The RSP system features an anatomic humeral neck-shaft angle. In 1977, Charles Neer and his colleagues invented the term “cuff tear arthropathy” and eventually provided the first detailed description of rotator cuff tear arthropathy (RCTA) in 1983. RCTA has three major characteristics: ☑️first, massive rotator cuff tear;☑️☑️ second, degenerative changes (i.e., glenoid erosion, loss of articular cartilage, osteoporosis of the humeral head, and eventually humeral head collapse); and☑️☑️☑️ third, superior migration of the humerus resulting in “femoralization” of the humeral head and “acetabularization” of the coracoacromial arch. Despite the different criteria used to define a massive rotator cuff tear, the result of a massive rotator cuff tear is the destabilization of the glenohumeral joint and the attritional destruction of the primary static stabilizers, leading to chondral wear and subsequent osteoarthritis. It is noteworthy that massive rotator cuff tears, although technically challenging to repair, are not necessarily Irreparable. 🔥⛔Signs of irreparability include static superior migration of the humeral head, a narrowed or absent acromiohumeral interval (AHI), and fatty infiltration affecting 50% or more of the rotator cuff muscles. ☢️The Hamada classification system de- scribes structural changes within the coracoacromial arch and changes in the acromiohumeral interval (AHI) on anteroposterior radiographs as the bases for Classification. This system divides massive rotator cuff tears into five radiographic stages, with consecutive stages indicating disease progression.
