It is good to hear from you my friend, and I am more than happy to share what information I have on the topic. Little research has been done in this area, which ought to be righted, but as it stands I view myself as the foremost expert in the topic. 
As you know, Malar is a common deity among lycanthropes. While in many cases Malarites contract lycanthropy before worshipping Malar, there are many cases of non-lycanthropic Malarites willingly being infected with lycanthropy as part of their worship. In a similar vein, there have been records of what one might call idiopathic lycanthropy among devoted Malarites, which is to say, lycanthropy without any clear cause of infection. 
Some have argued that the label idiopathic is incorrect, as the condition seems to be a potential effect of Malar worship or perhaps even gifted directly from Malar. Therefore, it has been proposed that the condition be labeled theopathic lycanthropy. However, arguments have also been made that the condition is not related to lycanthropy. Those suffering from the condition do not transform in any way that correlates to moon phases nor do they show a consistent vulnerability (or resistance)  like other were-beings. Despite this, the affected do have periods of involuntary animalistic transformation, predatory bloodlust, and hazy or missing memory surrounding transformations. For this reason, the condition is sometimes referred to as pseudo-lycanthropy rather than actual lycanthropy. 
While there have been some record of this condition, there is yet to be any agreement as to if the condition actually exists, or if it is simply a tall tale born of exaggerated tales of Malarite worship.
I have had the remarkable pleasure, however, to meet someone affected by theopathic pseudo-lycanthropy. On the road to Neverwinter, I came across a nomad which I first mistook for a druid. However their attire was nearly textbook for Malarite priests. As a storm rolled in, I took shelter with them in a temporary shelter they had erected. The Malarite was rather standoffish, but did their best to try to accommodate me. 
As the hours wore on and the storm showed no sign of letting up, I noticed my host growing increasingly sickly and restless. It was not yet nightfall when they bolted from the shelter with no explanation. Soon after I heard screams and the sickening crunch of bones, I was nearly too frightened to look, but eventually gathered the courage to peer outside. There I saw a hulking beast in the rain, lit by flashes of lightning. I watched as it rain into the distance, howling viciously. 
It was many hours later that my host returned, soaking wet with traces of blood on them. I slept that night with a silver dagger in my hand, lest they try to attack me, but they did not. In the morning, as the storm dissipated and my host cooked breakfast, I took a risk and asked them of their lycanthropy. They looked at me strangely, and while they admitted they were a Malarite, they repeatedly iterated that they were not suffering from lycanthropy—that their lack of the condition had in part led them to a more solitary life. Indeed, when preparing food, I saw them handle a silvered knife with no issue, they even had wolf’s bane flowers hanging in their shelter!

not great thats for sure! not really because she finds anything particularly "ugly" but he's just so unwell that everything feels disgusting and embarrassing. he does like to dress up and feels better doing that but just baseline body its like. i sure do have, one thats so annoying

Nivayah is a lesbian with nebulous gender and it definitely made things harder for them growing up but as it is now he's content with that if not anything else but it wouldnt effect his story much overall

Simultaneously none and also way way too much. why would he care what some lame normie says about them however in his online spaces he cares so so much what they say about him, if they dont like him its all over he'd explode

nini doesnt idolize anyone specifically but the idea of peoples perfect lifes. also i guess like actual idols and whatever character he's into at the time, he's like WWJD but with them

pyre wont act like anything has changed but does like a good theatrical show of revenge. she'd probably attempt to publicly humiliate someone whos pissed her off. wouldn't ruin her reputation by being outright but she'd def start some shit for them

Physically she knows shes attractive and is confident about that, about the like. lack of anything inside her though she doesnt think about it much if she can its like ignoring a cavity everyday until it just gets too bad.

7. How easily and frequently do they ask for help
Panini has to go through 5 trials of psychic torture only seen in greek myths before they even think of asking for any kind of help. If he can't do it himself he'd rather give on it entirely. Pyre might be a little slow but overall she knows that the best way to get anything done is just having lots of connections to lend a hand so why not use them?
19. What's their strongest emotion, the one which mostly rules them and compels them to action
Nini: Anger. It's normally pretty bottled up but once he's hit it its like a house on fire, a lot of his worst choices are made like this
Pyre: Desire. For most of her life she really wasnt allowed to consider her own wants but not that shes able to indulgence is her entire goal
37. Presented with three different ponds offering eternal health, strength, or wealth, which one would they choose to drink from, they can only drink from one
Pyre would choose wealth, not necessarily because she wants it all but wealth means you never have to want for anything, lifes a breeze with that. Panini wouldn't even answer that he says thats not enough info and gets debilitated by choice (they'd eventually land on strength)
45. What is their fatal flaw
Pyre's hedonism comes to the point where she doesn't actually give a shot about her own well being or if its right for anyone, even if it means breaking herself in the process if she gets it its good. Nivayah wants validation extremely badly, it makes her bitter, anxious, apprehensive, the problem is that there arent many people who they care about enough for it to mean anything from, not even himself.
