Ninety-right percent smooth. That's how I would rate myself for how well I performed yesterday during the validation of my nomination documents at Binangonan Elementary School, Binangonan, Rizal.

Maybe you're wondering why I used the word "performance". Well, it's actually my speaking performance I am referring to. I've been rating myself every time I speak in public or if there's a speaking situation I need to ace.

You see, for people with stutter (PWS) like me, speaking with people in authority, talking to strangers, finding ourselves in stressful situations are just common triggers for an episode. Some would avoid it, while others face it head on. For me, there's no turning back! Andito na tayo, so might as well fight!

So I started a little bit shaky (like I always do), but I managed my stutter just enough for me to share a lot of stories to the validators. With that, I left the room so relieved, replaying the positive comments from the validators. They won't stop resounding in my head. It has been two hours since the validation and we were on the way home, but I still can't move on.

These validators, who are both Public School District Supervisors (PSDSs) in San Mateo, Rizal, were so patient with me. I just wished everyone was like them. They listened to me intently, not minding my stutter, not even raising an eyebrow on me as they waited for me to finish speaking. And that's what made me feel at ease, that I started talking and talking whenever I had the chance. Syempre I had to avoid dead air.

Honestly, I feel like an idiot right now, for I wasn't able to take decent photos with my co-teachers and school head, and with the validators as well. Ganun pala talaga pag super nervous 'no?

This is another experience I won't ever forget. And I'm just so thankful for the overwhelming support I've been receiving not only from my family but from our PSDS Dr. Pitsberg De Rosas, the entire DepEd Baras District family, my students (both old and new), and from my friends from all over the planet.

External Validation of the Prognostic Nomogram (COMPASS) for Patients with Peritoneal Carcinomatosis of Colorectal Cancer

Abstract

Background

To optimize outcome, selection of patients for cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) is critical. Recently, Simkens et al.7 evaluated the peritoneal surface disease severity score (PSDSS) and suggested a new prognostic nomogram, the colorectal peritoneal metastases prognostic surgical score (COMPASS) based on age, peritoneal carcinomatosis index score, locoregional lymph node status, and signet ring cell histology. This COMPASS nomogram had better discriminative ability according to the Harrell c-index than PSDSS (c = 0.72 vs. 0.62). This study aimed to validate the COMPASS nomogram externally.

Methods

Data were retrieved from a prospectively maintained database, and all patients who underwent surgery between May 2005 and May 2016 were included in the study. For each patient, the PSDSS and COMPASS were calculated and then divided into subgroups. The discriminative ability of both scores for overall survival were quantified using Harrell c indices.

Results

A total of 153 patients underwent CRS + HIPEC for peritoneal carcinomatosis (PC) of colorectal cancer. The median overall survival (OS) was 46 months, and the mean PSDSS was 7.8 ± 3.7. When the PSDSS was divided into subgroups, the c index was 0.67. The mean COMPASS was 55.5 ± 25.6. When the patients were divided into four groups according to cutpoints of Simkens et al.7 a c index of 0.72 was obtained, showing its significant superiority over the discriminative ability of the PSDSS (p = 0.016).

Conclusion

External validation of the COMPASS confirms its moderate to good discriminative ability and its superiority over the PSDSS. Nevertheless, discrimination with the COMPASS score remains suboptimal, and further research on prognostic variables is essential for optimal patient selection.
