I FINALLY FOUND THIS POST AGAIN here someone take my ideas:

everything is still grimy and yellow-green filter with blinking fluorescent lights because of underfunding

The main character could be a new nurse who acts as the audience gaze, rattled but plucky and kind. She is not stunning, but she is soft and conventionally pretty

MC Nurse pans over paperwork about "new lean initiatives" and there's some foreshadowing about having to take on more dangerous clients

The program director is a slick and charismatic, seemingly warm, business type. He has a mentally ill family member. He also has a clear, uh, love of women. He says he is a feminist.

First scare is a rising tension with a Ringu-style lurcher grabbing Nurse in the hallway

SURPRISE the twist is, it's just an enby person with muscle issues due to their meds, they wanted to give new nurse a macaroni flower from Art Therapy

They slur and apologize for grabbing MC Nurse who comforts them by pinning the macaroni flower in her hair

This turns out to win her a lot of approving looks and reference from the patients. And then a clear turn toward a chilly reception from the other nurses

MC Nurse reprimanded for unprofessional conduct and is being gaslit for her charting quality. We begin to notice a lot of the other nurses look kind of the same, soft and conventionally pretty, only one of them is older than 40.

MC nurse notices a patient who also looks kind of the same. And then notices when she is suddenly gone. More gaslighting, but the patients know. And the patients know MC nurse knows.

The worst thing is that there is so much potential for exploring the horror of psych wards from the angle of medical abuse, ableism, forced treatment/drugging, loss of autonomy, power imbalance, demonization, dehumanization, etc, and YET the horror genre keeps defaulting to "insane asylums and psych wards are scary because there are mentally ill people in there"
