagined a worse thing to do as revenge.
The interrogator comes back and caretaker demands "just 5 minutes alone" through fuming tears. The interrogator notes caretakers hands in fists, their body shaking with anger, their eyes pure hatred.
"No." They gently shake their head.
"Why." Caretaker spat.
"Because I know what you can do in 5 minutes and I need them alive."

a caretaker turned whumper clipping their whumpee’s wings. No matter how loud or how many times whumpee begged caretaker to stop, telling them they were hurting them, caretaker just continued.
after caretaker clipped their whumpee’s wings, they just held them close no matter how hard whumpee tried to push away.
“I’m sorry. I’m so sorry, whumpee…” Caretaker quietly apologized over whumpee’s sobs, “but I have to do this…it’s the only way I can protect you…I can’t lose you. I won’t.”
caretaker hugged whumpee closer to them as whumpee’s sobs got louder.

Everything thinks Whumpee is dead, but they're just in a heavily/magically induced sleep. Whumpee's friends decide to cremate their body. Whumpee wakes up in the chamber they're being burned in and scream to get out, pounding on the case.
Bonus points if they have some form of immortality and they are unable to die, just going through it until Whumpee's friends realize what happened. The trust there is forever strained.

Whumper kidnaps Whumpee and tells Caretaker to come save them.
Simple, right?
NOPE. Caretaker does come... and they proceed to go after Whumper and torture them for what they have done to Whumpee, to the point that whatever Whumper had done to their captive was much nicer in comparison to what Caretaker would do to them.
BONUS: Whumpee is either amazed by how strong Caretaker is, or terrified that Caretaker could go that far.

Whumper tried anything to get on Caretaker's good side.
They promised to be better, to never hurt Whumpee or anyone again, and still Caretaker doesn't believe them.
Whumper knows that what they did was wrong, and wants to be better, to maybe have another chance to prove they can be good. But Caretaker doesn't even let them.
Whumper has to do anything Caretaker says, obey every order and behave, and that's not hard. Whumper can do that.
But soon, those simple orders like "don't raise your voice", "no fighting"; turn into "don't speak unless spoken to", "have you really earned today's meal?", "there's someone I don't like, take care of them". And Whumper wants to believe that's just so they can be good, but it isn't.
No matter how much they try, Caretaker's orders start to get to a whole new level. They can't even stop hurting people, because that's what Caretaker wants, and everything seems to be getting worse.
"Don't tell me you are feeling sad?"
"Stop crying. You didn't cry when Whumpee was hurt."
"Get up and do what I say. Don't you wanna be better?"
They do. They want to be better so bad.

CW: Vengful caretaker, whumper turned whumpee
Whumper was chained in a room kept in pure darkness. Caretaker abruptly flicked the light on, causing whumper to wince.
"Good morning." Caretaker said flatly, shuffling through a handful of papers. "Not that you know it's morning. You don't know anything."
"Wh-.... What do you w-want now..." Whumper hissed, spitting on the floor that had a tint of blood. 
"Just to update you on our little favorite. I'm about to bring whumpee home from the hospital. Were all so very excited." Caretaker smiled, they held up a picture of them and whumpee together. Whumpee was smiling a small simple smile; there was a healing bruise on their face almost faded, amongst other cuts and wounds.
"Look at them, precious aren't they? They've come a long way since-" they side-eyed whumper up and down with a fallen expression.
-Well, since you." They stared.
"If you want revenge, then take it already! I'm sitting right here..." Whumper scouled.
"Well I intend to." Caretaker leaned in closely as whumper reeled. "I'm undoing everything you did, break everything you built, rebuild everything you broke; and you're going to watch it all." Caretaker held the photo close to their chest tight enough it crinkled. 
"Then I'm going to take revenge. You can die knowing you achieved nothing."

The caretaker had always been protective of the whumpee, and they had hurt others in order to protect the whumpee plenty of times before. The whumper had gone too far though, and the caretaker was furious- they were going to hurt the whumper beyond what they had ever done before.

whumpee being taken off of feel good bliss drugs after being rescued but having the potential to such severe withdrawals that the doctors literally prescribe the whumpee with the same drug, just lowering in dosage each time so their body doesn’t go into shock.
caretaker is responsible for administration and whumpee has to deal with the fact that caretaker has to watch them go into the same messed up headspace they were in whilst capture and is ashamed, embarrassed, and mortified
tw bad caretaker/caretaker turned whumper, dubcon drugging (syringe), betrayal, manipulation, gaslighting, addiction
Whumpee couldn’t help squirming when Caretaker walked up to them with the needle. “Do you think– do you think I could start doing it myself? The dosage is pretty low by now, isn’t it?”
Caretaker gave them a sad smile. “I’m sorry, the doctor said it has to be administered by someone else to avoid–”
“Me abusing it. Yeah. I get it.” They shifted again, hesitant to put their arm out. “I just… I wouldn’t do it, you know. And I know how to… to find the vein.”
“Whumpee, it’s okay. Nobody’s judging you.”
“I’m judging me. I hate that you have to see me like this. It’s messed up. It’s embarrassing.”
Caretaker sighed. “I’m sorry. But I don’t find it embarrassing at all, it’s… it’s just medicine, yeah? I’ve never once looked at you and thought– well, anything negative. I hate that you’re going through this, but I’m just happy to be able to help, y’know?”
“Yeah,” they muttered, finally extending an arm so Caretaker could stab them for the hundredth time. “I guess there’s no way around it.”
“This doesn’t change my opinion of you. At all. I love you a lot, and I want to keep you safe.”
Whumpee nodded mutely, watching anxiously as Caretaker took their hand in their own. They couldn’t shake the thought that the substance in the syringe looked… exactly the same as the first day. It looked the same, it wasn’t less, it never felt any different, and it made Whumpee feel like they weren’t making progress at all.
“Can I see the box later?” they asked while Caretaker sanitised the area.
“Hm?”
“The box the meds come in. I’m not asking you to show me the meds, just, just the box– so I know where I am in terms of dosage.”
“Oh, I always throw the box away,” they said with an apologetic look. “I keep them in a different box, so it’s not super easy to spot. Sorry.”
Whumpee swallowed. So there was no way for them to keep track of that either. Great. “It’s fine,” they mumbled.
“You shouldn’t rush it.” They pushed the needle inside, and Whumpee hissed in pain before the feel-good chemicals invaded their mind once again. They could’ve sworn they saw Caretaker smile at the whimper they let out. “It’s doctor’s orders, after all.”
