Hi! I don’t think this is cross any boundaries but, regardless of if it is, feel free to not respond and delete this if you want to

My doctor suggested I might be intersex and I’m struggling to find out information (e.g., if I count/can be considered— for a lack of a better way to phrase it— intersex, more about it, etc). Do you have any suggestions on how to research this and/or any resources?

(Side-note: want to emphasis that I’m not asking for a diagnosis, just like tips and resources and whatnot to do my own research)

For context (if needed): I have hypoandrogenism/testosterone deficiency (that’s congenital) and I was AFAB (I’m also intolerant to my body’s default levels of oestrogen and progesterone, if that’s relevant?). Most of the stuff I found so far are like “women with hyperandrogegism” or “men with hypoandrogenism” which isn’t too relevant to me— I’ve seen mentions of “female androgen insufficiency syndrome”…but not much information or discussion about it

It seems like based on what you've shared and what your doctor has shared with you, that you have a form of hypogonadism. There are a lot of different intersex variations that are based on hypogonadism rather than hyper. I'll list a few intersex variations out that might be what you and your doctor have been talking about!

The most relevant one and what some people call "female androgen insufficiency syndrome" is estrogen insensitivity syndrome! It's also called estrogen resistance syndrome. With this, you can have XX chromosomes, vulva, vagina, and uterus, and ovaries. However, your body does not respond to estrogen despite producing high levels of estrogen. I haven't done enough research to know if this can also be comorbid with the intersex variations that cause testosterone deficiency, but from my limited search, it seems like it could be possible.

Aromatase Deficiency. With Aromatase Deficiency, people are born without the enzyme that converts androgens into estrogen. People with Aromatase Deficiency have XX chromosomes and usually have ovaries, uterus, vulva, and vagina, but might have larger clitoris. People might have more of a testosterone based secondary sex characteristics at puberty.

17-beta Hydroxysteroid Dehydrogenase 3 Deficiency. With 17 beta HSD, people don't have one of the enzymes they need to process and synthesize testosterone. With this variation, people have testes and XY chromosomes, but are often born with a vagina and vulva. People with this variation usually develop some testosterone influenced secondary sex characteristics at puberty.

Kallmann syndrome. This one is rarer, but is another form of hypogonadism. The key symptom for this diagnosis is that you don't have a sense of smell, so if that's something that's true for you, it might be relevant.

I'm a trans woman wondering if I'm intersex, and wanted some advice on what I should look into.

I currently don't know what my hormone levels were before starting HRT, but during it my T levels were always below 0.4 nmol/L (11.5 ng/dL) and I was set on a very low dose of anti androgens (which i probably didnt even need, in hindsight). My estrogen levels have also been on the low side, so the low T is not due to high estrogen. I'm currently post bottom surgery, and my T levels are so low I'm being recommended to start on a small amount of T in addition to the E I take.

My first puberty (the testosteron driven one), started around 13-14yr, and I was shorten than other boys. I'm suspecting I might have had a micropenis, although it's difficult to know / remember as I am post op. It also caused a problem during surgery, as there was not enough tissue for penile inversion. The genitals were not otherwise ambiguous.

I am short (5' 5") and have normal body proportions, so I don't think I have Klinefelter Syndrom.

Are there any test you can recommend? Also, do you know which other conditions causes low T levels?
