Although the affected area may change in appearance within 24 hours, the rash usually settles within a few days.
Doctors may refer to urticaria as either:
• acute urticaria– if the rash clears completely within 6 weeks
• chronic urticaria –in rarer cases, where the rash persists or comes and goes for more than 6 weeks, often over many years
A much rarer type of urticaria, known as urticaria vasculitis, can cause blood vessels inside the skin to become inflamed. In these cases, the weals last longer than 24 hours, are more painful, and can leave a bruise.

Visit your doctor/ General Practitioner (GP) if your symptoms don't go away within 48 hours.
You should also contact your doctor / GP if your symptoms are:
• severe
• causing distress
• disrupting daily activities

Who's affected by urticaria?
Acute urticaria (also known as short-term urticaria) is a common condition, estimated to affect around 1in 5 people at some point in their lives.
Children are often affected by the condition, as well as women aged 30 to 60, and people with a history of allergies.
What causes urticaria?
Urticaria occurs when a trigger causes high levels of histamine and other chemical messengers to be released in the skin.These substances cause the blood vessels in the affected area of skin to open up (often resulting in redness or pinkness) and become leaky. This extra fluid in the tissues causes swelling and itchiness.
Histamine is released for many reasons, including:
• an allergic reaction – such as a food allergy or a reaction to an insect bite or sting
• cold or heat exposure
• infection – such as a cold
• certain medications – such as non-steroidal anti-inflammatory drugs (NSAIDs)or antibiotics
However, in many cases of urticaria, no obvious cause can be found.
Some cases of long-term urticaria may be caused by the immune system mistakenly attacking healthy tissue. However, this is difficult to diagnose and the treatment options are the same.
Certain triggers may also make the symptoms worse. These include:
• drinking alcohol or caffeine
• emotional stress
• warm temperature

Diagnosing urticaria
Your GP will usually be able to diagnose urticaria by examining the rash. They may also ask you questions to find out what triggered your symptoms.
If your GP thinks that it's caused by an allergic reaction, you may be referred to an allergy clinic for an allergy test. However, if you've had urticaria most days for more than 6 weeks, it's unlikely to be the result of an allergy.
You may also be referred for a number of tests, including a full blood count (FBC), to find out whether there's an underlying cause of your symptoms.

Treating urticaria
In many cases, treatment isn't needed for urticaria, because the rash often gets better within a few days.
If the itchiness is causing you discomfort, antihistamines can help. Antihistamines are available over the counter at pharmacies – speak to your pharmacist for advice.
A short course of steroid tablets (oral corticosteroids) may occasionally be needed for more severe cases of urticaria.
If you have persistent urticaria, you may be referred to a skin specialist (dermatologist). Treatment usually involves medication to relieve the symptoms, while identifying and avoiding potential triggers.

Complications of urticaria
Around a quarter of people with acute urticaria and half of people with chronic urticaria also develop angioedema, which is a deeper swelling of tissues.
Chronic urticaria can also be upsetting and negatively impact a person's mood and quality of life.
Angioedema
Angioedema is swelling in the deeper layers of a person's skin. It's often severe and is caused by a build-up of fluid. The symptoms of angioedema can affect any part of the body, but usually affect the:
• eyes
• lips
• genitals
• hands
• feet

They carry a clear fluid called lymph. Lymph contains tissue fluid, waste products, and immune system cells. Lymph nodes are small, bean-shaped clumps of immune system cells. They are connected by lymphatic vessels. They contain white blood cells that trap viruses, bacteria, and other invaders, including cancer cells.
White blood cells are the cells of the immune system. They are made in one of your lymph organs such as the spleen or thymus, and in the bone marrow.
When your immune system doesn't work the way it should, it is called an immune system disorder. You may:
• Be born with a weak immune system. This is called primary immune deficiency.
• Get a disease that weakens your immune system. This is called acquired immune deficiency.
• Have an immune system that is too active. This may happen with an allergic reaction.
• Have an immune system that turns against you. This is called autoimmune disease.
Immune system disorders
Here are some common examples:
• Severe combined immunodeficiency (SCID). This is an example of an immune deficiency that is present at birth. Children are in constant danger of infections from bacteria, viruses, and fungi. This disorder is sometimes called “bubble boy disease.” In the 1970s, a boy had to live in a sterile environment inside a plastic bubble. Children with SCID are missing important white blood cells. This disease is deadly without treatment.
• Temporary acquired immune deficiencies. Your immune system can be weakened by certain medicines, for example. This can happen to people on chemotherapy or other medicines used to treat cancer. It can also happen to people after organ transplants who take medicine to prevent organ rejection. Also, infections such as the flu virus, mono (mononucleosis), and measles can weaken the immune system for a short time. Your immune system can also be weakened by smoking, alcohol, and poor nutrition.
• AIDS. HIV, which causes AIDS, is an acquired viral infection that destroys important white blood cells and weakens the immune system. People with HIV/AIDS can become seriously ill with infections that most people can fight off. These infections are called “opportunistic infections” because they take advantage of weak immune systems.
An overactive immune system
If you are born with certain genes, your immune system may react to substances in the environment that are normally harmless. These substances are called allergens. Having an allergic reaction is the most common example of an overactive immune system. Dust, mold, pollen, and foods are examples of allergens.
Some conditions caused by an overactive immune system are:
• Asthma. The response in your lungs can cause coughing, wheezing, and trouble breathing. Asthma can be triggered by common allergens like dust or pollen or by an irritant like tobacco smoke.
• Eczema. An allergen causes an itchy rash known as atopic dermatitis.
• Hay fever (allergic rhinitis). Sneezing, a runny nose, sniffling, and swelling of your nasal passages from indoor allergens such as dust and pets or outdoor allergens such as pollens or molds.
• Food allergy. Severe allergic reactions that are caused by foods.
Autoimmune disease
In autoimmune diseases, the body attacks normal, healthy tissues. Experts don't know why. It is likely a combination of a person’s genes and something in the environment that triggers those genes.
Three common autoimmune diseases are:
• Type 1 diabetes. The immune system attacks the cells in the pancreas that make insulin. Insulin removes sugar from the blood to use as energy.
• Rheumatoid arthritis. This type of arthritis causes swelling and deformities of the joints. An auto-antibody called rheumatoid factor is in the blood of some people with rheumatoid arthritis.
• Lupus. This disease attacks body tissues, including the lungs, kidneys, and skin. Many types of auto-antibodies are found in the blood of people with lupus.
