Shoulder dislocation and instability: a more in-depth approach
Simplified anatomy of the shoulder
To better understand these pathologies, it is useful to know the main elements of the shoulder joint:
the humeral head: the upper end of the arm bone (humerus), which articulates with...
Glenoid: rounded cavity of the shoulder blade.
Joint capsule: envelope that surrounds and stabilizes the joint.
Ligaments: fibrous structures that reinforce the capsule and limit movement.
Rotator cuff: group of muscles that ensure shoulder stability and rotation.
Mechanisms of dislocation In a dislocation, the humeral head protrudes from the glenoid, resulting in a loss of contact between the two articular surfaces. Anterior (forward) dislocations are the most common. Risk factors for instability :Repetitive trauma: micro-trauma associated with sporting activities (volleyball, handball, etc.). Ligament laxity: a certain genetic predisposition can lead to looser ligaments. Defects in coaptation: a malformation of the glenoid joint can lead to dislocation. Rotator cuff lesions: muscle weakness can impair shoulder stability.
Types of instability :Traumatic instability: linked to a specific event (fall, shock). Atraumatic instability: occurring without apparent cause, often linked to ligament laxity. Recurrent instability: repeated dislocations. Multidirectional instability: the shoulder may dislocate in several directions.
Diagnosis Diagnosis is based on
Clinical examination:
look for pain,
limitation of movement,
crepitus (cracking sound),
laxity.
Imaging examinations
X-rays, MRI, arthro-MRI to visualize lesions.
Treatment The choice of treatment depends on the severity of the instability, the patient's age and activities. It may combine :
Reduction of the dislocation: manual maneuver to restore the humeral head.
Immobilization: splint or sling to relieve pain and allow healing.
Physiotherapy: to strengthen muscles and restore mobility.
Surgery: in cases of recurrent instability or significant injury, various surgical techniques may be considered to stabilize the joint (ligament repair, lateroplasty, etc.).
Possible complications
Early osteoarthritis: premature wear of articular cartilage.
Nerve damage: axillary or supra-scapular nerve damage.
Joint stiffness: if immobilization is too prolonged or if rehabilitation is inadequate.
In short, shoulder dislocation and instability are pathologies that can be very disabling. Early diagnosis and appropriate treatment are essential to restore normal shoulder function.

Professional Home Physiotherapy Services in Cambridgeshire
The Benefits of Home Physiotherapy Services: Healing in Comfort
There has been an increase in demand for home physiotherapy services in recent years, as patients increasingly want to receive specialized care in the comfort of their own homes. Home physiotherapy offers qualified specialists' expertise right to the patient's door, allowing for tailored treatment programs, convenience, and better outcomes. In this article, we'll look at the advantages of home physiotherapy services and why they've become a popular option for people looking for effective rehabilitation and recovery.
1 Convenience and Comfort
One of the key benefits of home physiotherapy services is the convenience they provide. Patients no longer need to drive to a clinic or hospital, which saves time and energy, especially for individuals with mobility issues or restricted transportation options. The cozy and familiar setting of their own house also leads to a more comfortable healing process, which can improve their general well-being.
2- Personalized Care
Every patient's condition is unique, and home physiotherapy enables highly personalized treatment approaches. A physiotherapist can better grasp the patient's lifestyle, home setting, and individual issues when they visit the patient at home. This knowledge helps the physiotherapist to create a personalized rehabilitation program that meets the patient's individual needs and goals, resulting in more successful outcomes.
3- Increased Compliance and Motivation
Recovery from an injury or management of a chronic condition frequently necessitates persistent work and attention. Patients are more likely to stick to their treatment programs if they can receive physiotherapy at home. Patients feel more accountable and motivated during home-based sessions since they are actively involved in their treatment process in the comfort of their own homes.
4- Enhanced Family Involvement
Home physiotherapy also encourages more family involvement in the patient's healing process. Family members can witness the exercises and strategies performed during sessions, giving them the ability to provide support and assistance in between meetings. This collaborative approach frequently results in better outcomes and a faster healing process.
5- Ideal for Elderly and Pediatric Patients
Mobility and adaptation to clinical settings might be difficult for elderly people and young children. Home physiotherapy is a suitable alternative for these groups because it addresses their specific needs while also providing a comfortable and secure environment for treatment. Furthermore, this strategy helps to bridge the generation gap by strengthening the bonds between the elderly, children, and their caretakers.
Conclusion Home physiotherapy services have arisen as an innovative and successful method of providing patients with tailored care and rehabilitation. This strategy provides various benefits by delivering trained physiotherapists directly to patients' homes, including convenience, individualized care, increased compliance, and reduced infection risks. Patients can now begin their road to recovery with the help of their relatives and in the comfort of their own home. As the popularity of this service grows, it clearly marks a positive step forward in the evolution of modern healthcare.

Cuddling with specs hcs
Gn!reader
A/n Hello! Sorry I've been slacking with fics lately, I've been having some issues with my joints lately and it's been too painful to type for long periods of time :^( but I hope these hcs will tide you over for now! (Also I'm hopefully starting physiotherapy soon, which means typing won't be so painful!! Wish me luck hehe)
His absolute favourite cuddling position is the two of you laying face to face with his arms around your waist, holding you as tight as possible. He ESPECIALLY loves when you fall asleep and bury your face in his chest, he thinks of you like an adorable Teddy bear.
Sometimes he gets a little overstimulated by too much physical contact, but he still likes to lay beside you and hold your hand, or maybe just cuddle one of your tshirts so he can still feel close to you.
Even though you've been dating for a while, he's still not super experienced with relationships. Almost a year into your relationship, he still pulls the "yawn-and-stretch-his-arm-over-your-shoulder" move, and still gets giddy when you pull his arm over you properly and snuggle up to him.
He LOVES kisses, and often can't help himself from interrupting you as you speak with a kiss. You'll be laying in bed facing one another, just talking about comics or music, and all of a sudden his lips will be on yours. It's just that he finds you so endearing when you get passionate about something, and that's his way of showing you he cares about what you have to say.
(Slight nsfw) if he's feeling particularly cheeky, he likes to spoon you so he can cop a feel- you find it adorable how bad he is with subtlety. Although, both of you have learnt not to cuddle like this if you've got a big day ahead of you tomorrow, since it never really results in much sleep.
And finally, cuddling is not limited to behind closed doors, he pretty much always has a hand around your waist, or he's leaning his head on your shoulder, silently begging you for a kiss with his pretty green eyes

A Magnet For Trouble: Epilogue: Our Home
written for the @billybigbang2024 Read from CH 1
Art by @akichania HERE
Read here or on AO3
TW: Smut
Billy and Jason’s cohabitation lasts one week, during which they fight about everything, from the socks left on the floor to what they should order for takeout. Still, it’s the best week Jason has ever had. Once Billy gets back to his place, leaving Chrissy with a neat list of dos and don’ts, Jason starts to feel that his tiny apartment is way too big for him.
Something is missing. Or, even better, someone. But he hasn’t the courage to tell Billy how much he misses him, so they just keep hanging out, watching some stupid TV series together, and going to the HideOut when Jason’s wounds get better. Now, after two weeks at home, three months of physiotherapy and desk work, which Jason hated with all his heart, the policeman is finally back patrolling with Patrick.
His arm is still weak and he has signed to the local pool to strengthen his muscles, and it's just a coincidence if he chose the same pool Billy goes to. After all, it was the tattoo artist who suggested that swimming could have helped him regain muscle strength without straining his shoulder joint, and Jason isn't purposely drying himself slowly just to enjoy Billy's body covered in water drops while he steps out of the shower.
"You're getting faster," Billy comments, grabbing a towel and wrapping it around his waist.
"You're still way faster than me, but thanks."
“I have been a lifeguard for years and you have an injured arm. You can’t compete with me. At least not now.”
They share a Gatorade bottle, sitting on the same bench, "So you're going back to patrolling the streets, huh?" Billy asks, drinking some more, and Jason stares at his Adam's apple bob up and down a few times before replying.
"I am."
Jason knows that Billy isn't enthusiastic at the idea, but it's his job and he loves it and he's looking forward to working with Patrick.
"No stunt hero shit, right?"
"I’ll try to avoid it," Jason replies, "but you know I can't promise you not to do my job."
"Fuck me. I should have fallen in love with a florist." Billy complains, drying his curly hair with a towel.
"Maybe you should have. But you fell in love with me, didn't you, precious?"
Billy's cheeks get a nice shade of dark pink when he turns his head to avoid Jason's stare. To be a very self-confident man he gets really shy when treated with kindness.
"I could still change my mind," Billy replies, offended.
Jason chuckles, turning Billy's head toward himself, "You could… but I really hope you won't because I'm madly in love with you… And I couldn't survive without Gloria's cooking."
"Fine... I'll keep you around a little bit longer," Billy replies, kissing him back. Just a gentle peck on Jason's lips, "By the way, are you free tomorrow afternoon?"
"I have a morning shift but I should be home by four."
"Good. I'd like to take you somewhere."
"How mysterious. Are you going to kill me and hide my body in some seedy motel?"
"Fuck you!" Billy replies, throwing Jason his damp towel before grabbing his clothes to go to get changed.
"Ok. Ok. But don't make me wait!" Jason hollers, while Billy flips him the bird before closing the changing room door behind himself.
***
"Ok, keep your eyes closed," Billy says, keeping his hands in front of Jason's face.
"Ok, ok. But at least warn me about the steps!" Jason complains, chuckling.
"I already told you I'm sorry. I forgot they were there! Are you going to hold it against me forever?"
"Maybe not forever but long enough to make you feel guilty and force you to do something you don't want to."
"Like what?"
"Like coming with me to the movies." Jason immediately replies.
"Oh no! No way! I'm not going to watch the last Disney movie with you."
"But Max liked it!"
"Max is a sixteen-year-old girl, and I really hope we don't have her same tastes. Anyway, we are here."
"We’re here… where?" Jason asks, turning his head, trying to catch some familiar sound or scent but he gets nothing. That’s definitely the quietest place Billy ever chose for one of their dates.
"Ok. Listen. I hope I didn't bite off more than I can chew but…" Billy stutters, while he slowly removes his hands from Jason’s face and the policeman flinches at the sudden light.
In front of him, there's a lovely building in red blocks with a huge wooden door.
"What am I looking at?" Jason asks, confused.
"Home."
"Yeah. I get that it's a home, but…"
"Our home." Billy says, fishing a bunch of keys with a golden J as a key holder, "I... I might have rented an apartment here.” The tattoo artist quickly adds, “The neighborhood is nice, there's a park just a couple of blocks down there where you could run in the morning. It's close to the metro station and it's just a few stops from the police station and from my shop."
Jason turns toward Billy, his eyes wide and his mouth gaping in astonishment, "Billy. Are you asking me to come to live with you?"
"Yeah. No. I mean… I'm giving you the keys. It's a two-bedroom apartment. We could be housemates or you could just have a room to crash if needed. I don't… I don't want to force you but I think things are good between us and knowing that every night you'll come home to me will make me feel better. But that doesn't mean you have to. I mean... If you need time it's ok. I know it’s a big decision and your place is closer to Chrissy’s so I don’t know if you really want to move or…"
"Things are good huh?" Jason asks, quivering an eyebrow.
"Yeah. Well… at least I think so? Maybe I'm wrong. Oh fuck! You want to break up with me and I'm giving you the keys to the new apartment. Oh fuck! Fuck! Fuck!" Billy stutters, panicking.
"Hey, hey! Billy! Calm down! I'm joking, ok? I'm just joking. Things are good. Things are great actually. And I can't wait to move in with you."
“For real?” Billy asks, biting his lower lip.
"For real," Jason confirms, wrapping his arms around Billy's neck and tugging him closer to kiss him slowly.
"Let's go inside, I want to show you the apartment." Billy grins, grabbing Jason's hand and dragging him inside, smiling widely like a child.
The apartment is spacious and bright, with a nice living room with a cozy couch where Jason can already envision himself snuggling next to Billy to watch some old action movie, eating popcorn and drinking soda, a little kitchen, a very nice bathroom, and two bedrooms. One has Billy's movie posters on the walls, the other is pristine.
"So this would be my room?"
"If you want to."
"What if I don't?" Jason asks with a sly smile.
"Well you don't have to move in with me. I knew it! I knew it was a rush move but…"
"What if I want to sleep in this room?" Jason asks, sitting on Billy's bed.
"Oh... ok. We could switch rooms and-"
"No. I want to sleep in this room. With you." Jason declares, staring at Billy in the eyes. He feels his cheeks get warmer but he doesn't lower his eyes. For the first time, he's the bold one in their relationship.
"I... I'd like that. Or even better. I'd love that." Billy replies, pushing Jason on the bed.
The policeman bounces on the mattress, chuckling, while Billy doesn't lose any time to straddle him and block Jason’s wrists over his head.
"Are you feeling cocky, tiger?" Billy asks, staring at him with his dark blue eyes.
"Maybe…"
"Think you'll get lucky?"
"I don't know, but I didn't bring anything for the housewarming party… apart from myself." Jason winks.
"I was trying to be polite and look at what you did to me!" Billy whines in Jason's ear, rubbing his hard dick against Jason’s leg.
“It’s not my fault if you have no self-control, Billy.” Jason laughs, while Billy latches at his neck like a vampire.
“I’m going to leave so many bruises you won’t be able to hide them.” Billy roars in his ear. And maybe Jason should be worried about it. Maybe he should tell him that no, he can’t have bruises that will show with his uniform, but the truth is that he doesn’t care.
"Do you have lube?" Jason asks, looking around the room.
"Of course I do, silly man." Billy mocks him, "But first… I want to taste you."
"You can't say things like that!" Jason complains, hiding his face in the pillow, "I'm going to come untouched."
"That's cute. You think I would let you?" Billy asks him, unbuckling Jason's belt with sure hands and popping the jeans' button open.
When finally Billy frees Jason from his pants, kissing his stomach and his legs, worshiping every part of his body, on Jason's blue boxer there's a dark stain of precum.
"You're the devil himself," Jason complains, biting on his fingers.
"Sorry to break it to you, but you're not the first who told me something similar," Billy replies with a self-deprecating smile, before lowering the blue boxer just enough that Jason's dick springs free but keeps the elastic band on his balls.
Jason opens his mouth to reassure Billy that he was just joking, that Billy’s absolutely perfect, but all he can do is bite back a loud moan when Billy's velvety mouth engulfs him in one go.
It's not the first time that Billy blows him, but every time it takes him by surprise how swiftly he can take him in his mouth.
Billy sucks a few times, hollowing his cheeks before retreating and dedicating himself to licking the head of Jason's dick while stroking him with his hand harder and faster.
"You having a good time?" Billy asks, staring at Jason in the eyes with a thin trail of spit trailing down his lip.
"Billy... Billy…" Jason begs, trying to grab Billy's hair, but the blond tattoo artist chuckles.
"Not yet, tiger." Billy mocks him, tightening his grip around the base of Jason's dick and the policeman whines pitifully, "Yeah, I'm so bad for wanting you to come on my dick. Am I not?"
At those words, Jason lightens up, while Billy grabs his fancy lube and moves Jason's legs on his shoulder.
"I'm going to fuck you so slowly you're going to howl in pleasure and frustration," Billy whispers, before lowering down to kiss him hard and harshly, a mix of his and Jason's taste in his mouth.
Pouring way too much lube on Jason's hole, and grinning while the excess travels down Jason's crack, making him shiver at the cold feeling, Billy starts to stroke himself, getting even harder.
Jason's eyes are pinned on his boyfriend’s glistering dick when he slowly starts to thrust inside him.
As he promised, it’s the longest and most frustrating sex Jason has ever had. He has no leverage to press back while Billy's thick dick stretches him so nicely but so slowly that all he can do is beg him to put it in faster. Being the little shit that he's, Billy doesn't indulge him, making up silly excuses like Jason's too tight, so all the policeman can do is tighten his grip on the bed’s sheet and get what his boyfriend is willing to give him.
When Billy finally bottoms out they're both drenched in sweat, the tattoo artist from the effort of being slow and considerate, the policeman from the effort of keeping still.
They finally kiss, almost chest to chest, and the kiss is hot and sloppy as if they couldn't control themselves anymore.
"Ready for the ride?" Billy asks with a cocky smile.
"I have been ready for hours!" Jason complains, and that's the last coherent thought he has.
After that, his mind goes blank, while his body shakes with pleasure, his self-control snapped somewhere between the kiss and Billy's fast rhythm.
Jason doesn't hear the wet sound of their bodies clashing into each other, he doesn't notice the bed bumping into the wall, all he can hear are his loud moans and Billy's suffocated grunts of pleasure. He comes with a scream that should probably make him feel ashamed, but he doesn't care. His hole clenches Billy's dick rhythmically, milking him to the bones, and Billy collapses on his chest with a gentle "Oof."
For a few moments they lay on the bed, their bodies crushed together into the mythological creature with four arms and four legs, and then Billy slowly rolls on the side, still holding Jason's hand.
"So, do you like the apartment?" he asks, staring at the popcorn ceiling.
"I do. I hope the neighbors will like me too." Jason chuckles, turning to his side to draw mindless swirls on Billy's chest.
"It's four o'clock in the afternoon. We're alone."
"So this was your plan all along? Luring me into your cave so you can fuck me silly?"
"Not really, but I'm not complaining either." Billy replies, tugging Jason closer to him, so that his head is resting on Billy’s chest, the tattoo artist’s heart beating loudly under Jason's ear, "Now let's be serious. Do you really like it?"
"I like it. And I like you. And I want to live with you. Is that the answer you were looking for?"
Billy doesn't answer, he just kisses the crown of Jason's head, while they remain like that, resting in their embrace.
Jason sighs contented, he never thought he would have been so happy. He’s in love with a wonderful man who just asked him, in his grumpy way, to move in together. And maybe it is true that Billy is a magnet for trouble, but Jason never felt luckier in his life.

As most of you know I have been struggling with my health recently. This is my experience of HSD. Each person with the condition is affected differently and no two people are exactly the same with it. Just as a pre-warning I will be attaching photos at the end of injuries I have had but there is no blood. They are just dislocations and subluxations I’ve had. I also talk about some of the injuries as well. I am not writing this for sympathy but more as an explanation as to why I sort of disappear every so often.
Hypermobility Spectrum Disorders or HSD is a range of conditions that affect joint hypermobility. Often people are referred to as ‘bendy’, ‘flexible’ or ‘double jointed’. It is fairly common in children and young people but only 20% of people never lose the flexibility in their joints. More information about this condition is on The Ehlers-Danlos Society website.
This is my personal experience with the condition. Some people with HSD will experience these problems and others won’t.
When I was nine I had my first dislocation when my brother accidentally stood on my hand. I was taken to hospital by my parents and they sorted it pretty quickly with no alarms raised. However, when I was fourteen it got progressively worse. I kept having subluxations in my pinky finger on my right hand. A subluxation is an incomplete or partial dislocation of a joint. When I kept going to hospital they told me that it was a result of the injury when I was nine and they taught me how to manipulate it back into place. This happened almost everyday for two years at least.
The next year I dislocated my right thumb. Due to a medical error they missed the dislocation and the fracture on the x-ray and left it a week before noticing their mistake. I went back in to have it relocated a week after the injury. This is when I found out that I am immune to local anaesthetics so I had no pain relief when they did this. Due to a very observant doctor after my thumb dislocated inside of a plaster cast I was diagnosed with Joint Hypermobility Syndrome (the previous name for HSD).
After this I started having more subluxations and dislocations including in my ankle, both knees, my hip, my shoulder and my first and middle finger on my right hand.
Earlier this year I went to A&E with a swollen finger and they suspected rheumatoid arthritis. However, when I went to rheumatology I found that I was dislocating my finger in my sleep. That’s when my diagnosis name was changed to HSD. I scored a 6/9 on the Beighton Score. It is a system to measure joint flexibility. 0-3 is normal and 4-9 is when there is potentially a problem. I can hyperextend most of my joints and often I don’t notice when I am doing so.
I have now been receiving occupational therapy and I am on the waiting list for hypermobile physiotherapy.
My main symptoms include:
Fatigue but unable to sleep
Pain and stiffness in joints
Frequent strains and sprains
Frequent dislocations and subluxations
Poor balance
Bladder and bowel problems
I also suffer from scoliosis which I have had no treatment for and cluster headaches.
HSD is incurable and the only things I can do to help is have pain relief, hot baths, hot water bottles and hand warmers and use heat rub creams.
This condition affects my life quite severely. I haven’t got the worst symptoms someone can have but I am struggling to do daily tasks and even get out of bed some mornings. I use multiple types of joint supports as well as pen grips and back supports in my chair.
I am open to talking about this and will answer any questions about it.

Over a year and a half ago i was lucky enough to be part of an upper spasticity trial to have 5 weeks of intense hand therapy every weekday for 2-3 hours a day. it helped my arm, shoulder, and wrist. I saw immense improvements and actually had the best ever results recorded from the study at that time.
I think back and think to myself "it didn't work". I couldn't move my fingers by the end, and what is wrong with me. I had feelings of shame, guilt and hated myself for trying so hard and not getting the results i wanted.
I had my physiotherapist tell me recently that the problem wasn't in the joint, limb, ect itself, it was in my brain. Not long after that session I had my new psychologist tell me that my brain injury was a trauma in itself. two things i couldn't wrap my head around, because i thought the problem was with me and i just wasn't trying hard enough over the last 18 years to erase this huge part of me that i hated about myself.
Internalised ableism is something that has defined much of my life, and i didn't cause it, i was taught it. Ableism is defined as discrimination against disabled people and/or having a favour for able-bodied people. Internalised ableism is when you're disabled and have that belief about yourself. A short time after my stroke i went back to school a few days a week in a wheelchair where i was relentless bullied for being disabled. This was also a pivotal point in my life when self worth was being developed at the start of adolescence. It was taken away from me because i had the chance to form it like others my age. I was taught to hate myself and struggled coming to terms with what had happened to me, and i still do. I still whole heartedly believe that im living proof bullying can kill you; because it almost did. I spent over a decade of my life struggling with self harm, substance abuse and countless suicide attempts after that trauma, and now in recovery from drugs and alcohol, i can understand just how much that time of my life impacted me.
It's believed that damage to the basal ganglia in stroke victims can be reversed with neuroplascisity (rewiring your brain), yet i have to come to terms with the fact that wasn't a reality for me to a full extent. I am grateful to be able to walk without assistance (although my spatial awareness and coordination is poor), i can talk (with some disordered speech when fatigued and chronic dysphagia), and have somehow perfect vision aside from occasional nystagmus. I have dystonia, spascisity and high tone in my left arm/hand, and despite working endlessly hard on it, remains rigid and paralysed. I have myoclonic seizures on a daily basis and struggle to do some daily tasks, i do everything on my own yet do them slowly. I am very grateful to have a supportive family who cook for me, and ndis services to access the community. I am grateful for the progress i have made and one day hope to understand why this happened to me much like others with lifelong disabilities. After decades of being on pharmaceuticals i am so grateful to be living a holistic life full of amazing natural remedies that help me. This includes castor oil, magnesium oil, a clean diet and of course physiotherapy stretches and exercise.
I know like anyone that has suffered significant trauma in their lives, forging meaning and identity can be hard. Despite every obstacle i have come across in life i still work hard on my daily mobility. I hope one day to show others living with brain injuries and trauma that they too can have meaningful, impactful lives.

I swear I have made the same post over and over again about like... "augh I want to make stuff but my moderate to severe joint pain is so overwhelming that I just cannot even handle this fucking situation."
I have an appointment with my rheumatologist at the end of the month and I like. Absolutely have to make him understand that like... I am experiencing inflammation and pain all over my body and like. He's gotta treat it. Or refer me to someone who will because I am like. Sinus inflammation, joint inflammation, spine inflammation, eye inflammation, foot inflammation. Like. I got a feeling my inflammation is off the charts.
I need to sort some shit out and figure out who to talk to. It's very, very difficult to try and keep up with some of all this with like... this much pain and stress and maladaptive coping mechanisms... admitting to therapists some degree, the amount of dissociation going on, which I have been thinking does explain the muscle tension and pain in my jaw, neck, and shoulders... at least a little. I know I also got some terrible fucking posture. But I also know I been smothering my emotions down there.
There are big steps in between becoming self-aware of a problem and solving a problem, though. And like part of breaking them down to smaller steps is just like. Figuring out what even any of the solutions are, let alone how to go about achieving those solutions. I guess maybe physiotherapy and massage would be like... temporary help for the muscle tension... idk.

Journal Entry #56 (part two)
Journal Entry 56 (part one) // STORY INDEX
Yuri
In other news, Victor and I had a big day yesterday.
We got up early because he had an appointment at the hospital, first thing in the morning, for x-rays on his left arm and a consultation with Dr. Sato. I was glad that I was feeling well enough to come along, because it meant I got to hear Dr. Sato tell him that he's now able to resume all his normal activities. She gave him a letter that stated he was fit to return to work, and a written referral for physiotherapy, and he accepted both as if she'd presented him with a gold medal.
Then, I got to witness the moment when his second cast came off. He cried a little bit when the medical technician pulled the remnants of his cast away and he saw his naked left arm for the first time since January. It was not pretty. His left forearm looked somewhat shrunken compared to the other one, and the skin was sickly pale and peeling in several spots.
Victor couldn't seem to stop staring at it. His voice trembled as he said, 'It... It looks like an old man's arm. The other one didn't look like that."
"The other one was only in a cast for half as long as this one," the technician said. "Don't worry. This is totally normal."
"It doesn't look normal," Victor said.
I was standing at his right side, and I rested my palm on his shoulder. "It'll be okay," I said. I glanced up at the technician briefly, "Right?"
The medical technician smiled. "Yes, it'll be fine. We'll get that arm all cleaned up for you, check your mobility, and then you'll be good to go."
He made his way across the small room to where there was a sink and some cabinets. From a covered box next to the sink, he took out a washcloth, and then he selected a small towel from one of the cabinets. Once he'd made a few more preparations, he came back to us, and proceeded to wash and dry Victor's arm.
"Your arm's getting its very own spa treatment," I commented.
Victor still looked dubious, but he relaxed visibly as his arm began to look less grimy and damaged. It was still thin from disuse, obviously, but having nearly two months' worth of sweat, flaking dead skin and accumulated debris washed off it made a vast improvement in its appearance.
The technician applied moisturizer, and Victor made a face. I laughed. "Spa treatment," I reiterated in a stage whisper.
"You'll want to keep moisturizing a couple of times a day for the next week or so," the technician told him. "It'll help with the elasticity of your skin."
Victor turned to me. "You're the master of moisturizer. You can do that for me."
"Gladly," I said.
The technician checked Victor's mobility next, gently manipulating his wrist, fingers and thumb. He asked if it hurt or was uncomfortable in any way, and Victor said everything felt stiff but not painful. It was only the joint at the base of his thumb that he said was hurting, "A tiny bit."
We were instructed that Victor should take ibuprofen if his thumb continued to hurt for more than the next couple of hours. Then, with a final admonition to take it easy and to not miss any of his physio appointments, the technician released Victor from his care and we were able to go home.
As we walked out of the hospital, I held Victor's left hand. He kept squeezing my fingers gently every so often, possibly to reassure himself that his own were working.
"Everything okay?" I asked.
"Yeah," he said. "I'm just getting used to having both hands back. Letting it sink in, you know?"
By the time we got outside, the reality of having both his hands back must've sunk in completely, because he let go of me and started running around the parking lot like a crazy person, shouting "I'm free!"
I admonished him to calm down, not that I really thought it'd do any good, or that he was even listening. I smiled, watching him burn off his sudden burst of energy and feeling infinitely grateful to have him. I don't mind his nonsense most of the time. That's the sort of thing one has to accept as part of life with Victor. Sometimes he just can't contain himself.
We went home for lunch, where Victor had a grilled cheese sandwich, an apple and soup, and I had my formula and a small piece of apple that he cut off for me. I stole one of the hard maple sugar candies his grandfather had sent him too. I love those. They last a long time if I simply hold them in my mouth and let them dissolve slowly, so I get to enjoy the sugary goodness for a while. Upon reflection, I think I might have an above-average sweet tooth, which probably sounds strange coming from someone who has as complicated a relationship with food as I do.
But, anyway... I digress.
After lunch, Victor had another medical appointment, this time with his ophthalmologist, Dr. Ishida. I was tired, but I was still feeling okay, so I decided to come with him to that appointment as well.
Dr. Ishida reassessed Victor's vision, and she concurred with Dr. Sato that he was fit to do whatever he'd been doing before his accident. Then, she sent us a few doors down the street to the optician's office, so Victor could arrange a time to have his eyes tested for corrective lenses.
As it happened, their in-house optometrist had a cancellation and was able to do it straight away. Victor was beyond excited about that, and happily subjected himself to the optometrist's various tests, all the while chatting nonstop about everything he wanted to do once he got his new glasses. I'm sure the poor optometrist was glad to see the back of him, but she somehow managed to be gracious.
I helped Victor choose a nice set of frames, and was pleased that my earlier prediction was correct. He does look very cute with glasses. His frames are red, of course. I would've been surprised if he'd wanted some other colour.
The optician told us that Victor's glasses would be ready in twenty-four to forty-eight hours. They'd give us a call, she said, and Victor could come back to pick them up and have the fit adjusted, if necessary.
Victor was practically bouncing when we left there. This time, instead of telling him to settle down, I suggested that he should do something to redirect his energy. He decided that he'd like to go for a swim at the fitness centre and since he also needed to give Dr. Sato's letter to Tomiko, his boss. I told him that was a logical choice.
I went with him, not to swim but to cool my feet in the water and admire my gorgeous husband in his swimwear. I love watching him do anything athletic. He's so beautiful and graceful, and he makes it all look so easy.
I'll get back to swimming soon, I hope. I only learned last year, and I want to build up my confidence in the water so I won't be too nervous to enjoy the ocean when Victor and I finally get to take our trip to Sulani. We probably won't be going until later in the summer or in early autumn, but I've already checked with Dr. Kasongo to see if ocean swimming with my tube is allowed, just in case I still have it by then. She said yes, as long as I cover it to keep it safe and we thoroughly clean it afterwards.
Thinking about Sulani makes me happy. I can hardly wait to go. I want to explore everywhere and experience everything I can. Maybe I'll even try some new foods, if I'm feeling especially brave.
Victor says I should try pineapple. Apparently, there are places in Sulani where you can pick your own fruit, and Victor told me all about picking pineapples when he was there last spring. I've never eaten pineapple before, but I like fruit and I like the idea of eating something I harvested with my own hands, so we'll see.
By the time Victor was done in the pool, we were both exhausted and ready for some rest. I went straight to bed as soon as we got back to the house, and I skipped my dinnertime formula, even though I'm not supposed to do that.
Victor also skipped dinner. He got ready for bed and stayed in my room with me, but I think he was still too worked up from the day's events to sleep. He phoned his mother, heedless of how early in the morning it was in her time zone, and lay there cuddling me while he chatted with her on speakerphone. I fell asleep with Victor's hand in my hair, comforted by the nearness of him and by his and Dr. Grace's warm, familiar voices.
This morning, I slept late. Victor prepared my formula for me, and then he propped me up with a bunch of pillows so I could have the tube feeding equivalent of breakfast in bed. He climbed back into bed with me and lay with his head in my lap while I waited for my formula to finish going in. It takes about twenty to thirty minutes, usually. That's perhaps a little longer than it might take a normal person to eat a regular meal, but it's not as if we don't linger at the table for at least half an hour during most mealtimes anyway, so having my formula doesn't interfere all that much with my time during the day.
Victor is fascinated by it, which is a change from how he was when I first got out of the hospital. In the very beginning, he was nervous of it and he was hesitant to touch it. He didn't even like looking at it initially, but I think his curiosity about all things medical finally overpowered his discomfort. Now he wants to know everything about my experience with it.
"Can you feel it going in?" he asked. He trailed a forefinger across my belly, his touch feather-light, a few centimetres below my tube. "Does it feel weird?"
"I can't feel the formula going in, but I know when I'm full," I said.
"Does it feel the same as when you get full from actually eating?"
"Yes," I told him. "Stop tickling my belly, please."
"Sorry," he said, even though he quite evidently wasn't. He let his fingertip slide down toward the waistband of my pyjama pants. "Can I tickle somewhere else?"
"Not until I'm done with breakfast."
He fake pouted. “I guess I can wait till after breakfast, but not much longer than that. I mean, I have two working hands now, and if you’re feeling up to it, I thought we might… you know. Play a little.”
I thought about it, and realized that I was indeed feeling up to it. “As long as you’re gentle and we don’t move around too much.”
“You can just lie there and enjoy yourself, and I’ll give special attention to all your favourite spots, okay?”
“Okay, and then I can give some attention to yours.”
"Sounds good," he said, and hummed with satisfaction.
He rolled away from me and stretched languidly, his muscles rippling fluidly all along his bare upper body and arms. I wanted to touch him, to trace every perfectly sculpted contour of him. Something stirred in me that I hadn’t felt in months, and I was suddenly very impatient for my feeding to be done.
Victor peered up at me. His silver eyelashes fluttered over his half-closed eyes. Oceanic blue, my mother had once described his eye colour. I love that image; eyes the colour of a tropical sea, with a little dusting of silvery clouds above them. I'm often mesmerized by them.
“What?” Victor said.
I blinked. "Excuse me?"
“Your expression changed,” he said. “You… oh. Ohh.”
It was my turn to demand, “What?”
He grinned at me, mischievous and knowing. “You want me.”
I could not deny it.

Five Most Effective Physiotherapy Exercises for TMJ Pain Treatment
Has anyone ever gotten the compliment, “Your smile lights up the room”? Well, then it’s the T MJ muscles that you need to thank. TMJ muscles support your temporomandibular joint or TMJ, thereby helping you smile, yawn, chew, or even talk. TMJ pain can occur due to various reasons. But, the right physiotherapy exercises can help relieve the pain efficiently.
So, let’s check out the five most effective physiotherapy exercises for TMJ pain.
Chin tucks
Steps to do it:
Stand with your shoulders back and chest up.
Pull your chin such that you create a double chin.
Hold the position for three seconds.
Repeat ten times.
Relaxed jaw exercise
Steps to do it:
Place your tongue on the top of your mouth. It should be behind the teeth in your upper front jaw.
Relax your jaw muscles in this position.
Resist mouth-opening
Steps to do it:
Open your mouth.
Add gentle pressure to the chin while doing so.
Keep two fingers under your chin while opening the mouth.
Hold the position for a few seconds.
Close your mouth slowly.
Goldfish exercises (full opening)
Steps to do it:
Put your tongue on the mouth’s roof.
Keep one finger on your chin and another finger on one side of the TMJ.
Drop your lower jaw and restore your position.
Repeat six times for one set on each side.
Jaw forward movement
Steps to do it:
You need an object to perform this exercise.
The object should be ¼ inch thick.
Hold the object between your front teeth.
Move your jaw forward.
Keep the bottom teeth in front of your top ones.
You can switch the object with a thicker one once the exercise becomes easier.
You can try out the above-mentioned physiotherapy exercises for TMJ pain treatment. But, it is recommended to do these under the supervision of experienced physiotherapists.
Final Thoughts,

I think there's usually two versions of "doing x will help with y" that tend to create trouble for people.
1 - the obvious. This in fact does not help, will not help, and never help
2 - the frankly much more emotionally exhausting. This would help but only if it looked radically different than it has ever been offered to me
I think a lot about the movement one especially. For a long time I just got over and over again "just exercise and the pain will stop" and no one had an answer for when I said "okay but until that happens I'm in too much pain to move so what the fuck do my guy????" Then one day I just. I just went to see a personal trainer at a physiotherapy office. Not for physio! For a workout. And I was like. Okay here is what I can do and here is what hurts to do, please help me figure out how to x without pain. And she actually did! Like. I don't "just go move my body" in a way that anyone around me has ever treated like it counted. But she literally showed me joint mobilization exercises that reduced my pain right there in the session and for 72hrs after!!! No physio had ever shown me those options before, or taught me like the trainer did to start by re-mobilizing the joints, and THEN start doing full body movement exercises like rowing or elepitcal that had low-/no-impact on the joints. And I did. And it's at the point where I've already gotten back a TON of range of motion to places like my hips, my shoulders, etc. Places that, after 20yrs, I'd frankly given up on.
Sometimes you can't "just" move, you have to be taught the RIGHT movement to help you. Sometimes you can't "just" go anywhere outside, you have to know what kind of environment you might actually find comfortable with enough load-bearing needs met.
I've been learning more and more how widely the idea of accommodation can actually span and how many things I never knew could be experienced any other way, and I think when you're used to things not being for you it can be really hard to figure out how to build a life around yourself that works for you AND is fulfilling. I kinda had to learn how to even want things enough to ask myself if there was a version of the thing that actually worked for me because it felt easier to never want things than to want them and not be able to rely on them.
Either way, the important thing to come out of all this with is how personalized "helping" can be, and how important it is that we trust people to tell us what they need without trying to assert our own opinion over theirs.
Like "go exercise, you'll feel so much better once you move your body" doesn't apply to a lot of physically disabled people. And "just force yourself to socialize, you will have fun once you're there" doesn't apply to a lot of autistic people. And "your intuition will always tell you the truth" doesn't apply to a lot of psychotic people. And so on. And I'm not saying that this means that you can't or shouldn't promote these things, but they aren't universal solutions for everyone
