So, I'm a long-term migraine sufferer who has severe chronic vertigo (I've been spinning constantly since 2010). Not being able to keep things down is a regular occurrence for me especially when my vertigo goes full tornado.

I'm sure you've tried the non-medicine stuff like sucking on crystalized ginger, watered down Gatorade (at room temp), saltines, and broth. I know my doctor needed to know that non-medicine based options didn't work for me much of the time before they moved forward. One thing that I struggled with early on was hydration, which made me even more nauseous and which started a vicious circle. To get any water, I found that I couldn't drink actual tap water and that most bottled water didn't work. I had to go for spring water (for me Fiji is the best) to even have a hope of keeping it down. So now I keep bottles of the stuff stashed near my bed/couches in case an attack is triggered and I can't move.

Medicine-wise for over the counter stuff I've found that Tums in concert with famatodine works best--if my issue isn't vertigo or migraine based. Alka-seltzer I couldn't keep down--ginger ale/7-up was better TBH. For my migraine based stuff, I have a standing prescription for Promethezine (Phenergan). It can be taken either orally or rectally (which is good if the nausea is bad enough). There's also an injection, apparently, but I don't know as much about that. I've found that if I take promethezine at the first sign of aura that it can stave off the worst of the symptoms and make it so I can at least keep down really super bland foods like Chicken Noodle soup or saltines.

I saw that Joy mentioned Ondansetron (Zofran) as an option. And I've used that one too. It's really effective, but I have to be careful with it since the dissolvable pills contain an allergen of mine. This one also comes in a straight swallowable pill form (and they are tiny). It works fairly fast, but the down side is that one of the side effects is a headache. I haven't experienced that side effect, but it is a known one.

Both of those are available as generics, which should make your insurance happy. If your insurance doesn't cover them, the drug itself is fairly cheap (I paid something like $40 for a 30-day supply of promethezine when I was without insurance a few years ago)

You can also ask your doctor about medications that are used to treat things like hyperemesis gravidarum and post-surgery nausea--specifically Metoclopramide or Prochlorperazine. There's a lot of options.

I occasionally have a migraine that causes such intense nausea and vomiting that I can't keep down anything, including water or crackers or anything bland that I can usually stomach

I was wondering if there was any medicine you've tried that helped? my mom has recommended alka-seltzer but maybe you or one of your followers has experience with anything else?

I used to be able to preemptively stave it off by taking nexplanon, but insurance no longer covers it so 🤷‍♀️

A common anti-nausea med often prescribed as part of “migraine cocktails” at the hospital is zofran, but you can usually get a prescription for it from your regular doctor. I think the generic is ondansetron. I get prescribed that a lot for my migraine-induced nausea. I actually need to get it refilled.

Alia-seltzer works for some people because of the anti acids, but the aspirin isn’t always great long term on your stomach.

If you’ve tried zofran and it doesn’t work, it’s time to go back to your doctor and ask for a different antemetic.

After three attempts (and all of us having to sanitize my room after the first, uh, failed), I've finally managed to keep some promethezine down. Unfortunately it's not enough for me to handle anything more than water, but I'll take what I can get.

Also I'm gonna have to figure out a way to sleep in one of the armchairs. Trying to lie down seems to just make the nausea worse.
