Adrien has disordered eating caused, and disguised, by the strict diet his father has him on. He gets intensely anxious whenever eating outside of the strict parameters he's had set for him for years, which was never actually a problem before because it was designed by a dietitian to maintain a specific healthy weight, and it accounted for scheduled activities like basketball or fencing.

Then he became Chat Noir, and then it became a problem, because the strict diet certainly didn't account for all the calories burned as a superhero. And, unlike Félix, he doesn't sneak more food to account for it. Pushing him to do so gets a very defensive reaction.

Generally, Félix can cajole him to eating a least a little more- usually just getting him with sweets because despite all this, he has a sweet tooth, but it's rough at points, and he's gradually losing weight as Chat, which is not good.

But is also pretty normal, amongst models, so very few people ever question it, and Adrien is inclined to dismiss it and deflect whenever someone worries because he's grown up around all that.

He's not out here eating tissues to avoid eating like some other models he knows, so he's fine, really, he insists. It’s not like he actually has any issues with how he looks, so he couldn’t possibly have an eating disorder-

Even once an adult and no longer modeling, Adrien holds himself to the diet and panics if he feels like he's failing at that. Even though, rationally, he knows that there's literally no consequences for gaining weight anymore. It's not really until that point that Adrien realizes that he kinda maybe has a problem.

To be clear; I’m not saying his anorexic. He’s literally not. He has no distorted ideas of how he looks or anything like that; the technical name for his particular issue is “Avoidant/Restrictive Food Intake Disorder.”
