ATLA IS BACK! 20 new castings confirmed!

Today is a big day because Netflix has officially confirmed 20 new castings in the upcoming live-action Avatar: The Last Airbender series, which is now officially done both 7 months of principal photography and one month of reshoots, and now in post-production!

Four castings are brand-new:

Lieutenant Dang’s position in the Fire Navy is now confirmed to be Commander Zhao’s second-in comand.

The age of the character of King Bumi is confirmed to indeed still be an old man (“ancient” in the colloquial sense).

Avatar Kuruk

We have an official character description for Avatar Kuruk, which will be very interesting to anyone who has read the Chronicles of the Avatar novels: “A previous Avatar with a haunted past.”

(This could also be interpreted as being about Ummi, which of course factors in, but I do have confirmation that it’s related to the novels although I can’t say more since it’s spoilers for the end of the show!)

Three other castings were not confirmed today:

I still have very strong confirmation these are still true, they just weren’t in today’s “main” Season 1 cast announcement. There will of course be dozens more characters than the ones officially revealed so far!

But, what feels like the biggest deal today in some ways is that after more than 9 months of complete, top-secret radio silence (285 days!), live-action ATLA is back with official news! This is a really good sign for times to come.

It’s also cool to me that they made their own special announcement for this rather than including it in Netflix’s Tudum Global Fan Event with dozens of other shows and movies, which was only just a few days ago. Because this show is special, it’s set apart, and it’s gonna be BIG. Now we just wait...

MY ATLA LIVE ACTION S1E3 THOUGHTS

EP2 - EP4

This episode might simultaneously be the most and least conflicting to me. I largely enjoyed watching it, and a lot of my complaints are very nitpicky. That being said, if they move too many plot points around to write things out, they run the risk of the adaptation turning into something else entirely. 

If I, as a fan of the original, am able to understand why things are being changed, and where they’re going with those changes, then it is ultimately fine. So far I’m mostly able to understand the changes and where they might lead. This episode gets a little bit interesting in that aspect for me personally. (And it’s fine if people disagree!!)

The way they condensed the plot in episode two made sense, but I think this episode they were trying to weave a lot together, and I guess I can’t say yet if it worked out or not. To keep this section spoiler free, I won’t go into it, but I’ll def talk about this again once I’ve seen the whole show. 

Meanwhile, I’m still utterly perplexed by Zuko’s character. Like. He’s fine I guess. But they’re doing a few things with him here and there that I personally wouldn’t. I’ll reserve judgment until the end, I hope they can ultimately pull it together. 

In other news, as most of us know, because of her having been announced as being cast before the show was out, Azula is in this season. Her introduction and her involvement in book one is all original to this series, and so far I can’t find any fault with it. This actress is super believable as Azula, and because I’m enjoying her portrayal so much I just have to continue to hope that her insertion into book one’s events won’t interfere with the adaptation’s believability as Avatar the Last Airbender overall.

Netflix has announced 20 new cast members for the live-action Avatar: The Last Airbender series.

From the Water Tribe we have Amber Midthunder as Princess Yue, A. Martinez as Pakku, Irene Bedard as Yagoda, Joel Oulette as Hahn, Nathaniel Arcand as Chief Arnook, and Meegwun Fairbrother as Avatar Kuruk. From the Earth Kingdom we have Arden Cho as June, Utkarsh Ambudkar as King Bumi, Danny Pudi as the mechanist, Lucian-River Chauhan as Teo, and James Sie as the cabbage merchant. From the Fire Nation we have Momona Tamada as Ty Lee, Thalia Tran as Mai, Ruy Iskandar as Lt. Jee, Hiro Kanagawa as Fire Lord Sozin, C.S. Lee as Avatar Roku, François Chau as the Great Sage, and Ryan Mah as Lt. Dang. I've noticed you're learning a lot of languages! I'm a linguistics major and different languages have always been super interesting to me! (I speak several languages at home too lol) What languages do you already know how to speak/are learning? (Other than English ofc)

Oh wowwwww!! I had no idea that you were a linguistics major! It’s something that I’ve always been interested in but wouldn’t have the opportunity to study! What sort of topics do you cover and what’s something interesting that you’ve learned? Has it made you notice things about the way that people speak irl? Has it made you appreciate something about speech and language that you might have missed otherwise? Lol I’m turning the ask around on you sorry hahaha 

I’m going to add a bit about English anyway lol I have probably above average mastery amongst university-educated peers. Like any native speaker, there are always archaic or technical words which crop up when reading a textbook or reading an excellent fanfic every now and then. Amongst a descriptive 40K+ long chapter I read the other day, I had about 3 words I wasn’t sure of? For most books I don’t get more than 3-5 words that I’m not sure of because fiction books don’t use that complicated language tbh. Usually you can surmise the meaning through context, but I like looking them up just to get the dictionary meaning which is why I remember how many words I don’t know. I wasn’t particularly a voracious reader when I was younger until late high school, which is why I would say my vocabulary is above average comparing to university peers. My grammar is excellent though I would say. My active vocabulary is probably lacking haha

I speak Teo Chau at a conversational level (not well, but better than I used to). Listening is 99% (there are still words that I don’t know because they don’t tend to come up in day-to-day living and then suddenly I’m hearing a word my mum has never said before in my life). 

I’m also hoping to learn Mandarin to use at work, I’ve got the textbooks for beginners and intermediate level stuff, and I used to learn Chinese up til Grade 6 at Saturday school (they used immersion-based learning mostly), but my knowledge on that is holey because there are some complex words that I know and a bunch of simple characters I don’t know LOL I can listen well though, better than Japanese. 

I did Japanese Continuers for my HSC (got a Band 6 woo!) though I don’t use it at all because no one Japanese really lives in my area of Sydney (they tend to live around North East Sydney as far as demographics have shown). I can still listen and read fairly well.

Also I want to learn Korean because I’m a big kpop fan as everyone knows. I can listen and understand common words when watching k-dramas, but I haven’t really actively studied it apart from some super basic grammar, and I can read and type Hangul fairly effectively. According to 10fastfingers, I have a touch typing speed of 45 wpm so I can type quickly enough for my purposes since I’m not messaging anyone haha

I also know a bunch of food words in Vietnamese because my parents lived in Vietnam and cook it often.

Hepatitis E from Pork

This story from my perspective begins on the 28 December 2015- at 5.27 pm precisely. I know because that exact moment is captured on the WhatsApp chat on my handphone.

That’s my virology colleague remarking that he was signing out more Hepatitis E results than he expected. I’m no virologist but it did seem odd to me too. Hep E virus (HEV) is a positive-sense, single-stranded, non-enveloped, RNA virus that is spread by the faecal-oral route. When I was a microbiology trainee I was taught it was a disease mainly of developing countries due to ingesting contaminated water and shellfish. The sanitation in Singapore is relatively good. Unless travellers were importing the infection, there was really no reason to find many cases of Hep E in Singapore. So why should there be a spike in cases when there really should hardly be any cases in the first place?

You can follow our train of thought below as we approached this conundrum.

The suggestion that it was zoonotic was a bit of a stab in the dark on the basis that HEV is acquired by ingestion. Given we have access to clean water in Singapore, a contaminated food source seemed a more likely explanation. We were also influenced by recent experience of a large Group B Streptococcus acquired by eating raw fish.

With some judicious googling, my colleague soon stumbled on a link between pigs and HEV which I had hitherto been unaware of.

Even if you find the virus in food, it still needs to be ingested in an infectious form. Aren’t people normally aware that you should only eat pork that has been thoroughly cooked (to avoid parasites)? Coincidentally, a few days before this conversation I had first hand experience of a potential risk exposure event (top photo). One of the local dishes in Singapore is congee where raw pigs liver is placed on top of hot rice porridge. As anyone who likes eating offal knows, there is nothing more dreadful than overcooked liver, so the connoisseur will try and time it such that the liver is barely just cooked. It was still bloody when I bit into it.

So we had a working hypothesis. Over the next few days a bit of digging around revealed a number of interesting facts.

1. When we looked up the Ministry of Health’s Weekly Infectious Disease Bulletin, we found that there were more cases of Hep E (97) than Hep A (70) reported in 2015.

2. A lot of other developed countries were noticing unexpected increases of Hep E cases and these were increasingly linked to undercooked pork products (particularly sausages and pates).

3. Hep E had been already been studied in Singapore in 2013 (Tan LTC et al, 2013). At that time the association with pigs had in fact been considered but there was insufficient evidence to draw a firm conclusion about the food source, partly because the molecular tools to genotype HEV were not widely available at the time. HEV genotypes 1 and 2 are restricted to humans and cause the traditional outbreaks found in developing countries as a result of poor sanitation. Genotypes 3 and 4 may be found in humans but also have animal reservoirs, in particular pigs. 

So we embarked on a new study together with colleagues in the National Environmental Agency (now Singapore Food Agency) which has finally been published (Wong CC, 2019).

The findings can be summarised thus.

1. Hep E has been increasing in Singapore. From 0.92 cases per 100,000 population in 2009-2011 to 4.1 cases per 100,000 population in 2016.

2. The increase is predominantly in the Chinese population. Chinese (86%) were over-represented among those HEV IgM positive compared to Indians (5%) and Malays (2%), even taking into account the National ethnic distribution.

3. The majority (85%) of HEV were genotype 3. 15% were genotype 1 and 1 was genotype 4. Genotype 3 was found in both pig livers and human infections among the Chinese.

4. A different genotype of HEV which is not associated with animal sources was found in the Malays (unsurprising since Muslims do not eat pork).

During the long gestation time for this paper, a number of other local studies have been published.

A fatal case of chronic Hep E in  a solid organ transplant recipient (SOTR) in our hospital triggered a retrospective review of 16 cases, eight of which were SOTR (Teo EC et al, 2017)*. 88% were HEV genotype 3. 

Earlier this year another team of Singapore investigators  (Zhu YO, 2019) published on viral sequences from 15 immunocompromised patients with chronic Hep E at the National University Hospital. All were HEV genotype 3. Interestingly they found two patients who shared extremely similar viruses who had residential addresses, which were within one kilometre of each other suggesting  the possibility of common food sources.

So the conclusion is that eating raw or undercooked pork products may contribute to the majority of Hep E cases in Singapore.

What are the risks?

In most healthy people, Hep E is a mild infection and may not even be noticed.

However it may cause chronic infection in those with pre-existing liver disease and more severe infection in the immunocompromised.

There are implications here for public health and food for thought for those looking after immunocompromised patients and those running blood banks.

2. Các bài thuốc dân gian chữa viêm khớp cùng chậu

Hiện nay, cách điều trị viêm khớp cùng chậu bằng thuốc Tây chủ yếu là điều trị triệu chứng nhằm nhanh chóng cắt các triệu chứng của viêm: sưng, nóng, đỏ, đau để đẩy lùi cảm giác khó chịu của người bệnh.

Đun lửa nhỏ cho đến khi mễ nhân chín mềm ra thì thêm chút đường trắng vào ăn khi còn nóng.

Đun nhỏ lửa vì nếu lửa to hàm lượng tinh dầu bay hơi sẽ giảm tác dụng.

Nguyên liệu:

1 nắm lá lốt tươi

3 chén nước

Cách làm:

Cho nước vào nồi đun sôi, rồi cho nhỏ lửa đun còn 1 chén nước

Thái những lát mỏng

30g rễ cây trinh nữ

500ml nước sạch

Cách làm:

Xương chậu hẹp có sinh thường được không? Xương chậu nằm ở phần cuối của cột sống thắt lưng, nằm dưới thắt lưng bao quanh phần xương cột sống đoạn dưới. Xương chậu có hình cánh quạt gồm 4 bờ, 2 mặt và 4 góc, do 3 xương hợp thành: xương cánh chậu ở trên, xương mu ở trước, xương ngồi ở sau.

Khi từ bụng mẹ ra bên ngoài, thai nhi nhất định phải đi qua khung xương chậu. Cấu tạo của khung xương chậu hoàn chỉnh, cân đối và thường là đủ rộng để một thai nhi có thể tích trung bình có thể chui qua được dễ dàng. Một số trường hợp, khung chậu hẹp sinh thường được khi thai nhi nhẹ cân (<2500g) và ngôi thuận trên người chưa mổ lấy thai lần nào. Còn lại, đa phần các trường hợp các sản phụ có xương chậu hẹp cần phải mổ lấy thai để đảm bảo an toàn cho cả mẹ và con.

Khung xương chậu hẹp hay gặp ở những bà mẹ nhỏ bé, thấp, chiều cao dưới 1,45m. Còn nếu khung xương bị biến dạng thì nhìn hình dáng bên ngoài hoặc quan sát dáng đi cũng có thể nhận biết được. Các phương pháp thăm khám bao gồm:

Hỏi bệnh sử

Đây là phần thăm khám đầu tiên và quan trọng. Bác sĩ có thể tìm được nguyên nhân gây ra tình trạng xương chậu hẹp như: còi xương, suy dinh dưỡng, lao khớp háng, bại liệt, chấn thương…

Khám toàn thân:

Khám toàn thân thường thấy một sản phụ thấp bé < 1,50m, đặc biệt là dưới < 1,45m. Ngoài ra còn thấy những di chứng của các bệnh lý trước đây: ngực độ cao của còi xương tuổi thơ ấu, dáng đi khập khiễng, trật khớp háng, một bên chân ngắn một bên dài…

Khám khung chậu:

Đo các đường kính ngoài của khung chậu sẽ thấy giảm đáng kể. Độ trám Michaelis sẽ thấy mất cân đối trong khung chậu lệch.

Đo các đường kính ngoài của khung chậu (đại khung) sẽ thấy giảm đáng kể.

Đo các đường kính của tiểu khung đặc biệt quan trọng là nhô – hậu mu của eo trên sẽ thấy < 10,5cm.

Chụp X- quang:

Chụp X – quang giúp bác sĩ biết chính xác eo trên có hẹp hay không. Khi chụp cho tia X đi thẳng góc với eo trên, phía dưới đặt một lưới có kẻ ô từng cm2 và để khoảng cách tia thích hợp. Bác sĩ sẽ đánh giá được tương đối chính xác các đường kính eo trên khung chậu.

Theo các chuyên gia, các bà bầu cần thăm khám thường xuyên theo chỉ định của bác sĩ. Đặc biệt là khi có các dấu hiệu bất thường như: đau khớp xương chậu kèm theo biểu hiện tê cứng chân, đau dai dẳng ở vùng chậu hông giữa hai mông, chân vòng kiềng dẫn đến chân to, mông xệ, khoảng cách giữa hai chân lớn là dấu hiệu của giãn xương chậu, cơn đau lan dần xuống đùi và có dấu hiệu teo cơ vùng mông đùi, khi quan hệ thấy đau, sốt hoặc rét run người, buồn nôn, choáng váng… cần đến khám ngay tại các cơ sở y tế uy tín.

Group B Streptococci in sushi and sashimi

I have previously posted about a large community Group B Streptococcus (GBS) outbreak in Singapore (Kalimuddin S, 2017).

In that post I alluded to the fact that my original hypothesis was that people were getting GBS by eating raw tilapia in sushi and sashimi as a result of a trade practice known as fish species substitution, where a cheaper fish species  is substituted for a more expensive one and given a similar sounding name.

Somewhere on that plate is a serotype Ia ST7 GBS.

The GBS outbreak eventually turned out to be due to serotype III ST283 GBS found in bighead carp, giant snakehead and tilapia. People were getting sick consuming raw fish dishes in local Chinese stalls rather than Japanese-style restaurants.

However the question over whether GBS could be found in sushi and sashimi continued to bother me. I got a chance to study this when 2 students from Temasek Polytechnic were attached to our laboratory in 2015 (Koh TH, 2017). These students were required to do a small project so I set Terry and Fionicca off to purchase sushi and sashimi from the cheaper Japanese-style food outlets  and supermarkets dotted across Singapore. At the end of the day we collected 17 fish samples sold ready-to-eat. Twelve samples were salmon, 4 were sold as ‘Tai’, and one was actually sold as tilapia. The fish were processed according the method detailed in a previous post.

We isolated 2 GBS. One was serotype II ST1 GBS resistant to tetracycline from a Salmon sample. The other was serotype Ia ST7 GBS susceptible to tetracycline from the tilapia sample.  We were also able to find molecular evidence for serotype Ia ST7 GBS in a sample sold as ‘Tai’ even though we were unable to isolate the organism in culture. We did not find any serotype III ST283 GBS though this may be a limitation of the small sample size.

Besides serotype III ST283 GBS, there are two other types of GBS that are important fish pathogens, serotype Ib ST260 and serotype Ia ST7.

Serotype Ib ST260 (originally named Streptococcus difficile) is the original GBS pathogen of fish that was described as early as 1966.  They are characteristically non-haemolytic. This type affects many species of fish and is widely distributed. However, there is some data to suggest that this GBS type is not found in Singapore, Malaysia, and Thailand, but may be found in Indonesia. When I first started looking out for zoonotic GBS infections in 2008, I was focused on serotype Ib ST260. In retrospect this was probably a mistake. This type is so niche-adapted that it has a reduced genome content, and so far has only been found in poikilothermic (cold blooded) animals like fish and frogs (Delannoy CMJ, 2016). It has never been described in humans and probably poses no zoonotic risk.

The origins of serotype Ia ST7 GBS are less certain. This type has caused human infections in neonates in Japan (Evans JJ, 2008), and adults in Iceland (Bjornsdotir ES, 2016), but on the whole is rarely found in humans (note both Japan and Iceland have populations known for high fish consumption). On the other hand, serotype Ia ST7 GBS have caused many outbreaks in fish, especially tilapia, in asia and the middle east. This type has also recently caused an outbreak locally in Singapore among farmed golden pomfret (Trachinotus blochii) (Chong SM, 2017). Interestingly in that manuscript, the authors say that GBS had not been detected in cultured marine and freshwater fish in Singapore prior to 2014. To confuse matters, an old American type culture collection strain of GBS (ATCC A909) was recently found to be serotype Ia ST7 (Tettelin H, 2005). This was isolated from a human neonate with sepsis in 1934! The genome of ATCC A909 was recently compared with that of serotype Ia ST7 GBS isolated from tilapia in China and found to be similar (Liu G, 2013).

So is serotype Ia ST7 GBS a zoonosis or do fish get infected as a result of human effluent polluting the environment? Delannoy et al suggest that serotype Ia ST7 and serotype III ST283 GBS that cause fish infections probably have a primarily homeothermic (warm blooded) host range and have acquired fish-associated virulence factors not usually found in human or bovine isolates (Delannoy CMJ, 2016). So perhaps an anthroponosis first then a zoonosis later?

What of the serotype II ST1 GBS resistant to tetracycline from Salmon? We think this was probably the result of human contamination of fish (human clones of GBS tend to be tetracycline resistant whereas fish isolates tend to be tetracycline susceptible).

Going back to the issue of fish species substitution and mis-labelling of fish. This is a widespread practice in the fish trade. Customers may not know the subtle difference between dory fish (Vietnamese catfish Pangasius spp. ) and the rather more prized John Dory (Zeus faber).

A butterfish (Escolar, Lepidocybium flavobrunneum) in Singapore is not the same as a butterfish in South Australia (Mulloway, Argyrosomus japonicas). It is important to know the difference! As a cautionary tale I refer you to a blogpost by MIPHIDIC.

One of the more interesting parts of our study was trying to identify the actual fish species that was being sold as ‘Tai’ which most people would probably assume was the fish you normally see on ‘Japan Hour’* which is the Red Sea Bream (Pagrus major).

We did this by a method known as fish barcoding. This involved sequencing the cytochrome c oxidase subunit I gene of the fish meat and submitting the sequences to the Fish-Bol database.

Of the 4 ‘Tai’ samples, 2 were indeed Red Sea Bream (Pagrus major), 1 was Crimson Snapper (Lutjanus erythropterus), this was the sample that had molecular evidence for serotype Ia, ST7 GBS. The other sample was a Nile tilapia (Oreochromis niloticus). So if you want to be sure of what fish you are eating, order the whole fish or go to a reputable restaurant (and be prepared to pay more).
