Haven't really been around for a couple of days,
because i found myself dealing with a nasty UTI for whatever reason. This is the third one ever in 40+ years, and the only one so far that isn't the direct result of a catheter from surgery. No Foleys since 2020, thank goodness. And my blood sugar control has been pretty consistently at nondiabetic levels--not that it being totally out of control for years was enough to set them off for me. ¯\_(ツ)_/¯
I was trying to wait it out, but yeah today I've been actively sick enough with a miserable fever (measured at 103.6F/39.8C so far between doses of meds for that and the pain tonight), but yeah I think I am going to need to try to see someone and try to get antibiotics out of them. Getting some pain over my kidneys since last night, too.
I've spent too much of the day today sacked out in the bed, alternately shivering and sweating disgustingly with no energy. My blood sugar I ate a little crispbread with cheese earlier, but then later suddenly yakked up some miso broth that I drank this evening partly for the electrolytes, during one particularly violent shivering and shuddering episode. That was fun. Thankfully I did have a plastic bag in my backpack by the bed.
This will be the first time I have needed to seek out any kind of treatment for a "normie" health issue since we moved here, and honestly that does not seem to have as good a reputation as it could. They are apparently pretty stingy with meds for anything that might possibly clear up on its own. Which is understandable up to a point, but not always the best call--and of course I have some bad history with that in the UK with their just pennypinching unwisely. And refusing to give me a long enough course of antibiotics to totally clear up that last UTI from the hospital in 2020. Thankfully, it was easier to obtain various necessary meds through semi-legit online private consultations there. Not so much here, AFAICT.
But yeah, I actually consulted what 1177 was advising for urinary tract infections:

And yeah, at least they are making a very clear distinction between cystitis without a fever, and the kind with a fever that's probably gone to your kidneys, may turn more dangerous, and really does need antibiotics. So, I am hoping they will actually follow their own recommendations and prescribe some without too much problem.
And since I really doubt I'll be in any shape to drag my gross sweaty ass through the shower before somehow getting it to the local primary care, maybe that would actually work in my favor to help convince them that I am legitimately sick and feverish. I do not plan to take any paracetamol/Tylenol before going, unpleasant as that would be. But, let 'em see the shaking and sweating in its full glory. Though my temperature was still just over 100F/37.8C earlier with some anti-fever still on board.

I’ve noticed many times at this point that there’s a disjoint between the treatment trans people receive from supposedly queer-affirming institutions and nonprofits, and the treatment we receive from for-profit services that only cater to the richest among us.
At sliding scale queer clinics and LGBTQ community centers, I’m treated like something of an inscrutable freak or an incompetent much of the time. People condescend to me, remember nothing about me, and misgender me even when my identity and pronouns are marked on my chart. Other basic facts about me and my health get regularly messed up too. Even my own doctor keeps forgetting I’m on testosterone gel, not injections, and multiple times I’ve had the wrong blood labs ordered for me or had paperwork misplaced.
I can easily contrast that to the treatment I get at the spas, tanning salons, gay bars, and plastic surgery centers I’ve visited, all of which cater to a wealthier, more privileged clientele. I’m gendered correctly more often at LA Tan than I am at the queer clinic. The technician who performed my laser hair removal was more respectful toward my trans body than any nurse I’ve met at Planned Parenthood. At the gender clinic, I was a forgettable number on a spreadsheet whose phone calls go unreturned. At the plastic surgeon, I'm served fancy coffee and asked about my aesthetic preferences.
Wealth unlocks access to a realm where queer people’s bodies and identities are respected, where we are trusted to manage our own healthcare and are remembered and prized. I desperately wish this weren’t the case. I’m sometimes a bit disgusted at myself for how much I enjoy receiving this lavish-seeming treatment, even though I know it’s only basic dignity.
The only way to escape erasure or institutional control under our present system is to have enough wealth to transform from mere patient into coveted consumer. With enough money, you can skip the clinic in all its bureaucracy, order hormones from an online service, book a major surgery within mere weeks, order UTI meds from an app, and pay a driver to take you home from the hospital rather than begging a friend.
Right now, I can afford to be a consumer, which means I am allowed to be a person and to do with my body whatever I wish. And I don’t know what to do with how good that feels.

Kazu has been diagnosed with Diabetes & has started treatment. He had just had bloodwork and urinalysis done the middle of December, because I felt like he was off then. Not his normal, active playing self like Romeo & Quincy. All of that bloodwork and urinalysis came back completely normal. His blood glucose was right in the middle of the range 127. The range is 72-175. So since she couldn’t find anything wrong with him, we just had to wait till he developed any more symptoms. February 16 Kazu started being treated for a UTI. I also noticed he was drinking more water. About 3 days into his UTI treatment I still didn’t think he was right so I took him for more bloodwork and another urinalysis. Thursday 22nd morning my vet called called and said Kazu has diabetes my heart sank. His blood glucose was almost 400. She wanted me to bring him in to learn how to give injections and go get him a prescription for insulin to start asap. Kazu started out getting 1 unit of insulin every 12 hours. It is the tiniest little amount of insulin not even a fourth of an inch. We are also adjusting his diet and probably everyone else’s diet to make things easier and possibly help prevent Kazu’s other two brothers from getting diabetes as well I hope since this seems to be genetic. There is a possibility that Kazu could go into remission since we caught this very early so I am hopefully. Kazu’s went to the vet yesterday morning 2/28 for 9am to have glucose curve monitoring. His numbers still stayed in the low 400s for his first 2 checks then high 300s for the second 2 checks. Kazu was on 1 unit of insulin every 12 hours, now I gave him 1.5 units last night and then starting this morning he now gets 2 units of insulin every 12 hours. He has been really good about the shots. We just distract him with some of the lickable tube treat and he doesn’t even know he’s getting an injection. Kazu goes back for his next glucose curve monitoring in two weeks unless something change. I have been reading and researching a lot online about cat diabetes. I want to be fully educated and able to care for him the best I can. Kazu does act like he feels a bit better so I will keep you posted on how he’s doing! I can’t believe that he also has diabetes just like what happened to Forrest. At least this time I knew what to watch for. It still kills me that I didn’t know what was happening to Forrest until it was too late. I will always feel like I failed Forrest because I didn’t know what was happening. Why isn’t no one talking about toilet paper? When I visited UK, the transition was very hard. I used to shower every time I went to loo for BMs because even wet wipes didn’t feel okay. Yeast infections and UTIs are common among women. Here in India, women avoid proper treatment because of the stigma attached to it. Like how are they any different from conservatives?
oh my god yeah literally tho. for one im surprised theres even people teaching others online NOT to wipe back to front. youd think thats common sense but w/e i dont know ppl's life circumstances. secondly simply wiping is honestly inadequate lol like the bacteria is still there u just made sure its not all wet and messy and wont get all over ur undies. i think the fact that in my country women & men literally wash down there after using the toilet AND wipe helps minimise the risk of yeast infections & UTIs. but its also not gonna prevent it totally, bc i know a lot of women + girls who got it regardless (and no they weren't "filthy whores"...some of them are incredibly young & virgins. its extremely backwards that such things would even need to be specified).
also women's vaginal health is highly stigmatised in my country too. statements further stigmatising it from westerners online does not help. they couldve talked about how sexual activity plays a role & men being dirty plays a role without being weird about it & making it all about sexual activity (bc it isnt all about that at all.)

I don't think I've had a week without a fucking issue ever since I got here
To be fair doctors missed the fact that my psoriasis was getting erythrodermic back in France but they weren't dermatologists because no fucking dermatologist takes new patients there anymore
But anyway. New GI team determines my last treatment wasn't working. Wants me on the most expensive treatment there is. OK. Get the implant because I can't be on the pill AND the new treatment.
Get a UTI. Get antibiotics. Antibiotics cause yeast infection. UTI returns. Repeat cycle. Not sure if it's gone to this day but at least I don't have kidney pain anymore.
Had an appointment booked for dermatologist... Gets fast tracked to it because half my body is covered in a red rash that starts peeling. I ask doctors on my online chat, suspecting a fungal infection - nope, erythrodermic psoriasis, could die, lol.
Gets seen in dermatology the new week. Put on two creams including one steroid. Says he'll discuss Stelara with GI, who had in the meantime decided on another med.
Eventually they both agree on going with Stelara... And my insurance denies it. Motherfucker, we picked Kaiser specifically because they're both the insurance and the medical team. After some back and forth, GI fixes it and I get my first infusion. Yay.
A couple weeks after that, I start getting sores in my mouth, mostly under my tongue. I tell my GI on the phone checkup, and she doesn't think it's linked to the new medication.
Either way I'm seeing the dentist a week after the call because I need my wisdom teeth extracted. Dentist has no clue what the sores are, prescribes a mix of antifungal + numbing agent + something else to hopefully fix it. It helps for two days but there's not enough medication and it simply returns.
And now I'm here. First Stelara injection next week at the hospital, which I'm not looking forward to. I fucking hated prefilled pens but I'm going to hate that shit even more.
I have a GP appointment the next day and hope they can either tell me what's wrong or direct me to someone who can. I've asked for a magic mouthwash refill to tide me over. That's $80 BTW.
But for now eating, brushing my teeth and even speaking hurts like hell. Just needed to write out the timeline because I feel insane.
