Darling

"What do you want for lunch, Darling?" Michael says to the air next to him as he sits on a bench:To Michael, though, he's talking to the love of his life, the only woman who could truly understand him and care for him, his darling. By talking openly to himself, he's made himself a target for bullies like Dave.

Dave and his group targeted the outcasts: the theater kids, the queer folk, the band kids, and the biggest outcast of them all… Michael the lunatic. But when they weren't tormenting others, they were a normal friend group who did normal friend group things. As Dave heard Michael talk to himself again, he scoffs and says, "Maybe a real girlfriend would help tighten the loose screws in his head," and the friend group roars with laughter.

They head off, leaving Michael alone. Michael is not phased by this because his girlfriend told him that she's all he'll ever need; he needs no one else; he has her and her alone. Dave and the friend group go to classes 2–3 to get a vision from the class psychic. Once there, Dave is first, and he puts his cross neckless to the side as he lies down on the makeshift desk table and the psychic instructs him to close his eyes.

Dave opens his eyes to see a heart-shaped door in front of him. When he tries to go through, he's stopped by a mannequin pointing at a sign that says, "TICKETS BEFORE ENTERING RIDE." Dave reaches into his pockets, and he has a ticket in it. On it, the ride name is "THE TRAGIC TALE OF DAVE MILLERS LOVE LIFE." He drops the ticket in shock, but the mannequin grabs it before it hits the floor. Then it shoves Dave into the ride and starts it.

The cart goes slowly as it passes through little stages with the woes of Dave's love life. Including the first time he had been confessed to, he didn't feel anything towards the girl and simply rejected her. The first time he saw a man and woman kiss in public, he felt repulsed by the idea of a romance like that. The first time he saw, "NO, STOP THIS," Dave screamed as he kicked the cart, trying desperately to get free. He knows what's next; he doesn't want to admit the truth.

The cart stops at one of the stages, and Dave looks down, refusing to look at the stage and its content. Robotic arms appear from the cart and force his head up and open his eyes. In front of him is the memory of when he saw him. Once he saw him, he felt warm, nervous, and confused. Why did he feel this way? But then it hit him like a rock, and he wanted to throw up. It was love. But the fear of his family and everyone he knows figuring out and shunning him scares him, so he decides to make the boy's life hell just to prove he feels nothing for him.

Dave wakes up in a sweat, and the psychic asks how his vision went. Dave punches the psychic. Some of his friends catch the psychic from falling; one of them shouts, "What the hell was that!?!" Dave gathers his composure. "Sorry… Just I…didn't like what I saw…" Everyone gets ready for classes since lunch is almost over.

As the sun sets and everyone heads home, Darling and Michael are walking down a bridge on the route they always take. They pass by a couple cafés, then are greeted by a barking dog in the residential area. Soon they get to Michael's house. Michael opens the door and goes inside, trying to close the door as quietly as possible, but the door still makes noise.

His mother awakes, the reek of booze following her. Michael tries to run to his room, but he's not fast enough. His mother grabs him by the hair and asks, "What did I say about HIC making a shit ton of noise when you get back?" Michael keeps apologizing while starting to tear up. "SHUT UP, YOU FUCKING DISGRACE." His mother throws him on the floor as Michael keeps apologizing. "I should've aborted you; then I'd still have my own damn life," she says, taking out a cigarette and lighting it, "but now I'm here stuck taking care of a piece of living garbage." Her words are like venom to Michael, but at least his Darling can help him pull that venom out and cure his wounds. His mother goes out for fresh air, yet Michael knows she's just run out of rum. Michael takes this time to go up to his room and spend time with Darling, lying in her arms as she caresses his hair. If only he could feel these things.

The passing weeks are a constant loop of going to school, being an outcast, and hiding in his own house. Michael is sitting on his and Darling's bench during lunch while he writes a story down in his notebook. Dave and his friends notice him writing and decide that the world should see his wonderful writing. One of the girls goes up and snatches the notebook. Michael tries to grab it, but she throws it to Dave, and Dave starts reading aloud a fanfic of him and Darling living in a digital world where everything is wacky and full of adventure. Michael desperately tries to get the notebook back, but sadly, Dave is a head higher than him, so he can't reach it. While reading, Dave notices Michael's huge bruise on his forehead. It startles him enough that he drops the book. Michael picks up the books and starts running, seemingly for his life.

One of the guys starts asking why he dropped the book, and Dave mutters, "I… I don't know." The friend group is sad their little prank ended without Dave even finishing the story, but they don't let it ruin their day and move on to go do something else. Meanwhile, Michael is hiding in a broom closet, while his Darling comforts him by telling him that you got the book back and that you stopped them before they got any further. Michael hugs his Darling and cries. If only he could feel that hug.

When school ends and the friend group goes and does their own thing, Dave is heading to a new café he saw online. While sitting there waiting for his order and another friend to show up, he sees Michael walk past the café, soaking wet and still smiling. And even now, he doesn't know why, but he gets the urge to follow him. Dave follows Michael, making sure to keep a distance and letting curiosity get the better of him. When entering a residential area, a dog starts barking at him when he walks by. He gets startled and hides behind a wall. After a few seconds, he catches up to Michael, still keeping somewhat of a distance. Michael stops at his house; Dave crouches down nearby and watches. As Michael opens the door, a bottle smashes against his head. His mother curses him out for scaring her; she grabs him by the hair as Michael keeps apologizing profusely.

All Dave could do was sit there in shock. He knew horrible parents existed, but he never thought he'd see someone mistreat their child that publicly. He gets up and walks back the way he came, past the dog and past the café, and while walking on the bridge, he just thinks of everything he's done. Not only that, but he thinks back to how much of a bitch he's been to Michael ever since freshman year; he thinks about how much he's made this guy's life worse than it already is. He starts to regret everything he's done and said. By the time Dave makes it back to his house, the sun has already set, and his mother is waiting nervously outside the front door. When she sees him, she runs over, calling his name as she does so, and hugs him, asking where he's been. Dave hugs his mother back, thanking her for being there for him. His mother was confused about this sudden affection, but was glad for it anyway. They went back inside and had a nice family dinner.

Therapeutic Efficacy and Considerations:

Asthma/COPD: 😐 After preliminary evidence noted benefit for improvement of lung function, one high-quality trial demonstrated increased anti-inflammatory activity in patients maintained on corticosteroids. Currently, there is not enough evidence to generally recommend use for this indication.

Migraine Prophylaxis: 😊 One trial demonstrated that butterbur decreased number of migraines, migraine days, and severity of symptoms versus placebo, with no reports of adverse effects. Butterbur may be an option in patients who have failed other treatments or insist upon a nonprescription alternative. Patients must be made aware that long-term safety data is lacking and of the UPA hazard (see note below). The dose for this indication is 50-100 mg rhizome extract standardized to 7.5 mg perasin and isopetasin given twice a day for 4-6 months, then taper dose until migraine incidence decreases.

Seasonal Allergic Rhinitis: 😊 In a placebo-controlled study, butterbur reduced response to allergen challenge and demonstrated comparable efficacy to cetirizine in reduction of symptoms in a second high-quality study, with a lower incidence of sedation. Evidence does support butterbur as a short-term option for this indication. Safety considerations prevent it from being recommended for chronic allergic rhinitis.

Note: Butterbur herb contains unsaturated pyrrolizidine alkaloids (UPA), a toxic substance that is lethal at high doses. Standardized extracts are processed to remove UPA. Only products from reputable manufacturers that are certified to be UPA-free should be used. Patients should be made aware of symptoms of UPA toxicity (see Adverse Effects) and warned to immediately stop use and seek medical attention if they experience these symptoms. The dose for allergic rhinitis is the extract standardized to 8-16 mg petasin given 3-4 times daily.

Chemistry/Pharmacology: The most active component is petasin. Other active ingredients include oxopetasin esters, sesquiterpene compounds, and isopetasin. Anti-inflammatory effects are due to inhibition of leukotriene synthesis. It also decreases mast cell activation and blood levels of histamine and leukotrienes. Animal studies have noted some spasmolytic activity.

Drug Interactions: Do not use with other UPA-containing herbs (e.g., borage, coltsfoot, dusty miller, ragwort, etc.). May have additive adverse effects with other anticholinergics and antimigraine agents.

Contraindications/Precautions: Use is contraindicated during pregnancy and in nursing mothers (due to possible teratogenicity, hepatotoxicity, and excretion in breast milk), in patients with severe gastrointestinal or bladder motility conditions (due to possible worsening of symptoms), in patients with current or past hepatotoxicity, and in patients allergic to the Asteraceae/Compositae family (e.g., chrysanthemums, ragweed, daisies, and marigolds).

Adverse Effects: Diarrhea or constipation, headache, belching, GI distress, dysphagia, itchy eyes, eye discoloration, stool discoloration, fatigue, asthma, pruritus, skin discoloration, and drowsiness have been reported to occur in less than 1% of patients using UPA-free extracts. Note: UPAs are hepatoxic, mutagenic, nephrotoxic, carcinogenic, and can cause veno-occlusive disease, the symptoms of which are right upper abdominal pain and distention, sometimes accompanied by reduced urine output.
