Three years and five referrals later, I think I am finally actually on the waiting list for strabismus surgery!
The journey so far:
1. Notice your weird eye thing is happening more frequently. Do some research and learn it has a name - strabismus. Ask for a referral to an ophthalmologist next time you’re at the walk-in clinic getting your meds refilled
2. The walk-in clinic sends the referral, but it’s rejected because you have to go ask an optometrist about it first. You go to an optometrist. She says she can help, but recommends something called “vision therapy” and gives you bifocals. You do some research and discover that vision therapy is only supported by evidence for a specific manifestation of strabismus that is clearly not what you have. It is also not covered by your insurance and is very expensive. You tell her no thanks but wear the bifocals because she told you to. They do not help.
3. You go back to the walk in clinic and say you saw an optometrist and she didn’t help, and you would like a referral to an ophthalmologist. They agree and send the referral
4. You wait a number of months and get no response. You call the walk-in clinic and ask about the referral. “What referral?” they say. They have lost the referral. You go back to the walk-in clinic and get another referral.
5. The referral comes through and you have an appointment with an ophthalmologist! He tells you he isn’t really an expert in strabismus and refers you to someone else who is.
6. Your referral to the other ophthalmologist goes through. He tells you the bifocals are doing nothing and you can stop wearing them, and gives you a fresnel prism to wear. This is a plastic sticker that goes over one lens. It compensates for your double vision, but also makes that eye extremely blurry. He says he can put you on the waiting list for surgery.
7. You wait to hear about surgery but hear nothing. A few months after you were told you’d hear something, you call the office. They tell you the ophthalmologist has moved to Ontario and all his patients have been referred to other people. You want to scream.
8. About a year after getting the fresnel prism you give up and get prisms built into your glasses instead. It’s expensive, but helps a lot. Not completely though, and you’re at the maximum they can possibly do to compensate for your strabismus.
9. At some point you call the new ophthalmologist’s office to ask how long this referral will take. They tell you that they mostly see children, so it may be 2 or more years. They believe they are the only ophthalmologist in the area dealing with strabismus. You wish doctors had caught this when you were a kid instead of assuming you’d grow out of it.
9. Eventually, something like 3 years after going to the walk in clinic, you get in to see the new ophthalmologist on a cancellation without having to wait the 2 years for the referral. You are put on the surgery list. Again. One of the techs tells you it’s impressive that your glasses can fit so much prism in them and be actually wearable. You hope this time you will have surgery and your ophthalmologist will not move away.

Understanding Retinopathy: Causes, Symptoms, and Treatment Options
Tumblr media
Introduction Retinopathy is a medical condition that affects the retina, a critical component of the eye responsible for vision. In this comprehensive guide, we will delve into the various aspects of it, including its causes, symptoms, and available treatment options. Whether you’re seeking to expand your knowledge or have recently been diagnosed with retinopathy, this article will provide you with valuable insights to better understand this condition.
What is Retinopathy? It specifically diabetic retinopathy, is a condition that primarily affects individuals with diabetes. It occurs when prolonged high blood sugar levels damage the small blood vessels in the retina. However, it’s important to note that it can also be associated with other medical conditions, such as hypertensive and sickle cell .
Tumblr media
The Causes of Retinopathy Understanding the underlying causes of it is crucial in its prevention and management.
1. Diabetes Mellitus The primary factor leading to it is frequently diabetes mellitus. High blood sugar levels in individuals with diabetes can weaken and damage the blood vessels in the retina over time.
2. Hypertension Hypertension, or high blood pressure, can lead to hypertensive retinopathy, where the blood vessels in the retina are affected due to increased pressure.
Tumblr media
3. Sickle Cell Disease Sickle cell disease can cause sickle cell retinopathy, as the abnormal shape of red blood cells can block blood flow to the retina.
4. Other Medical Conditions Retinopathy can also occur due to other medical conditions like autoimmune diseases and certain infections.
Recognizing the Symptoms Identifying the symptoms of retinopathy is essential for early diagnosis and prompt treatment.
1. Blurred Vision Blurred vision, especially when reading or focusing on distant objects, is a common early symptom of retinopathy.
2. Floaters and Flashes Patients may experience floaters—tiny specks or threads that appear in their field of vision. Additionally, flashes of light may occur.
3. Gradual Vision Loss Without treatment, retinopathy can lead to gradual vision loss or even blindness.
Tumblr media
Diagnosis and Treatment Early diagnosis and appropriate treatment are crucial for managing retinopathy effectively.
1. Regular Eye Examinations People with diabetes or hypertension should undergo regular eye examinations to detect retinopathy in its early stages.
Tumblr media
2. Laser Therapy Laser therapy is a common treatment method for retinopathy. It helps seal leaking blood vessels and prevent further damage.
Tumblr media
3. Anti-VEGF Injections Anti-VEGF injections are another option, which can reduce abnormal blood vessel growth in the retina.
4. Vitrectomy In advanced cases, a vitrectomy may be necessary to remove blood and scar tissue from the eye.
Conclusion Retinopathy is a serious eye condition that demands attention and care. By understanding its causes, recognizing symptoms, and exploring treatment options, individuals can take proactive steps to manage this condition effectively. Remember that early diagnosis and treatment play a crucial role in preserving your vision.

re: tags on my last post
you know what else grownups need too? eye exams. glasses. prescription medication. therapy. ND assessments.
if you're between 18-65 in Ontario you get like jack shit from OHIP. like, yes, obviously kids need these things and increasing healthcare to provide dental care for children is a good thing (they already had coverage vision- & pharma-care). but it's not enough.
I've been on a waitlist for a family doctor since 2018. I submitted an application for a (new since I left for grad school) clinic but they prioritize precariously housed people & people in much less stable situations for me (which is fair & reasonable bc limited resources). I haven't had a family doctor since my last one retired in 2006. There's a telemedicine clinic around the corner but the doctor zooms in from Toronto & won't prescribe ANY medication if he has "concerns" about just one of them. 1 of the 2 walk-in clinics in the region just closed. that means there's just 1 walk-in clinic to service nearly 90,000 people.
BUT even if I do get a doctor soon and I do get the assessments I need & renewed asthma/allergy prescriptions, it won't matter unless I somehow suddenly start making a metric fuck tonne of money well above a living wage. or get married & have a bunch of kids I don't want.
Because I can't afford $1800-$6000 for an ADHD or ASD assessment. Supposedly there's a way to get an assessment for $300 but fuck if I can afford that. Or my $350 (last time I bought it in 2018)/month Timothy grass allergy pills, not to mention my other allergy and asthma meds. I haven't had an eye exam since 2019 but I can't afford that either, let alone new glasses (the ones I have now were emergency-replacement-cheapest-pos-frames-I-could-find after breaking my last pair). luckily my driver's licence is expired because my RX is so out of date it's technically illegal for me to drive with these glasses. I haven't been to a dentist since I was 26 and aged out of my dad's work coverage because it's so expensive.
But I'm over 18. and under 65. and not a parent. and unemployed. so I continue to fall through the cracks. I'm not valuable enough to the government to deserve health coverage.

Learn about Newcomers  Health Insurance in Canada
The cost of health and medical bills in Canada is shared by taxpayers because public healthcare is subsidised. Due to Canada's single-payer system, eligible users often do not have to pay for the majority of medical treatments and hospital stays. One may apply for public health insurance through their province or territory if they are a Canadian citizen or permanent resident, as well as some foreigners with work permits and some international students. Newly arrived permanent residents or Newcomers Health Insurance Canada in several provinces will immediately be covered by the government health insurance programme. There may be a three-month wait in some other cases.
youtube
What benefits do newcomers get in Canada regarding their health-insurance?
If they are eligible for public healthcare in their province of residency, newcomers to Canada may additionally opt to obtain private health insurance to cover costs for services not covered by provincial healthcare. Prescriptions, dental and vision care, as well as other types of ancillary medical care, are all covered by insurance (such as physical therapy or mental health services). As a condition of your employment, many (but not all) employers will also provide you this form of coverage; however, you might only be eligible for it if you successfully complete a probationary period.
The kind of health insurance in Canada if you are a resident outside your province
You can receive fundamental healthcare services in any other Canadian province with medical coverage. For instance, if you require emergency medical care while physically present in Manitoba and are covered by the province of Ontario, you may receive such services. You should be aware that there are some limitations to this, and that if you obtain certain treatments outside your province of residency, you may be charged for them. Examples include ambulance transportation, some emergency dentistry services, and the purchase of prescription pharmaceuticals. Knowing this is essential to comprehending Canadian healthcare.
Because prescription medications can be expensive, it is critical that you know what and how much of the cost is covered for you in the form of Newcomers Health Insurance Canada so that you can manage your health effectively. Visit the website of einsured.ca to know more.

Note
I wasnt interested in CAN politics until I stumbled onto your blog but anyways what are the main reasons you should vote NDP vs the others?
The NDP is the most left major party in Canada.
The NDP support expanding our healthcare system to cover prescription medication, dental, vision and hearing and boost funding for healthcare in general (including mental health); they want to tax the rich more to pay for these expanded social programs. They want to get rid of our broken electoral system First Past the Post and replace it with Mixed Member Proportional Representation.
The NDP want to eliminate all subsidies to the fossil fuel industry, retrain fossil fuel workers so that they can work in renewable energy fields, opposes all oil pipelines in Canada and wants to massively invest in renewable energy. They want to make large investments in public transit including making public transit free. They want to invest in electric vehicles and other public transit like trains. They want to eliminate post secondary education related tuition and otherwise reduce student debt.
The NDP want to increase supports for those living in poverty, people with disabilities and seniors. They want to increase access to EI (Employment Insurance) for unemployed workers. They want to ban internships federally and raise the federal minimum wage to $15/hour. They want to ban conversion therapy, take in more LGBTQ refugees, end employment discrimination towards LGBTQ people and remove the ban on blood donations by gay men and trans people. They want to fully cover and make transgender gender confirmation surgeries accessible for everyone.
The NDP want to expand access to Abortion across the country. They will ban the racist police practice of carding. They want to build 500,000 new units of affordable housing over the next decade. They want to cut the prices of cell phone bills by targeting the companies that have been gouging Canadians for years. The NDP wants to decriminalize all possession of drugs to fight the opioid crisis and save lives. The NDP will fund a Basic Income pilot project and take over Ontario’s existing pilot project.
Very few of these things will be done by any other political party in Canada. The Liberals and Conservatives are the status quo. We need to vote for progress.
Lots more here:

The thing about mental health... (Blog #5)
The thing about living with mental illness is that individual experience can be extremely difficult to articulate to anyone who’s not experienced the condition. There is a sense of isolation that accompanies a diagnosis, a fear of remaining in whatever mental state is causing you distress. Alongside this is the voice that taunts you, telling you that if you express your innermost feelings you will never be able to take them back. Your illness will begin to define you in the eyes of others, and soon you will become your darkness. Although I’m speaking from personal experience, as well as to the experiences of some close people in my life, it is undeniable that a stigma exists surrounding mental health, despite the impact varying from person to person. This is the just one reason why many people don’t reach out to claim the assistance they need in order to begin their healing process. The barriers, whether mental, social, financial etc. that impede on one getting help are difficult to maneuver, and students at the University of Guelph are seeming to have a particularly disadvantageous experience with the services offered on campus.
Tumblr media
Being a University student is a time of growth; an experience to help one identify a sense of self, a drive, a passion. It is a time to create new connections and live each day with a sense of purpose and hope. However, there is also an immense amount of pressure that looms above all of this positivity. Exams, assignments, financial distress, physical and mental health complications, and personal issues are faced daily in the lives of many University students. In these stressful periods, it is so important for campuses to have spaces and resources available in order to alleviate some of these anxieties. At Guelph, however, many students are less than happy with the mental health assistance provided. Two months ago, an article was released by online news site Narcity outlining the reasons students distress and concern on this issue, with students even going so far as to claim that the school has "the worst mental health support of all the universities in Ontario". I will attach the link to the full article below, but essentially the main point of concern was the fact that the school only has ONE psychiatrist, subjecting students to MONTHS of wait-time to get an actual appointment. For students who are unable to afford help from outside sources, this time unassisted could be detrimental to their wellbeing. There are 17 registered therapists, however, they are not certified to diagnose or give out prescriptions as psychiatrists are. The therapy services on campus also have an extremely long waitlist, and I actually just had my first appointment today after waiting over a month. My next appointment will not take place until the very end of March. In addition, a concern was brought up about the lack of a fall reading week. This prohibits many students from refilling necessary prescriptions with doctors located in their hometowns, and doesn’t allow for a short and necessary mental break during the stressful time that is exams. The following are screen grabs of some tweets from current students, clearly upset and disturbed by the circumstances.
Tumblr media
I believe the most important thing students at the University of Guelph need to realize is that they are not alone in their struggles. Each and every person deals with pain in a unique way, it may just be hard to see at times. And I believe the most important thing the University of Guelph student health department needs to realize is that their system is currently failing students and a solution needs to be found. I have therefore come up with an idea for a collaborate installation, which will serve the purpose of communicating and advocating for mental health awareness on campus. This installation will be made out of string, weaving and intertwining around each other, attached to four wooden poles for stability. Here are some examples that are similar to the vision I’m describing:
Tumblr media Tumblr media Tumblr media
Beside the main installation will be desks set up with pieces of paper, pens, and clothespins. Students and faculty alike will be asked to fill out a piece of paper and attach it to the web wherever they please. In their piece of writing could be words of encouragement, their own personal experiences with mental health, information on helpful resources, advice, etc. They may keep their note anonymous or sign their name, whatever suits their comfort. Passersby will then be able to take notes off of the installation and read them, putting it back on the web where they found it when they’re done. It will be a fairly large piece, placed in the main space in the UC where a majority of students and faculty flow throughout the day. This installation will serve as a reminder that you don’t have to be alone in your fight. Each note written and read will create a sense of connectivity and support with those around us; all human beings simply trying their best in this life.
Tumblr media
So now I ask you, have you guys had/heard about a similar experience with Guelphs mental health services? Or have your experiences been completely opposite to those described? I would love to hear your thoughts…
Until next time,
Claire

Dry Eyes: Causes, Symptoms, and Effective Remedies
Tumblr media
Dry eyes are a common and often uncomfortable condition that occurs when the eyes do not produce enough tears or when the tears evaporate too quickly. While it might seem like a minor annoyance, dry eyes can lead to significant discomfort and even affect your vision if left untreated. In this article, we’ll explore the various aspects of , including its causes, symptoms, and effective remedies. Understanding Dry Eyes What are Dry Eyes? It scientifically known as keratoconjunctivitis sicca, refer to a condition where the eyes fail to produce an adequate amount of tears or the tears evaporate too quickly. Tears play a crucial role in maintaining the health of the eyes, providing lubrication, reducing the risk of infection, and ensuring clear vision.
Tumblr media
How Do Tears Work? Tear composition encompasses a fusion of aqueous elements, oils, mucosal components, and distinct proteins.. Each component serves a specific purpose. The water layer moisturizes the eyes, while oils prevent rapid evaporation. Mucus helps even spreading of tears, and the proteins combat infections.
Causes of Dry Eyes It can occur due to a variety of factors:
Environmental Factors Exposure to dry and windy climates, air conditioning, and heating systems can contribute to rapid tear evaporation.
Aging and Hormonal Changes As we age, tear production tends to decrease. Hormonal changes, such as those during pregnancy or menopause, can also lead to dry eyes.
Tumblr media
Medical Conditions Certain conditions, like diabetes, rheumatoid arthritis, and thyroid disorders, are linked to dry eyes.
Medications Histamine blockers, nasal decongestants, mood-regulating medications, and specific drugs for managing blood pressure have the potential to diminish the production of tears.
Common Symptoms Persistent Discomfort People with dry eyes often experience a constant sensation of grittiness or foreign particles in their eyes.
Itchiness and Redness Dry eyes can cause itching and redness due to the irritation of the eye’s delicate tissues.
Tumblr media
Sensitivity to Light Increased sensitivity to light, known as photophobia, is another symptom of dry eyes.
Blurred Vision Blurred vision, especially during activities that require intense focus, can indicate dry eyes.
Diagnosing  Eye Exams Ophthalmologists conduct thorough eye examinations to evaluate tear production, eye surface damage, and overall eye health.
Tumblr media
Measurement of Tear Production The Schirmer test measures tear production by placing a small strip of filter paper under the lower eyelid.
Managing and Treating  Artificial Tears and Lubricating Eye Drops Over-the-counter artificial tears and lubricating eye drops can provide temporary relief by moisturizing the eyes.
Tumblr media
Lifestyle Changes Staying hydrated, maintaining a balanced diet rich in omega-3 fatty acids, and taking regular breaks during screen time can alleviate dry eye symptoms.
Prescription Medications In severe cases, prescription medications like corticosteroid eye drops or cyclosporine may be recommended.
Advanced Therapies Advanced treatments include blocking tear ducts temporarily to retain tears and using intense pulsed light therapy to improve tear production. conclusion Dry eyes can significantly impact your comfort and visual acuity. By understanding the causes, symptoms, and available treatments, you can effectively manage and alleviate dry eye discomfort. Incorporating small lifestyle changes and seeking professional advice when needed can go a long way in maintaining eye health.

What am I going to do with myself? What can I possibly hope to accomplish when all I have to work with is a bag of bones that is anxious and angry? How easily the switch can be flipped by unaware hands in a normal conversation This is what therapy was for - this was suppose to be easier for me But still that creeping feeling of unrest has made its way behind my eyes Clouding my vision of all the beauty and love in my life right now Because storm clouds Because rain and lightning Because thunder The booming roaring cracking sounds of the sky being ripped apart It washes over me wave after wave after wave And I am torn because there would be nothing to be gained from letting myself get washed away by these gruesome waves But the struggle needed to fight them is draining me more and more as I grow older And older is quite the thing itself. I have aged for sure but there are parts of me that refuse to and I’m unsure if it is my own stubborn desire to be young for as long as I can Or if these are the parts poisoned by outside hands, stuck in some sickening stasis beyond my control I roar into muted pillows in hopes that the beast will be sated if I give into the vibrations Allowing them to rip through my chest, hard and rough, filled with things I can’t quite recognize Distorted memories and feelings are unbalanced by this stomping manifestation A giant shrouded figure - only the crooked gnashing teeth are visible from where I am Usually I run, I avoid these darker ideas and shame myself for them and their existence But not today, today I open my arms and embrace this beast (at London, Ontario) https://www.instagram.com/greenthumbblacksole/p/BuUMLEjnkOY/?utm_source=ig_tumblr_share&igshid=qx75u60gffeq

CONCUSSION AND LONG-TERM DISABILITY CLAIMS – WHAT YOU NEED TO KNOW
There has recently been a lot of discussion surrounding concussions, particularly in sports. Yet, a concussion can happen to anyone who has sustained a blow to the head, face, neck, or upper body. While most people recover from a concussion within a month, there are some people who have long-lasting symptoms that prevent them from returning to work.
In this article, we will discuss what a concussion is, how it is diagnosed, the symptoms associated with a concussion, treatment, prolonged symptoms, and how long-term disability insurers treat concussion claims.
What is a concussion
A concussion or a mild Traumatic Brain Injury (mTBI) is an injury that affects how your brain functions. It is usually caused by a blow to the head, face, neck, or upper body.
Your brain has the consistency of gelatin. It is cushioned and protected by cerebrospinal fluid inside your skull. A strong blow to the head, neck, or upper body can cause the brain to slide back and forth against the inner walls of your skull. Sudden acceleration or deceleration of your head can also cause your brain to hit the walls of your skull, causing injury. These injuries affect brain functions.
Falls, workplace injuries, falling objects, contact sports, motor vehicle collisions, pedestrian or bicycle accidents, or being a victim of physical abuse are examples of actions that can cause a concussion.
Diagnosis
If you think that you may have a concussion, it is important to get assessed by a healthcare professional immediately. Your doctor will evaluate your symptoms, review your medical history, and conduct a neurological examination. Your doctor will diagnose you with a concussion by ruling out more severe types of injury to the brain or spine. They will assess your ability to see, walk, think, and remember. They will also assess your balance, reflexes, muscle strength, sleep patterns, and mood. If necessary, your doctor may send you for diagnostic scans of your head and neck.
Symptoms of concussion
The signs and symptoms of a concussion can be subtle and may not be apparent immediately. Some symptoms may appear right away. Other symptoms may appear after several hours or several days. The symptoms of a concussion can range from mild to severe and can last hours, days, weeks, or even months.
According to the Ontario Neurotrauma Foundation’s Guideline for Concussion/Mild Traumatic Brain Injury & Persistent Symptoms, common symptoms of concussion include:
Cognitive
• Feeling foggy
• Hard time remembering and focusing
• Speed of information processing
• Trouble thinking clearly
• Trouble finding words
• Judgement
• Executive functioning, such as decision making, planning, and motivation
Physical
• Headache/migraine
• Dizziness and balance problems
• Nausea
• Fatigue
• Sleep disturbance
• Blurred vision
• Sensitivity to light or noise
• Hearing problems
Behavioural
• Depression
• Nervous or anxious
• Agitation
• Irritability
• Impulsivity
• Aggression
• Angry
• Sad
• Sleep more or less than you normally would
• Hard time falling asleep or staying asleep
• Ringing in the ears
• Seizure
• Temporary problems with smell and/or taste
• Numbness and tingling
• Neck pain
Treatment
Rest is the most effective way of allowing your brain to recover from a concussion. You need to physically and mentally rest to recover from a concussion.
Your brain is more susceptible to further damage after you have suffered a concussion. Therefore, while you are recovering, it is important to avoid activities that may cause another head injury. It is important to allow yourself time to recover and to gradually return to your regular activities.
If you are diagnosed with a concussion, your doctor should give you a management plan to help you recover. This management plan should be personalized and based on your symptoms and assessment results and to best fit your lifestyle. Some examples of treatment that may be recommended are vision therapy, specialized glasses, vestibular therapy, pain medication for headaches, physiotherapy for headaches, etc.
Many factors can influence how a person will recover from a concussion. Each concussion is unique and should be treated on a case-by-case basis. It is important to tell your doctor if you have a history of mental health issues as this may affect your symptoms and your recovery.
Prolonged Symptoms
The length of time for recovery varies from person to person. Most people recover from a concussion within a month. Some people will have symptoms from 1 – 3 months. In about 15% of cases, symptoms can last longer than 3 months. Symptoms lasting longer than 3 months are called “prolonged symptoms”. If your symptoms are prolonged, you may be diagnosed with “post-concussion syndrome”.
According to the Ontario Neurotrauma Foundation, it may take you longer to recover from a concussion if you:
Are a teenager or an older adult;
Are female;
Go back to school, work, or exercise too soon;
Have had a concussion before;
Have a history of migraine headaches;
Have mental health issues such as depression or anxiety;
Have trouble sleeping; or
Are showing signs of vestibular or visual abnormalities (e.g. blurred vision, dizziness, difficulty focusing, motor sensitivity)
Post-concussion syndrome can significantly affect a person’s daily functioning, including returning to work. Because of this, prolonged symptoms need to be closely monitored by your doctor. Your family doctor may refer you to other healthcare providers to treat your specific symptoms including mental health specialists, rehabilitation providers, and/or a specialized concussion clinic.
Long-term disability and concussions
When you are planning your return to regular activities, including work, you need to consider both cognitive and physical activities which may make your concussion worse. It is important that you not return to work before you are cleared by your doctor. If your doctor feels that you are ready to return to work, it is important that your employer accommodates your physical and cognitive limitations and restrictions. Your doctor will help you identify any limitations and restrictions that will require accommodations at work.
If your symptoms persist and you are unable to return to work, you should apply for short-term disability benefits. If you have not recovered after the short-term disability period ends, you need to apply for long-term disability benefits. Returning to work before you are recovered can aggravate your concussion symptoms and make your condition worse, prolonging or even limiting your full recovery.
Since concussions are “invisible injuries”, disability claims are often denied by insurance companies. The insurance company may take the position that there is no “objective medical evidence” such as a CT scan, MRI or blood test to prove that you are suffering from post-concussion syndrome. Insurance companies often expect that you will recover from your concussion within medically prescribed timelines and should you not, they may question your credibility, your treatment and your ability to work and deny your claim. At MK Disability Lawyers we have represented many clients with concussions. We know the type of medical evidence that is needed to persuade the insurance company that you are unable to work.
We also understand that clients with concussion symptoms will require very specific accommodations throughout the litigation process. At MK Disability Lawyers, we strive to make those accommodations readily available to our clients; whether it be by communicating in a way that does not aggravate concussion symptoms (in-person or by telephone, avoiding electronic communications), darkening our meeting room, following up our meetings in writing detailing our discussion, not setting a time limit on our discussions or meetings, among other accommodations, and by generally exercising extra patience and compassion with the understanding that concussion symptoms can be frustrating and limiting to our clients.
If you have suffered a concussion and if your claim for short-term or long-term disability has been denied or terminated, contact us for a complimentary consultation.
